    Treatment strategy for skin and soft tissue infections caused by nontuberculous mycobacteria following various procedures

    Background The early diagnosis and treatment of skin and soft tissue infections caused by nontuberculous mycobacteria (NTM) are important, especially as infections with rapidly growing mycobacteria (RGM) are rare and difficult to diagnose and treat. Recently, we identified 22 cases of NTM infections; in this study, we suggest treatment strategies by analyzing the demographic characteristics and treatment progress of these patients. Methods A retrospective study of patients with NTM infections from 2009 to 2019 was conducted. To identify NTM infections, acid-fast bacillus (AFB) staining, Gram staining, polymerase chain reaction (PCR), and cultures of mycobacteria were performed. Empirical treatment with a combination of antibiotics or surgery was performed; species identification and drug susceptibility tests were performed by the Korean National Tuberculosis Association. The final regimen was determined after obtaining the test results. Results The mean incubation time of NTM was 4.32Β±2.88 weeks. RGM were detected in mycobacterial cultures in 21 of the 22 NTM patients. The results of AFB staining were negative in all patients, although PCR was positive for NTM in one patient. Fourteen patients were hospitalized for treatment with intravenous antibiotics and surgery. Treatment with a combined regimen of oral antibiotics was maintained for a mean of 5.41Β±1.85 months. Conclusions The unusual clinical manifestations of skin and soft tissue infections caused by NTM make them difficult to diagnose. Suspicion of NTM based on clinical presentation and a detailed examination should be followed by proper treatment involving multiple antibiotics and surgery in these patients.ope

This approach is expected as a promising therapeutic method in the future medicine by utilizing differentiation of stem cells into desired cells or tissues using biomaterial. For this regenerative medicine, there exist lots of attempts to construct optimized structures of various shapes and sizes that can regulate the stem cell fate. In this review, we will empathize the topographic effect as stem cell niche on the mesenchymal stem cell (MSC) response (cell attachment, proliferation, and differentiation) according to the shape and size of the structure of the substrates, and comprehensively analyze the importance and the effect of shape and size of the surface topography.ope

    3D CT Image Processing for 3D Printed Auricular Reconstruction of Unilateral Microtia Patient

    Purpose Microtia is congenital anomaly of external ear and the reconstruction method for the external ear of microtia patient was based on autogenous costal cartilage framework. The application of 3D printing technique in medical science has made more possibility of human tissue restoration, and we tried to apply this technique in auricular reconstruction field. Materials and Methods As for unilateral microtia patient, the contralateral side ear is normal and reconstructive surgeon tried to mimic it for reconstruction of affected ear. So, we obtained facial CT scan of microtia patient and made mirror image of normal side ear. Moreover, to make the 3D scaffold based on the mirror image of normal ear and to apply this scaffold for the auricular reconstruction surgery, we included auriculocephalic sulcus and anterior fixation part. Results We could successfully obtain mirror image of normal ear, auriculocephalic sulcus and anterior fixation part for 3D scaffold printing. Conclusions Using this CT image processing and 3D printing technique, we will be able to make the scaffold for auricular reconstruction of unilateral microtia patient, and perform auricular reconstruction in near future.ope

    Glomus Tumors: Symptom Variations and Magnetic Resonance Imaging for Diagnosis

    BACKGROUND: The typical clinical symptoms of glomus tumors are pain, tenderness, and sensitivity to temperature change, and the presence of these clinical findings is helpful in diagnosis. However, the tumors often pose diagnostic difficulty because of variations in presentation and the nonspecific symptoms of glomus tumors. To the best of our knowledge, few studies have reported on the usefulness of magnetic resonance imaging (MRI) in diagnosing glomus tumors in patients with unspecific symptoms. METHODS: THE INCLUSION CRITERIA OF THIS STUDY WERE: having undergone surgery for subungual glomus tumor of the hand, histopathologic confirmation of glomus tumor, and having undergone preoperative MRI. Twenty-one patients were enrolled. The characteristics of the tumors and the presenting symptoms including pain, tenderness, and sensitivity to temperature change were retrospectively reviewed. RESULTS: Five out of 21 patients (23%) did not show the typical glomus tumor symptom triad because they did not complain of pain provoked by coldness. Nevertheless, preoperative MRI showed well-defined small soft-tissue lesions on T1- and T2-weighted images, which are typical findings of glomus tumors. The tumors were completely resected and confirmed as glomus tumor histopathologically. CONCLUSIONS: Early occult lesions of glomus tumor in the hand may not be revealed by physical examination because of their barely detectable symptoms. Moreover, subungual lesions may be particularly difficult to evaluate on physical examination. Our cases showed that MRI offers excellent diagnostic information in clinically undiagnosed or misdiagnosed patients. Preoperative MRI can accurately define the character and extent of glomus tumor, even though it is impalpable and invisible.ope

    The influence of nanopillar structure size to fibroblast’s proliferation and alpha-smooth muscle actin (Ξ±-SMA) expression

    Backgound : With advances in medicine and materials engineering, biomaterials, such as silicone and titanium are used in many tissue regeneration cases. Many studies are being conducted to confirm the reactivity of cells to various biomaterials. Among these studies, fibroblast proliferation and differentiation are known as important factors related to capsulation. In this study, we attempted to determine how various sized polyurethane acrylate (PUA) nanopillar structures act on fibroblast proliferation and differentiation. Methods : The scaffolds were prepared in four groups: flat and nanopillar (350 nm, 500 nm, 1,000 nm). Flat group was used as a control. To evaluate the effect of scaffolds of nanopillar structures on fibroblasts, quantitatively analyzed for the degree of adhesion, proliferation and alpha-smooth muscle actin (Ξ±-SMA) expression of cells. Results : As a result of the MTT assay, the proliferation was lower than that of the flat group in the nanopillar group. As a result of staining, the intensity of F-actin was significantly different from that of the flat group. And the intensity of Ξ±-SMA was different from that of the flat group but not significantly. Conclusions : The nanopillar structure of PUA material seems to play a role in inhibiting the proliferation and differentiation of fibroblasts regardless of the size of the structure. This suggests that the use of PUA nanopillar implants may be effective in suppressing fibrosis due to fibroblast proliferation and differentiation.ope

    Diagnosis and treatment of positional plagiocephaly

    Positional plagiocephaly is increasing in infants. Positional plagiocephaly is an asymmetric deformation of skull due to various reasons; first birth, assisted labor, multiple pregnancy, prematurity, congenital muscular torticollis and position of head. Positional plagiocephaly can mostly be diagnosed clinically and by physical examinations. The simplest way to assess the severity of plagiocephaly is to use a diagonal caliper during physical examination, which measures the difference between the diagonal lengths on each side of the head. Plagiocephaly can be treated surgically or conservatively. Positional plagiocephaly, which is not accompanied by craniosynostosis, is treated conservatively. Conservative treatments involve a variety of treatments, such as change of positions, physiotherapy, massage therapy, and helmet therapy. Systematic approaches to clinical examination, diagnosis and treatment of positional plagiocephaly can be necessary and the age-appropriate treatment is recommended for patients with positional plagiocephaly.ope

    The Treatment of Trochanteric Pressure Sore Using Superior or Inferior Gluteal Artery Perforator Flap

    Purpose: Management of pressure sores has been improved, along with development of musculocutaneous flaps and perforator flaps. Nowadays, the treatment of pressure sore with perforator flaps has shown several advantages, including minimal donor site morbidity, relatively versatile flap design not only in primary cases but also in recurred cases and minimized anatomical rearrangement of regional muscle position. In this study, we report our clinical experience of gluteal perforator flap used in the treatment of a greater trochanteric pressure sore. Methods: A clinical study was performed on 7 patients who underwent total 10 operations. 1 superior gluteal artery perforator flap and 9 inferior gluteal artery perforator flaps were used to reconstruct the defect, followed by the mean observation duration of 22 months. Results: There were no total flap loss. We treated 2 cases of partial flap loss with debridement and primary repair. 2 recurred cases were successfully treated using the same method. Donor sites were all primarily repaired. Conclusion: The gluteal perforator flap could be considered as a safe and favorable alternative in the treatment of soft tissue defects in the greater trochanteric area. The advantages of the flap include low donor site morbidity and the possibility of versatile flap design not only in primary cases but also in recurred cases.ope

    Gene therapy using hepatocyte growth factor expressing adenovirus improves skin flap survival in a rat model

    Hepatocyte growth factor (HGF) is a potent angiogenic factor that can stimulate the production of blood vessels in ischemic tissue. We investigated whether gene therapy using HGF-expressing adenovirus could enhance skin flap survival. Sprague-Dawley rats were randomly divided into three groups. Rats were subdermally injected with HGF-expressing adenovirus (HGF virus group), recombinant HGF (rhHGF group), or phosphate buffered saline (PBS group) 2 days before and immediately after 3 Γ— 9 cm caudal flap elevation. The survival area of the skin flap, the ratio of blood flow, CD31-positive vessels and, VEGF expression were examined. Skin flap viability was significantly increased in the HGF virus group compared to the rhHGF and PBS groups (71.4% Β± 5.9%, 63.8%Β± 6.4%, and 39.2% Β± 13.0%, respectively) (P = 0.025). Furthermore, the blood flow ratio was significantly increased in the HGF virus group. In the HGF virus group, the number of CD31-positive vessels and vascular endothelial growth factor (VEGF) expression were significantly increased. Gene therapy using HGF-expressing adenovirus increase VEGF expression, the number of viable capillaries, and blood flow to the flap, thereby improving skin flap survival.ope

    Perforating patterns of cutaneous perforator vessels in anterolateral thigh flaps for head and neck reconstruction and clinical outcomes

    Background: Anterolateral thigh (ALT) flaps are versatile soft tissue flaps that have become the standard soft-tissue flaps used for head and neck reconstruction. They provide a long vascular pedicle, constant vessel diameter, abundant soft tissue coverage, and minimal donor site morbidity. The ALT flap was initially designed on the basis of a septocutaneous (SC) perforator. However, more recent research has shown that a substantial number of ALT flaps are now based on musculocutaneous (MC) perforators, and the ratio between MC and SC perforators varies among studies. In this study, we analyzed the perforating pattern of ALT flaps along with their clinical outcomes during head and neck reconstruction in the Korean population. Methods: From October 2016 to July 2020, 68 patients who had undergone an ALT flap procedure for head and neck reconstruction were enrolled retrospectively. The perforating pattern of the cutaneous perforator vessel (MC perforator/SC perforator/oblique branch), pedicle length, and flap size were analyzed intraoperatively. Patient demographics and flap necrosis rates were also calculated. Results: The highest number of cutaneous perforator vessels supplying the ALT flap were the MC perforators (87%). The proportion of MC perforators was significantly higher than that of the SC perforators and oblique branches. Flap necrosis occurred in seven cases (11.86%); sex, hypertension, diabetes mellitus, coronary artery disease, perforator course, and history of radiotherapy did not significantly affect flap necrosis. Conclusion: The ALT free flap procedure remains popular for reconstruction of the head and neck. In this study, we observed that the majority of cutaneous vessels supplying the flaps were MC perforators (87%). When using the MC perforator during flap elevation, careful dissection of the perforator is required to achieve successful ALT flaps because intramuscular dissection is difficult. Perforator pattern and history of radiotherapy did not affect flap necrosis.ope
