    Comparative study of oncologic outcomes for laparoscopic vs. open surgery in transverse colon cancer

    PURPOSE: Laparoscopic resection for transverse colon cancer is a technically challenging procedure that has been excluded from various large randomized controlled trials of which the long-term outcomes still need to be verified. The purpose of this study was to evaluate long-term oncologic outcomes for transverse colon cancer patients undergoing laparoscopic colectomy (LAC) or open colectomy (OC). METHODS: This retrospective review included patients with transverse colon cancer who received a colectomy between January 2006 and December 2010. Short-term and five-year oncologic outcomes were compared between these groups. RESULTS: A total of 131 patients were analyzed in the final study (LAC, 84 patients; OC, 47 patients). There were no significant differences in age, gender, body mass index, tumor location, operative procedure, or blood loss between groups, but the mean operative time in LAC was significantly longer (LAC, 246.8 minutes vs. OC, 213.8 minutes; P = 0.03). Hospital stay was much shorter for LAC than OC (9.1 days vs. 14.5 days, P < 0.01). Postoperative complication rates were not statistically different between the two groups. In terms of long-term oncologic data, the 5-year disease-free survival and overall survival were not statistically different between both groups, and subgroup analysis according to cancer stage also revealed no differences. CONCLUSION: LAC for transverse colon cancer is feasible and safe with comparable short- and long-term outcomes.ope

According to the current situation of the health of young children, Domestic studies have looked at factors that affect the health of early life mainly in the field of children study and health science, most of which were socioeconomic status of parents. Overseas studies have continued to report that family structure affects the health of children in their early years of life. In this situation, there is a family structure that we should pay attention to. Its a unwed single father family. The reasons are as follows. There are problems with birth registration of children of unwed fathers and social vulnerability of single father family. Also there is a problem that children of unwed fathers are unlikely to be affected by their biological mother from the time they are born, which is a point that distinguishes them from other single father families. In the absence of study on the health of young children of unwed fathers, it is necessary to seek the direction of policymaking to improve their health. The purpose of this study is to explore in depth the health-related factors and contexts among young children of unwed fathers. Accordingly, the data were collected through 1:1 in-depth interviews with 10 unwed fathers and were analyzed according to the inclusive analytic procedures. The codes derived under the inclusive anlaytic procedures are classified based on similarities and differences and are named subcategories and categories. The researcher derived a total of 98 codes, 38 subcategories, and 12 categories from the analysis. Because the categories were scattered, the researcher described them in three main areas. The three areas are the health concept of young children recognized by fathers, the health-related factors of young children, and the fathers response strategy for the health of young children. Area is not a term used in the inclusive anlaytic procedures and is a term used arbitrarily by this researcher. The health concepts of young children recognized by fathers were the fulfillment of a childs basic needs, the good growth and development of a child, the balance of physical, mental and social health, and the healthy environment around a child. Based on the health concepts of young children recognized by fathers, factors such as the difficulty of workfamily reconciliation, the world of only a child and me, fathers mental health and absence of care means, the lack of resources and opportunities for unwed single father family and the unfulfilled basic human rights were negatively affecting the health of young children of unwed fathers. The fathers were responding in their own way. They were responding to the preparation for childrens meal in their own way, were trying to establish material stability for their children, and were doing their best for relationship with children and their childrens social relationship. Based on the results of the analysis, this study suggested the following policies for the health of young children of unwed fathers. We need to find ways to manage mental health of unwed fathers. In addition, initial response-related support from field offices such as community service centers, hospitals, and single-father facilities is required. This could also be considered for online forms such as sites for unwed single father family or emergency and counseling calls. Ultimately, the issue of birth registration of young children of unwed fathers should be resolved. Finally, support for workfamily reconciliation of unwed fathers is required.     Text orientation estimation by using distributed label

    MasterThis paper proposed a method to increase the accuracy of the predicted orientation of text. We modified cross-entropy loss by distributing the label weight. After predicting the initial angle by using the deep-learning method, the rotation angle was more accurately predicted with a post-processing algorithm. Among the various distribution, when the label was distributed by using triangular distribution, the accuracy was the highest. The proposed model increased the accuracy of text orientation prediction, and reduced the error by using triangular distribution, and consequently increased the accuracy of final character recognition

    Short- and long-term outcomes of laparoscopic surgery for intestinal Behcet's disease: a comparative study with open surgery

    INTRODUCTION: Surgical treatment of intestinal Behcet's disease (BD) is not well established. Specifically, it is still difficult to assess the clinical value of laparoscopic surgery to address this condition. We aimed to evaluate the clinical course and the characteristics of laparoscopic surgery for intestinal BD compared with open surgery. METHODS: We reviewed charts of 91 patients who underwent surgical treatment for intestinal BD between January 1995 and December 2012. We retrospectively compared the laparoscopic group (LG, n = 30) and the open group (OG, n = 61) in terms of patient demographics, clinical features, operative data, postoperative course, complications within 30 days after operation, and long-term follow-up data. RESULTS: There were more females in the LG than in the OG (63.3 vs. 36.1%, p = 0.014), and oral/genital ulcers were more frequent in the LG (76.7 vs. 54.1%, p = 0.038; 60 vs. 36.1%, p = 0.031). Intractability with medical treatment was dominant in the LG (76.7 vs. 45.9%, p = 0.02), while intestinal perforation or fistula were more prevalent in the OG (10 vs. 44.3%, p = 0.001). Most patients received an ileocecectomy or a right hemicolectomy as their first surgery. In the LG, the patients had a shorter operation time (162.0 vs. 228.5 min, p < 0.001) and had less blood loss (61.7 vs. 232.3 ml, p = 0.003). There were no significant differences in postoperative complications, reoperation, mortality, and hospital stay between the groups. During the follow-up period, the mean number of operations was less in the LG than in the OG (1.3 vs. 2.1, p = 0.011). Analysis indicated that 20% of patients in the LG and 50.8% in the OG underwent more than two operations (p = 0.005). CONCLUSION: Laparoscopic surgery is feasible and safe for selected intestinal BD patients. However, there were no better short-term outcomes in LG compared with OG.restrictio
