Therapeutic Leukapheresis for the Management of Hyperleukocytosis: Ten-year Experience in a Tertiary Care Hospital

Background : Hyperleukocytosis is a medical emergency that is characterized by increased blood viscosity and predisposition to various neurological, pulmonary, and gastrointestinal complications. In addition, patients are at risk of the tumor lysis syndrome because of the increased tumor burden. Therapeutic leukapheresis is an important treatment for these emergent states. In this study, we retrospectively analyzed therapeutic leukapheresis procedures that were performed in our institution during the last 10 yr. Methods : We retrospectively analyzed therapeutic leukapheresis procedures conducted from July 2005 to March 2015 at a tertiary care hospital. We present our observations, especially the procedural characteristics and hematological parameters before and after the aforementioned procedures. Results : Seventy-two patients underwent a total of 146 therapeutic leukapheresis procedures. The average presenting white blood cell (WBC) count was 268×103/?L, and ranged from 54×103/?L to 673×103/?L. After an average of two sessions, a statistically significant drop in the WBC counts was observed. The average WBC removal rates during the initial and entire therapeutic leukapheresis procedures of each patient were 33% and 46%, respectively. The platelet count and hemoglobin concentration were significantly reduced. Conclusions : Therapeutic leukapheresis significantly reduces peripheral WBC counts and is a safe and effective procedure for the treatment of hyperleukocytosis.ope

Therapeutic Plasma Exchange in a Patient with Hemophagocytic Lymphohistiocytosis

A 22-year old female patient with systemic lupus erythematosus presenting microangiopathic hemolytic anemia was treated with therapeutic plasma exchange 23 times. The patient’s condition and laboratory findings (aspartate aminotransferase, alanine aminotransferase, ferritin, total bilirubin, and lactate dehydrogenase) did not improve despite the initial 18 therapeutic plasma exchange treatments. Thrombotic thrombocytopenic purpura was ruled out due to normal ADAMTS-13 activity test result; hemophagocytic lymphohistiocytosis was diagnosed based on fever, splenomegaly, pancytopenia, hypertriglyceridemia, hyperferritinemia, and hemophagocytosis in bone marrow aspiration. The patient’s condition improved rapidly upon treatment with a combination of immunosuppressants and cytotoxic agents, and more therapeutic plasma exchanges were performed five consecutive times with prolonged intervals in between. We observed that therapeutic plasma exchange treatment alone was not effective enough to treat hemophagocytic lymphohistiocytosis, unlike thrombotic thrombocytopenic purpura. Therefore, it is necessary to determine and start drug administration promptly in the treatment of hemophagocytic lymphohistiocytosis with thrombotic microangiopathy.ope

The Significance of Garam Lee Byung-gis Studies through the Analysis of the Literature Data of Garam Mungo : Focusing on Garam Mungo in Kyujanggak

가람 이병기(1981~1968)는 그가 평생 수집한 장서를 서울대학교 도서관에 기증하였다. 그중 한국학 관련 주요 고문헌들은 규장각한국학연구원 소장 가람文庫'(1,612책)로 보관되어 있다. 본 연구는 가람 이병기의 기증 장서 가람文庫'를 중심으로 그 규모와 양상을 조사하고, 그의 창작과 저술에 활용된 문헌들을 추적하여, 그의 장서가 가람 학문의 기초 동력으로써 다양한 학문과 창작 저술들에 활용되고 연구 적용된 바를 고찰하였다. 이를 위해 가람이 발표한 「조선어문학명저해제」와 「한국명저해제」와 『書目』과 가람文庫의 목록을 비교하고 가람文庫' 목록을 조사 분석하여 그가 수집한 고서들의 특성들을 살펴보았다. 또한 가람이 생산한 저술의 총량 및 다양성과 가람文庫와의 관계를 살피면서 문헌-서지학, 국어학, 국문학(고전문학, 현대문학), 구비문학․민속학, 교육학, 역사학 등 분야에 걸쳐서 다양한 분야에 다양한 내용과 형식으로 가람文庫' 서적들이 활용되었음을 보여 주었다. 그의 국문학 저술 『國文學全史』는 대표적인 예로 그가 평생 고서들을 수집하는데 그치지 않고 이를 충실히 연구하고 활용하였음을 확실하게 증명해주고 있다. 또한 그의 창작 수필에도 가람文庫' 고서는 가장 빈번하게 발견되는 소재로 그 작품의 내용과 인용된 가람文庫'의 고서들의 활용을 고찰하였다. 가람이 수집하여 소장하고 기증한 귀중한 장서들이 문화유산으로서 보전해야할 가치도 크지만, 더불어 이러한 가람文庫' 자료 조사를 통하여 가람의 저술과 연관 지어 그것이 과거 유산으로서가 아니라 현재와 미래의 재생산 가치로 그의 학문에 나타난 현대적 가치와 의의가 있음을 고찰하였다

(A) study on determinants of the first birth interval among the Korean married women

국제보건학과/석사[한글] 현재와 같이 출산력 수준이 낮을 때에는 조그마한 출산력 수준의 변화에도 사회가 민감하게 반응할 수밖에 없다. 인구학자들 사이에서 이러한 출산력의 변화와 관련하여 출산기간에 관심이 모아져 왔다. 현재의 낮은 출산력 수준에 영향을 미칠 수 있는 출산기간의 문제－이 연구에서는 결혼부터 첫 자녀출산까지 걸리는 시간인 첫 출산간격－에 영향을 미치는 요인의 효과에 대한 연구는 오히려 저 출산의 시대에 더 큰 중요성을 갖는다. 이 연구는 한국보건사회연구원이 조사한 2000년 전국 출산력 및 가족보건 실태조사 자료를 이용하여, 사회 경제적인 특성들과 결혼연령, 결혼코호트, 첫 임신시 인공유산 경험, 남아선호, 가족의 세대형태에 따라 첫 출산간격의 변화를 분석하였다. 이 연구는 조사당시 임신중인 경우를 포함하여 첫 출산간격이라는 기간을 종속변수로 모형화 할 수 있는 생존분석을 이용했다. 분석결과 결혼연령이 상승할수록 첫 출산간격이 감소한다는 일반적인 주장과는 달리 1985년 결혼코호트이후에는 23-25세 준거범주에 비해 늦게 결혼할수록 첫 출산간격이 길어진다고 밝혀졌다. 결혼코호트의 효과를 잘 살펴보면 1980-84년을 기준으로 그 이전의 결혼코호트들에서 각각 모수 측정치가 음수로 준거범주보다 첫 출산간격이 길게 나타나지만 첫 출산간격 자체는 점점 짧아져 오는 경향을 보이며 그 이후의 결혼코호트들에서 첫 출산간격이 다시 점점 길어지고 있음을 볼 수 있다. 남아선호와 가족의 세대형태에 따라서도 첫 출산간격에 차이가 나타났으며 이는 전통적인 제도나 가치도 첫 출산간격에 영향을 미치는 것을 시사한다. 강한 남아선호 사상을 가진 여성들은 그렇지 않은 여성들에 비해 첫 출산간격이 짧아지는 것으로 나타나고 있고, 3세대 미만의 가정이 그렇지 않은 가정의 여성들에 비해 첫 출산간격이 길게 나타났다. 첫 임신시 인공유산경험을 한 경우의 여성들이 그렇지 않은 여성들에 비해 첫 출산간격이 길게 나타났다. 저 출산력 시대에 개인의 생애과정과 가족의 변화, 출산력을 비롯한 인구학적 측면에서 중요한 영향을 미칠 수 있는 첫 출산간격에 대해 지속적인 연구와 관심을 기울일 필요가 있다. [영문] Korea society is susceptible to even tiny changes of the fertility rate because currently the fertility rate is low. So demographers are interested in the fertility duration. So the studies on the effects of factors influencing on the fertility duration which can affect the degree of low fertility (on this study fertility duration means the interval between marriage and the first child birth) get more important meanings in the era of low fertility rate. This study is based on the Year 2000 Korea National Fertility Survey carried out by Korea Institute of Health and Social Affairs. We analysed socioeconomic characteristics, marriage age, marriage cohort, artificial abortion experiences in the first pregnancy, son preference notion and the types of family generations as independent variables. A dependent variable was the first birth interval and the survival analysis was used. The pregnant women were also included in the analysis. This analysis showed the opposite results to the general opinion that the first birth interval decreases as the marriage age goes up. Based on the age of 23-25 As the marriage age gets higher, the first birth interval goes longer since 1985. The results of the study about the effects of marriage cohort are as follows. Before 1980-84, we can observe that the parameter has minus value and the first birth interval gets longer than the standard year. But the intervals between the measurements gradually get smaller. From the latest marriage cohort, we can see the first birth interval gets longer and longer. The son preference tendency and the types of family generations also give influences to the firth birth interval. This means that the traditional systems and values can affect the first birth interval. The women who strongly prefer boys to girls have shorter first birth interval than the others. And the women who consist of less than 3 generations have longer first birth period than the others. Also the women who experienced artificial abortion have longer first interval. Our analysis suggests the necessities of ongoing research and concerns about the first birth intervals which can have major influences on personal life courses, changes of family and fertility rates in low fertility society.ope

Story comprehension abilities by 4, 5, 6 year-old children : focusing on task and error types

언어병리학 협동과정/석사아동들이 가장 쉽게 접할 수 있는 담화의 형태 중 하나인 이야기는 학령기 학업 수행 능력의 중요한 예측 인자이다. 아동의 이야기를 평가하고 분석하기 위해서는 먼저 일반 아동의 이야기 이해 특성을 살펴볼 필요가 있다. 따라서 본 연구에서는 세 가지 유형의 이야기 이해과제(사실적 정보이해 과제, 텍스트 연결추론 과제, 빠진 정보추론 과제)를 이용하여 학령전기 일반 아동의 이야기 이해 능력에 대한 특성을 알아보고, 이와 더불어 연령에 따라 이야기 오류유형(사실적 이해 실패, 질문범위 오류, 엉뚱한 대답, ''모른다'' 반응, 잘못된 추론)에서 어떠한 특징을 보이는지를 알아보았다. 이를 위하여 만 4, 5, 6세 일반 아동 각각 15명씩(남자 7명, 여자 8명), 총 45명을 대상으로 이야기를 들려주고 이야기 이해 능력을 검사하였다. 본 연구에서는 선행연구에서 사용되었던 이야기를 학령전기 아동의 수준에 맞게 수정하여 사용하였으며, 아동에게 제시한 이야기 이해과제 역시 선행연구에서 사용되었던 과제를 사용하되, 이야기 수정 과정 중 생략된 내용은 연구자가 제작하였다. 본 연구의 결과 및 논의점은 다음과 같다. 첫째, 4세 아동이 세 가지 유형의 이야기 이해과제 점수와 총점에서 가장 낮은 점수를 획득하였으며, 각각의 이야기 이해과제 점수와 총점은 연령에 증가함에 따라 높아졌다. 이러한 결과가 나타난 이유는 연령이 증가함에 따라 이야기 문법 지식, 작업기억 용량, 어휘 및 구문이해 능력과 추론 능력이 발달하여 아동들이 이야기를 보다 잘 이해할 수 있기 때문으로 보인다. 둘째, 세 연령 모두에서 사실적 정보이해 점수가 추론 과제인 텍스트 연결추론 및 빠진 정보추론 점수보다 유의하게 높았다. 이는 사실적 정보이해 과제가 추론 과제보다 단순한 인지처리과정을 요구하는 과제 유형이기 때문일 것이다. 셋째, 세 연령에 따른 이야기 오류유형 비율에는 유의한 차이가 있었다. 사실적 이해 실패와 엉뚱한 대답의 오류비율은 연령이 증가함에 따라 낮아졌고, 질문범위 오류는 연령이 증가함에 높아졌다. 이는 아동의 연령이 증가함에 따라 이야기 이해능력이 발달하기 때문으로 보인다. ''모른다'' 반응의 경우, ''모른다'' 반응이 나타난 이유에 대한 심층 검사가 이루어지지 않았기 때문에, 그리고 잘못된 추론의 경우, 선행 연구에서 사용된 이야기 이해 과제와의 차이로 인하여 연령 증가에 따라 오류비율이 높아지거나 낮아지는 경향성이 관찰되지 않은 것으로 보인다. 본 연구는 정상발달 아동의 이야기 이해 발달 특성에 대한 지표를 제공하였으며, 이야기 치료 시, 언어와 인지가 통합된 접근이 이루어져야 하는 근거를 제시하였다는 점에서 의의가 있다. 향후 아동언어 장애군을 연구 대상으로 확장하여 정상군과의 비교 연구, 읽기 과제를 통한 이야기 이해과제 수행에 대한 연구가 진행되기를 기대해 본다.restrictio

Job Satisfaction and Role Conflict of Nurses in Out-Patient Department

간호학교육 전공/석사[영문] [한글] 21세기 미래의 건강관리 체계에 대한 예측에 의하면 의학기술의 발달과정보화 사회로의 급속한 진전으로 환자의 진료형태가 입원보다는 통원치료의 형태로 점차 확대될 것이고 이에 따른 외래 기능의 확대 및 변화가 기대된다. 그러나 현 외래간호사의 업무활동을 보 면 전문직 간호사로서 독립된 기능을 하고 있다고 보기 어려우며 이에 따른 역할갈등이 예기된다. 따라서 본 연구는 외래간호사의 업무수행과 관련하여 역할갈등과 직무만족도를 파악하 였다. 연구대상은 서울시내에 소재하고 있는 3차 진료기관인 대학부속병원의 외래 간호사 169 명이었으며, 1999년 5월 3일부터 1997년 5월 15일까지 질문지를 통해 자료를 수집하였다. 연구도구는 선행연구에서 김숙자(1992)가 사용한 직무만족측정도구와 김문실, 박상연(1 994)이 개발한 역할갈등 측정도구를 수정 보완하여 사용하였다. 수집된 자료는 전산 처리하여 일반적 특성은 실수와 백분율, 직무만족 정도와 역할갈등 정도는 백분율, 평균, 표준편차를 구하였다. 일반적 특성에 따른 직무만족과 역할갈등 정도는 ANOVA와 t-test, Tukey HSD를 사용하였고, 직무만족과 역할갈등간의 관계는 단순 상관관계(Pearson Correlation)와 회귀분석(Regression)을 이용하였다. 연구결과를 요약하면 다음과 같다. 1. 대상자의 직무만족도를 분석한 결과 최대평점 5점에서 2.98로 나타났으며, 일반적 특성에 따른 직무만족도는 학력이 높을수록(P<.05), 외래경력이 높을수록(P<.001), 직위 가 높을수록(P<.001) 높게 나타났다. 2. 대상자의 역할갈등 분석결과는 최대평점 5점에서 3.29로 나타났으며, 일반적 특성에 따른 역할갈등 정도는 간호사 총 경력이 낮을수록(P<.05) 통계적으로 유의하게 높게 나 타났다. 요인별 역할갈등 정도는 역할모호 요인이 가장 높게 나타났으며(3.43), 능력부족 (2.99), 협조부족(2.95), 환경장애(2.90)순이었다. 3. 직무만족과 역할갈등간에는 통계적으로 유의한 역 상관관계를 나타내었으며(r=-.253 , p<.01), 역할갈등 요인 중 직무만족에 가장 영향을 미치는 요인은 환경장애로 61.6% 설 명하였으며 역할모호가 6.6%, 협조부족이 4.2%,능력부족이 2.3%를 각각 설명해 주는 것으 로 나타났으며, 총 설명력은 74.7%이었다. Job Satisfaction and Role Conflict of Nurses in Out-Patient Department Lee, Kyoung Ae Department of Nursing Education Graduate School of Education Yonsei University According to prediction from health management systems, The 21th century's health care service is more likely to be done through ambulatory settings than through in-patient settings owing to the rapid development of medical science and communication techniques, and consequently a broader function for out-patient department is indispensible. However, the role of the out-patient department nurses at the present Is rather dependent and assistant and this is expected to cause role conflict. This study is designed to understand the job-related role conflict and satisfaction level of nurses in the out-patient departments. The subjects of this study were the 169 nurses from three general hospitals and data were collected for 13 days from May 3 till May 15,1999 through survey questionnaires, Revised forms of "the Index of Work Satisfaction" by Kim Suk Ja(1992) and "Role Conflit Scale" by Kim Mun Sil and Park Sang Yun(1994) were used as survey tools. The characteristics of the subjects were analyzed using frequencies and percentile scores arid the level of job satisfaction and role conflict, using percents, means and standard devations. The characteristics related job satisfaction and role conflict were analyzed by means of ANOVA, t-test, and Tukey HSD. The relation between job satisfaction and role conflict was analyzed by Pearson correlation coefficient and multiple Regression. The results of this study are as follows 1. The respondents' average job satisfaction was 2.98 of possible score of five Characteristics related to higher job satisfaction were education(P<.05), longer work experience in out-patient departments(P<.001), and higher job status(P<.001). 2. The respondents' role conflict was 3.29 of possible score of five Characteristics related to more role conflict which were statistically significantly included the nurses having a shorter period of total work experience(P<.05). The factors' role ambiguity'(3.43), followed by 'lack of capability'(2.99),'lack of cooperation'(2.95), and 'non supportive environmental system'(2.90), were the most influential factor on the role con(dict scores. 3. Job satisfaction and role conflict were negatively correlated(r=-.253,p<.01). Among role conflict factors, the most influential factor on job satisfaction score was 'non supportive environmental system'(61.6%), followed by 'role ambiguity'(6.6%),',lack of cooperation(4.2%),and 'lack of capability'(2.3%) factors .restrictio
