    Factors affecting the healthcare utilization of hospital types among hypertensive outpatient service users: Using the Korea Medical Panel data from 2010 through 2016

    Background: The prevalence of chronic disease such as hypertension, diabetes and hyperlipidemia has been gradually increasing, and among them the hypertension is one of the main factors for ischemic heart disease and cardiovascular disease (CVD), which rank the first and the second out of top ten death causes in the world. In Korea, awareness, treatment and control of hypertension have been steadily improved, but it is required to pay attention and improve them. In the case of clinic type medical institutions, patients could use the institutions so conveniently that it is possible for them to access continuous medical service and it has the advantage of treatment improvement for chronic hypertension as cost benefit. However, since the health care system in Korea allows patients to choose a medical institution freely and health care delivery system is not established as well, it is a severe social problem that many patients want to go to large hospitals. So, by identifying characteristics of hypertension outpatients in a hospital type medical institutions, this study provided basic data for practical health policy plans through quality improvement in primary care so that patients could select and utilize the clinic type medical institutions. Methods: This study quoted data from Korea Medical Panel database from 2010 to 2016. In order to verify relative factors of outpatient in hospital types, it analyzed all of 3,877 patients who are over 30 years old and have used medical institutions more than one time, and the number of analysis is 15,040 according to subject repeat measurement. Considering correlation according to the subject repeat measurement of each year, this study conducted generalized linear mixed model analysis. Also, this study additionally conducted subcategory analysis by dividing gender. In every analysis, it utilized SAS version 9.4 (SAS Institute, Cary, NC, United States), and it set significance level as 5%. Results: As a result of operating multivariable analysis by controlling demographic factors, health behavior factors, and health status factors, odds ratio of outpatient utilization is lower in hospital type medical institutions compared to clinic type medical institutions when it is women than men in the case of gender (OR=0.44, 95% CI=0.32-0.61), it was 65 or more than under 65 (OR=0.71, 95% CI=0.57-0.89). Furthermore, in some factors odds ratio of outpatient utilization is higher in hospital type medical institutions compared to clinic type medical institutions when they live in Busan, Ulsan, and Gyeongnam province than Seoul, Gyeonggi, Incheon, and Jeju (OR=1.40, 95% CI=1.05-1.86), they are the highest annual income of households than the lowest annual income of households (OR=1.73, 95% CI=1.30-2.29), they are medicaid aid recipients than health insurance subscribers (OR=1.94, 95% CI=1.34-2.83), they have three or more complex chronic diseases than they do not have it (OR=1.59, 95% CI=1.19-2.11), they accompany diabetes (OR=1.81, 95% CI=1.41-2.32), and they accompany ischemic heart disease or cerebrovascular disease (OR=6.80, 95% CI=5.28-8.76). As the results of analysis which analyzed data by considering gender, odds ratio of outpatient utilization is lower in hospital type medical institutions when in the case of men they have a spouse than they do not have it (OR=0.58, 95% CI=0.35-0.97), and in the case of women they live in a household with three generations than a household with one generation (OR=0.73, 95% CI=0.53-0.99). Conclusions: When reflecting research results, if hypertension patients are men using outpatient in hospital type medical institutions depending on preference, are under 65 years old, live in Busan, Ulsan, and Gyeongnam province, and are in a high annual income level, it should be a prior issue to improve recognition on medical system, such as it is necessary to prepare hypertension management plans enhancing individual requirement and convenience and to establish reliability that they could get prompt care when they need it in the more advanced medical institution. In addition, if hypertension patients are medicaid aid recipients using outpatient in hospital type medical institutions because they are not able to manage their diseases as a vulnerable class, men without a spouse, and women who live in a household with one generation, it is necessary to pay attention and prepare policies through primary medical reinforcement to make them utilize medical service continuously and improve their blood pressure management rate. Its doctrinal aspects, which encompass the different perspectives of various Buddhist schools, have had a major influence on the process of the establishment of East Asian Buddhism. In modern Korean Buddhist scholarship, research on the Awakening of Faith has been conducted focusing on Wลnhyos ๅ…ƒๆ›‰(617 ๏ฝž686) commentaries, such as the Kisillon so ่ตทไฟก่ซ–็– and the Taesลญng kisillon pyลlgi ๅคงไน˜่ตทไฟก่ซ–ๅˆฅ่จ˜. As a result, Korean scholarship has reached a certain level of understanding of the Awakening of Faith based on the constant study on Wลnhyos commentaries. However, the broader doctrinal significance of the Awakening of Faith does not appear to have been properly considered. Given that the Awakening of Faith has a long historical trajectory in East Asia, the study on the treatise should be conducted in the broader context of East Asian Buddhism. Furthermore, Wลnhyos thoughts on the Awakening of Faith may be better understood from the broad perspective. Considering the need to extend the scope of research on the Awakening of Faith, this article presents an overview of the process of accepting the Awakening of Faith in China, Korea, and Japan up until modern times in the broader context of East Asian Buddhism. By doing this, I disclose that there have been various doctrinal perspectives revolving around the Awakening of Faith in East Asia, and, in this light, suggest that Wลnhyo-centered Korean Buddhist scholarship on the Awakening of Faith should be reconsidered
