A cross-sectional study of patients participating in the Nevada State Health Division’s Women Health Connection Program (WHC) was conducted to assess patient satisfaction for cervical cancer screening. In this study, 528 WHC program patients provided information regarding their satisfaction with the treatment services they received, accessibility issues, breast and cervical cancer health education and information, and overall program satisfaction. A large majority of patients reported high levels of satisfaction with the program services and clinic personnel. Significant positive correlations were found when comparing satisfaction with services received at the clinic to treatment received by physicians, nurses, and admissions personnel, how well information provided to patients eased their concerns, and when results were discussed with patients. Wait time for admission and to see a physician were negatively correlated to satisfaction. Those who reported that they would not use services again indicated lower levels of satisfaction with the information received and treatment from caregivers and admissions personnel when compared to those who would use the program again

Static and non-static quantum effects in two-dimensional dilaton gravity

We study backreaction effects in two-dimensional dilaton gravity. The backreaction comes from an R2R^2 term which is a part of the one-loop effective action arising from massive scalar field quantization in a certain approximation. The peculiarity of this term is that it does not contribute to the Hawking radiation of the classical black hole solution of the field equations. In the static case we examine the horizon and the physical singularity of the new black hole solutions. Studying the possibility of time dependence we see the generation of a new singularity. The particular solution found still has the structure of a black hole, indicating that non-thermal effects cannot lead, at least in this approximation, to black hole evaporation.Comment: 10 pages, no figure

The Russian Court Chapel Choir, 1796-1917

The Russian Court Chapel Choir: 1796-1917. Throughout the nineteenth century the Russian Court Chapel Choir - the Court Kapella - with its rich, sonorous voices, virtuosic technique and strict discipline, enjoyed an enviable reputation. Its singers performed not only in liturgical services and official state ceremonies in the imperial churches, but also played an active role in the musical life of St. Petersburg outwith the court. The music school within the Kapella provided the singers with an excellent choral training as well as tuition in instrumental performance, harmony, orchestration and composition; indeed, until the second half of the nineteenth century it was the only Institution to offer this level of musical education. The Court Kapella also played a vital role in the publication of music for the Orthodox Church at this time: from 1816 the director of the Kapella had the power to authorize or prohibit every edition, performance and publication of sacred music in Russia and, as a result, could significantly influence the development of sacred music. This study deals with the period from 1796, the year in which Dmitry Bortnyansky was appointed director of the Kapella, until 1917 when the institution underwent such radical reorganization as to render it almost unrecognisable. The study is divided into three sections: the first deals with the structural organization of the Kapella as an educational establishment and focuses particularly on the development of the curriculum under a changing directorate; the second deals with the structure of the liturgy used in the Russian Orthodox Church and the emergence and development of harmonized court chant; the third deals with the development of sacred vocal music in Russia during the period 1796-1917, and is Illustrated by an examination of the music composed specifically for the use of the Court Kapella and its relationship with the liturgy which it enhanced. Despite the wealth of documentation concerning the Court Kapella held in the archives of the Ministry of the Imperial Court in the State Historical Archive in St. Petersburg, there is no detailed study of this extremely complex subject which is of such great relevance to so many aspects of Russian musical life. Many of the most influential figures of the time were closely involved with the Kapella - for example Glinka, Balakirev, and Rimsky-Korsakov - but little research has been done on this area of their work. The educational importance of the institution has always been overshadowed by the achievements of the St. Petersburg Conservatoire, and the consequences of the monopoly exerted by the Kapella in the area of music publication have never been investigated. A survey of this topic is essential not only for our understanding of the inestimable wealth of Russian sacred music, but also for a comprehensive view of Russian musical life at this time

Effective Smoking Cessation Methods among Smokers with Diabetes in NV

Smoking and diabetes could both be prevented if individuals would abstain from smoking, eat healthy, and exercise regularly. Smokers with diabetes have an increased risk of serious health outcomes, hence effective smoking cessation interventions are critical. The transtheoretical model was used in this quantitative study analyzing secondary data from the state of Nevada Quitline to examine the relationships between smoking cessation method (counseling versus counseling and medication) and quitting smoking for 720 smokers with/without diabetes. Participants were Nevada residents, ages 18+, men and women, English or Spanish speakers. Descriptive statistics, logistic regression, and a test of two proportions were conducted. The majority of the participants had not quit (67.5%). Quit rates did not differ between smokers with/without diabetes, however, individuals who received counseling and medication were 1.94 times as likely to quit compared to those who received counseling alone. Among diabetes smokers, age was significantly related to quit status; for every 1 year, the likelihood of quitting increased by 1.03 times; and Hispanics were 7.50 times more likely to quit smoking compared to Caucasians. Findings from this study could help healthcare providers, public health practitioners, and scholars develop effective smoking cessation programs to meet the needs of smokers with diabetes

Anti-malarial prescription practices among outpatients with laboratory-confirmed malaria in the setting of a health facility-based sentinel site surveillance system in Uganda.

BACKGROUND: Most African countries have adopted artemisinin-based combination therapy (ACT) as the first-line treatment for uncomplicated malaria. The World Health Organization now recommends limiting anti-malarial treatment to those with a positive malaria test result. Limited data exist on how these policies have affected ACT prescription practices. METHODS: Data were collected from all outpatients presenting to six public health facilities in Uganda as part of a sentinel site malaria surveillance programme. Training in case management, encouragement of laboratory-based diagnosis of malaria, and regular feedback were provided. Data for this report include patients with laboratory confirmed malaria who were prescribed anti-malarial therapy over a two-year period. Patient visits were analysed in two groups: those considered ACT candidates (defined as uncomplicated malaria with no referral for admission in patients ≥ 4 months of age and ≥ 5 kg in weight) and those who may not have been ACT candidates. Associations between variables of interest and failure to prescribe ACT to patients who were ACT candidates were estimated using multivariable logistic regression. RESULTS: A total of 51,355 patient visits were included in the analysis and 46,265 (90.1%) were classified as ACT candidates. In the ACT candidate group, 94.5% were correctly prescribed ACT. Artemether-lumefantrine made up 97.3% of ACT prescribed. There were significant differences across the sites in the proportion of patients for whom there was a failure to prescribe ACT, ranging from 3.0-9.3%. Young children and woman of childbearing age had higher odds of failure to receive an ACT prescription. Among patients who may not have been ACT candidates, the proportion prescribed quinine versus ACT differed based on if the patient had severe malaria or was referred for admission (93.4% vs 6.5%) or was below age or weight cutoffs for ACT (41.4% vs 57.2%). CONCLUSIONS: High rates of compliance with recommended ACT use can be achieved in resource-limited settings. The unique health facility-based malaria surveillance system operating at these clinical sites may provide a framework for improving appropriate ACT use at other sites in sub-Saharan Africa

A Lean Enterprise Architecture Approach as an Enabler for Organizational Agility : Case: Metso Outotec

In the era where delivery speed is perceived more important than IT landscape integration, consistency and long-term planning, different architectural approaches have become important considerations of information systems management. Moreover, recent studies have shown that the need for a holistic EA is often overlooked, when organizations try to apply agile development models, which may lead to several problems, such as technical debt, redundant rework, inconsistent communication, decentralized and siloed architecture design, unsustainable architecture, and inconsistence in coding style. Hence, with the growing deployment of scaling agile methods there is a need for purpose-fit approaches to integrate EA frameworks to enable organization agility while maintaining long-term vision. This study aims to explore how EA activities are put into practices in a company deploying large-scale agile development methods – namely EA deliverables, EA benefits, EA concerns and EA enablers. In total, 13 semi-structured interviews were conducted from a case company, and an analysis was done using the Gioia method. As a result, EA deliverables (business objective deliverables, intentional architecture deliverables, and emergent design deliverables), EA benefits (organizational agility and organizational robustness), EA concerns (immaturity, disengagement, urgency, and resistance and anti-patterns), and EA enablers (communication and collaboration, Lean EA, and EA culture) were identified. The enterprise architecture practices used by the case company were in line with the guidelines and best practices recommended by the literature and industry experts. Moreover, a literature review provided some theoretical constructs and suggestions, namely the Lean EA development (LEAD) method and the design principles of architectural thinking for supporting organizational agility, which can be recommended to be applied by the case company or any other organization scaling agile

Prevalence and Antimicrobial Agent Susceptibility of Methicillin-resistant Staphylococcus aureus in Healthy Pediatric Outpatients in Las Vegas

Colonization and infection by community-associated resistant strains of Staphylococcus aureus are being reported in epidemic proportions. The purpose of this study was to determine the local prevalence of methicillin-resistant Staphylococcus aureus (MRSA) colonization in children and to characterize the MRSA isolates in the laboratory with regard to antimicrobial agent susceptibility patterns, and the presence of the mecA and the Panton-Valentine leukocidin (PVL) genes. Nasal swabs were collected at two pediatric clinics from a total of 505 children during health maintenance visits. A brief questionnaire was administered to collect demographic data and pertinent medical, family, and social history. Samples were cultured onto 2 selective media for S. aureus and MRSA. Potential MRSA isolates were further evaluated by real-time polymerase chain reaction (PCR), and for susceptibility to eight antibiotics by disk diffusion. Culture results showed that MRSA was present in 15 of the 505 specimens (3.0%). Six different antimicrobial susceptibility profiles were observed among the MRSA isolates. PCR amplification results showed that all 15 MRSA isolates were positive for the presence of the mecA gene, and 10 MRSA isolates contained the PVL gene. Understanding local prevalence rates and the role of colonization in infection are needed to develop effective interventions to reduce MRSA infections

Enabling the transfer of information technology support knowledge to enterprise customers using web-based self-service systems : critical success factors from the support organisation perspective

    Get PDF
This paper explores critical success factors (CSFs) in the transfer of after-sales information technology (IT) support knowledge from IT product and service firms that provide IT support services (“IT support organisations”) to enterprise customers, when Web-based Self-service Systems (WSSs) are used. The research explores the topic from an IT support organisation perspective in an interpretive multi-stage study involving six large multinational IT support businesses. Key findings reported in the paper include six categories of CSFs, and twenty-seven CSFs distributed across the six categories. The findings suggest that researchers and practitioners consider planning and implementing WSSs for after-sales IT support provision within a complex network of IT support organisations, business partners and enterprise customers
