    Clinico-economic analysis of PD-1 inhibitors and combinations of BRAF and MEK inhibitors for the treatment of metastatic melanoma with a BRAF V600 gene mutation

    The high prevalence of BRAF V600 gene mutation in the Russian Federation and the cost of treatment the metastatic melanoma (MM) with immuno-oncology and combined targeted drugs make it relevant to assess the clinical and economic feasibility of their use. Objective. Cost-effectiveness analysis (CEA) and budget impact analysis (BIA) of PD-1 inhibitor pembrolizumab was performed in comparison with nivolumab and combinations of BRAF and MEK inhibitors in the 1st line therapy of MM with BRAF v600 gene mutation. Methodology. CEA and BIA was performed using the results of a network meta-analysis and based on the Markov model, which estimated the number of life years saved (LYS), duration of progression-free survival and direct medical costs (DC). Results. The cost of treatment per year in the 1st line therapy on pembrolizumab was 3.32 million RUB, what is 29 % cheaper than combination of dabrafenib + trametinib (4.67 million RUB), 60 % than vemurafenib + cobimetinib (8.30 million RUB), 1.7 % than nivolumab (3.38 million RUB). Total 3 years DC for the 1st line therapy on pembrolizumab (3.80 million RUB) were 43.9%, 68.2% and 1.8 % lower than the cost of dabrafenib + trametinib (6.76 million RUB), vemurafenib + cobimetinib (11.93 million RUB) and nivolumab (3.86 million RUB). CER per 1 LYS for pembrolizumab was 2.43 million RUB, nivolumab – 2.48 million RUB, dabrafenib + trametinib – 4.24 million RUB and vemurafenib + cobimetinib – 7.49 million RUB increase in the use of pembrolizumab to 50 %, by reducing the share of the use of combinations of BRAF and MEK inhibitors, will result in budget savings up to 1 507,5 million RUB (26.4 %). Conclusion. The use of pembrolizumab in the 1st line of MM therapy with BRAF V600 gene mutation allows to save the budget and is economically feasible

    Heterotopic Gastric Mucosa in Cervical Oesophagus: Clinical Observations

    Aim. Description of the endoscopic and clinical traits of heterotopic gastric mucosa (HGM) observed in cervical oesophagus.Key points. HGM in proximal oesophagus can be asymptomatic or have various clinical manifestations. A 40-yo female patient consulted a gastroenterologist with complaints of cough and globus sensation. For several years she was visiting an otorhinolaryngologist and psychotherapist, with therapy ineffective. Esophagogastroduodenoscopy (EGDS) at the last visit revealed several foci of HGM in cervical oesophagus of 1.2 x 0.8 cm maximal size. The patient was prescribed a combined prokinetic — proton pump inhibitor therapy, which relieved the symptoms. EGDS in a 21-yo patient without active complaints revealed a 2 cm-wide HGM of 4/5 cervical oesophageal lining with acidproducing zones.Conclusion. Two different scenarios of cervical oesophageal HGM are described, the first one manifested with laryngopharyngeal reflux, and the second devoid of clinical manifestations despite a large heterotopic site

    Esophageal Lichen Planus as a Cause of Dysphagia: Literature Review and Clinical Observation

    Aim: to analyze the literature data, and to raise awareness of doctors of various specialties about the methods of diagnosis and treatment of esophageal lichen planus (ELP).Key points. In a 67-year-old female patient with complaints of difficulty swallowing solid food and weight loss, esophagogastroduodenoscopy revealed subcompensated stenosis of the middle third of the esophagus and signs of fibrinous esophagitis. Based on the characteristics of the endoscopic picture and the detection of apoptotic Ciwatt bodies in esophageal biopsies, a diagnosis of ELP was established. Treatment with glucocorticosteroids led to relief of symptoms and positive endoscopic dynamics. ELP is rare and the least studied, data on this disease in the literature are presented mainly in the form of clinical observations and analysis of series of cases. Typical clinical manifestations include dysphagia and odynophagia. Despite the low prevalence, ELP can be associated with serious complications: stenosis and esophageal squamous cell carcinoma. Endoscopic examination reveals characteristic signs in the esophagus: swelling, thickening and increased vulnerability of the mucosa, often with fibrin, formation of membranes and strictures. The histological picture is represented by epithelial dyskeratosis with exfoliation, lichenoid lymphocytic infiltration. The most specific histological sign is the presence of apoptotic Civatte bodies. Recommendations for the treatment of ELP are limited to the results of a series of clinical observations and include the prescription of systemic corticosteroids. The issue of supportive therapy is the least studied.Conclusion. Analysis of the literature data and the clinical case demonstrate that lichen planus of the esophagus is one of the rare causes of dysphagia. Characteristic endoscopic and histological signs are key for the diagnosis. The management of patients with esophageal lichen planus is insufficiently defined and today includes taking of glucocorticosteroids, endoscopic dilation of stricture and dynamic endoscopic observation, given the high risk of squamous cell carcinoma in this category of patients

    MuSR method and tomographic probability representation of spin states

    Muon spin rotation/relaxation/resonance (MuSR) technique for studying matter structures is considered by means of a recently introduced probability representation of quantum spin states. A relation between experimental MuSR histograms and muon spin tomograms is established. Time evolution of muonium, anomalous muonium, and a muonium-like system is studied in the tomographic representation. Entanglement phenomenon of a bipartite muon-electron system is investigated via tomographic analogues of Bell number and positive partial transpose (PPT) criterion.     Prospects for the use of a <i>Bacillus subtilis</i> metabolites-based feed additive in dairy farming

    Laboratory and field experiment on use of the feed additive based on Bacillus subtilis endo- and exometabolites for the cows in different physiological periods are described in the paper. The feed additive impact on main body systems of the tested cows (n = 30), milk production parameters, growth rate of the calves (n = 18) born to the said cows were examined. The feed additive was added to the diets for the cows of test groups, 15 g per cow. The feed additive was found to have a positive effect on immunohematological and metabolic processes in postpartum cows. Neutrophils’ phagocytic activity increased by 12.5% and 14.6% in the animals of test group 1 and test group 2, respectively, as compared to that one in control animals (42.8 ± 1.9%). Neutrophil absorbency increased by 2.5 times, 3.2 times and 2.1 times in the animals of test group 1, test group 2 and control group, respectively. The proportion of T-lymphocytes in blood of animals in test group 1 and test group 2 was 44.5 and 48.9%, respectively, proportion of T-lymphocytes in blood of control animals equaled to 37.5%. Trend for increase in total protein concentration in cow sera owing to increase in albumin fraction was observed in postpartum period: it was 72.91 ± 3.45 g/l in test group 1; 75.54 ± 4.12 g/l in test group 2; 70.95 ± 4.25 g/l in control group. Average daily milk yield in cows of test group 1, test group 2 and control group for the 150 days of lactation was 24.50 ± 1.86 kg; 25.33 ± 1.45 kg and 22.75 ± 4.41 kg, respectively. Higher growth rate was reported for the calves born to the cows received the diet supplemented with the said feed additive. Heifers of test group 1 and test group 2 have reached body weight of 193.51 ± 5.76 and 195.33 ± 3.76 kg and in control group – of 187.33 ± 4.98 kg within 6 months. Feed additive based on endo- and exometabolites of Bacillus subtilis is recommended for cow diets for highly productive dairy herd creation and food-producing animal welfare maintenance
