Factors influencing sexual satisfaction in patients with breast cancer between support group and non support group

간호학과/석사[한글] 유방암 환자들은 암으로 인한 공포뿐만 아니라 수술로 인한 여성미의 상실로 인해 부부에게 있어 가장 중요하며, 인간의 기본욕구인 성생활과 관련된 문제점들을 겪고 있다. 성생활은 개인의 삶의 질을 측정하는 지표들 중의 하나로 만족스런 성생활은 삶의 중요한 부분이지만, 사회문화적 배경이나 의료진의 무관심 등으로 인해 공개적으로 표현되고 있지 못하는 형편이며, 이에 대한 중재 또한 제대로 이뤄지지 못하고 있는 실정이다. 자조집단은 공통의 경험, 사고 및 느낌을 가진 대상자들이 제 문제를 서로 나눔으로써 정보의 교환과 보편성, 소속감, 수용감 및 응집력이 형성되어 긍정적 결과를 가져오는 중재방법으로 유방암 환자의 치료과정 단계뿐만 아니라 추후관리과정 단계에도 다양하게 적용되고 있다. 이에 본 연구는 자조집단 참여여부에 따른 유방암 환자의 성생활 만족 영향요인을 비교하고 그 차이를 발견함으로써 유방암 환자의 성생활 적응을 도울 수 있는 보다 근거 있는 중재를 마련하기 위함이며, 유방암 환자의 질적인 삶을 증진시킬 수 있는 중재 방안으로 자조집단을 보다 활성화시키는 전략개발을 위한 기초 자료를 제공하고자 시도되었다. 본 연구의 대상은 2004년 4월 1일부터 6월 1일까지 유방암 환자의 자조집단이 있는 서울 소재의 S병원에서 자조집단에 참여한 유방암 환자 63명과 자조집단에 참여하지 않은 유방암 환자 76명을 표본으로 하였다. 본 연구의 도구는 구조화된 질문지로서, 성생활 만족과 성생활관련 요인, 그리고 일반적 특성 및 질병, 성생활관련 특성을 측정하였고, 자가보고법에 의해 자료수집되었다. 수집된 자료는 부호화한 후 SAS 프로그램을 이용하여 서술 통계, χ2-test, t-test, ANOVA, Pearson correlation과 Spearman rank correlation, Stepwise multiple regression으로 분석하였다. 본 연구의 결과는 다음과 같다. 1. 자조집단 참여군과 비참여군의 성생활 만족과 관련된 요인의 분포와 동질성을 검증한 결과, 두 군은 동질한 것으로 나타났다. 2. 자조집단 참여군과 비참여군 간의 성행위, 신체상, 사회적 지지를 비교해 본 결 과, 성행위와 신체상은 통계적 유의성이 없었으며(p = .855; p = .576), 사회적 지지는 통계적 유의성이 나타났다(p = .045). 사회적 지지 중 “내가 병으로 자리 에 눕게 되면 도와줄 사람이 있다”(p = .030), “내가 스스로 식사준비를 할 수 없을 때 대신 해 줄 사람이 있다”(p = .000), “내가 아플 때 집안일을 도와줄 사 람이 있다”(p = .025), “내가 필요한 사람이라고 느끼게 해주는 사람이 있 다”(p = .028), “내가 신경 쓰지 않도록 도와줄 사람이 있다”(p = .024), “나의 문 제를 이해해 줄 사람이 있다”(p = .042)의 문항에서 두 군간의 통계적 유의성이 나타났다. 3. 자조집단 참여군과 비참여군 간의 성생활 만족을 비교해 본 결과, 자조집단 참여 군이 33.24점(±7.23)으로 자조집단 비참여군의 31.67점(±7.60)보다 높게 나타났으 나, 통계적 유의성은 없었으며(p = .218), “나와 배우자의 성생활은 단조롭다(역 문항)”의 문항에서 두 군간의 통계적 유의성이 나타났다(p = .038). 4. 자조집단 참여군과 비참여군의 일반적 특성과 질병, 성생활관련 특성에 따른 성 생활 만족을 비교해 본 결과, 자조집단 참여군의 일반적 특성 중 연령에서 연령 이 낮을수록 성생활 만족이 높게 나타나, 통계적 유의성이 나타났다(p = .037). 또한 일반적 특성에 따른 자조집단 참여군과 비참여군의 성생활 만족을 비교해 본 결과, 대상자의 교육수준과 배우자의 교육수준 중 대졸이상인 군에서 성생활 만족이 높게 나타나, 통계적 유의성이 나타났다(p = .022; p = .012). 5. 자조집단 참여군과 비참여군 간의 성생활 만족과 유의한 상관관계를 보인 요인 을 비교해 본 결과, 자조집단 참여군은 성행위, 연령, 수술 후 성교횟수의 변화, 신체상, 대상자의 교육수준 순이었고, 자조집단 비참여군은 성행위, 사회적 지지, 신체상 순이었다. 6. 자조집단 참여군과 비참여군 간의 성생활 만족 영향요인을 비교해 본 결과, 자조 집단 참여군은 성행위가 성생활 만족의 36.9%를 설명하였고, 자조집단 비참여군 은 성행위, 신체상, 사회적 지지가 성생활 만족의 38.0%를 설명하였다. 이를 종합해 보면, 성생활 만족 영향요인은 자조집단 참여여부에 따라 다르며, 자조집단 참여여부에 따른 성생활 만족은 차이가 없는 것으로 나타났다. 본 연구가 자조집단과 성생활 만족 영향요인을 관련지어 설명할 수 있는 근거자료를 제시했다는 점에서는 의의가 있지만, 성생활 만족의 직접적인 중재효과로써 자조집단을 설명하기엔 충분하지 못하므로 자조집단에 대한 중재의 효과로 정당성을 설명하기 위해서는 성생활과 관련된 직접적 프로그램을 운영하여 그 효과를 측정하는 연구가 뒷받침되어져야 한다. 또한 유방암 환자를 위한 중재는 유방암 치료과정 중 뿐만 아니라 치료과정 후에도 지속적이어야 하므로 자조집단 참여를 통한 지속적 교육과 상담은 유방암 환자의 긍정적 성생활을 도모하는데 기여할 수 있을 것이다. [영문]The purpose of this study was to identify the influence of sexual behavior, body image, social support, and other characteristics (general, disease-related, sexual life-related) on sexual satisfaction in patients with breast cancer according to their participation in support group. This study attempted to provide basic data for the development of strategies to activate support groups and develop well-grounded interventions to improve the quality of life among breast cancer participants in a support group. Data were collected by self-report questionnaries from April 1 to June 1, 2004. Breast cancer patients wew recruited from one hospital in Seoul, Korea, and participants included 63 patients attending a support group and 76 patients who did not participate in the support group. The questionnaire set consisted of sexual satisfaction, sexual behavior, body image, social support and information on general characteristics, disease-related characteristics, and sexual life-related characteristics. Data were analyzed using the SAS program. The results of this study were as follows; 1. There was no statistically significant difference in general characteristics, disease-related characteristics, and sexual life- related characteristics between support group participants and non support group participants. 2. There was no statistically significant difference in sexual behavior and body image between the two groups(p = .855; p = .576). Social support scores were significanty higher in the support group participants(p = .045). 3. There was no statistically significant difference in sexual satisfaction between the two groups(p = .218). 4. Sexual satisfaction was positively related with sexual behavior, post-op change of sexual intercourse frequency, body image, and patient''s education level, and negatively related with age in support group participants. Sexual satisfaction was positively related with sexual behavior, social support and body image in non support group participants. Sexual behavior explained 36.9% of the sexual satisfaction in support group participants. Sexual behavior, body image, and social support explained 38.0% of the sexual satisfaction in non support group participants. The result of this study provides basic data which can explain that support group participation and factors influencing sexual satisfaction are related, although study findings are insufficient in explaining support groups as having a direct impact on sexual satifaction among breast cancer patients. Implications point to the need for the development and implementation of programs that focus specifically on sexual life issues for breast cancer patients, as well as further reseaech measuring the effects of such intervention programs. Interventions for patients with breast cancer must be offered not only during the treatment process but afterwards as well. Continuous education and counseling through participation in support groups can contribute to promote and affirm a healthy sexual life for patients with breast cancer.ope
