Effect of Enflurane Anesthesia on the Patients who Treated with Isoniazid

의학과/석사[영문] [한글] Enflurane이 간의 microsomal enzyme system인 cytochrome P-450 효소에 의해 대사되며 , 이 대사과정에서 효소 유도(enzyme induction)를 유발하여 enflurane 마취시 isoniazid , ethanol, chlorpromazine 등의 약물 투여나 당뇨병시 enflurane 대사가 증가된다고 한 다. Isoniazid를 동물과 인간에게 일정기간 투여 후 enflurane 마취시 enflurane의 대사가 증가하며, 혈중 불소 이온이 50μM 초과시는 신기능 저하를 초래하기도 하고, INH를 장기 간 투여시는 이 약물로 인한 hepatitis가 발생하기도 한다. 본 연구는 결핵 유병율이 높은 우리나라에서 isoniazid를 하루 300mg 이상식 5일 이상 치료받은 결핵환자 10명과 비복용환자 11명을 대상으로 하여서, enflurane 마취시 enflur ane의 대사 증가 정도를 혈장과 소변의 불소 이온치료 수술 전, 마취 후 2시간, 수술 직 후, 수술 후 24시간에 측정하고, 불소이온으로 인한 신기능 변화 정도를 BUN과 creatinin e으로, INH로 인한 간기능 변화 정도는 혈청내 SGOT와 SGPT로 측정 관찰하여 다음과 같은 결과를 얻었다. 1. INH 투여군과 대조군에서 enflurane 마취시 시간에 따라 혈장내 불소 이온이 증가하 였고, INH high F**- 군에서 특히 현저한 증가를 보였다. 2. 소변내 불소 이온은 INH high F**- 군에서 마취 후 유의 있게 증가하였다. 3. INH 복용환자에서 신기능과 간기능 검사상 유의한 변화가 없었다. 이상의 실험결과로 INH 투여 환자에게 enflurane 마취가 가능하다고 사료된다. Enhanced biotransformation might occur after enflurane itself and several drugs, INH, ethanol and chlorpromazine. Increased inorganic fluoride, one of the enflurane metabolites, could impair renal function. The possibility of increased enflurane defluorination following treatment with isoniazid group(n=10) and control group (n=11) was investigated with the measuring serum and urine F**- on the properative period, 2 hr after anesthesia, immediate postoperative period and 24th postoperative hour. According to serum inorganic fluoride concentration, isoniazid group was divide again into INH high F**- and INH low F**- groups. On the preoperative, immediate postoperative periods and 24th postoperalive hous, the changes of renal function were measured with the BUN and creatinine, liver function with the SGOT and SGPT. The results are as follows: 1. Serum imorganic fluoride increased in enflurane anesthesia significantly in all three groups and decreased 24th postoperative hour. And enhanced defluorination was the highest in INH high F**- group among three groups. 2. Urine inorganic fluoride was increased only in INH high F**- group. 3. There was no changes in renal and hepatic function after enflurane anesthesia. Our study indicated that enflurane does not do any harmful effect on the INH treated patient.restrictio

산림인증제는 환경적，사회적，경제적으로 지속해 갈 수 있는 산림경영을 위해 제3의 기관에서 적절한 기준으로 심사·인증하고 그렇게 인증된 산림에서 생산된 목재를 시장에서 차별 화함으로써 바랍직한 산림경영·관리를 유도하는 제도를 말한다. 이 산림인증제는 산립을 지속가능하게 관리할 수 있는 정책수단의 하나로 인식되어 1990년대부터 환경관련 단체들에 의해 주도적으로 연구 및 홍보가 이루어져 왔으며 현재 전 세계적으로 활발한 논의가 이루어지고 있는 실정이다. 본 연구는 원목올 구매하여 제품을 생산하는 목재가공업체와 인증 목제품을 구매하는 소비자들에 대해 산림인증제에 대한 인지여부，인증목재에 대한 목재가공업자들의 구매의사， 인증 목제품에 대한 소비자들의 추가지불의사，산림인증제의 국내 활성화 여부，시기，조건 등을 조사함과 동시에 그 결과들과 연령，근무부서，근무기한，원목 또는 제품 구매량과의 관계를 밝히는데 목적이 있다. 또한 목재가공업체(제재업체，합판 • 보드업체，가구업체)와 소 비자 간의 산림인증제에 대한 인식의 차이를 알아보고자 한다. 이렇게 얻어진 조사결과를 통해 산림인증제의 국내 도입 가능성을 검토하는데 궁극적인 목척 이 있다
