An Estimation of Optimal KTX Fares: The Case of Seoul~Busan Travel

학위논문 (석사)-- 서울대학교 환경대학원 : 환경계획학과, 2013. 2. 김성수.2004년 4월 1일 경부고속철도의 1단계 구간(서울~대구) 개통에 이어 2010년 11월 1일 2단계 구간(동대구~부산)의 개통으로 서울에서 부산까지 2시간 18분 만에 이동할 수 있게 되었다. 2011년 12월 기준 월 330만 명이 고속철도를 이용하고 있으나 이달 고속철도의 운임이 인상되었고 다음 해 수서발 KTX의 민간 운영 시 현재 운임보다 인하가 가능하다는 논란이 야기되면서 고속철도의 적정 운임수준에 대해 연구가 필요한 실정이다. 현재 철도운임은 물가상승률과 수송 원가를 바탕으로 운임을 산정하고 있으며 공공성이 배제되어 있다는 한계가 있다. 대중교통수단의 운임은 운임수준에 따른 수요와 공급을 고려해야 하며 통행자에게 편익을 제공해야 한다. 또한 일정 수준 이상의 운영기관 이익을 보장하는 수준에서 설정되어야 하며 운행 시 발생하는 대기오염 또는 소음 등의 외부비용이 반영되어야 한다. 따라서 본 연구에서는 철도운영기관의 관점과 사회적 관점에서 고속철도의 적정 운임수준을 추정하는 방법론을 제시하고 서울~부산 통행에 적용하는 것을 목적으로 하였다. 본 연구의 시간적 범위는 2005년 1월부터 2011년 12월까지이며 월별로 총 72개의 관찰점을 가진 집계 자료를 사용하였다. 적정 운임수준 추정의 기준연도는 자료의 가장 최근 시점인 2011년 12월로 하였다. 공간적 범위는 고속철도의 통행시간 절감이 큰 장거리 노선인 서울~부산 통행이며 고속버스, 고속철도, 일반철도, 항공 등 4가지 지역 간 대중교통수단을 대상으로 하였다. 고속철도의 적정 운임수준을 추정하는 방법은 다음과 같다. 먼저 고속철도의 운임변화에 따른 수단분담률의 변화와 운행횟수 변화를 통해 추정하기 위해 서울~부산 통행을 대상으로 수요모형을 구축하였다. 이때 운임변화로 인한 수요변화는 공급변수인 운행횟수를 변화시키며 운행횟수 변화는 다시 수요량을 변화시키므로 이러한 과정을 반복 수행하여 수단분담률과 운행횟수가 더 이상 변하지 않는 균형점을 도출하였다. 도출된 값을 바탕으로 철도운영기관의 관점과 사회적 관점에서 적정 운임수준을 추정하였다. 고속철도의 운임을 5%씩 변화시키며 철도운영기관의 이익변화와 사회적 후생변화를 살펴보고 각 변화량의 최대가 되는 고속철도의 적정 운임을 산출하였다. 여기서 철도운영기관의 이익변화는 고속철도와 일반철도의 이익변화로 구성된다. 한편, 사회적 후생변화는 통행자 편익변화, 운송업체의 운임수입변화와 사회적 비용변화, 외부비용(대기오염비용)변화, 세수(부가가치세)변화를 통해 추정되었다. 서울~부산 수단분담모형 추정결과, 모형 전체의 설명력을 나타내는 우도비는 0.304로 추정되어 설명력이 높은 것으로 나타났다. 통행시간, 소득이 반영된 운임, 운행횟수 변수 모두 기대부호와 일치하고 통계적으로 유의한 것으로 나타났다. 철도운영기관의 관점과 사회적 관점에서 서울~부산 간 고속철도의 적정 운임수준 추정결과는 다음과 같다. 첫째, 철도운영기관의 관점에서 고속철도의 이익변화만 고려하면 현재 운임(48,100원)의 145%수준인 69,745원이 적정운임인 것으로 나타났다. 둘째, 고속철도와 일반철도를 운영하고 있는 철도운영기관의 관점에서는 현재 운임의 145%인 69,745원일 때 운영기관은 최대 이익을 가져 고속철도만 고려했을 때와 동일한 결과로 나타났다. 이는 일반철도의 수요가 고속철도 운임과 운행횟수 변화에 덜 민감하며, 일반철도의 운임은 매우 낮아 수요가 증가해도 운임수입은 크게 증가하지 않기 때문인 것으로 볼 수 있다. 셋째, 사회적 관점에서 고속철도의 적정 운임수준은 31,265원으로 현재 운임의 65% 수준인 것으로 추정되었다. 이는 서울~부산 간 고속철도의 현재 운임이 사회적으로 적절한 운임 수준보다 높은 수준에서 운행되고 있다는 것을 의미한다. 이러한 결과가 도출된 가장 큰 이유는 고속철도의 운임을 낮추면 철도운영기관의 운임수입이 줄어들지만 다른 운송업체의 운임수입은 증가하기 때문인 것으로 판단된다. 특히 통행자 편익이 현재보다 많이 증가해 사회적 후생변화가 201,271천원/일로 현재 운임의 65% 수준에서 최대가 되는 것으로 나타났다. 본 연구는 고속철도의 적정 운임수준을 추정하는 방법론을 제시하고 적용한 점, 고속철도 1, 2단계 개통 후 자료를 사용하여 전국 단위가 아닌 특정 노선을 대상으로 수단분담모형을 추정한 점에서 의의가 있다.As the KTX(Korea Train Express) opened its first section from Seoul to Daegu in April 1st, 2004 and the second from Dongdaegu to Busan in November 1st, 2010, the travel time from Seoul to Busan has been shortened to 2 hours and 18 minutes. As of December 2011, despite the three millions passengers who had been taking the KTX, the fares increased in December, 2011. In addition, little controversy about the possible fare decrease if a private agency operates KTX departing from Suseo has called upon the need to further research. The current KTX fares are estimated from the rate of inflation and operating costs and at the same time, they have to be set while excluding the publicity. The fares of public transportation should take the supply and demand, also provide benefits to passengers. They also should guarantee profits to the operating agencies and reflect the external costs that arise from operation such as air pollution or noise cost. Therefore, this study suggests the methodology on estimating the optimal KTX fares from the perspective of both the railway operating organization and the society and aims to apply the methodology to Seoul to Busan travel. The time frame of this study is from January 2005 to December 2011 and the study utilizes data from a monthly total of 72 perspectives travel. The reference tear for the estimation of fares has been set to the earliest point which is December 2011. The space frame is from seoul to Busan, the long distance travel which has a large reduction in travel time. The subjects are 4 modes of public transportation, bus, KTX, railway and air. The method of estimation of optimal KTX fares is the following. First, a demand model for Seoul to Busan was developed to estimate the fares based on the changes in the mode share and operation frequencies, which are caused by the changes in the KTX fares. The change in demand from the change in the fare affects operation frequency, which in turn affects the demand. This process was repeated until the equilibrium was estimated, where the mode share and the operation frequencies would no longer change. Based on the calculated equilibrium values, the optimal KTX fares were estimated from the perspective of both the railway operating organization and the society. By changing the fares by 5% each time, the variations in the revenue of the rail way operating organization and of the social welfare were estimated. The optimal fares for which each of these variations was maximized could be estimated. The variations in the revenues of the railway operating organization consisted of the revenue change from the KTX and railway. Meanwhile, the social welfare revenue variation was measured from the variations in passenger benefits, fare revenues and social costs of the operating organization, external costs and tax. The modal split showed the value of rho-sq was calculated to be 0.304, suggesting the model explained the travel. The variables of the travel time, fare and operation frequency, all coincided with the expected signs and were estimated resonable statistically. The results of the estimation of optimal KTX fares from the operating organization and the society are as the following. Firstly, from the organization's perspective which only considers the variations in its revenue, the optimal fare was estimated to be 69,745 won which is 145% of the present fare(48,100 won). Secondly, from the perspective of railway organization that operates the KTX and the railway, the optimal fare of 69,745 won, which is 145% of the current fare rate(48,100 won) allows the organization to take the maximal revenue and this result is the same as the result when only the KTX was considered. This result can be explained by the facts that the demand of railway is less sensitive compared to the KTX fare and operation frequencies, and also that the railway fare is too low to be raised greatly by the increase in the demand. Lastly, from the perspective of the society, the optimal KTX fare was estimated to be 31,265 won, 65% of the current fare rate. This suggests the present fare rate for Seoul Busan Travel is higher than the appropriate fare rate. The main reason for such a result is that lowering KTX fare decreases the revenue of operating organization but increases the revenue of other transport agencies. Passenger benefits in particular increase greatly compared to the present, showing the variation in social welfare to be 201,271 thousand won per a day which means the maximum is 65% of the current fare rate. This study is significant as it suggests the methodology for estimating the optimal KTX fares, and it estimates the modal split of a specific line rather than a national level, based on the latest data after the 1st and 2nd openings of KTX. Concept development of grief : focusing on the process of spousal bereavement : 배우자 사별과정 중심으로

간호학과/박사[한글] 죽음은 인간이면 그 누구도 회피할 수 없는 사건으로서 이러한 죽음은 사별을 야기하고, 사별은 남겨진 사람들로 하여금 '애도'를 경험하게 한다. 애도는 인간의 일상과 간호현장에서 지속적으로 접하게 되는 현상임에도 불구하고 간호영역에서 간과되고 있는 실정이다. 본 연구는 한국적 상황에서 배우자 애도 개념에 대한 명확한 이해를 위해 시도된 서술적 연구로서, Schwartz-Barcott & Kim (1986)이 제시한 Hybrid model을 이용하여 개념분석을 통한 애도 개념을 개발하고자 시도하였다.현장작업 단계에 참여한 대상자는 배우자 애도 개념을 잘 반영할 수 있다고 판단된 배우자와 사별한지 3년 이내의 남성 8명과 여성 7명이었다. 자료수집 기간은 2004년 9월부터 2005년 4월까지였으며, 자료수집 방법은 심층면담과 참여관찰이었고, 녹음된 자료와 연구자의 참여관찰 내용을 즉시 기록하여 분석하였다. 자료의 분석은, 이론적 단계에서는 선행 문헌을 토대로 애도의 속성, 경험적 지표를 파악하여 배우자 애도의 작업적 정의를 내렸으며, 현장작업 단계에서는 현장관찰과 녹취된 면담 자료를 내용분석하고 조직화된 현장노트와 비교․분석하여 배우자 애도의 차원, 속성, 경험적 지표 및 애도 과정을 제시하였다. 최종분석 단계에서는 이론적 단계에서 수집된 자료와 현장작업 단계에서 관찰된 자료를 비교,분석,통합하여 배우자 애도의 과정에 따른 속성과 경험적 지표를 제시하고 재정의된 배우자 애도의 정의를 제시하였다.본 연구의 다음과 같다.배우자 애도의 선행요인은 배우자와의 사별이며, 배우자 애도의 차원은 자신과 관련된 내면적 차원, 가족/타인과 관련된 관계적 차원, 존재의미와 관련된 실존적 차원으로 구분할 수 있다. 배우자 애도의 속성은 신체로 표현되는 고통, 인지능력 저하, 마음의 슬픔(傷心), 새로운 삶에 대한 기대와 갈등, 사회적 낙인, 유가족간의 사랑과 갈등, 이웃 사랑, 삶의 의미 상실, 절대자 의지 또는 원망, 고통과 존재의미 추구, 및 새로운 정체성 확립이다. 배우자 애도의 경험적 지표는 신체․인지․정신․사회․영적 건강상태이다. 배우자 애도에 영향을 미치는 요인은 성, 연령 및 결혼 기간, 유가족들과의 관계, 자부심, 성격, 경제력 등의 자립자원, 지지원, 순명(順命), 고인과의 관계 질, 이중적 위기, 죽음의 질, 및 투병기간이며, 배우자 애도의 과정은 충격-해방기, 고통기, 및 통합기로 구분된다.따라서, 최종분석 단계에서 재정의된 배우자 애도의 정의는, '배우자 사별로 인해 경험하게 되는 자기 자신, 가족․타인, 및 존재의미와 관련된 차원에서 신체로 표현되는 고통, 인지기능 저하, 마음의 슬픔(傷心), 사회적 낙인, 유가족간의 사랑과 갈등, 절대자 의지 또는 원망 등의 혼돈과 고통 속에서 새로운 통합된 자아로 나아가는 각 개인의 독특한 변화 과정'이다.이상의 연구 결과, 배우자와 사별한 여성에서의 사회적 낙인과 남성의 성적욕구와 관련된 갈등은 본 연구의 현장작업 단계에서 두드러지게 관찰된 애도의 속성으로 파악되었으며, 급속도로 고령화되어가고 있는 한국 사회에서 배우자와 사별한 노인들의 심각성이 크게 대두되었다. 생존한 배우자의 삶에 결정적인 것은 시간과 노력이 요구되는 고통의 애도과정에서 보이는 개인의 반응이며, 애도는 각 개인에게 미치는 영향요인에 따라 다양하게 표현될 수 있고, 일정한 한 시점에서 끝나는 것이 아니라 여생동안 애도 과정에서 살아가는 법을 터득함으로써 더 건강한 총체로서 통합된 한 인간이 될 수 있음이 확인되었다. 따라서, 간호사들은 건강하고 긍정적인 애도과정을 경험할 수 있도록 대상자를 지지할 중요한 위치에 자신이 있음을 명심하고, 애도에 영향을 미칠 수 있는 요인들을 사정하고 각 애도 과정에서 직면하는 애도의 속성을 파악하여 그에 적절한 간호중재를 제공할 수 있어야 한다. [영문]Bereavement is inevitable for all humans, and grief is experienced by every bereaver. However, the phenomenon of grief is neglected in the practice and study of nursing. The purpose of this study was to develop the concept of grief focusing on the process of spousal bereavement in Korea, applying the Hybrid model suggested by Schwartz-Barcott and Kim (1986) for analysis according to the 3 phases- i.e., Theoretical, Field, and Final analytic phases. First an extensive literature review was done for the Theoretical phase. In-depth interviews were conducted with 15 participants (8 males and 7 females) included whose spouse died within the past 3 years in the Field phase of this study. They fully understood the purpose of the study and agreed to participation by signing the consent form. The interviews were tape-recorded and transcribed and data analysis was progressed. In the Final analytic phase, the attributes and empirical referents in the Theoretical and the Field phases were compared, analyzed, and integrated according to the process of grief.The final results of study were as follows:The antecedent of the concept of spousal grief was spousal death. The dimensions of grief were classified to inner dimensions related to oneself, relational dimensions related to family and others, and existential dimensions related to meaning of being. The attributes of spousal grief were physical suffering, decline of cognitive ability, heartbreaking sorrow, expectations and conflicts of a new life, social stigma, love and conflict among family members, loss of meaning of life, dependence on or resentment towards God, seeking meaning of suffering and existence, and construction of new identity. The empirical referents of grief were physical, cognitive, mental, social, and spiritual health status. The influential factors of grief were gender, age and duration of marriage, quality of relationship in the bereaved family, social support and resources, compliance to fate, quality of the relationship with the dead, concurrent crisis, quality of the death, and duration of struggle against disease. The grieving progressed through 3 phases- shock-emancipation, suffering, and integration. Therefore, the final definition of spousal grief was identified as a unique changing process to a newly integrated self in the midst of shock and suffering such as physical suffering, decline of cognitive ability, heartbreaking sorrow, expectations and conflicts of a new life, social stigma, love and conflict among family members, loss of meaning of life, and dependence on or resentment towards God.The results of this study show that individual grief is a process that requires a lot of time and effort and is manifested differently according to various influential factors. Also, there is no ending point in the grieving process, and that bereavers can become better balanced through learning the way to survive during the grieving process throughout the rest of their lives.Nurses should recognize the importance of their unique position as supporters for bereavers, and should try to assess individual influential factors, to identify the attributes of grief in the process of grief, and to provide tailored nursing interventions.restrictio

간호학과/석사[한글] 간호학은 총체적인 인간을 대상으로 하는 학문이며, 인간은 신체·마음·영이 서로 유기적인 관계를 형성하여 서로 상호 작용하는 독특한 존재이다. 특히 암환자와 같이 신체·정신적으로 위기를 경험할 때 영적 차원은 보상기전으로서 더욱 중요하고 핵심적인 인간의 본질로서 작용한다. 그럼에도 불구하고 간호이론, 연구, 실무에서 영적 측면이 간과되어 왔다. 암환자들의 영적 고뇌를 이해하고, 그들의 영적 요구를 정확히 사정하여 적절한 영적 간호를 제공하기 위해서는 영적 간호에 있어서 가장 기본적인 개념이라 할 수 있는 ''영성''에 대한 분명한 개념정립이 요구된다. 이에 본 연구는 암환자에 있어서의 영성의 차원, 선행요인, 속성, 지표를 파악하고 암환자 영성을 재 정의하기 위한 개념분석 연구이다. / 본 연구는 Schwartz-Barcott, Kim(1986)이 제시한 혼종모형을 이용하여 기독교, 불교, 무교를 가진 암환자를 대상으로 영성 개념분석을 시도하였다. 연구대상자는 암을 진단 받고 투병중인 30세 이상의 암환자로 항암요법을 받거나 질병의 악화로 병원에 입원한 환자 중 각 종교를 대표한다고 암병동 수간호사가 추천한 기독교 2명, 불교 3명, 무교(無敎) 3명을 대상으로 하였다./ 자료수집은 해당 병원 관계자의 승낙을 얻은 후, 2002년 4월초부터 6월초에 걸쳐 이루어졌으며, 연구자가 직접 대상자들에게 연구자의 연구의도를 밝히고 대상자들의 동의를 구한 후 진행하였다. / 자료분석은 이론적 단계에서는 광범위한 문헌고찰을 통해 암환자 영성의 차원, 속성, 지표를 제시하고 작업적 정의를 내렸으며, 현장작업단계에서는 Schatzman, Strauss(1973)의 기록체계인 관찰기록, 이론적 기록, 방법론적 기록을 이용하여 현장 노트를 작성하여 자료를 조직화하였다. 이 자료를 토대로 사례별로 참여관찰기록과 심층면담내용을 삽입하여 현장관찰을 통한 종교별, 사례별 암환자의 영성 차원에 따른 속성, 지표를 제시하였다. 또한, 최종분석단계에서는 이론적 단계에서와 현장작업단계에서 파악된 암환자 영성의 차원에 따른 속성과 결과를 비교, 분석하였다. 또한, 종교를 비롯한 각 개인의 특성에 따른 영성 속성의 공통점과 차이점을 비교, 분석하였으며 최종적으로 암환자 영성의 차원, 속성, 지표를 제시하고 암환자 영성을 재 정의하였다. 연구결과는 다음과 같다. 1. 암환자 영성의 차원 / · 절대적 존재와 관련된 수직적 차원/ · 타인·이웃·외부환경과 관련된 수평적 차원/ · 의미추구와 관련된 실존적 차원 2. 암환자 영성의 속성/ · 암투병의 고통 속에서 더 찾고 강화될 수 있는 역동적이고 독특한 과정/ · 절대자와의 관계성 또는 자신에 대한 의지와 믿음/ · 현실 초월성/ · 내적 에너지 원/ · 삶의 의미와 목적 추구/ · 미래지향성 3. 암환자 영성의 지표 1) 내면적 지표/ · 사랑/ · 절제 / · 자기 초월/ · 평안/ · 자아존중/ · 자기수용/ · 희망/ 2) 행동적 지표 / · 용서 / · 기도/ · 종교적 실천/ �기쁨과 감사의 삶/ · 이웃에 대한 봉사 4. 암환자 영성의 정의 / 모든 인간이 가진 본질인 ''영''의 외적 발현으로서 암투병의고통 속에서 더 찾고 강화될 수 있는 역동성과 개인의 종교, 연령, 과거경험, 가족에 대한 부담감에 따라 독특하게 표현되는 절대자, 타인·이웃·외부환경, 의미추구와 관련된 차원에서 나타나는 절대자와의 관계성 또는 자신에 대한 의지와 믿음이며 현실 초월, 미래지향, 삶의 의미와 목적을 추구하게 하는 내적 에너지원이다. 이상의 이론적 단계와 현장작업 단계를 통한 암환자 영성 개념분석을 통하여 암환자의 영성은 각 종교, 연령, 과거 경험, 가족에 대한 부담감에 따라 각기 다르게 표현될 수 있으며 또한, 암진단과 전이라는 생의 고통 속에서 더욱 강화될 수도 있음을 확인할 수 있었다. 그러므로, 간호사는 암진단과 질병의 악화는 영적인 위기인 동시에 영적 성장을 위한 좋은 계기임을 인식하고 독특하게 표현되는 각 대상자들의 영성을 정확히 사정하고 그에 적절한 간호중재를 제공하기 위한 노력을 기울여야 할 것이다 [영문] All nurses should provide spiritual care for their clients as holistic persons consisted of body, mind, and spirit. It is especially important to care spiritually for cancer patients facing the crisis of their life. Therefore, the purpose of this study is to analyze the concept of spirituality which is one of the basic concept for spiritual care in cancer patients. The subjects of this study were eight persons; 2 Christians and 3 Buddhists, and 3 persons who do not have any religion. They were cancer patients, and in-patients recommended by the head-nurse of an oncology department because they represented adherents to these different religious faith. The data was collected from April 9, 2002 to June 4, 2002 through using the method of in-depth interview and participant observation with the participants'' consent. The interviews were recorded on audio tapes and then transcribed. There were 3 phases for data analysis- Theoretical phase, Fieldwork phase, Final analytical phase- according to the Hybrid Model suggested by Schwartz-Barcott, and Kim(1986). In the Fieldwork phase, the data of participant observations and the content of the in-depth interviews were presented and then enumerated as dimensions, attributes, and outcomes of spirituality in the cases. Also in final phase, the attributes and outcomes of the Theoretical phase and the data of fieldwork phase were compared and analyzed according to the dimensions of the spirituality of cancer patients and then, the commonalities and differences according to religion, age, past experiences and burden of family were discussed. The final results of study were as follows : 1. Spirituality is internal energy manifested in 3 dimensions- related to the absolute being, other person·neighbor·environment and seeking of meaning- connectedness with the absolute being or will and believe oneself, transcendence of reality, future oriented, and seeking meaning and purpose of life. The result of this study shows that an individual spirituality is manifested differently by his(her) religion, age, past experiences and burden of family, and able to be strengthened in suffering due to struggle with cancer. Therefore, nurses should recognize that diagnosis and deterioration of cancer is not only a spiritual crisis but a good chance for spiritual growth and should try to assess the spiritual problems of their clients.ope

(A) study on the physical health problems and management of the health problems of the elderly.

간호학교육전공/석사[한글] 최근 경제성장에 따른 전반적인 생활수준의 향상과 의학기술의 발전으로 사망률이 감소되고 평균 수명이 현저하게 증가하여 노인 인구가 급증하고 있는 추세이다. 그러나 이러한 평균수명의 연장으로 오히려 노인들의 만성질환 유병율은 높아지고 있으며 이에 대한 관리실태 또한 미흡한 실정이다. 이에 본 연구는 노인들의 신체적인 건강문제를 조사하고 각 건강문제에 따른 관리실태를 파악함으로써 건강문제를 보다 포괄적이고 효율적으로 관리하기 위한 기초자료를 제공하는데 목적이 있다. 자료수집은 2001년 4월 10일부터 4월 30일까지 20일 동안 서울시 서대문구 홍제동에 거주하는 65세 이상의 재가 남녀 노인 100명을 임의표출하여 구조화된 질문지를 이용하여 면담을 통해 자료를 수집하였다. 노인의 신체적인 건강문제를 알아보기 위한 연구도구로는 Brodman과 그의 동료들(1951)에 의해서 개발된 CMI(Cornell Medical Index)를 근간으로 총 38문항의 질문지를 사용하였으며, 각 건강문제에 관리실태를 파악하기 위하여 건강문제 발생시 일차적으로 행하는 관리법을 선택하여 답하도록 연구자가 개발한 도구를 사용하였다. 수집된 자료는 SPSS/PC+를 이용하여 분석하였으며, 대상자의 일반적인 특성과 건강문제별 분포 및 건강문제에 따른 관리실태는 실수와 백분율로 분석하였으며, 일반적인 특성에 따른 건강문제 분포는 t-test, ANOVA, χ²-test의 방법으로 분석하였다. 또한 일반적 특성에 따른 건강문제 관리실태는 χ²-test 방법으로 분석하였다. 본 연구결과를 요약하면 다음과 같다. 1. 대상자의 일반적인 특성중 성별은 남자 52%, 여자 48%로 비슷하였으며, 연령은 65세이상 74세가 69%, 75세 이상이 31%로 평균연령은 72.9세였다. 2. 대상자의 건강문제 분포는 '눈이 잘 안 보인다'가 67%로 가장 높게 나타났으며, 그외 '본래의 치아가 약해졌다'가 65%, '손이나 다리에 쥐가 자주난다'가 60%, '관절이 붓고 아플 때가 있다'가 57%의 순이었다. 3. 각 건강문제에 대한 관리실태는 병의원 방문은 당뇨가 있을 경우에서 100%로 가장 높았으며, 그외 혈압이 높을 경우 64.6%, 본래의 치아가 약해졌을 때 50.8%, 허리가 아플 때 30.6%, 관절이 붓고 아플 때 26.3%의 순으로 병의원을 방문하였다. 약국이용은 소화가 잘 안될 때 82.6%로 가장 높게 나타났으며, 그외 두통 66.7%, 어깨나 등살이 빳빳해질 때 24.2% 순이었다. 자가관리는 손이나 다리에 쥐가 날 때가 53.3%로 가장 높게 나타났으며, 그외 어깨나 등살이 빳빳해질 때 51.1%, 몸의 일부가 저리거나 전기가 날 때 47.5%, 현기증이 날 때 45.7%의 순이었다. 치료를 받지 않는 경우는 귀가 잘 안 들릴 때 82.9%로 가장 높았으며, 그외 눈이 잘 안보일 때 66.6%, 치아가 약해졌을 때가 47.4%의 순이었다. 보조기의 사용은 눈이 잘 안 보일 경우 과반수 이상인 73.1%가 안경을 착용하고 있었으며,치아가 약해진 경우 55.4%의 노인이 의치를 사용하고 있었다. 4. 일반적 특성에 따른 건강문제 분포를 비교해 본 결과 성별에서는 여자가 남자보다 더많은 건강문제를 갖고 있었으며(t=-3.60, p< .05), 배우자 생존 여부에서는 배우자가 사망한 집단에서 더 많은 건강문제를 가지고 있었다(t=-2.23, p< .05) 5. 일반적 특성에 따른 관리실태와의 관계는 '본래의 치아가 약해졌다'에서는 성별, 교육정도, 참여모임유무에 따라 관리실태에 유의한 차이가 있었는데, 여자보다 남자가(χ²=3.360, p< .1) 그리고 교육정도가 초등학교 졸업인 집단이 다른 집단보다(χ²=8.077, p < .05), 참여모임이 있는 집단이 없는 집단에 비해(χ²=3.491, p< .1) 전문의료 기관을 방문하는 비율이 높게 나타났다. '손이나 다리에 쥐가 자주 난다'에서는 배우자 생존여부에 따라 관리 실태에 유의한 차이가 있었는데, 배우자가 생존해 있는 집단이 배우자가 사망한 집단에 비해 전문의료기관을 방문하는 비율이 높게 나타났다(χ²=3.064, p< .1). '관절이 붓고 아플 때가 있다' 에서는 교육정도에 따라 관리실태에 유의한 차이가 있었는데 교육정도가 초등학교 졸업인 집단이 다른 집단보다 전문의료기관을 방문하는 비율이 높았다(χ²=7.817, p< .05). '혈압이 높다고 의사가 말한 적이 있다'에서도 교육정도에 따라 관리실태에 유의한 차이가 있었는데, 교육정도가 초등학교 졸업인 집단이 다른 집단보다 전문의료기관을 방문하는 비율이 높게 나타났다(χ²=10.093, p< .05). '늘 소화가 안되고 헛배가 부르다'에서도 교육정도에 따라 관리실태에 유의한 차이가 있었는데, 교육정도가 초등학교졸업인 집단이 다른 집단보다 전문의료 기관을 방문하는 비율이 높게 나 타났다(χ²=7.886, p< .05). 이상의 연구결과를 종합해 볼 때 65세 이상 노인들의 건강문제 유병율이 높음에도 불구하고 이에 따른 관리실태는 미흡함을 알 수 있었다. 또한 일반적 특성에 따라 노인의 건강문제와 관리실태에 차이가 있는 것으로 나타나 노인들의 건강문제를 이해하고 관리하는데 있어서 노인의 여러 일반적인 특성을 함께 고려하여 접근하는 것이 요구된다. [영문] The number of older people has increased because of the development in medicine and the changes in socioeconomic status. With a decrease in physical functioning, the prevalence of chronic diseases has increased in the elderly. The related problems of health will become a very important issue in the future. Thus, this study was to analyze the physical health problems and the management of the health problems among the elderly. This is a descriptive survey study. The sample consisted of 100 persons over the age of 65 years old, who were living at home in Hong Jae Dong, Seoul. Data were collected from April 10 to April 30, 2001 through a personal interview of the elderly using a structured questionnaire. The physical health problems were measured using the Cornell Medical Index, which Brodman(1951) designed and was modified by Nam, Ho Chang(1965). Two questions were subtracted and five questions were added to the tool for this research. The collected data were analyzed using the statistics package SPSS, yielding frequency, percentage, mean, χ²-test. The result of the analysis were as follows: 1. Among the 38 physical problems, visual difficulties problems with occurrence of 67% rated highest. The second most frequent problem was dental problems(65%), numbness of hands and feet(60%) rating third. 2. Analysing the management of the physical health problems, it appeared that these subject visit a medical center(general hospital, clinic, chinese clinic) for the following reasons, diabetes (100%) was the highest, and high blood pressure(64.6%), dental problems(50.8%), back pain(30.6%) and joint pain(26.3%). The visit to a pharmacy were related to indigestion(82.6%) was the highest and headache (66.7%), muscle pain(24.2%). Self-care were related to numbness of hands and feet(53.3%) was the highest and muscle pain(51.1%). Use of assistive devices: eye glasses(73%), dentures(55.4%). 3. Considering the results of the demographic characteristics, health problems were significantly increased in people having the following characteristics: females(t= -3.60, p< .05): no spouse(t= -2.23, p< .05). 4. Considering management patterns of the demographic characteristics, in case of dental problems: males(χ²=3.360, p< .1), educational level (elementary graduation: χ²=8.077, p< .05), engaging group(χ²=3.491, p< .1) were significantly high in visiting a medical center. In case of numbness of hands and feet: with spouse(χ²=3.064, p< .1) was significantly high in visiting a medical center. In case of joint pain: educational level (elementary graduation: χ²=7.817, p< .05) was significantly high in visiting a medical center. In case of high blood pressure: educational level (elementary graduation: χ²=10.093, p< .05) was significantly high in visiting a medical center. In case of indigestion: educational level (elementary graduation: χ²=7.886, p< .05) was significantly high in visiting a medical center. From the result of this study the following conclusions can be drawn: In order to improve the management of the health problems in the elderly, it is necessary to develop the health promotion and care program component. Also continuous health education in home or/and community unit must be essential factor for effective geriatric health management. The result of this study provide information which will improve nursing practice and nursing intervention in geriatrics.ope
