(The) effects of cold therapy on pain in patients with chest tube insertion

간호학과/석사[한글] 흉관삽입한 환자들의 흉관과 관련된 통증완화를 위해 진통제 투약하고 있지만 효과적이지 못하다. 따라서 의료진은 이러한 통증을 완화시킬 수 있는 방법을 모색해야만 한다. 본 연구는 냉요법이 흉관삽입 환자의 통증에 미치는 영향을 규명하기 위한 유사 실험연구로 비동등성 대조군(non-equivalent control group) 전·후 시차설계이다. 연구기간은 2003년 2월 1일부터 11월 30일 까지였고, 연구 대상자는 수원 소재, A 종합병원 응급실로 내원하여 일차성 자연기흉진단을 받고 흉관을 삽입한 환자로 실험군 15명, 대조군 15명으로 총 30명이었다. 통증감각은 본 연구자가 한국어로 번역하여 수정한 S-F MPQ(Short Form McGill Pain Questionnaire)로 측정하였고, 통증강도는 시각적 상사척도인 VAS(visual analogue scale)로 측정하였다. 그리고 통증관련 생리적 지수로 수축기 혈압, 이완기 혈압, 맥박을 측정하였다. 연구절차는 응급실에서 흉관을 삽입하기 전 대상자를 방문하여 연구의 목적을 설명하고 동의서를 받은 후 일반적 특성을 조사하였다. 이후 대조군과 실험군은 흉관삽입 30분후 사전조사가 이루어졌고, 실험군의 경우 냉요법을 30분 적용하였다. 냉요법이 끝난 직후와 냉요법 적용이 끝난 30분후, 60분후에 각 각 통증감각과 통증강도, 혈압과 맥박을 총4회 측정하였다. 대조군은 처치 없이 측정만 하였다. 대조군과 실험군 모두 흉관삽입으로 인한 통증을 중재하기 위한 방법으로 삽입 전에 진통제투약과 국소마취를 시행하였다. 자료분석은 SPSSWIN 11.0프로그램을 이용하였고, 실험군과 대조군의 동질성 검증은 χ2-test로 검증하였으며, 냉요법이 흉관삽입 후 통증이 미치는 효과를 파악하기 위한 가설검증은 반복측정 자료에 대한 분산분석(repeated measures ANOVA)으로 검증하였다. 도구의 신뢰도를 검증하기 위하여 Cronbach''s alpha를 구하였다. 연구결과는 다음과 같았다. 1. 실험군과 대조군의 일반적 특성으로 연령, 결혼, BMI, 삽입의사, 삽입부위와 통증감각, 통증강도 그리고 통증관련 생리적 지수인 혈압과 맥박에 대한 동질성 검증은 통계적으로 유의한 차이를 보이지 않았다. 2. 가설검증 결과는 다음과 같았다. 1) 제 1가설인 ‘흉관삽입 후 냉요법을 적용한 환자와 적용하지 않은 환자는 통증감각에 차이가 있을 것이다.’는 지지되었다. 냉요법 적용 전·후 실험군과 대조군의 집단간 차이가 있는 통증감각은 ‘칼로 찌르듯이 아프다’, ‘쑤신다’, ‘누르면 아프다’로 실험군이 대조군보다 통증감각의 강도가 낮았다. 그리고 시간이 지남에 따라 통증감각에 유의한 차이를 보였다. 냉요법 적용이 끝난 직후 통증감각이 감소하다가 증가하는 통증감각은 ‘쓰라리다’, ‘꽉 쥐듯이 아프다’, ‘따끔하고 화끈거린다,’ ‘쑤신다’, ‘누르면 아프다’였고, 냉요법 적용이 끝난 30분후 통증감각이 감소하다가 증가하는 통증감각은 ‘쏘듯이 아프다’와 ‘칼로 찌르듯이 아프다’였다. 그러나 냉요법 적용이 끝난 60분후 ‘쿡쿡 쑤신다’, ‘쏘듯이 아프다’, ‘칼로 찌르듯이 아프다’, ‘쓰라리다’는 실험군과 대조군의 통증감각점수 증가폭이 비슷하였고 ‘꽉 쥐듯이 아프다’, ‘따끔하고 화끈거린다’, ‘쑤신다’, ‘누르면 아프다’는 통증감각점수의 증가폭이 실험군에서 대조군보다 더 컸다. 2) 제 2가설인 ‘흉관삽입 후 냉요법을 적용한 환자와 적용하지 않은 환자는 통증강도에 차이가 있을 것이다.’는 지지되었다. 냉요법 적용 전·후 실험군과 대조군의 통증강도는 유의한 차이를 보였는데, 실험군이 대조군보다 통증강도가 낮았다(F=4.23, p<0.05). 시간이 지남에 따라 통증강도에 유의한 차이를 보였다(F=27.54, p<0.00). 냉요법을 적용한 실험군이 대조군보다 점수의 증가폭이 작았는데, 냉요법 적용이 끝난 직후 실험군은 0.2점, 대조군은 1.06점 증가하여 5배로 작았고, 30분후 실험군은 0.47점, 대조군은 1.07점 증가하여 2배로 작았다. 그러나 냉요법 적용이 끝난 60분후에는 실험군이 1.26점, 대조군이 0.47점 증가하여 실험군이 2배로 더 커졌다. 3) 제 3가설인 ‘흉관삽입 후 냉요법을 실시한 환자와 실시하지 않은 환자는 수축기 혈압, 이완기 혈압, 맥박에 차이가 있을 것이다.’는 지지되지 않았다. 냉요법 적용 전·후 생리적 지수의 집단간 차이는 보이지 않았다. 실험군과 대조군에서 시간에 따라 유의한 차이가 나타났으나, 측정값들이 모두 정상 범위 내에 있어 임상적으로 의의가 없는 것으로 나타났다. 이상의 연구결과를 요약하면 다음과 같은 결론을 얻을 수 있다. 냉요법을 적용한 경우 ‘칼로 찌르듯이 아프다’, ‘쑤신다’, ‘누르면 아프다’ 통증감각은 느끼는 정도가 냉요법을 적용하지 않은 경우보다 약하였다. 또한 냉요법 적용이 끝난 직후 ‘쓰라리다’, ‘꽉 쥐듯이 아프다’, ‘따끔하고 화끈거린다,’ ‘쑤신다’, ‘누르면 아프다’와 같은 통증감각이 감소하였고, 냉요법이 끝난 30분후 ‘쏘듯이 아프다’와 ‘칼로 찌르듯이 아프다’ 와 같은 통증감각이 감소하여 덜 아프게 지각하였다. 또한 냉요법은 통증강도를 줄이는 것으로 나타났다. 그러나 통증감각과 통증강도에서 냉요법 적용이 끝난 60분후부터 냉요법의 효과가 떨어지는 것으로 나타났다. 따라서, 임상에서 흉관삽입 후 통증을 완화시키기 위한 간호중재 방법으로써 냉요법이 효과적으로 활용될 수 있는 가능성을 제시하였으나, 냉요법의 간호중재 효과를 정확히 규명하기 위해서는 좀 더 대상자수를 늘리고, 냉요법을 반복적으로 적용한 추후 연구가 필요하리라 사료된다. [영문]Pain related to chest tube is a major problem during chest tube drainage but there are no guidelines to manage this problem except give analgesics. The objective of this study was to identify the effects of cold therapy on pain in patients with chest tube. A quasi-experiment, non-synchronized design with non-equivalent control was used to test the effects of cold therapy on pain in patients with chest tube. Thirty patients undergoing chest tube insertion for treatment of primary pneumothorax from February 1 through November 30, 2003 at one university hospital participated in the study. Fifteen patients were randomly assigned to the experimental group and the remaining patients(15) to the control group. The instrument used to measure pain sensation was a modified S-F MPQ done by the researcher. For pain intensity the VAS was used. Systolic and diastolic blood pressure and pulse rate were measured as indicators of physiological reaction to pain. The pretest data were obtained 30 minutes after chest tube insertion. Patients in the experimental group only, received cold therapy for 30 minutes. Patients in the control group did not receive any therapy. The posttest data were collected at three points, right after the cold therapy was finished, 30 minutes after the cold therapy and 60 minutes after the cold therapy. The data were analysed using χ2-test, Cronbach''s alpha and repeated measures ANOVA with the SPSSWIN11.0 program. The results of the study are as follows; 1. Hypothesis 1, that ''there will be a significant difference between the two groups in pain sensation level after chest tube insertion'' was supported. The pain sensation level for the experimental group was significantly lower than that of the control group for pain that was ''stabbing'', ''aching'' and ''tender''. The pain sensation level for the experimental group was lower right after that cold therapy was finished for pain that was ''sharp,'' ''cramping'', ''hot-burning'', ''aching'' and ''tender''. The pain sensation level for the experimental group was lower at 30 minutes after cold therapy for pain that was ''shooting'', ''stabbing''. At 60 minutes after cold therapy, the pain sensation level in the experimental group was the same as, or lower than the control group. 2. Hypothesis 2, that ''there will be a significant difference between the two groups in pain intensity level after chest tube insertion'' was supported. The pain intensity level of the experimental group was significantly lower than that of the control group (F=4.23, p<0.05). The pain intensity level in the experimental group was lower than that of the control group right after cold therapy was finished. The pain intensity level increased by 0.2 points in the experimental group and 1.06 points in the control group. At 30 minutes after cold therapy, it was 0.47 points in the experimental group and 1.07 points in the control group. At 60 minutes after cold therapy, the pain intensity level had increased by 1.26 points in the experimental group but only 0.47 points in the control group. 3. Hypothesis 3, that ''there will be a significant difference between the two groups in physiological indicators of pain (systolic, diastolic blood pressure and pulse rate) after chest tube insertion was not supported. The findings of this study indicate that cold therapy can be effective for pain in patients with chest tube insertion. Therefore, cold therapy could be recommended as an effective nursing intervention for relieving pain related to chest tube. A larger patient population is required for the current findings to be applied to the general population. Further research is needed to identify the effects of cold therapy applied repeatedly.ope
