The main purpose of this study was to have a thorough grasp of resilience of adolescents of problem-drinking parents which conduce to maintenance their school and social life positively and achievement of developmental tasks in adolescents normally. Therefore the concrete purposes of this descriptive study were to describe resilience of adolescents, to verify resilience of adolescents according to the problem-drinking in parent, and to find out related factors and predictive factors of resilience of adolescent of problem-drinking parents.
Empirical data for testing were collected conveniently from 482 adolescents who were students of 4 middle schools in Seoul and a local city using self-administered questionnaires between March 26 and April 8, 2003. The 31-revised resilience measurement appropriate to adolescents from Kim(2003)''s instrument, the Korean Version of the Children of Alcoholics Screening Test(CAST-K, Kim, Chang, & Kim, 1995), Rosenberg''s Self-Esteem Scale(Chang, 1991), Family Adaptability and Cohesion Evaluation Scale โ
ข(FACES โ
ข, Ahn, 1988), and measurements of relationship with friends and teachers at a school(Cheon, 2003; Roh, 1998) were employed to measure resilience as a dependent variable, problem-drinking level of parents, self-esteem, family adaptability and cohesion, and relationships with friends and teachers at a school as independent variables. SPSS 10.0 WIN Program using descriptive statistics, T-test, ANOVA, Pearson correlation, and multiple regression was used for data analysis.
The major findings are as follows.
The mean score of adolescent resilience was 92.99ยฑ13.49, among components of the resilience measurement in three domains, the score of intrapersonal characteristics was highest, secondly the score of interpersonal characteristics, and the score of characteristics of coping was lowest which was lower than the total mean score of resilience. This means that the intervention in coping skill can improve resilience of adolescents.
As s result of CAST-K, adolescents of non-drinking or social drinking parents who scored below 5 were 300(62.2%), adolescents of heavy drinking parents who scored 6 through 12 were 95(19.7%), and adolescents of problem-drinking parents who scored over 13 were 69(14.3%). The mean score of resilience of adolescents of non-drinking or social drinking parents was 94.82ยฑ13.06, the score of adolescents of heavy drinking parents was 91.35ยฑ12.72, and the score of adolescents of problem-drinking parents was 87.36ยฑ14.38 which was lowest among 3 groups(p=.000).
The related factors to resilience of adolescents of non-drinking or social drinking parents were self-esteem, family adaptability and cohesion, and relationship with friends and teachers at a school, gender, existence of siblings, an exercise as a hobby, the number of close friends and respectable teachers at a school(p<.05), in case of adolescents of heavy drinking parents were self-esteem, family adaptability and cohesion, and relationship with teachers at a school, an exercise as a hobby, gender, monthly income(p<.05). The related factors to resilience of adolescents of problem-drinking parents were self-esteem, family adaptability and cohesion, and relationship with friends at a school, a job of a father, gender(p<.05).
According to the results of multiple regression analysis, the predictive factors of adolescent resilience were significantly self-esteem and family adaptability and cohesion in all of 3 groups(p=.000).
In conclusion, the results of this study provided evidence that potently influential factors to adolescent resilience which help them to overcome crisis situations were self-esteem and family adaptability and cohesion. These factors are needed to adolescents of problem-drinking parents the most. The results of this study suggest that nursing interventions utilizing the methods of self-esteem improvement and family intervention are needed to promote resilience of adolescents of problem-drinking parents. In addition, interventions at various kinds including the utilization of individualized strengths and resources are also needed.
The contribution of this study is to give a wholistic understanding of the adolescents of problem-drinking parents and to provide a foundation for the development of programs for adolescents of problem-drinking parents.ope

[์๋ฌธ]The purpose of this study was to compare nonpulsatile perfusion with pulsatile
perfusion on hemodynamics and kidney function during and after cardiopulmonary
bypass(CPB) in the patients undergo coronary artery bypass grafting(CABG).
This study was a nonequivalent control group pretest- posttest design with
nonpulsatile perfusion group(NP) and pulsatile perfusion group(PP).
The subjects of this study were 20 patients who were received CABG at Y
University Cardiovascular Center in Seoul from March 30, 2000 to May 30, 2000 and
were selected using selection criteria. The subjects were allocated to nonpulsatile or
pulsatile group by a matching method.
Hemodynamic data were measured at pre induction, post induction, immediately after
aorta cross clamp on(ACC on), 60minutes after the start of CPB(CPB 60'), after the
closure of sternum, on arrival at ICU, and post operative day 1(POD 1). Hemodynamic
variables were composed of mean arterial pressure, peripheral vascular resistance, plasma
catecholamine(epinephrine, norepinephrine), dosage of vasodilator(Nitroprusside), and
cardiac index. Kidney function variables were urine output, serum BUN and creatinine
and were measured only at pre CPB, during CPB, post CPB, and POD 1.
The collected data were analyzed using the SPSS-PC.
The results are summarized as follows.
1. In both groups, mean arterial pressures were decreased significantly at ACC on
but progressively were increased at CPB 60', after the clousure of sternum and on
arrival at ICU. On POD 1, mean arterial pressures were decreased in both groups.
Comparing with two groups, mean arterial pressure was significantly higher in NP than
PP, 72.8ยฑ13mmHg versus 61.3ยฑ6.6mmHg, p <.05 at CPB 60'. There was no statistical
difference in both groups at the other stages.
2. Peripheral vascular resistances were decreased significantly at ACC on but
increased at CPB 60', after the clousure of sternum and on arrival at ICU in two
groups. Peripheral vascular resistances in two groups were decreased to the normal
range on POD 1. No significant difference was detected in peripheral vascular resistance
between two groups.
3. In both groups, plasma epinephrines were increased significantly at ACC on. The
maximum plasma epinephrine levels were detected at CPB 60'. After the clousure of
sternum, plasma epinephrines were decreased in both groups. No significant difference
was found plasma epinephrine between two groups.
4. Plasma norepinephrines were increased significantly at ACC on and CPB 60'. After
the clousure of sternum, plasma epinephrine was decreased in NP but increased in PP.
Plasma norepinephrine was significantly higher in NP than in PP at ACC on and CPB
60' and it was statistically significant.
5. The dosage of vasodilator(Nitroprusside) was increased significantly higher in NP
than in PP (p < .05).
6. In both groups, cardiac indices were decreased after induction but increased
significantly on POD 1. No significant difference was found in cardiac index between
two groups.
7. In both groups, urine outputs were increased significantly during CPB but
decreased at post CPB. On POD 1, urine outputs were decreased significantly in both
groups. Urine output was higher in NP than in PP during CPB but was higher in PP
than in NP at post CPB. But no significant difference was detected in urine output
between two groups.
8. In both groups, serum BUNs were decreased significantly at post CPB. On POD
1, serum BUN was increased in NP but was decreased in PP. Serum BUN was
higher in NP than in PP at pre CPB but was higher in PP than in NP at post CPB
and POD 1. But no significant difference was detected in serum BUN between two
groups.
9. At post CPB, serum creatinine was increased in NP but was decreased
significantly in PP. On POD 1, serum creatinine was decreased at the level of pre CPB
in NP but was maintained at level of post CPB in PP. There was no significant
difference in both groups.
In conclusion, it was suggested that pulsatile perfusion, compared with nonpulsatile
perfusion, can attenuate hemodynamic changes by decreasing release of plasma
norepinephrine, peripheral vascular resistance, mean arterial pressure and dosage of
vasodilator during cardiopulmonary bypass. Also pulsatile can prevent renal dysfunction
by increasing urine output and decreasing serum BUN and serum creatinine after
cardiopulmonary bypass.ope
