    Appraisal of the Participatory and Decentralized approach to Rural Community Development in Cameroon

    This paper examines the core concepts and principles that underpin the participatory and decentralized approach to rural development using the Grassfield Participatory and Decentralized Rural Development Project (GPDERUDEP) in Cameroon as a case study. It describes how the local development fund (LDF) was used within GP-DERUDEP to shift resources and responsibilities for investment planning, contracting and implementation at the level of rural communities thus strengthening service provision, community and economic development. The paper posits that a critical appraisal of the mechanisms and structures of decentralization within the context of poverty alleviation strategies can provide clues in facing the challenges encountered in implementing projects in resource poor economies. The paper illustrates a framework that links social capital and community development to economic development outcomes. Data were obtained through project evaluations, field observations and focus group discussions with project stakeholders. The project provides an innovative approach to engage the state and local stakeholders in collective strategic decision-making in rural development. Project achievements are visible in socio-economic infrastructure and social capital but challenges persist in implementing decentralization policies due to entrenched top-down values and dependence on foreign donors to fund and sustain rural development programme in Cameroon.Keywords: Community Development, Decentralization, Participation, Grassfield Project, Local Development Fun

    State Manoeuvres in Hotel Acquisitions in Cameroon: An Analysis of the Ringway Hotel Saga, 1962- 1992

    Since the independence and reunification of Cameroon in 1960/61, state engagements in hotel provisioning has been one of the essential socio-economic devotions. Decision making in this sector has most often largely depended on administrative preferences than well-versed managerial pointers. Consequently, some state ventures in this direction have been characterised by, inappropriate forecasting and mismanagement. These setbacks in the long run culminate in the collapse of public hotel investments. Based on primary and secondary sources and informed by qualitative analysis, this article profiles a crucial case of a public hotel investment, the Ringway Hotel in the chief town of Bamenda (Cameroon) as it quivered and writhed for survival. It argues that the purchase and management of the hotel was trapped in controversies involving but not limited to the strategic interest of the state and different stakeholders. The state purchase and operational principles of the hotel did not fluidly align with the desire for which its purchase was envisioned.     GMP compliant isolation of mucosal epithelial cells and fibroblasts from biopsy samples for clinical tissue engineering

    Engineered epithelial cell sheets for clinical replacement of non-functional upper aerodigestive tract mucosa are regulated as medicinal products and should be manufactured to the standards of good manufacturing practice (GMP). The current gold standard for growth of epithelial cells for research utilises growth arrested murine 3T3 J2 feeder layers, which are not available for use as a GMP compliant raw material. Using porcine mucosal tissue, we demonstrate a new method for obtaining and growing non-keratinised squamous epithelial cells and fibroblast cells from a single biopsy, replacing the 3T3 J2 with a growth arrested primary fibroblast feeder layer and using pooled Human Platelet lysate (HPL) as the media serum supplement to replace foetal bovine serum (FBS). The initial isolation of the cells was semi-automated using an Octodissociator and the resultant cell suspension cryopreservation for future use. When compared to the gold standard of 3T3 J2 and FBS containing medium there was no reduction in growth, viability, stem cell population or ability to differentiate to mature epithelial cells. Furthermore, this method was replicated with Human buccal tissue, providing cells of sufficient quality and number to create a tissue engineered sheet

    Understanding of research, genetics and genetic research in a rapid ethical assessment in north west Cameroon

    BACKGROUND There is limited assessment of whether research participants in low-income settings are afforded a full understanding of the meaning of medical research. There may also be particular issues with the understanding of genetic research. We used a rapid ethical assessment methodology to explore perceptions surrounding the meaning of research, genetics and genetic research in north west Cameroon. METHODS Eleven focus group discussions (including 107 adults) and 72 in-depth interviews were conducted with various stakeholders in two health districts in north west Cameroon between February and April 2012. RESULTS Most participants appreciated the role of research in generating knowledge and identified a difference between research and healthcare but gave varied explanations as to this difference. Most participants' understanding of genetics was limited to concepts of hereditary, with potential benefits limited to the level of the individual or family. Explanations based on supernatural beliefs were identified as a special issue but participants tended not to identify any other special risks with genetic research. CONCLUSION We demonstrated a variable level of understanding of research, genetics and genetic research, with implications for those carrying out genetic research in this and other low resource settings. Our study highlights the utility of rapid ethical assessment prior to complex or sensitive research

    Grain morphological characterization and protein content of sixty-eight local rice (Oryza sativa L) cultivars from Cameroon

    Rice (Oryza sativa L.) cultivated in Cameroon is appreciated by consumers for its nutritive quality and good taste. Diversity of 68 local rice cultivars was investigated via grain morphology and protein content characterization. The size and shape of grains were determined and used with yield parameters to classify the cultivars and perform Principal Component Analysis (PCA). Total protein content and glutelin content of eight selected cultivars (CMRGNd, CMRGDn, CMRGTĂŻ, CMRTBa, CMRDWb, CMRDTc3, CMRDTx5 and CMRDTx6) were evaluated by Bradford assay and correlation analysis of all the parameters studied was performed. Long size grains (42) were predominant over extra-long (16), medium (9) and short (1) grains. Slender shaped grains (36) were distinguished as well as medium (28) and bold (4) grains. The 68 cultivars were grouped into four clusters independent of their origins. PCA revealed three principal components accounting for 74.4% of total variation. Highest total protein content was observed in CMRGNd (14.3%) and highest glutelin content in CMRGDn (10.1 mgEqvBSA/g DW). Pearson correlation of the different variables revealed no significant correlation between total protein and glutelin contents with the agro-morphological parameters evaluated in this study. This suggests that none of these parameters could be descriptor for protein content. Positive correlation between grain length and yield (r = 0.7) suggests grain length as yield descriptor

    Predominance of N-acetyl transferase 2 slow acetylator alleles in children less than ten years experiencing adverse treatment events following artemisinin-based combination therapy in North and South West Regions of Cameroon

    The human N-acetyltransferase II (NAT2) gene may vary between individuals resulting in variability in the incidence of adverse drug reactions. We set out in this adhoc analysis to determine the distribution of allele frequencies of NAT2 gene variants among children less than ten years treated with artemisinin-based combinations in Cameroon. Children with uncomplicated malaria were deparasitized with artemisinin based combination therapy (artesunate amodiaquine, dihydroartemisinin-piperaquine and artemether-lumefantrine) and followed up for 42 days and adverse events recorded. Blood was spotted on filter paper prior to drug administration for DNA extraction by chelex method. Standard nested PCR followed by restriction enzyme analysis with KpnI, TaqI, and BamHI for detection of polymorphisms in the NAT2 was performed. Allelic frequencies and acetylator phenotypes were compared between participants with or without adverse drug events. The prevalence of slow, intermediate and fast acetylators was 54, 34 and 12% respectively. Significant clustering of mutant alleles (NAT2*5, NAT2*6 and NAT2*7) occurred among those who experienced skin rash and tiredness (OR = 5.765, P = 0.046) (OR = 13.280, P = 0.020). No significant difference was observed between fast and slow acetylators with respect to the development of other recorded adverse events. Overall, producers of the slow acetylator alleles were more likely to develop any adverse events (OR = 8.286, P = 0.017) during the study period. Mutant alleles of the wild type NAT2*4 associated with the slow acetylator phenotype were the most predominant NAT2 allelic type and individuals with the phenotype were more likely to develop adverse events to ACTs.Keywords: N-Acetyltransferase 2, Artemisinin based combination therapy, adverse events, slow acetylators, allel

    Hands-on training in structural biology, a tool for sustainable development in Africa series 4.

    Structural biology is an essential tool for understanding the molecular basis of diseases, which can guide the rational design of new drugs, vaccines, and the optimisation of existing medicines. However, most African countries do not conduct structural biology research due to limited resources, lack of trained persons, and an exodus of skilled scientists. The most urgent requirement is to build on the emerging centres in Africa - some well-established, others growing. This can be achieved through workshops that improve networking, grow skills, and develop mechanisms for access to light source beamlines for defining X-ray structures across the continent. These would encourage the growth of structural biology, which is central to understanding biological functions and developing new antimicrobials and other drugs. In this light, a hands-on training workshop in structural biology series 4 was organised by BioStruct-Africa and the Malaria Research and Training Center (MRTC) in Bamako, Mali, to help bridge this gap. The workshop was hosted by MRTC from the 25th to 28th of April 2022. Through a series of lectures and practicals, the workshop enlightened the participants on how structural biology can be utilised to find solutions to the prevalent diseases in Africa. The short training gave them an overview of target selection, protein production and purification, structural determination techniques, and analysis in combination with high-throughput, structure-guided, fragment-based drug design

    As the study of disease occurrence and health indicators in human populations, Epidemiology is a dynamic field that evolves with time and geographical context. In order to update African health workers on current epidemiological practices and to draw awareness of early career epidemiologists on concepts and opportunities in the field, the 3rd African Epidemiology Association and the 1st Cameroon Society of Epidemiology Conference was organized in June 2-6, 2014 at the YaoundĂŠ Mont Febe Hotel, in Cameroon. Under the themeÂŤPractice of Epidemiology in Africa: Stakes, Challenges and PerspectivesÂť, the conference attracted close to five hundred guest and participants from all continents. The two main programs were the pre-conference course for capacity building of African Early Career epidemiologists, and the conference itself, providing a forum for scientific exchanges on recent epidemiological concepts, encouraging the use of epidemiological methods in studying large disease burden and neglected tropical diseases; and highlighting existing opportunities

    Community-directed delivery of doxycycline for the treatment of onchocerciasis in areas of co-endemicity with loiasis in Cameroon

    <p>Abstract</p> <p>Background</p> <p>Severe side effects following ivermectin treatment of onchocerciasis in areas of co-endemicity with loaisis have been an impediment for the work of the African Programme for Onchocerciasis Control (APOC) in forested regions of several countries. Doxycycline has been shown to be effective in the treatment of onchocerciasis and has the added advantages of killing adult <it>Onchocerca volvulus </it>but neither adult <it>Loa loa </it>nor their microfilariae. This drug therefore offers great potential for the treatment of onchocerciasis in areas of co-endemicity with loiasis. The limitation of use of this drug is the duration of treatment that may pose a potential problem with therapeutic coverage and compliance with treatment. To benefit from the advantages that doxycycline offers in the treatment of onchocerciasis, it will be necessary to establish an effective distribution system that can access remote communities. This study assessed the feasibility of a large-scale distribution of doxycycline for the treatment of onchocerciasis in areas of co-endemicity with loiasis using a community-directed approach.</p> <p>Methods</p> <p>The study was carried out in 5 health areas co-endemic for <it>Onchocerca volvulus </it>and <it>Loa loa </it>which had no prior experience of the Community Directed Treatment with Ivermectin (CDTI). The community-directed delivery process was introduced using a cascade mechanism from the central health system that passed through the regional health delegation, health district and the health areas. Community health implementers (CHIs) were trained to deliver doxycycline to community members and, under the supervision of the health system, to monitor and document drug intake and side effects.</p> <p>Results</p> <p>The community members adhered massively to the process. Of the 21355 individuals counted, 17519 were eligible for treatment and 12936 were treated with doxycycline; giving a therapeutic coverage of eligible population of 73.8%. Of the 12936 who started the treatment, 97.5% complied by the end of six weeks. No serious side effect was registered during the six week treatment.</p> <p>Conclusion</p> <p>This study indicates that when empowered the community health implementers can successfully deliver doxycycline for six weeks for the treatment of onchocerciasis in areas of co-endemicity with loiasis. The therapeutic coverage and the compliance treatment rate achieved in this study coupled to the known efficacy of doxycycline on <it>O. volvulus</it>, are indicators that the strategy involving the mass administration of doxycycline can be used to control onchocerciasis in those areas of co-endemicity with loiasis where ivermectin may be contraindicated.</p

    Evolution‐Guided Engineering of Non‐Heme Iron Enzymes Involved in Nogalamycin Biosynthesis

    Microbes are competent chemists that are able to generate thousands of chemically complex natural products with potent biological activities. Key to the formation of this chemical diversity has been the rapid evolution of secondary metabolism. Many enzymes residing on these metabolic pathways have acquired atypical catalytic properties in comparison to their counterparts found in primary metabolism. The biosynthetic pathway of the anthracycline nogalamycin contains two such proteins, SnoK and SnoN, belonging to non‐heme iron and 2‐oxoglutarate‐dependent mono‐oxygenases. In spite of structural similarity, the two proteins catalyse distinct chemical reactions; SnoK is a C2–C5′′ carbocyclase, whereas SnoN catalyses stereoinversion at the adjacent C4′′ position. Here we have identified four structural regions involved in the functional differentiation and generated 30 chimeric enzymes to probe catalysis. Our analyses indicate that the carbocyclase SnoK is the ancestral form of the enzyme from which SnoN has evolved to catalyse stereoinversion at the neighboring carbon. The critical step in the appearance of epimerization activity has likely been the insertion of three residues near the C‐terminus, which allow repositioning of the substrate in front of the iron center. The loss of the original carbocyclization activity has then occurred with changes in four amino acids near the iron center that prohibit alignment of the substrate for formation of the C2–C5′′ bond. Our study provides detailed insights into the evolutionary processes that have enabled Streptomyces soil bacteria to become the major source of antibiotics and antiproliferative agents.</div
