The Changes of Right Ventricular Function and Hemodynamic Parameters During Coronary Anastomosis in Beating Heart Surgery
Background: Hemodynamic derangement during the displacement of the beating heart in off-pump coronary artery bypass graft surgery (OPCAB) might be related with right ventricular (RV) dysfunction. This study evaluated the influence of displacing and stabilizing the heart, for the anastomosis of coronary arteries, on hemodynamic alterations and RV function in patients undergoing OPCAB. Methods: Twenty patients with triple vessel coronary artery disease underwent OPCAB using single pericardial sutures: a tissue stabilizer was included. The hemodynamic variables and right ventricular ejection fraction (RVEF) were obtained using a right-heart ejection fraction thermodilution pulmonary artery catheter after the induction of anesthesia, before and after anastomosis of each coronary artery and after sternal closure. Results: No significant hemodynamic changes were observed during the displacement of the heart or the placement of a stabilizer on all of the coronary arteries, except the obtuse marginal artery (OM) before anastomosis. RVEF, left ventricular stroke work index (LVSWI), stroke volume index and cardiac index (CI) decreased and mean pulmonary artery pressure increased significantly whist positioning the graft to the OM. Right ventricular volumes were not significantly changed, although central venous pressure and pulmonary capillary wedge pressure increased. Changing CI had a close relationship with LVSWI (r2 = 0.537, P ๏ผ 0.05) but not with RVEF (r2 = 0.118). These hemodynamic compromises recovered to baseline values after sternal closure. Conclusions: The displacement of the beating heart for positioning during anastomosis of the graft to the OM caused significant hemodynamic instability and LV functional changes in addition to RV functional changes seemed to be responsible for hemodynamic derangements.ope

[์๋ฌธ]The uncontrolled proliferation of cancer cells causes the formation of hypoxic regions within solid tumors due to lack of adequate blood supply. Hypoxia exists to varying degrees in most solid tumors, and clinically, tumor hypoxia has been associated with poor therapeutic response and patient prognosis. Hypoxia-inducible factor (HIF) is a heterodimeric transcription factor that mediates responses to hypoxia by binding to a hypoxia-response element (HRE) present within target genes. Therefore, the HIF/HRE system can be utilized to specifically target therapeutic gene expression to tumors.Tumor necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL), a member of the TNF family, is a type II transmembrane cytokine molecule. Soluble TRAIL has been shown to induce apoptosis in a wide variety of cancer cells in vitro and to suppress tumor growth specifically without damaging normal cells and tissues in vivo. In our previous report, we have used that adenovirus expresses and secretes highly apoptotic secretable trimeric TRAIL into the culture supernatant3. Here, as an approach to TRAIL-based cancer gene therapy, Secretable trimeric TRAIL (stTRAIL) was previously cloned into YKC11 under the modified mTERT promoter to form YKC11-stTRAIL, which had much better cytopathic effect than YKC11. YKC11-stTRAIL adenovirus potently induced apoptosis in vitro in cancer cell lines such as U87MG, U343, U251N in hypoxic environment. Our data reveal that a gene therapy using YKC11-stTRAIL has a promising potential to treat human cancers including gliomas. Taken together, these lines of evidence demonstrate that the newly generated adenovirus expressing stTRAIL induces efficient cell lysis in hypoxic environment, which is critical for adding therapeutic value to replicating adenovirus for cancer gene therapy.ope

A sexual education program to enhance the sexual knowledge and satisfaction of the patients with lumbar disc surgery was developed and its effect was evaluated using a quasi-experimental study.The sexual education program for the patients with lumbar disc surgery was develop by analyzing sexual counseling cases, in Y university hospital department of neurosurgery, from Dec. 2001 to Jan. 2003 and from related articles. Six professionals in this field were consulted and a pre-evaluation was performed with 10 patients and revised into the final program. The effect of sexual education program was evaluated from, April 1 to May 3, 2003, in total of 54 patients with simple posterior lumbar laminectomy or posterior lumbar fusion, 1 month after their discharge, in Y university hospital department of neurosurgery, using a nonequivalent control group pretest-posttest design.24 patients in the experimental group received one session of 30 to 40min sexual education and the control group did not. The evaluation of educational effect was performed, 1month after the education, using a questionnaire developed from Derogatis sexual function inventory translated by BS Jang and YW Kim. Data were analyzed using SAS 8.1. General characteristics are in real numbers and percents. X2- test and t-test were used to verify the hypothesis and homogeneity between the experimental and the control group. Pearson's correlation coefficient was used to analyze the relationship between sexual knowledge and sexual life satisfaction.The results of the study are as follows. 1. Hypothesis 1, โthere is a difference in the sexual knowledge between spine surgery patients with sexual education and withoutโ, was statistically significant(t=7.82, p=.00). 2. Hypothesis 2, โthere is a difference in the sexual satisfaction between spine surgery patients with sexual education and withoutโ, was statistically significant(t=7.20, p=.00).3. Sexual knowledge and Sexual life satisfaction had significant correlation(pre education r=.57, p=.00 post education r=.60, p=.00).In summary, a systemic sexual education program for the patients with lumbar disc surgery increases the sexual knowledge and sexual life satisfaction. The sexual knowledge and sexual life satisfaction has a significant relationship. Therefore the sexual education program developed by interview results of the spine surgery patients will not only be a specific guidebook of sexual life satisfaction for the spine surgery patients, but also serve as a foundation for process of developing education programs.ope
