Relationship between grade/stage and liver iron deposition in chronic B viral hepatitis

의학과/석사[한글] B형 간염 바이러스는 감염 후 다양한 임상경과를 나타내며, 만성 B형 바이러스성 간염으로 진행된 환자에서 만성으로 진행하지 않은 환자에 비하여 혈중 철 농도가 높다. 또한 철의 과부하를 유발시키는 원인이 없는 경우에도 만성 B형 바이러스성 간염에서 종종 철 침착이 관찰되며, 이와 같은 철 침착은 간내 모든 부위에서 가능하지만 대부분 문맥 내피세포에 생긴다. 만성 바이러스성 간염 환자에서 인터페론 치료에 대한 반응과 혈중 철 농도 및 문맥역내 철 침착 정도는 밀접한 역상관관계가 있어, 혈중 철 농도 및 문맥역내 철 침착을 관찰함으로써 인터페론 치료 반응에 대한 예측이 가능하다. 우리나라에서 만성 B형 바이러스성 간염의 유병률이 높고 인터페론 치료가 흔히 사용되지만 현재까지 국내에서는 만성 B형 바이러스성 간염 환자에서 간내 철 농도에 대한 연구가 없었다. 이에 본 연구는 만성 B형 바이러스성 간염 환자의 간문맥역내 철 침착 여부가 인터페론 투여 후 치료 효과를 예측하는데 도움이 되는지 알기 위한 기초적 연구로써 만성 B형 간염 환자에서 간의 문맥역내 철 침착을 관찰하였다. 그리고, 만성 간염의 등급 및 진행정도에 대하여 관찰함으로써 문맥역내 철 침착과 간염의 등급 및 진행정도 상호간에 어떠한 관계가 있는지를 알아보고자 하였다. 본 연구는 복강경 직시하 간조직 검사를 시행한 만성 B형 바이러스성 간염 환자 37예와 비바이러스성 만성 간질환 20예의 간 조직생검으로 만성 간염의 등급 및 진행정도 그리고 간문맥역내 내피세포와 조직구에 침착된 헤모시데린을 관찰하였고 간기능, 혈중 철농도 등을 검사하였다. 혈중 철 농도를 검사한 만성 B형 바이러스성 간염 15예와 비바이러스성 만성 간질환 6예를 대상으로 비교한 총 철 결합능은 통계적으로 의미있게 만성 B형 바이러스성 간염 환자에서 감소되어 있었으나 (p-value < 0.05), 혈장 철, transferrin 포화도, 혈장 ferritin은 양 군간에 유의한 차이가 없었다. 만성 B형 바이러스성 간염 37예 중 13예(35%)에서 비바이러스성 만성 간 질환자 20예 중 1예(5%)에서 문맥역내 헤모시데린 침착이 관찰되어 만성 B형 바이러스성 간염 환자에서 통계적으로 유의하게 헤모시데린 침착이 증가되어 있었다(p-value < 0.05). 만성 B형 바이러스성 간염 환자 37예 중에서 문맥역내 헤모시데린이 침착되어 있는 13예에서 침착이 없었던 24예에 비하여 통계적으로 유의하게 간염의 진행정도(p-value < 0.01) 및 문맥/문맥주변부 활성도(p-value < 0.05)와 유의한 상관관계를 보인 반면, 혜모시데린 침착정도와 간소엽 활성도와는 상관관계가 없었다. 혈중 철 농도를 측정한 만성 B형 바이러스성 간염 15예에서 문맥역 헤모시데린 침착 여부에 따른 혈중 철농도의 차이는 없었다. 이상의 결과로 만성 B형 바이러스성 간염에서 혈중 철농도는 대조군인 비바이러스성 만성 간질환과 차이를 보이지 않은 반면, 간문맥내 혜모시데린 침착은 대조군에 비하여 유의하게 증가되어 있어, 우리나라에서도 만성 B형 바이러스성 간염에서 간내 철 농도를 인터페론 치료에 대한 예후 인자로 이용할 수 있을 것으로 생각되며, 문맥역내 철 침착이 관찰될 때, 만성 간염의 문맥/문맥주변부 활성도와 진행정도가 심하였다. 만성 B형 바이러스성 간염 환자에서 문맥역에 철 침착이 관찰될 때, 간경변으로의 진행할 가능성이 높다는 것을 알 수 있으며, 따라서 이러한 경우에 간경변으로의 이행가능성에 유의하여 적극적인 추적 관찰이 필요할 것이다. 향후 헤모시데린 침착에 의한 문맥 내피세포의 면역학적 생리적 기능에 대한 좀 더 많은 연구를 통하여 간내 철 침착에 따른 만성 B형 바이러스성 간염의 병태 생리를 확인할 수 있을 것으로 생각된다. [영문] Increased serum and hepatic iron may be observed in chronic viral hepatitis in the absence of recognized iron overload state. In chronic B viral hepatitis, hemosiderin deposition is mainly noted in the endothelial cells of portal tracts. Elevated iron stores in liver are predictive of a poor response to interferon. In Korea, although interferon therapy is commonly used, there are no reports on the distribution of hemosiderin in the liver with chronic B viral hepatitis. The serum iron status and deposition of hemosiderin in portal tracts were evaluated in patients with chronic B viral hepatitis as a basic study for interferon therapy. The relation between the serum iron status and hemosiderin deposition in the portal tracts with other histological features of chronic hepatitis was also evaluated. Liver biopsy specimens were obtained through the peritoneoscopy from 37 patients with chronic B viral hepatitis and 20 patients with non viral chronic hepatitis as control group. More than 10 portal tracts were available for histologic examination from 37 patients with chronic B viral hepatitis and non viral chronic hepatitis as control group. Total iron binding capacity (TIBC) was decreased in patients with chronic B viral hepatitis as compared with patients in control group(p-value < 0.05). Whereas, there were no difference of serum iron, ferritin and transferrin saturation between chronic hepatitis and non viral hepatitis. 13 of 37 cases (35%) of chronic hepatitis B and only 1 of 20 cases(5%) of non viral hepatitis showed hemosiderin deposition in the endothelial cells of portal tracts. The difference was statistically significant between chronic hepatitis and non viral hepatitis. In chronic B viral hepatitis, grade and porto-periportal activity were positively correlated with hemosiderin deposition (p-value < 0.05), but serum iron concentration was not correlated with hemosiderin deposition in portal tract. These findings suggest that iron deposition in portal tracts is useful in predicting the likelihood that chronic hepatitis B will also respond to interferon therapy in Korea as like other country. This hemosiderin deposition was positively correlated with the degree of porto-periportal activity and fibrosis in chronic B viral hepatitis. When there is iron deposition in endothelial cells of the portal tracts, close follow-up will be needed with awareness of possible progression to liver cirrhosis.restrictio
