공기업의 보수 복리후생과 재무건전성과의 관계

(The) study on normal endometrial and concomitant ovarian histology of Korean women following normal menstruation

의학과/석사[한글] 정상적인 월경주기를 가진 사람의 각 시기별 자궁내막의 조직상과 난소의 변호를 이해하면 자궁내막 또는 난소의 기능적인 질환을 가진 환자를 접할 대 어는 단계의 이상인지를 판정하는 데 크게 도움이 된다. 정상 자궁내막 및 난소의 이러한 표준적인 조직상에는 인종 또는 환경요소가 관여할 가능성을 배제할 수 없음에도 불구하고 아직 한국인을 대상으로 한 연구가 거의 없다. 그러므로 지난 3년간 원주기독병원 산부인과 및 병리학교실과 3년 5개월간 영동세브란스병원 산부인과 및 병리과에서 검색하였던 정상 월경주기를 지닌 가임연령의 여성에서 난소 및 자궁내막질환 이외의 수술적응증이 있어 자궁 및 기능성 난포가 포함된 난소를 동시에 절제한 예들을 대상으로 이들의 자궁내막과 난소가 알려진 월경주기에 따라 어떤 조직상을 나타내는 가를 조사한 바 다음과 같은 결론을 얻었다. 1) 총 184예의 한국여성의 정상 월경주기에 따른 자궁내막의 변화는 기존의 국내외의 보고와 거의 동일하였다(Noyes, 1950; 지 및 최, 1980). 2) 연구조건에 합당한 난소는 모두 116예 였으며, 황체형성이 되기 전 단계인 전배란기에서는 난포의 하루하루를 구별할 수 있는 뚜렷한 조직학적 특징을 찾을 수 없었다. 배란일로부터 월경이 시작하기까지의 황체의 변화에 있어서도 때에 따라서는 하루하루를 구별할 수 있는 조직학적 특징이 뚜렷하지 않았으나 자궁내막 소견을 참고하여 비교적 일관성 있는 소견을 관찰할 수 있었으며 이들도 역시 이미 보고된 다른 저자들의 것과 대동소이하였다.(Brewer, 1942; Corner, 1945, 1956). 3) 보다 더 정확한 형태학적 특징을 제시하기 위하여는 더 많은 수의 검색 자료와 정확한 배란일 측정이 필요하다고 사료되었다. [영문] Normal endometrial and concomitant ovarian histology during menstrual cycle was obtained, bases on 184 normal uteri with or without ovaries including functional follicles that were removed surgically. These uteri and ovaries were selected from all the hysterectomy specimens with or without adnexa for the period of 3years and 5months at Young dong Severance Hospital and of 3years at Won ju Christian Hospital. Only those uteri that showed no gross or microscopic abnormalities except for a minimal lesions confined within the cervix were included in the study. The ovaries had to include functional follicles without pathologic lesions, and the lesions were minimal, if any. Microscopic findings of each endometrium were described according to the criteria set beforehand, and then were matches with menstrual history. The result was much the same as the previous reports by Noyes, et al.(1950) and Chi and Choi in Korea(1980). Cases selected for the histological dating of the corpus luteum were markedly limited(116 cases). Prior to ovulation, it was impossible to observe the characteristic histologic findings of the functional follicle matched with menstrual history. But each histologic finding of the corpus luteum between the day of ovulation and the standard cycle day 28 was distinguishable by its characteristic histologic findings, and the result of the histological dating of the corpus luteum was relatively corresponded to the previous report by Corner(1956). Further studies must be performed with much more specimens and with much more detail ovulation days for making a more accurate norm and for evaluating abnormal histology of the ovaries.restrictio
