    A Case of Pure Autonomic Seizure Presenting as Sinus Tachycardia

    Tachycardia rarely consists of an ictal symptomatology associated with autonomic symptoms such as sialorrhea, even if it commonly precedes or combines with a limbic aura of complex partial seizure. A 14-year-old boy with recurrent palpitation, chest discomfort, and sialorrhea suffered from a pure autonomic seizure presenting as sinus tachycardia with a neural origin, which had been misdiagnosed as paroxysmal supraventricular tachycardia without improvement by cardiac medication. The right mesial temporal origin was revealed by video CCTV-EEG with the profile of the heart rate during ictus.ope

    Beneficial Effects of Stroke-Unit Care in Stroke Patients with Atrial Fibrillation

    PURPOSE: Continuous cardiac monitoring in a stroke unit (SU) may improve detection of atrial fibrillation (AF), and SU care may improve the rate of anticoagulation by better adherence to a standardized treatment protocol in patients with AF. We investigated the effects of the SU on the detection of AF and the rate of warfarin therapy in patients with AF. MATERIALS AND METHODS: Acute stroke patients who had been admitted before or after the opening of the SU were included in our study. SU patients were monitored continuously with electrocardiography. Rates of AF and warfarin therapy were compared between patients admitted to the SU (SU group) and those admitted to the general ward (GW) prior to the opening of the SU (GW group). RESULTS: Total 951 patients had been admitted to the GW prior to the opening of the SU (from January 2000 to November 2002), and 2349 patients to the SU (from January 2003 to December 2008). AF was found in 149 patients (15.7%) in the GW group and in 487 (20.7%) in the SU group. Most of AF detected during admission was paroxysmal AF (84.8%). The frequency of newly detected AF was higher in the SU group than the GW group (2.5% vs. 0.7%, p=0.001). The rate of anticoagulation consideration was also higher in the SU group. CONCLUSION: SU care improved the detection of AF and the rate of anticoagulation consideration in acute stroke patients. Our findings support the benefits of continuous cardiac monitoring in the SU for stroke patients.ope

    학위논문(석사) -- 서울대학교대학원 : 공과대학 항공우주공학과, 2021.8. 여재익.본 연구에서는 스크램제트 제어모델 정립을 위한 근사 1차원 연소기 해석 솔버가 구축되었다. 유체에 대한 지배방정식 및 아레니우스 식 기반의 연소모델, 연료분사모델이 솔버 내에 구현되었으며, 구축된 솔버를 바탕으로 한 해석이 수행되었다. 솔버의 검증을 위하여 0차원 점화지연 문제 및 1차원 스크램제트 연소해석 문제가 도입되었으며, 상용 수치해석 프로그램인 ANSYS Fluent의 2차원 해석 결과가 현 코드의 근사 1차원 해석결과와 비교되었다. 현 솔버가 선행 문헌의 결과들을 성공적으로 재현해 내고 있으며 근사 1차원 해석의 타당성을 확보하였다. 이어서 아음속 조건에서의 해석을 위한 Ram mode 알고리즘이 구축되었으며 열질식 위치를 통해 Ram mode에서 연소기 입구 마하수를 확정하는 해석이 수행되었다. Ram mode에서 PCST (Precombustion shock train) 해석을 위한 모델이 도입되었으며 천이구간 해석을 위한 알고리즘이 도입되었다. 또한 코드 내 Ram mode 해석의 적절성을 판단하기 위해 격리부 내 의사충격파 길이를 통해 불시동 발생 여부가 분석되었고 구축된 코드를 통해 연소기의 당량비에 따른 성능분석 연구가 수행되었다.In this work, a one-dimensional combustor solver was constructed for the scramjet control model. The governing equations for fluid flow, Arrhenius based combustion kinetics, and the injection model were implemented into the solver. In order to validate the solver, the zero-dimensional ignition delay problem and one-dimensional scramjet combustion problem were considered and showed that the solver successfully reproduced the results from the literature. The 2D analysis result of ANSYS Fluent, a commercial numerical analysis program, was compared with the quasi 1D analysis result of the current code. The current solver has successfully reproduced the results of previous literature and secured the validity of quasi one-dimensional analysis. Subsequently, a ramjet analysis algorithm under subsonic speed conditions was constructed, and a study on the inlet Mach number of the combustor was carried out through the thermal choking locations at ram conditions. In such conditions, a model for PCST (precombustion shock train) analysis was implemented, and the algorithm for transition section analysis was introduced. In addition, in order to determine the appropriateness of the ram mode analysis in the code, the occurrence of a unstart was studied through the length of the pseudo-shock in the isolator.     Clay-organic intumescent hybrid system for the synergetic flammability of polymer nanocomposites

    A polyethylene vinyl acetic acid (EVA) nanocomposite comprising of an intumescent agent that incorporates ammonium phosphate monobasic, mono-pentaerythritol, and melamine with a cationic nanoclay was set up through solution blending and melt blending to assess the fire retardancy utilizing a cone calorimeter. The results demonstrated that there was a significant reduction in the peak heat release rate of 70% contrasted with pure EVA. The fire retardancy of the clay-organic intumescent mixture framework composite was more successful than a same amount of additional nanoclay and intumescent agent.     2D Inorganic-Antimalarial Drug-Polymer Hybrid with pH-Responsive Solubility

    Artesunic acid (ASH), an antimalarial drug, has low oral bioavailability due to its low aqueous solubility. To overcome this problem, artesunate (AS) was intercalated into zinc basic salt (ZBS) via co-precipitation. AS was immobilized with a tilted double layer arrangement, which was also confirmed by XRD and 1-D electron density mapping. In order to decrease the release rate of AS under gastrointestinal conditions and to simultaneously increase the release rate of AS under intestinal conditions, ZBS-AS was coated with EU-DRAGIT L100 (ZBS-AS-L100). Finally, we performed an in-vivo pharmacokinetic study to compare the oral bioavailability of AS of ZBS-AS-L100 with that of ASH. Surprisingly, it was found that the former is 5.5 times greater than the latter due to an enhanced solubility of AS thanks to the ternary hybridization with ZBS and EUDRAGIT L100. Therefore, the present ZBS-AS-L100 system has a great potential as a novel antimalarial drug formulation with pH selectivity and enhanced bioavailability

    Molecular orientation of intercalants stabilized in the interlayer space of layered ceramics: 1-D electron density simulation

    In this review, an attempt is made to calculate one-dimensional (1-D) electron density profiles from experimentally determined (00l) XRD intensities and possible structural models as well in an effort to understand the collective intracrystalline structures of intercalant molecules of two-dimensional (2-D) nanohybrids with heterostructures. 2-D ceramics, including layered metal oxides and clays, have received much attention due to their potential applicability as catalysts, electrodes, stabilizing agents, and drug delivery systems. 2-D nanohybrids based on such layered ceramics with various heterostructures have been realized through intercalation reactions. In general, the physico-chemical properties of such 2-D nanohybrids are strongly correlated with their heterostructures, but it is not easy to solve the crystal structures due to their low crystallinity and high anisotropic nature. However, the powder X-ray diffraction (XRD) analysis method is thought to be the most powerful means of understanding the interlayer structures of intercalant molecules. If a proper number of well-developed (00l) XRD peaks are available for such 2-D nanohybrids, the 1-D electron density along the crystallographic c-axis can be calculated via a Fourier transform analysis to obtain structural information about the orientations and arrangements of guest species in the interlayer space. © 2014 Hitotsubashi University
