    Fast Neural Network Predictions from Constrained Aerodynamics Datasets

    Incorporating computational fluid dynamics in the design process of jets, spacecraft, or gas turbine engines is often challenged by the required computational resources and simulation time, which depend on the chosen physics-based computational models and grid resolutions. An ongoing problem in the field is how to simulate these systems faster but with sufficient accuracy. While many approaches involve simplified models of the underlying physics, others are model-free and make predictions based only on existing simulation data. We present a novel model-free approach in which we reformulate the simulation problem to effectively increase the size of constrained pre-computed datasets and introduce a novel neural network architecture (called a cluster network) with an inductive bias well-suited to highly nonlinear computational fluid dynamics solutions. Compared to the state-of-the-art in model-based approximations, we show that our approach is nearly as accurate, an order of magnitude faster, and easier to apply. Furthermore, we show that our method outperforms other model-free approaches

    Design for producing fiberglass fabric in a lunar environment

    The purpose of this project was to design a method of producing a fabric material on the lunar surface from readily available glass fibers. Various methods for forming fabrics were analyzed to determine which methods were appropriate for the lunar conditions. A nonwoven process was determined to be the most suitable process for making a fabric material out of fiberglass under these conditions. Various resins were considered for adhering the fibers. A single thermoplastic resin (AURUM) was found to be the only applicable resin. The end product of the process was determined to be suitable for use as a roadway surfacing material, canopy material, reflective material, or packaging material. A cost analysis of the lunar process versus shipping the end-product from the Earth suggests that the lunar formation is highly feasible.     Model Order Reduction for Rotating Electrical Machines

    The simulation of electric rotating machines is both computationally expensive and memory intensive. To overcome these costs, model order reduction techniques can be applied. The focus of this contribution is especially on machines that contain non-symmetric components. These are usually introduced during the mass production process and are modeled by small perturbations in the geometry (e.g., eccentricity) or the material parameters. While model order reduction for symmetric machines is clear and does not need special treatment, the non-symmetric setting adds additional challenges. An adaptive strategy based on proper orthogonal decomposition is developed to overcome these difficulties. Equipped with an a posteriori error estimator the obtained solution is certified. Numerical examples are presented to demonstrate the effectiveness of the proposed method

    Continuous infusion of cefiderocol in a critically ill patient with continuous venovenous haemofiltration

    Cefiderocol is a broad-spectrum cephalosporin antibiotic and is indicated in patients with difficult-to-treat Gram-negative bacterial infections. Cefiderocol is applied as a 2–4-times daily prolonged 3-h infusion. The therapeutic target of cefiderocol suggests that continuous infusion (CI) may be advantageous, since it is more likely to achieve 100% of time of the unbound concentration above the minimal inhibitory concentration (MIC). However, limited information on cefiderocol as CI has been assessed. We present a case of a critically ill 37-year-old woman with continuous venovenous haemofiltration (CVVH) treated with a CI of cefiderocol for multidrug-resistant Pseudomonas aeruginosa. She received 4 g per 24 h, in accordance with the recommendations for the total daily dose during CVVH with an effluent flow rate of 2.1–3 L/h. We evaluated intraperitoneal, plasma arterial pre- and postfilter and ultrafiltrate (urine) total cefiderocol concentrations and discussed the pharmacokinetics in respect to the CVVH settings. The predicted unbound plasma concentrations during CI resulted in 6.8–9.5-fold higher concentrations than the adopted MIC of 2 mg/L for cefiderocol against P. aeruginosa. The optimal time of the unbound concentration >MIC target of cefiderocol was met during the sampling period, suggesting adequate exposure during the total treatment period. The obtained intraperitoneal concentration indicated adequate cefiderocol exposure at the site of infection. Continuous infusion of 4 g cefiderocol per 24 h led to sufficient plasma concentrations in our anuric critically ill patient treated with CVVH. This case is supportive to the use of cefiderocol as continuous infusion

    This review explored the following question: Are the comprehensive treatment models Makoto Therapy, Brain Gym, and Interactive Metronome effective interventions for improving occupational performance including improving executive function, academic performance, and physical coordination in children and adolescents with Autism Spectrum Disorder (ASD)? Because current research on Interactive Metronome, Brain GymÂź, and Makoto Therapy fails to address children and adolescents with autism spectrum disorder, presents multiple flaws in research design, and does not measure occupational outcomes such as occupational performance, we recommend that these interventions should not be used as comprehensive treatment models in occupational therapy. We recommend that more occupational-based, methodologically-sound research involving youth with ASD be conducted before implementing these interventions in occupational therapy practice

    Low-Dose Antithymocyte Globulin Has No Disadvantages to Standard Higher Dose in Pediatric Kidney Transplant Recipients: Report from the Pediatric Nephrology Research Consortium

    Introduction Rabbit antithymocyte globulin (rATG) dosing strategies for induction in pediatric kidney transplantation vary between centers. It is not known whether a lower rATG induction dose provides safe and effective immunosuppression compared with a “standard” higher dose. Methods We performed a retrospective multicenter study of all isolated first-time kidney transplant recipients \u3c 21 years old who received rATG induction between 1 January 2010 and 31 December 2014 at 9 pediatric centers. An a priori cutoff of a 4.5-mg/kg cumulative rATG dose was used to identify low (≀ 4.5 mg/kg) and standard (\u3e 4.5 mg/kg) exposure groups. Outcomes examined included 12 months posttransplant graft function (estimated glomerular filtration rate [eGFR]); the occurrence of acute rejection, donor-specific antibody (DSA), neutropenia, and viral infection (cytomegalovirus [CMV], Epstein-Barr virus [EBV], and BK virus); and 24-month outcomes of posttransplant lymphoproliferative disorder (PTLD) occurrence and patient and graft survival. Results Two hundred thirty-five patients were included. Baseline features of the low and standard rATG dose groups were similar. By 12 months, the rATG dose group had no significant impact on the occurrence of neutropenia, positive DSA, or viral polymerase chain reaction (PCR). Graft function was similar. Acute rejection rates were similar at 17% (low dose) versus 19% (standard dose) (P = 0.13). By 24 months, graft survival (96.4% vs. 94.6%) and patient survival (100% vs. 99.3%) were similar between the low- and standard-dose groups (P = 0.54 and 0.46), whereas the occurrence of PTLD trended higher in the standard-dose group (0% vs. 2.6%, P = 0.07). Conclusion A low rATG induction dose ≀ 4.5 mg/kg provided safe and effective outcomes in this multicenter low immunologic risk pediatric cohort. Prospective studies are warranted to define the optimal rATG induction dose in pediatric kidney transplantation

    From fibers to satellites: Lessons to learn and pitfalls to avoid when optical communications move to long distance free space

    peer reviewedThe paper summarizes the recent investigation on feasibility of adapting state-of-the-art coherent fiber-optics (FO) systems for Free Space Optical (FSO) scenarios. This investigation is critically dependent on the intertwined aspects of architecture, as well as device and propagation impairments (including the channel) appearing in the aforementioned systems. Towards this, the work identified the key system differences between the two systems. Particularly, the FSO channel model was investigated, impact of atmospheric turbulence on FSO was discussed and a channel series was generated. Subsequently, relevant FO techniques including coherent detection, wavelength division multiplexing and Time-Frequency packing (TFP) were reviewed. Another departure from FSO works was the emphasis on coherent reception; receiver architectures and diversity schemes were first investigated. The former strived to make fair comparison amongst the receivers considering the diverse nature of perturbation added, while the latter indicated gain in performance through increase of diversity order (2-4 dB gain). An immediate conclusion is a suggestion on adaptation of wavelength diversity when coherent receivers. The investigation also evaluated the capacity and outage of fast and slow fading channels with parameters motivated by the channel modelling work. The shaping gain was evaluated and an LDPC code design example was provided for FSO downlinks. Finally, TFP enabled a remarkable performance gain when applied to coherent detection schemes, but only marginal with direct detection. The paper concludes by pointing to the next steps that build on this investigation and the need to corroborate with measurements

    Internet-based search of randomised trials relevant to mental health originating in the Arab world

    BACKGROUND: The internet is becoming a widely used source of accessing medical research through various on-line databases. This instant access to information is of benefit to busy clinicians and service users around the world. The population of the Arab World is comparable to that of the United States, yet it is widely believed to have a greatly contrasting output of randomised controlled trials related to mental health. This study was designed to investigate the existence of such research in the Arab World and also to investigate the availability of this research on-line. METHODS: Survey of findings from three internet-based potential sources of randomised trials originating from the Arab world and relevant to mental health care. RESULTS: A manual search of an Arabic online current contents service identified 3 studies, MEDLINE, EMBASE, and PsycINFO searches identified only 1 study, and a manual search of a specifically indexed, study-based mental health database, PsiTri, revealed 27 trials. CONCLUSION: There genuinely seem to be few trials from the Arab world and accessing these on-line was problematic. Replication of some studies that guide psychiatric/psychological practice in the Arab world would seem prudent
