약국을 통한 공적 마스크 배부 정책을 중심으로

학위논문(석사) -- 서울대학교대학원 : 보건대학원 보건학과(보건정책관리학전공), 2023. 2. 김창엽.The purpose of this study is to analyze the publicly supplied mask system, which was a policy to distribute quarantine masks during the COVID-19 infectious disease crisis, from the perspective of collaborative governance. There are growing number of wicked problems that are difficult for single government agencies to effectively address. To solve these wicked problems, various partners such as the central government, local government, civic groups, and corporations are interested in collaborative governance which forms a horizontal and voluntary network. This is because the health care system was unified into the national health insurance system, not only the need for cooperation is not great but also the public-private were less likely to cooperate in the private health care service supply system. In addition, the healthcare sector has been solving problems mainly by experts with high level of expertise because the healthcare sector has a wide gap in information depending on that level of expertise. The publicly supplied mask system' refers to a policy that restricts the number of masks purchased per person and requires them to purchase masks on a designated date according to the year of birth in order to distribute masks as fairly and efficiently as possible to citizen in the face of shortage of masks due to the COVID-19 infectious disease crisis. The publicly supplied mask system is of great significance as an example of collaborative governance in the healthcare sector in that government agencies, quasi-public institutions, and private healthcare institutions worked closely together to solve the problem of mask distribution, which was an urgent public problem at the time. Therefore, by analyzing the publicly supplied mask system, a major quarantine policy for COVID-19, from the perspective of collaborative governance, we tried to analyze how collaborative governance worked in the process of introducing and operating public masks, and to discover the characteristics of collaborative governance in the health care sector. This study examines the starting conditions, institutional design, facilitative leadership, collaborative process, and outcomes in which collaborative governance could be formed in a single case of publicly supplied mask system by applying Ansell & Gash's collaborative governance theoretical model. The publicly supplied mask system was implemented from March 9 to July 12, 2020, but the temporal scope of the cases is from January 2020, when the introduction of the system began to be discussed, to June 2021, when discussions on follow-up measures to the system were concluded. In order to analyze the starting conditions, institutional design, cooperation process and results of collaborative governance, an extensive literature review was conducted, including government press releases, newspaper articles, government departments' COVID-19 response white papers, Ministry of Health and Welfare reports, and meeting materials. Subjective or contextual elements that are difficult to grasp through literature review, such as incentives that have not been officially revealed, precedents of conflict or cooperation that have not been revealed, degree of spontaneity in institutional operation, facilitative leadership, and goal awareness of each actor, have been explored through in-depth interviews. As subjects of in-depth interviews, three government officials, three officials from the Korean Pharmaceutical Association, four local pharmacy pharmacists, and three distributors who were directly involved in the publicly supplied mask system were selected, and a semi-structured interview was conducted based on a literature review in advance. As a result of the analysis, collaborative governance worked effectively in the publicly supplied mask system, and the main success factor was awareness of the disaster situation and strong shared understanding. However, in this case, unlike the order in Ansell & Gash's model, the collaborative process is in the order of shared understanding → face-to-face dialogue → trust building → commitment to process → intermediate outcomes. This is because clear goals and visions were shared first due to the infectious disease crisis. Another factor was the sense of duty was formed among all actors of private sector cooperation due to the uniqueness of the healthcare sector. Although the institutional design had inclusiveness and clear rules for participation, the frequent changes in detailed rules caused confusion. In addition, the leadership of the Korean Pharmaceutical Association and the heads of distributors, and the leadership of the President facilitated the collaborative process. Through this study, it is revealed that the publicly supplied mask system is a case in which public problems can be solved well through the problem-solving method of collaborative governance in the health care sector, and by identifying the formation process of collaborative governance, success factors and limitations, it can be used to promote collaborative governance among the government agencies, quasi-public institutions and private health care institutions. Furthermore, it is expected that it will be a clue to understanding the characteristics of collaborative governance in the health care sector.본 연구는 코로나19 감염병 위기 시 방역 마스크를 분배하기 위한 정책이었던 공적 마스크 제도를 협력적 거버넌스 관점에서 분석함을 목적으로 한다. 하나의 정부 기관이 효과적으로 해결하기 어려운 사악한 문제(wicked problems)가 증가하고 있다. 사악한 문제의 해결을 위해 중앙정부, 지방자치단체, 시민단체, 기업 등 다양한 파트너들이 수평적이고 자발적인 네트워크를 형성하는 협력적 거버넌스에 관한 관심이 증가하고 있다. 행정학, 정치학, 사회학 등 다분야에서 협력적 거버넌스 연구도 늘어나고 있다. 그러나 보건의료 부문에서는 상대적으로 협력적 거버넌스에 관한 연구가 적은 편이다. 그 이유는 보건의료체계가 국민건강보험 체계로 단일하여 협력의 필요성이 크지 않은 동시에 민간 위주의 보건의료서비스 공급 체계에서 민관의 협력 가능성도 적었기 때문이다. 또한, 보건의료 부문은 높은 전문성을 요구하고, 그에 따른 정보의 격차가 크기 때문에 주로 높은 수준의 전문성을 가진 전문가들에 의해 문제를 해결해 왔던 것도 하나의 요인이다. 공적 마스크 제도란 코로나19 감염병 위기로 인한 마스크 품귀 상황에서 시민들에게 최대한 공평하고 효율적으로 마스크를 배분하기 위해 1인당 마스크 구매 수량을 제한하고, 출생 연도에 따라 지정 일자에 마스크를 구매하도록 했던 정책을 말한다. 마스크 수급난이 시작되던 때 정부의 1, 2차 긴급수급조정조치에도 불구하고 마스크 수급 문제는 해결되지 않았다. 마침내 정부는 마스크 공급을 통제하게 되었고, 이를 위해 민간의 공적 판매처가 필요했다. 공적 판매처로 편의점, 약국 등의 선택지가 있었으나 최종적으로 민간보건의료기관인 약국과 의약품 유통업체와의 협력적 거버넌스를 형성하여 문제를 해결하였다. 공적 마스크 제도는 당시 긴박한 공공문제였던 마스크 수급 문제를 협력적 거버넌스를 통해 해결했다는 점에서 보건의료 부문의 협력적 거버넌스 사례로서 의미가 크다. 따라서 코로나19의 주요한 방역 정책인 공적 마스크 제도를 협력적 거버넌스 관점에서 분석하여 공적 마스크 도입과 운영과정에서 협력적 거버넌스가 어떻게 작동했는지 분석하고, 보건의료 부문의 협력적 거버넌스의 특징을 발견하고자 하였다. 본 연구는 문헌 분석, 심층 면담을 이용한 질적 사례연구이다. 공적 마스크 제도는 2020년 3월 9일부터 7월 12일까지 시행되었으나 제도의 도입이 논의되기 시작한 2020년 1월부터 후속 조치가 마무리되는 2021년 6월까지를 사례의 시간적 범위로 하였다. 협력적 거버넌스의 초기조건, 제도적 설계, 협력과정과 결과를 분석하기 위해 정부 보도자료, 신문기사, 정부 각 부처의 코로나19 대응 백서, 보건복지부 보고서, 대한약사회 회의 자료 등의 광범위한 문헌 조사를 수행하였다. 비공식적인 인센티브, 외부에 드러나지 않았던 갈등 또는 협력의 전례, 제도 운용의 자발성 정도, 촉진적 리더십, 각 행위자의 목표 인식 등 문헌 조사로 파악하기 힘든 주관적이거나 맥락적인 요소는 심층 면담을 통해 탐구하였다. 심층 면담 대상자로 공적 마스크 제도에 직접 관여했던 행위자였던 정부 관계자 3인, 대한약사회 관계자 3인, 지역약국 약사 4인, 유통업체 관계자 3인을 선정하여 사전에 문헌 조사를 바탕으로 반구조화된 인터뷰를 진행하였다. 본 연구는 연구의 목적에 알맞은 Ansell & Gash의 협력적 거버넌스 이론모형을 적용하여 공적 마스크 제도라는 단일한 사례에서 협력적 거버넌스가 형성될 수 있었던 초기조건, 제도적 설계, 리더십, 협력과정, 결과에 대해 분석하였다. 분석 결과 공적 마스크 제도에서 협력적 거버넌스는 효과적으로 작동했고, 주요 성공 요인은 재난 상황에 대한 인식과 강한 이해의 공유다. 다만 협력과정에서 Ansell & Gash의 모형에서의 순서와 다르게 이해의 공유→면대면 대화→신뢰구축→몰입→중간결과의 순서로 협력이 진행되었다. 감염병 위기 상황으로 인해 명확한 목표와 비전이 우선적으로 공유되었기 때문이다. 또한, 보건의료 부문이 가지는 특수성으로 인해 민간 협력 주체 모두에게 사명감이 형성되었던 점도 성공 요인이다. 제도적 설계에서 참여의 포괄성과 명확한 규칙을 가지고 있었으나 세부 규칙이 자주 변경되면서 혼란을 빚기도 했다. 그리고 초기조건을 형성하는 과정에서 대한약사회와 유통업체 대표들의 리더십이 협력과정을 촉진했다. 본 연구는 공적 마스크 제도가 보건의료 부문에서 협력적 거버넌스라는 문제 해결 방식을 통해 공적 문제를 잘 해결할 수 있었던 사례임을 밝히고, 협력적 거버넌스의 형성 과정, 성공 요인과 한계를 규명함으로써 향후 정부 기관, 준공공기관, 민간 보건의료기관 간의 협력적 거버넌스를 촉진하는 데에 활용될 수 있을 것이다. 나아가 보건의료 부문에서 협력적 거버넌스의 특징을 이해할 수 있는 실마리가 될 것을 기대한다.제 1 장 서론 1 제 1 절 연구배경 1 제 2 절 연구의 필요성 2 제 3 절 연구목적과 연구질문 4 제 2 장 이론적 배경 및 선행연구 6 제 1 절 협력적 거버넌스 6 1. 협력적 거버넌스의 정의 6 2. 협력적 거버넌스의 모형 9 제 2 절 선행연구 11 1. 협력적 거버넌스 선행연구 11 2. 재난 상황에서의 협력적 거버넌스 선행연구 13 제 3 장 연구 방법 17 제 1 절 연구설계 및 분석의 틀 17 1. 사례연구 17 2. 연구의 범위 및 대상 17 3. 연구 분석틀 18 제 2 절 자료 수집 19 1. 문헌 자료 19 2. 심층 면담 20 제 3 절 자료 분석 24 1. 자료 분석 방법 24 2. 타당도와 신뢰도 확보 24 3. 연구자의 배경 및 신념 25 제 4 장 연구 결과 27 제 1 절 공적 마스크 제도의 개요 27 1. 공적 마스크 제도의 배경 27 2. 공적 마스크 제도의 경과 31 제 2 절 협력적 거버넌스 사례 분석 38 1. 초기조건 38 2. 제도적 설계 51 3. 촉진적 리더십 56 4. 협력과정 59 5. 결과 69 제 5 장 종합 사례 분석 및 논의 75 제 1 절 사례분석 요약 75 제 2 절 논의 81 제 3 절 연구의 의의 및 한계 84 참고문헌 86 Abstract 92석
