This paper conjectures that such a mobility is due to the relatively low economic status of the GFRI researchers. A regression analysis shows that the wage is not lower in GFRIs, which seemingly defies the conjecture. But it turns out that the economic status, when measured in terms of life-time earnings, is significantly lower for researchers in GFRIs. The main cause of such a disparity is the shorter work-life of GFRI researchers: they have to retire earlier than college professors. A recent policy measure enacted in 2012 extends the mandatory retirement age for GFRI researchers is expected to attenuate the uni-directionality of mobility by balancing life-time earnings of GFRI researhers

Healthcare organizations are facing a financial crisis due to more competitive relationships among themselves as well as low health insurance fees. As patients'' healthcare needs are increasing, healthcare organizations make numerous efforts to maintain or improve the quality of care with low cost. Into this changing healthcare universe has emerged a new management system: the case management system, which is an effective approach to reduce healthcare cost while maintaining or improving the quality of care. Under the current fee-for-service reimbursement system, many healthcare organizations have introduced the case management system to improve their financial management status. The purposes of the current study are: (1) to develop a data warehouse-based system for evaluating and monitoring the case management activities, and (2) to measure and analyze its effects. In order to collect the data for the study, the database on discharged patients was utilized at a university hospital located in Seoul from June 1, 2002 through December 31, 2002. Initially, a data warehouse was built for the case management system. The case management activities were analyzed using structured methodology to establish the case management system. The findings of the study are summarized as follows: (1) The case management system was developed to make it possible to monitor the quality of care and resource utilization by possessing the monitoring functions in the utilization reviews, critical pathways, and clinical indicators. (2) Utilizing the case management system, unplanned readmissions were examined among total discharged patients for two months (November 1, 2002 thru December 31, 2002). The unplanned readmission rate was 2.3% in a total of 11,960 discharges and 0.07% of total discharges displayed the identical diagnoses and physicians. While no significant differences were found in the demographic variables such as gender and age among the patients, there was a significant difference between the admission routes at the first admission and the second one (ฯ‡2=17.696, p < 0.001). The patients with short length of stay at the first admission also demonstrated short length of stay at their second admission (ฯ‡2=23.819, p < 0.001). (3) It was estimated that the total length of stay in the hospital would be reduce to about 2.9% after implementing the case management system. In this case, the cost-benefit analysis showed that the net benefit would be 2,796 thousand Won and the value acceleration analysis displayed that cumulative benefit for the prospective five years would be 1,414 million Won. Also, the value linkage analysis demonstrated that the cumulative benefit for the prospective five years would be 3,405 million Won and net benefit for one year would be 1,646 million Won. In conclusion, the importance of the present study would be not only implementing the knowledge management system into existing case management activities, but also evaluating its effects and estimating its financial benefits. The study suggested that the case management system would be a supportive tool for monitoring and improving the quality of care, and a cost-effective tool for healthcare organizations. It is recommended that the development of various information systems may contribute in improving the quality of healthcare as well as the economics of healthcare organizations.ope

    Purpose: To investigate health problems in child day care centers, needs for Child Care Health Programs, and management of sick children by day-care staff. Methods: A cross-sectional descriptive study. Day-care staff (N=206) from 33 day care centers in Seoul, completed a structured questionnaire. Results: Almost half of the day-care staff (53.4%) experienced sick events in their child at least once a month. The most frequently reported difficulty in having a sick child was in not having a health professional to consult, and almost all day-care staff (99.5%) had asked parents not to bring a sick child to the day care center. The biggest reason of exclusion was due to infectious disease including measles, chicken pox, mumps, diarrheaโ€คvomiting, and pink eye. The majority of day-care staff (97.6%) agreed that there is a need for Center-based Child Care Health Program (CCCHP), and expected general health & illness management, and special care for chronic disease from CCCHP. Conclusion: These results showed a high incidence of sickness in children in day care centers, and there were great demands for a Center-based Child Care Health Program for day-care staff. Also the results of this study can be utilized as a basis for the establishment of child care policy in Korea
