    Therapeutic Leukapheresis for the Management of Hyperleukocytosis: Ten-year Experience in a Tertiary Care Hospital

    Background : Hyperleukocytosis is a medical emergency that is characterized by increased blood viscosity and predisposition to various neurological, pulmonary, and gastrointestinal complications. In addition, patients are at risk of the tumor lysis syndrome because of the increased tumor burden. Therapeutic leukapheresis is an important treatment for these emergent states. In this study, we retrospectively analyzed therapeutic leukapheresis procedures that were performed in our institution during the last 10 yr. Methods : We retrospectively analyzed therapeutic leukapheresis procedures conducted from July 2005 to March 2015 at a tertiary care hospital. We present our observations, especially the procedural characteristics and hematological parameters before and after the aforementioned procedures. Results : Seventy-two patients underwent a total of 146 therapeutic leukapheresis procedures. The average presenting white blood cell (WBC) count was 268ร—103/?L, and ranged from 54ร—103/?L to 673ร—103/?L. After an average of two sessions, a statistically significant drop in the WBC counts was observed. The average WBC removal rates during the initial and entire therapeutic leukapheresis procedures of each patient were 33% and 46%, respectively. The platelet count and hemoglobin concentration were significantly reduced. Conclusions : Therapeutic leukapheresis significantly reduces peripheral WBC counts and is a safe and effective procedure for the treatment of hyperleukocytosis.ope

    Therapeutic Plasma Exchange in a Patient with Hemophagocytic Lymphohistiocytosis

    A 22-year old female patient with systemic lupus erythematosus presenting microangiopathic hemolytic anemia was treated with therapeutic plasma exchange 23 times. The patientโ€™s condition and laboratory findings (aspartate aminotransferase, alanine aminotransferase, ferritin, total bilirubin, and lactate dehydrogenase) did not improve despite the initial 18 therapeutic plasma exchange treatments. Thrombotic thrombocytopenic purpura was ruled out due to normal ADAMTS-13 activity test result; hemophagocytic lymphohistiocytosis was diagnosed based on fever, splenomegaly, pancytopenia, hypertriglyceridemia, hyperferritinemia, and hemophagocytosis in bone marrow aspiration. The patientโ€™s condition improved rapidly upon treatment with a combination of immunosuppressants and cytotoxic agents, and more therapeutic plasma exchanges were performed five consecutive times with prolonged intervals in between. We observed that therapeutic plasma exchange treatment alone was not effective enough to treat hemophagocytic lymphohistiocytosis, unlike thrombotic thrombocytopenic purpura. Therefore, it is necessary to determine and start drug administration promptly in the treatment of hemophagocytic lymphohistiocytosis with thrombotic microangiopathy.ope

[์˜๋ฌธ] Korea society is susceptible to even tiny changes of the fertility rate because currently the fertility rate is low. So demographers are interested in the fertility duration. So the studies on the effects of factors influencing on the fertility duration which can affect the degree of low fertility (on this study fertility duration means the interval between marriage and the first child birth) get more important meanings in the era of low fertility rate. This study is based on the Year 2000 Korea National Fertility Survey carried out by Korea Institute of Health and Social Affairs. We analysed socioeconomic characteristics, marriage age, marriage cohort, artificial abortion experiences in the first pregnancy, son preference notion and the types of family generations as independent variables. A dependent variable was the first birth interval and the survival analysis was used. The pregnant women were also included in the analysis. This analysis showed the opposite results to the general opinion that the first birth interval decreases as the marriage age goes up. Based on the age of 23-25 As the marriage age gets higher, the first birth interval goes longer since 1985. The results of the study about the effects of marriage cohort are as follows. Before 1980-84, we can observe that the parameter has minus value and the first birth interval gets longer than the standard year. But the intervals between the measurements gradually get smaller. From the latest marriage cohort, we can see the first birth interval gets longer and longer. The son preference tendency and the types of family generations also give influences to the firth birth interval. This means that the traditional systems and values can affect the first birth interval. The women who strongly prefer boys to girls have shorter first birth interval than the others. And the women who consist of less than 3 generations have longer first birth period than the others. Also the women who experienced artificial abortion have longer first interval. Our analysis suggests the necessities of ongoing research and concerns about the first birth intervals which can have major influences on personal life courses, changes of family and fertility rates in low fertility society.ope

Job Satisfaction and Role Conflict of Nurses in Out-Patient Department Lee, Kyoung Ae Department of Nursing Education Graduate School of Education Yonsei University According to prediction from health management systems, The 21th century's health care service is more likely to be done through ambulatory settings than through in-patient settings owing to the rapid development of medical science and communication techniques, and consequently a broader function for out-patient department is indispensible. However, the role of the out-patient department nurses at the present Is rather dependent and assistant and this is expected to cause role conflict. This study is designed to understand the job-related role conflict and satisfaction level of nurses in the out-patient departments. The subjects of this study were the 169 nurses from three general hospitals and data were collected for 13 days from May 3 till May 15,1999 through survey questionnaires, Revised forms of "the Index of Work Satisfaction" by Kim Suk Ja(1992) and "Role Conflit Scale" by Kim Mun Sil and Park Sang Yun(1994) were used as survey tools. The characteristics of the subjects were analyzed using frequencies and percentile scores arid the level of job satisfaction and role conflict, using percents, means and standard devations. The characteristics related job satisfaction and role conflict were analyzed by means of ANOVA, t-test, and Tukey HSD. The relation between job satisfaction and role conflict was analyzed by Pearson correlation coefficient and multiple Regression. The results of this study are as follows 1. The respondents' average job satisfaction was 2.98 of possible score of five Characteristics related to higher job satisfaction were education(P<.05), longer work experience in out-patient departments(P<.001), and higher job status(P<.001). 2. The respondents' role conflict was 3.29 of possible score of five Characteristics related to more role conflict which were statistically significantly included the nurses having a shorter period of total work experience(P<.05). The factors' role ambiguity'(3.43), followed by 'lack of capability'(2.99),'lack of cooperation'(2.95), and 'non supportive environmental system'(2.90), were the most influential factor on the role con(dict scores. 3. Job satisfaction and role conflict were negatively correlated(r=-.253,p<.01). Among role conflict factors, the most influential factor on job satisfaction score was 'non supportive environmental system'(61.6%), followed by 'role ambiguity'(6.6%),',lack of cooperation(4.2%),and 'lack of capability'(2.3%) factors .restrictio
