Factors affecting the healthcare utilization of hospital types among hypertensive outpatient service users: Using the Korea Medical Panel data from 2010 through 2016

Background: The prevalence of chronic disease such as hypertension, diabetes and hyperlipidemia has been gradually increasing, and among them the hypertension is one of the main factors for ischemic heart disease and cardiovascular disease (CVD), which rank the first and the second out of top ten death causes in the world. In Korea, awareness, treatment and control of hypertension have been steadily improved, but it is required to pay attention and improve them. In the case of clinic type medical institutions, patients could use the institutions so conveniently that it is possible for them to access continuous medical service and it has the advantage of treatment improvement for chronic hypertension as cost benefit. However, since the health care system in Korea allows patients to choose a medical institution freely and health care delivery system is not established as well, it is a severe social problem that many patients want to go to large hospitals. So, by identifying characteristics of hypertension outpatients in a hospital type medical institutions, this study provided basic data for practical health policy plans through quality improvement in primary care so that patients could select and utilize the clinic type medical institutions. Methods: This study quoted data from Korea Medical Panel database from 2010 to 2016. In order to verify relative factors of outpatient in hospital types, it analyzed all of 3,877 patients who are over 30 years old and have used medical institutions more than one time, and the number of analysis is 15,040 according to subject repeat measurement. Considering correlation according to the subject repeat measurement of each year, this study conducted generalized linear mixed model analysis. Also, this study additionally conducted subcategory analysis by dividing gender. In every analysis, it utilized SAS version 9.4 (SAS Institute, Cary, NC, United States), and it set significance level as 5%. Results: As a result of operating multivariable analysis by controlling demographic factors, health behavior factors, and health status factors, odds ratio of outpatient utilization is lower in hospital type medical institutions compared to clinic type medical institutions when it is women than men in the case of gender (OR=0.44, 95% CI=0.32-0.61), it was 65 or more than under 65 (OR=0.71, 95% CI=0.57-0.89). Furthermore, in some factors odds ratio of outpatient utilization is higher in hospital type medical institutions compared to clinic type medical institutions when they live in Busan, Ulsan, and Gyeongnam province than Seoul, Gyeonggi, Incheon, and Jeju (OR=1.40, 95% CI=1.05-1.86), they are the highest annual income of households than the lowest annual income of households (OR=1.73, 95% CI=1.30-2.29), they are medicaid aid recipients than health insurance subscribers (OR=1.94, 95% CI=1.34-2.83), they have three or more complex chronic diseases than they do not have it (OR=1.59, 95% CI=1.19-2.11), they accompany diabetes (OR=1.81, 95% CI=1.41-2.32), and they accompany ischemic heart disease or cerebrovascular disease (OR=6.80, 95% CI=5.28-8.76). As the results of analysis which analyzed data by considering gender, odds ratio of outpatient utilization is lower in hospital type medical institutions when in the case of men they have a spouse than they do not have it (OR=0.58, 95% CI=0.35-0.97), and in the case of women they live in a household with three generations than a household with one generation (OR=0.73, 95% CI=0.53-0.99). Conclusions: When reflecting research results, if hypertension patients are men using outpatient in hospital type medical institutions depending on preference, are under 65 years old, live in Busan, Ulsan, and Gyeongnam province, and are in a high annual income level, it should be a prior issue to improve recognition on medical system, such as it is necessary to prepare hypertension management plans enhancing individual requirement and convenience and to establish reliability that they could get prompt care when they need it in the more advanced medical institution. In addition, if hypertension patients are medicaid aid recipients using outpatient in hospital type medical institutions because they are not able to manage their diseases as a vulnerable class, men without a spouse, and women who live in a household with one generation, it is necessary to pay attention and prepare policies through primary medical reinforcement to make them utilize medical service continuously and improve their blood pressure management rate. Depending on those results, health policy plans should be required in order to efficiently manage hypertension patients by considering characteristics of subjects. 배경 : 인구의 고령화와 더불어 고혈압, 당뇨병 및 고지혈증 등의 만성질환 유병률은 지속적으로 증가하는 추세이고 그 중 고혈압은 세계 10대 사망원인의 1위, 2위를 차지하는 허혈성 심질환과 뇌혈관질환의 주요 위험요인이다. 우리나라에서 고혈압의 인지율과 치료율, 조절률은 꾸준히 향상되고 있으나 여전히 지속적인 관심과 개선이 필요하다. 의원급 의료기관의 경우 환자가 편리하게 이용할 수 있어 지속적인 의료서비스 이용이 가능하고, 비용 효과적으로 만성질환인 고혈압 환자의 치료개선에 장점이 있다. 그러나 우리나라의 보건의료체계는 환자의 선택에 따라 의료기관을 자유롭게 이용할 수 있으며 의료전달체계가 제대로 정립되어 있지 않아 대형병원 쏠림현상이 심각한 사회문제로 부각되고 있다. 이에 본 연구는 병원급 의료기관 외래를 이용하는 고혈압 환자의 특성을 파악하여 의원급 의료기관을 선택하고 이용할 수 있도록 일차의료의 질 향상을 통한 실효성 있는 보건정책방안을 마련하는데 기초적인 자료가 되기를 기대한다. 설계 : 2010년부터 2016년까지의 한국의료패널 연간데이터를 이용하였다. 고혈압 환자의 병원급 의료기관 외래이용 관련 요인을 파악하기 위해 1회 이상 의료기관 이용 유형이 조사된 30세 이상 고혈압 환자 3,877명을 모두 분석에 포함하였으며, 연구대상자 반복측정에 따른 분석건수는 15,040건 이었다. 연구대상자의 연도별 반복측정에 따른 상관성을 고려하여 일반화 선형 혼합모형 분석을 실시하였다. 또한, 성별을 구분한 하위집단 분석을 추가로 실시하였다. 모든 분석은 SAS version 9.4(SAS Institute, Cary, NC, United States)를 사용하였으며 유의수준은 5%로 설정하였다. 결과 : 인구사회적 요인, 건강행태 요인, 건강상태 요인을 모두 통제하여 다변수 분석을 실시한 결과, 성별 및 연령에서는 남성에 비해 여성 (OR=0.44, 95% CI=0.32-0.61), 65세 미만에 비해 65세 이상 (OR=0.71, 95% CI=0.57-0.89)에서 의원급 대비 병원급 의료기관 외래이용 오즈비가 낮게 나타났다. 그밖에 고혈압 환자에서 의원급에 비해 병원급 의료기관의 외래이용 오즈비가 높게 나타난 요인은 서울·경기·인천·제주지역보다 부산·울산·경남지역 거주 (OR=1.40, 95% CI=1.05-1.86), 가장 낮은 소득가구에 비해 가장 높은 소득가구 (OR=1.73, 95% CI=1.30-2.29), 건강보험 가입자에 비해 의료급여 수급자 (OR=1.94, 95% CI=1.34-2.83), 복합만성질환을 보유하지 않을 때보다 3개 이상 보유 (OR=1.59, 95% CI=1.19-2.11), 당뇨병 동반 (OR=1.81, 95% CI=1.41-2.32), 허혈성 심장질환 또는 뇌혈관질환 동반 (OR=6.80, 95% CI=5.28-8.76)이었다. 성별을 구분하여 분석한 결과에서는 남성의 경우 무배우자보다 유배우자 (OR=0.58, 95% CI=0.35-0.97), 여성의 경우 1세대가구보다 3세대가구에 속해 있을 때 (OR=0.73, 95% CI=0.53-0.99) 병원급 의료기관의 외래이용 오즈비가 낮게 나타났다. 결론 : 본 연구결과를 비추어볼 때 선호에 따라 병원급 의료기관 외래를 이용하는 것으로 판단되는 남성, 65세 미만, 부산·울산·경남지역 거주, 고소득 고혈압 환자에게는 개인의 요구도와 편의성을 제고한 고혈압 관리방안을 마련할 필요가 있으며, 상급의료기관에서 진료가 필요한 경우 신속하게 진료를 받을 수 있다는 신뢰를 구축하는 등 의료시스템에 대한 인식개선이 우선되어야 할 것이다. 또한. 취약계층으로 질병의 관리가 잘 안되어 병원급 의료기관 외래를 이용하는 것으로 판단되는 의료급여 수급자, 무배우자 남성, 1세대가구의 여성 고혈압 환자 등에 대해서는 지속적으로 의료서비스를 이용하고 혈압관리율을 향상시킬 수 있도록 일차의료강화를 통한 관심과 정책이 필요하다. 이러한 연구결과를 바탕으로 대상자 특성을 반영하여 효과적인 고혈압 환자 관리를 위한 보건의료정책 방안을 마련할 필요가 있다.open석

A study on factors to influence medical expenses of hospitalized hospice by using multi-level analysis

보건통계전공연구배경 말기암환자의 무의미한 치료를 줄이고 존엄한 임종 지원과 건강보험 재정의 합리적 지출을 위해 입원형 호스피스 제도가 시행되었다. 호스피스 제도의 지속적 확대를 위해 본사업 시행 전·후 진료비 변화를 알아보고, 다수준분석을 통하여 개인 수준뿐만 아니라 의료기관 수준까지 고려하여 입원형 호스피스 진료비에 영향을 미치는 요인의 집단내 상관성을 분석하고자 하였다. 대상 및 방법 본 연구는 2014년 1월 1일부터 2016년 12월 31일까지 총 3년간 건강보험심사평가원에 등록된 입원형 호스피스 전문기관의 요양급여비용 청구명세서를 이용하였다. 입원 에피소드 단위로 합산하여 마지막 입원 진료에 대해 총 24,595명을 대상으로 본사업 시행 전·후로 나눠서 입원일당 진료비, 입원건당 진료비, 재원일수, 마약성진통제 투여비용의 차이를 Two Sample t-test로 분석하였다. 입원형 호스피스 진료비에 영향을 미치는 요인을 확인하기 위해 연령, 성별, 암 종류, 수혈, 완화의료 관련 시술 등을 포함한 개인요인과 지역특성, 의료시설·인력현황을 반영한 의료기관요인을 동시에 고려할 수 있는 “다수준 로지스틱 분석 모형”을 이용하였다. 연구결과 완화의료 본사업 전·후의 차이를 분석한 결과 입원일당 진료비는 시범사업 162,380원에서 본사업 295,877원으로 82.21% 증가, 입원건당 진료비는 시범사업 2,932,631원에서 본사업 6,436,328원으로 119.47% 증가, 재원일수는 시범사업 17.5일에서 본사업 시행 이후 20.9일로 19.30% 증가, 마약성진통제 투여비용은 시범사업 52,317원에서 본사업 253,842원으로 385.20% 통계적으로 유의하게 증가하였다(p<.0001). 다수준분석 결과 입원일당 진료비와 입원건당 진료비, 재원일수, 마약성진통제 투여비용, 입원 후 30일 이내 사망에 대해 개인수준 변수만을 포함한 모델에서의 분산보다 의료기관수준 변수까지 포함한 모델에서 설명력이 증가하였고 통계적으로 유의하였다(p<0.05). 개인수준 변수 중 완화의료 관련 시술 관련된 모든 항목과 보험형태가 의료급여인 경우 교차비가 매우 높게 나타났고, 의료기관수준 변수에서 완화의료 전담간호사수 및 사회복지사등급의 병원간 차이가 통계적으로 유의하게 나타났다(p<0.05). 결 론 다수준분석을 통해 성별, 암 종류, 보험형태, 완화의료 관련 시술, 완화의료 전담의사수, 간호사등급, 사회복지사등급 등이 완화의료 진료비에 영향을 미치는 요인임을 확인하였다. 본 연구는 성별, 연령 등 개인요인의 관련성과 의료기관요인의 특성을 반영함으로 입원형 호스피스 진료비에 영향을 미치는 요인에 대해 의료기관별로 통제한 최초 연구라는 것에 의의가 있다. 정액수가 도입에 따른 완화의료 서비스 질을 공정하고 효율적으로 평가하고 관리할 수 있는 의료 질 관리체계 구축이 필요하다. 또한, 완화의료 적정성 평가 및 가감지급 적용 방안 검토를 위해 질 관리에 대한 연구가 꾸준히 이루어져야 할 것이다open석

An Overview of Acceptance of the Awakening of Faith in East Asian Buddhism

은 동아시아 불교사상전통에 있어서 핵심적 논서이다. 다양한 교리적 입장들을 포용하는 의 이론적 요소와 이 논서의 여래장如來藏 개념은 동아시아 불교사상전통의 정립에 지대한 영향을 미쳤다. 근현대 불교학계에 있어서도 의 사상은 논의의 중심주제 중 하나이다. 현대 한국불교학계에 있어서는 에 대한 연구가 원효元曉(617∼686)의 와 를 중심으로 꾸준히 진행되어 왔고, 그 결과 원효의 주석에 대한 연구의 괄목할 만한 성장과 성과가 이루어졌다. 하지만 한국불교학계에서 원효의 와 를 중심으로 에 대한 체계적 이해가 가능하게 되었다 하더라도, 원효의 주석을 매개로 이해가 진행됨에 따라 원효 혹은 한국의 틀을 넘어선 사상의 의미는 상대적으로 소홀히 다루어졌다고 보여진다. 은 6세기 중반에 등장한 이후 긴 사상적 궤적을 가진 논서임을 고려할 때 의 연구가 더 넓은 범위에서 수행되어야 할 것임은 분명하다. 게다가, 원효의 사상은 동아시아 전체의 맥락에서 조망될 때 그 사상적 의의가 더 명확해질 수 있음 또한 명백하다. 이 논문에서는, 의 연구범위의 확장에 대한 이러한 필요성을 바탕으로 하여, 중국, 한국, 일본에서 현대에 이르기까지 전개된 에 대한 논의를 동아시아 전체의 맥락에서 개괄, 정리하려 한다. 이러한 사상 연구사의 포괄적인 조망을 통해 전체 동아시아 불교전통에서의 논의의 다양성을 고찰하고, 이러한 맥락 안에서 한국불교의 연구의 현재 위치를 조명해 보고자 한다.The Awakening of Faith is a seminal treatise in the East Asian Buddhist tradition. Its doctrinal aspects, which encompass the different perspectives of various Buddhist schools, have had a major influence on the process of the establishment of East Asian Buddhism. In modern Korean Buddhist scholarship, research on the Awakening of Faith has been conducted focusing on Wŏnhyos 元曉(617 ～686) commentaries, such as the Kisillon so 起信論疏 and the Taesŭng kisillon pyŏlgi 大乘起信論別記. As a result, Korean scholarship has reached a certain level of understanding of the Awakening of Faith based on the constant study on Wŏnhyos commentaries. However, the broader doctrinal significance of the Awakening of Faith does not appear to have been properly considered. Given that the Awakening of Faith has a long historical trajectory in East Asia, the study on the treatise should be conducted in the broader context of East Asian Buddhism. Furthermore, Wŏnhyos thoughts on the Awakening of Faith may be better understood from the broad perspective. Considering the need to extend the scope of research on the Awakening of Faith, this article presents an overview of the process of accepting the Awakening of Faith in China, Korea, and Japan up until modern times in the broader context of East Asian Buddhism. By doing this, I disclose that there have been various doctrinal perspectives revolving around the Awakening of Faith in East Asia, and, in this light, suggest that Wŏnhyo-centered Korean Buddhist scholarship on the Awakening of Faith should be reconsidered

The Meaning of Buddha Nature as Middle Way

우리는 많은 불교 전적에서 모든 중생에게 佛性이 있다는 구절을 흔히 접한다. 다시 말하여 많은 경전에서는 모든 중생에게 佛性이 있지만 중생은 자신의 미혹으로 인하여 佛性을 깨닫지 못한다고 하고 있다. 그렇지만 여기서 이 佛性이란 것은 무엇을 말하는가? 애초에 석가모니는 諸法無我를 설하여 어떠한 自我도 없음을 주장하였다. 그런데 후의 많은 대승경전, 특히 如來藏·佛性계통의 경전에서는 다시 佛性을 내세우고 있는 것이다. 이것은 새로운 自我가 있음을 말하는 것이 아닌가? 만약 그렇다면 佛性또는 如來藏사상은 석가모니의 無我說과는 완전히 상반되는 이론은 아닌가? 비록 여래장계 경론에서는 佛性이나 여래장 개념이 空과 다르지 않는 것이라고 반복 서술하고 있지만 無我또는 空이라는 부정적인 테제에서 佛性이라는 긍정적인 테제로의 전환은 그 내용은 차치하고 형식만으로서라도 혁신적인 전환이다. 더구나 경론 상에서 佛性을 空이라고 언명한다 할지라도 空을 佛性이라는 긍정적 개념의 틀에서 다시 조명해야만 했던 필요성과 함께 空사상과 배치되는 佛性외의 여러 가지 존재론적인 표현들을 다시 고려하지 않을 수 없다
