Makorin Ring Finger Protein 1 as Adjunctive Marker in Liquid-based Cervical Cytology

To assess the utility of makorin ring finger protein 1 (MKRN1) as a marker of cervical pathology.A PROspective specimen collection and retrospective Blinded Evaluation study was conducted. Liquid-based cytology samples were collected from 187 women, embedding all residuals as cell blocks for immunohistochemical staining of MKRN1 and P16 . Results of liquid-based cervical cytology, immunostained cell block sections, and human papillomavirus (HPV) hybrid capture (with real-time polymerase chain reaction) were analyzed. Clinical outcomes were analyzed overall and in subsets of specimens yielding atypical squamous cells of undetermined significance or low-grade squamous intraepithelial lesions.Makorin ring finger protein 1 positivity and grades (1-3) of cervical intraepithelial neoplasia (CIN) increased in tandem (CIN1, 32.4%; CIN2, 60.0%; and CIN3, 80.0%), reaching 92.3% in invasive cancer. Sensitivity, specificity, positive predictive value, and negative predictive value in detecting CIN2+ via MKRN1 were 73.8%, 76.8%, 75.6%, and 75.0%, respectively. The performance of liquid-based cytology was poorer by comparison (61.3%, 69.5%, 66.2%, and 64.8%, respectively), and HPV assay (versus MKRN1 immunohistochemical staining) displayed lower specificity (67.7%). Combined HPV?+?MKRN1 testing proved highest in sensitivity, specificity, positive predictive value, and negative predictive value (71.8%, 85.5%, 82.3%, and 76.5%, respectively), whereas corresponding values for cytology?+?HPV (60.6%, 81.8%, 75.4%, and 69.2%) and cytology?+?MKRN1 (58.8%, 84.1%, 78.3%, and 67.7%) were all similar. In instances of atypical squamous cells of undetermined significance or low-grade squamous intraepithelial lesions, the HPV?+?MKRN1 combination performed best by above measures (100%, 72.7%, 73.9%, and 100%), followed by cytology?+?MKRN1 (100%, 50.0%, 60.7%, and 100%).Makorin ring finger protein 1 displayed greater sensitivity and specificity than liquid-based cytology and proved more specific than HPV assay. In combination testing, MKRN1?+?HPV showed the highest sensitivity and specificity levels. The MKRN1 biomarker may be a useful adjunct in primary cervical cytology screening.ope

Clinical Significance of CA125 Level after the First Cycle of Chemotherapy on Survival of Patients with Advanced Ovarian Cancer

PURPOSE: To determine the most powerful cancer antigen 125 (CA125)-related prognostic factor for advanced epithelial ovarian cancer (EOC) and to identify cut-off values that distinguish patients with a poor prognosis from those with a good prognosis. MATERIALS AND METHODS: We included 223 patients who received staging laparotomy and were diagnosed with stage IIC-IV serous EOC. Cox regression analysis was used to determine the most significant prognostic factor among the following variables: serum CA125 before surgery and after the first, second, and sixth cycles of chemotherapy; the nadir CA125 value; the relative percentage change in CA125 levels after the first and second cycles of chemotherapy compared to baseline CA125; CA125 half-life; time to nadir; and time to normalization of the CA125 level. RESULTS: The CA125 level after the first chemotherapy cycle was the most significant independent prognostic factor for overall survival (OS). Time to normalization (p=0.028) and relative percentage change between CA125 levels at baseline and after the first chemotherapy cycle (p=0.021) were additional independent prognostic factors in terms of OS. The CA125 level after the first chemotherapy cycle (p=0.001) and time to normalization (p<0.001) were identified as independent prognostic factors for progression free survival (PFS). CONCLUSION: Among well-established CA125-related prognostic factors, serum CA125 levels after the first cycle of chemotherapy and time to normalization were the most significant prognostic factors for both OS and PFS.ope

On the factors influencing the length of stay in military hospital

보건관리학과/석사[한글] 이 연구는 군 병원의 재원기간에 영향을 미치는 요인을 분석하기 위해서 1900년도 퇴원환자중 폐결핵, 급성충수염, 만성간질환경변, 무릎관절내부장애, 추간원판장애 환자 1300명을 대상으로 조사하여 다음과 같은 결과를 얻었다. 1. 입원한 환자의 상위 5개 질병인 폐결핵, 만성간질환경변, 무릎관절내부장애, 추간원판장애가 차지하는 비율이 25.4% 였으며, 각 질병의 비율은 3.5%, 3.1%, 1,9%, 3.3%, 13.7% 였다. 2. 질병별 평균재원기간을 보면 급성충수염 27.53일, 폐결핵 78.9일, 만성간질환경변 53.54일, 추간원판장애 66.29일로 민간의료기관의 재원기간보다 각각 2.9배, 3.38배, 2.61배, 3.27배가 높았다. 3. 군 입원환자 특성별 상병별 평균재원기간은 5개질병 모두 타군(해·공군)재원일수가 길었고, 계급별로는 모두 간부에 비해 사병이 길었으나 추간원판장애의 경우는 간부가 사병보다 13일 길었다. 급성충수염의 경우는 간부가 사병에 비해서 약 2배가 짧았다. 4. 군 특수요인별 재원기간은 폐결핵의 경우 발병시기에서 근무외 발생환자가 92.5일, 일반 후송시 17일 이었고, 급성충수염의 경우는 전공상별로는 비전공상 환자가 49.1일, 일반 후송시 24.7일 이었고, 무릎관절내부장애의 경우는 근무외 발생 환자가 86.6일 이었으며, 추간원판장애의 경우는 공상환자가 62.4일, 근무외 발생환자가 71.1일, 일반 후송시 24.7일 이었다. 5. 군 병원 특성별 평균재원기간은 모두 통계학적으로 유의한 결과를 나타내고있는데 폐결핵은 이동외과병원, 야전병원, 수도·후방병원별로 각각 4.79일, 12.69일, 29일, 127.74일 이었고, 급성충수염의 경우는 12.24일, 24.10일, 34.62일, 31.7일, 만성간질환경변의 경우는 각각 4.39일, 19.8일, 36.20일, 97.62일 이었으며, 무릎관절내부장애의 경우는 11.29일, 31.4일, 44.96일, 125.27일, 추간원판장애의 경우는 15.27일, 37.1일, 46.30일, 103.24일로 나타났다. 이는 질병별로 병원의 특성에 따라 유의한 차이가 있음을 나타내며 급성충수염의 경우에는 수도·후방 병원이 약32일로 이동외과병원보다 2.61배 높음을 의미한다. 6. 중회귀 분석의 결과 군병원의 재원기간에 영향을 미치는 요인의 경우는 폐결핵은 병원의 특성에서 후송병원과 수도·후방병원이, 급성충수염의 경우에는 후송수단이, 만성간질환경변의 경우에는 후송병원와 수도·후방병원이, 추간원판장애의 경우에는 야전 후송 수도·후방병원이 영향을 미치는 요인으로 작용하였으며 환자 개인적 특성요인은 군별에서, 군특수 요인은 후송수단이 영향을 미치는 요인으로 작용하였다. 위의 결과로 보아 현 군병원 재원기간에 영향을 미치는 요인은 선정된 5개 질병에서 보면 군병원인 야전병원, 후송병원, 후방병원등의 군병원 특성이, 재원기간에 영향을 미치고, 군환자 개인특성중 일부인 군별에 따라 차이가 있고, 군 특수요인으로 생각되는 후송수단이 일부 영향을 미치는 것으로 생각된다. 따라서 현 군진료체계의 질적향상과 효율성의 제고를 위해서 위의 결과를 토대로 질병별 재원일수를 선정하여, 재원기간을 단축하여, 병원관리를 하는 것이 필요하다고 생각한다. [영문] This study was carried out to analyze that the factors influencing the length of a hospital stay the 1,300 patients suffering from the diseases such as Pulmonary tuberculosis, Acute appendicitis, Chronic hepatocirrhosis, Internal derangement of knee and Intervertebral disc disorders in the military hospital. 1. The ratio of upper 5 diseases of inpatient was 25.5% ; each diseases was 3.5%, 3.1%, 1.9%, 3.3% and 13.7%. 2. The average hospitalized period of each diseases was 27.53 days in Acute appenditis, 78.9 days in Pulmonary tuberculosis, 53.54 days in Chronic hepatocirrhosis and 66.29 days in Intervertebral disc disorders. The days of hospital treatment was 2.9 times, 3.3 times, 2.6 times, 3.27 times as long as private medical institution. 3. Average hospitalized period by characteristics and diseases was long than other force's, soldier's was long than officer's in most case. In the case of Acute appendictis, officer's was 2 times as short as soldier's. 4. Hospitalized period of Pulmonary tuberculosis attacted in the before enlistment and off time was long 4 days, 29 days more than other cases in specific factors, Statistically significant differences were observed between wounded and evacuation condition in Acute appendictis, attack point and evacuation condition in Chronic Hepatocirrhosis and off time in Internal derangement of knee. In the case of Intervertebral disc disorders, statistically significance was observed. 5. Statistically significant differences were observed between hospitalized period and characteristics of the military hospital. The hospitalized period of each diseases as follow ; Pulmonary tuberculosis was 4.79, 12.69, 29 and 127.7 days, Acute appendicitis was 12.24, 24.10, 34.60 and 31.7 days, Chronic hepatocirrhosis was 4.39, 19.8, 36.20 and 97.62 days, Internal derangement of knee was 11.29, 31.4, 44.96 and 126.27 days and Intervertebral disc disorders were 15.27, 37.01, 46.30 and 103.24 days in MASH, Field Hospital, Goneral Hospital. 6. According to the results of multiple regression analysis, the factor affecting hospitalization period were evacuation hospital and National rear hospital for Pulmonary tuberculosis ; evacuation means for Acute appendicitis ; evacuation hospital and National rear hospital for Chronic hepatocirrhosis ; field, evacuation hospital and National rear hospital for Intervertebral disc disorders. Usefulness of Makorin Ring Finger Protein1 (MKRN1) immunocytochemistry in liquid-based cervical cytology

의학과/석사목적 MKRN1이 자궁경부 병변에서 면역반응성이 증가하는지 확인하고 MKRN1 면역염색이 자궁경부암 진단의 정확도를 높이는데 유용한 검사법인지 확인하기 위하여 본 연구를 시행하였다. 방법 2013년 6월 강남세브란스병원 임상시험심사위원회의 승인을 받은 이후 2013년 7월부터 전향적으로 검체수집을 하였다. 자궁경부 액상세포검사 (cytology)와 인유두종 바이러스검사 (HPV test)를 시행하고 남은 cytology 검체를 이용하여 세포블록을 제작한 뒤 MKRN1과 p16INK4a 면역염색을 시행하였다. 4가지 검사 결과는 최종 조직검사 결과와 비교하였다. 각 검사법의 CIN2 이상 (CIN2+)의 병변을 진단하는데 있어 진단 정확도를 비교하기 위해 민감도, 특이도, 양성예측도, 음성예측도, 정확도를 계산하였고 ASCUS/LSIL군만 따로 케이스 선택하여 같은 분석을 반복 시행 하였다. 결과 연구에 등록된 환자는 189명 이었으나 2개의 검체에서 자궁경부 세포가 관찰 되지 않아 최종 분석에서 제외하였다. 전체 환자 187명 중 비정상 cytology를 보인 환자는 140명 (74.9%), 정상 cytology를 보인 여성은 47명 (25.1%) 이었다. Cytology와 HPV test는 각각 CIN1에서 78.9%, 86.8%, CIN2에서 90.6%, CIN3/CIS에서 93.6%, 97.9%, cancer에서 92.9%의 양성률을 보였고 MKRN1과 p16 INK4a는 각각 CIN1에서 31.6%, 60.5%, CIN2에서 62.5%, 65.6%, CIN3/CIS에서 74.5%, 76.6%, invasive cancer에서 92.9%, 85.7%로 중증도에 비례해서 증가하는 양상을 보였다. MKRN1의 CIN2+를 진단하는데 있어 민감도, 특이도, 양성예측도, 음성예측도, 정확도는 각각 73.1%, 74.5%, 73.9%, 73.7%, 73.8%였다. 그러나 cytology와 HPV test는 민감도면에서는 각각 92.5%, 94.6%로 MKRN1보다 높았으나 특이도가 42.6%, 54.3%로 낮았다. p16INK4a의 민감도, 특이도, 정확도는 74.2%, 53.2%, 63.6%로 4가지 검사 방법 중 정확도가 가장 낮았다. 두 가지 검사를 조합하여 보았을 때, cytology+HPV는 90.3%의 민감도를 보였으나 특이도가 66%로 가장 낮았던 반면 MKRN1+HPV는 민감도는 68.8%였으나 특이도가 87.2%로 가장 높았고 두 조합의 진단의 정확도는 같았다 (78.1%). ASCUS/LSIL의 경우에는 HPV+MKRN1가 민감도 60.0%, 특이도 81.3%로 가장 정확도가 높았다 (73.1%). 결론 MKRN1은 cytology, HPV test보다 민감도는 낮았으나 특이도가 높았다. 그리고 두 가지 검사를 조합하였을 경우 HPV+MKRN1은 ASCUS/LSIL군에서 cytology+HPV보다 진단의 정확도가 더 높았고 전체 환자군에서는 정확도가 같았다. 따라서 MKRN1 면역염색은 특이도가 낮은 cytology 또는 HPV 검사의 단점을 보완하여 자궁경부암 진단의 정확도를 높이는데 보조적인 검사로 유용하게 이용될 수 있을 것으로 생각된다.ope

Choledochal cyst rupture with an intrahepatic pseudocyst mimicking hepatic mesenchymal hamartoma in an infant

Spontaneous choledochal cyst rupture and consequent biliary peritonitis are rare in children. Here, we report a unique case of intrahepatic bile duct rupture of choledochal cyst type IV that resulted in loculated biliary peritonitis with an intrahepatic pseudocyst mimicking a hepatic mesenchymal hamartoma in a 3-month-old boy. Magnetic resonance cholangiopancreatography can help differentiate these conditions. Regardless of the location, pseudocysts resulting from biliary peritonitis should be considered in cases of choledochal cysts with adjacent fluid collection or cystic lesions.restrictio
