    Karo Kari : the murder of honour in Sindh Pakistan : an ethnographic study

    This paper aims to discuss the wider context, in which honour murders occur, the social structures which contribute to the occurrence and perpetuation of the practice of honour murders. An ethnographic fieldwork was conducted in Jacobabad Sindh, Pakistan. The study found that honour murders were not solely driven by customs and traditions, but also by a feudal culture, male-dominated social structures, the complicit role of state institutions and law enforcement agencies and a web of vested interests. Therefore, honour murders may be prevented by reducing the influence and interference of feudal lords on state institutions, in particular law enforcement agencies, and by promoting education that challenges a patriarchal and feudal mind-set in the community

    Isolated Sacral Metastasis Mimicking a Chordoma Presenting as a First Manifestation of Thyroid Malignancy: Case Report

    Fifty-nine years old female presented with the clinical manifestations of lumbosacral plexopathy. MR imaging revealed a large destructive pelvic mass of sacral origin, mimicking a sacral chordoma. Histopathological findings are suggestive of follicular variant of papillary carcinoma of thyroid origin. There was no clinically palpable thyroid lesion or any clinical feature suggestive of thyroid dysfunction. This report describes an isolated sacral metastasis of thyroid origin without prior clinical features of thyroid malignancy.     A novel MAC Protocol for Cognitive Radio Networks

    In Partial Fulfilment of the Requirements for the Degree Doctor of Philosophy from the University of BedfordshireThe scarcity of bandwidth in the radio spectrum has become more vital since the demand for wireless applications has increased. Most of the spectrum bands have been allocated although many studies have shown that these bands are significantly underutilized most of the time. The problem of unavailability of spectrum bands and the inefficiency in their utilization have been smartly addressed by the cognitive radio (CR) technology which is an opportunistic network that senses the environment, observes the network changes, and then uses knowledge gained from the prior interaction with the network to make intelligent decisions by dynamically adapting transmission characteristics. In this thesis, recent research and survey about the advances in theory and applications of cognitive radio technology has been reviewed. The thesis starts with the essential background on cognitive radio techniques and systems and discusses those characteristics of CR technology, such as standards, applications and challenges that all can help make software radio more personal. It then presents advanced level material by extensively reviewing the work done so far in the area of cognitive radio networks and more specifically in medium access control (MAC) protocol of CR. The list of references will be useful to both researchers and practitioners in this area. Also, it can be adopted as a graduate-level textbook for an advanced course on wireless communication networks. The development of new technologies such as Wi-Fi, cellular phones, Bluetooth, TV broadcasts and satellite has created immense demand for radio spectrum which is a limited natural resource ranging from 30KHz to 300GHz. For every wireless application, some portion of the radio spectrum needs to be purchased, and the Federal Communication Commission (FCC) allocates the spectrum for some fee for such services. This static allocation of the radio spectrum has led to various problems such as saturation in some bands, scarcity, and lack of radio resources to new wireless applications. Most of the frequencies in the radio spectrum have been allocated although many studies have shown that the allocated bands are not being used efficiently. The CR technology is one of the effective solutions to the shortage of spectrum and the inefficiency of its utilization. In this thesis, a detailed investigation on issues related to the protocol design for cognitive radio networks with particular emphasis on the MAC layer is presented. A novel Dynamic and Decentralized and Hybrid MAC (DDH-MAC) protocol that lies between the CR MAC protocol families of globally available common control channel (GCCC) and local control channel (non-GCCC). First, a multi-access channel MAC protocol, which integrates the best features of both GCCC and non-GCCC, is proposed. Second, an enhancement to the protocol is proposed by enabling it to access more than one control channel at the same time. The cognitive users/secondary users (SUs) always have access to one control channel and they can identify and exploit the vacant channels by dynamically switching across the different control channels. Third, rapid and efficient exchange of CR control information has been proposed to reduce delays due to the opportunistic nature of CR. We have calculated the pre-transmission time for CR and investigate how this time can have a significant effect on nodes holding a delay sensitive data. Fourth, an analytical model, including a Markov chain model, has been proposed. This analytical model will rigorously analyse the performance of our proposed DDH-MAC protocol in terms of aggregate throughput, access delay, and spectrum opportunities in both the saturated and non-saturated networks. Fifth, we develop a simulation model for the DDH-MAC protocol using OPNET Modeler and investigate its performance for queuing delays, bit error rates, backoff slots and throughput. It could be observed from both the numerical and simulation results that when compared with existing CR MAC protocols our proposed MAC protocol can significantly improve the spectrum utilization efficiency of wireless networks. Finally, we optimize the performance of our proposed MAC protocol by incorporating multi-level security and making it energy efficient

    Peritumoral cystic meningioma

    A case of 39 years old male is described who presented with headache, right-sided focal fits and decreased power in the right hand. CT scan brain showed a left fronto-parietal cystic lesion with centrolateral intramural nodule with homogenous enhancement. At surgery, the extra-axial lesion with cyst containing xanthochromic fluid had a well-defined capsule that could easily be separated from the peri-lesional cortical surface. The dural-based nodule with its cyst wall was resected in toto. The histopathology of mural nodule was reported as meningioma while the cyst wall histology revealed meningothelial cells

    MULTI-HAZARD RISK ASSESSMENT OF QURUMBAR VALLEY, GHIZER, GILGIT BALTISTAN, PAKISTAN

    Being located in a seismically active zone, these mountain valleys are exposed to different hydro-meteorological hazards like rockfall, debris flow, landslide, ice avalanches, and glacial lake outburst flood (GLOFs).  The present study is to investigate different multi-hazards and their prevailing risk in the particular area of Qurumbar valley Ishkoman. In the mid-nineteen and twenty centuries at least six devastating glacial lake outburst floods (GLOFs) has been recorded, initially, only the Qurumbar glacier was considered as the main cause of this outburst flood, later field investigation and interview from local inhabitant revealed that nine more tributary glacier are existing in the area. The recent outburst of flood in the Badswat area of Qurumbar Ishkoman is also considerable, submerging 12 houses and a stretch of roads. Apart from it, the physical vulnerability of the area is increasing, as new areas are being used for housing with the increase in population. The study area is exposed to several other natural hazards like rock fall, debris flows, ice avalanches, and bank erosion. The present study is based on quantitative and qualitative approaches in assistance with GIS/RS an emergent application. The following methodologies were adapted to gather the primary and secondary data for GIS/RS processing. The primary data comprises GIS data and procurement and development and field data. The field data contain ground-truthing /validation. The community-based Hazard Vulnerability Risk Assessment (HVRA) was also carried out to know about human perception. Secondary data has been collected from different literature. Both the data were put into GIS for the processing which gives us our final developmental tools in the form of maps. The hazard and risk map of the Qurumbar Ishkoman depict that flash floods, debris flow, bank erosion, and GLOFs account for major hazards in Qurumbar valley. It is concluded based on our outcomes i.e., hazard and risk maps that earthquake is the main hazard of the area while flash floods, GLOFs, debris flow, bank erosion and the bank collapsed are the main hazards of the area. It is very important to systematically integrate map information into the planning and management process which contributes to a safer environment

    Tuberculosis of the scapula

    Tuberculosis (TB) of the scapula is an extremely rare presentation of osteoarticular tuberculosis. In a tuberculosis endemic setting with a rising burden of acquired immunodeficiency syndrome, this rare presentation may become more frequent. The common presentation is with longstanding Complaints of pain and swelling in the shoulder region, typically in young adults. Classic radiographic features include a well defined radiolucent lesion with minimal sequesteration. The erythrocyte sedimentation rate is usually elevated and initially these patients are frequently mis-diagnosed and thus a high index of clinical suspicion is required. Medical therapy with antituberculous drugs is the standard modality of treatment. This case serves to higlight the salient features of scapular TB

    Growth and production of Nile tilapia (Oreochromis niloticus Lin.) in irrigated Boro rice under floodplain environment

    The experiment was conducted at BRRI Regional Station, Habiganj during 1994-95 to evaluate the growth and economic performance of Nile tilapia, Oreochromis niloticus, fish reared in the field of irrigated boro rice with different fertilizer levels. Grain yield of rice was not affected by fish culture. It was observed that 50% of recommended fertilizer was enough to produce increased rice yield (8-10 t/ha) at floodplain environment and additional yield was obtained with the increasing fertilizer rates. Results further indicated that O. niloticus could successfully be reared in the field of irrigated boro rice with recommended fertilizer level. Larger size of fingerlings at release had improved recovery percent, body weight gain and higher fish yield. Results also revealed that rice + fish production system produced higher net return than the system with rice alone

    Family reported outcomes, an unmet need in the management of a patient's disease: appraisal of the literature

    Abstract: Background: A person’s chronic health condition or disability can have a huge impact on the quality of life (QoL) of the whole family, but this important impact is often ignored. This literature review aims to understand the impact of patients' disease on family members across all medical specialities, and appraise existing generic and disease-specific family quality of life (QoL) measures. Methods: The databases Medline, EMBASE, CINHAL, ASSIA, PsycINFO and Scopus were searched for original articles in English measuring the impact of health conditions on patients' family members/partner using a valid instrument. Results: Of 114 articles screened, 86 met the inclusion criteria. They explored the impact of a relative's disease on 14,661 family members, mostly 'parents' or 'mothers', using 50 different instruments across 18 specialities including neurology, oncology and dermatology, in 33 countries including the USA, China and Australia. These studies revealed a huge impact of patients' illness on family members. An appraisal of family QoL instruments identified 48 instruments, 42 disease/speciality specific and six generic measures. Five of the six generics are aimed at carers of children, people with disability or restricted to chronic disease. The only generic instrument that measures the impact of any condition on family members across all specialities is the Family Reported Outcome Measure (FROM-16). Although most instruments demonstrated good reliability and validity, only 11 reported responsiveness and only one reported the minimal clinically important difference. Conclusions: Family members' QoL is greatly impacted by a relative's condition. To support family members, there is a need for a generic tool that offers flexibility and brevity for use in clinical settings across all areas of medicine. FROM-16 could be the tool of choice, provided its robustness is demonstrated with further validation of its psychometric properties.Peer reviewe

    Impacts of habitat variability on the phenotypic traits of Aconitum violaceum Jacq. ex Stapf. at different altitudes and environmental conditions in the Ladakh Himalaya, India

    Aconitum violaceum Jacq. ex Stapf is endemic to the Northern Himalayan regions of India, Pakistan and Nepal. To adapt and endure the rigorous stressful situations, phenotypic features of a specific plant species vary across different climatic conditions and elevations. The aim of this study was to reveal the impact of altitude and environmental conditions on the morphological attributes of a threatened species A. violaceum. Plants at high elevations (Maan-man, 4010m asl) were short; whereas the plants grown at ecological conditions of population-I (Khawous, 3220m asl) showed better growth and development in all morphological attributes. It has been noticed that there is a considerable reduction in the plant height, rhizome, leaf dimension, flower number and leaf number at higher elevations. Regression analysis and Pearson’s correlation coefficient demonstrated a positive relationship between plant height and all other morphological attributes. Principal component analysis (PCA) revealed that the habitat, ecological conditions and elevations of population-I (Khawous) and Population-II (Numsuru) were proved to be relatively better for the luxuriant growth and development of an endemic plant species A. violaceum. From the observations, it can be inferred that A. violaceum grows successfully at an elevation ranges between 3000 to 3500m ASL. This study provides a comprehensive account of phenotypic variability concerning environmental conditions and suggested a suitable environment for sustainable cultivation of this important medicinal plant species in the Ladakh Himalaya
