보건간호학과/석사[한글] 암환자들이 겪고 있는 두려움에는 죽음에 대한 걱정, 가정생활의 곤란, 경제적 압박, 현재와 미래의 일에 대한 불확실성, 제어할 수 없는 통증들이 있다. 특히 암으로 인한 통증은 만성적인 성격을 갖고 있으므로, 지속적으로 통증관리가 되지 못하는 경우에는 통증에 대한 두려움과 고통에 시달리게 되고, 통증조절도 스스로 해결할 수 없다는 무력감 때문에 좌절하게 되어 삶이 의욕을잃게되기 쉽다. 따라서 암환자의 통증은 원인이 적절히 사정되고 예방적인 접근이 취해지지 않는다면, 통증조절이 효과적으로 이루어지지 않게되며, 환자는 이로인해 많은 고통을 경험하게 된다. 실제로 현대의 다양한 치료법과 의약수단으로 완전한 고통의 제거나 혹은 일상생활 등 불편없이 살아가도록 도와줄 수 있음에도 불구하고, 많은 암환자들이 경감되지 않은 통증으로 고통을 받고 있는데, 이는 많은 부분이 의료인의 통증완화를 위한 중재법에 대한 지식부족과 통증에 대한 인식부족에 기인한다. 따라서 이 연구는 암환자의 통증관리 실태 및 과정과 간호사, 의사의 통증관리 및 마약성 진통제 사용에 대한 지식과 태도를 조사하여 통증관리 현황과 문제점을 파악함으로써, 합리적이고 체계적인 통증관리방법을 모색하는데 도움이 되는 기초자료를 마련하기 위하여 시도 되었다. 연구대상은 서울시내 일개 종합병원 암병동에 입원한 통증이 있는 암환자 31명과 암병동에 근무하는 간호사14명, 의사 6명을 대상으로 하였다. 연구도구는 통증사정, 중재에 대한 간호과정을 파악하기 위하여 의무기록지 검토, 관찰, 면담을 실시하였고 Watt-Watso n이 개발한 통증관리에 대한 지식 측정 도구를 이용하여 통증관리에 대한 지식과 태도를 조사하였다. 자료수집기간은 1995년 3월 16일부터 4울 30일까지 였으며,자료분석은 의무기록지 검색으로 수집된 자료는 사정, 중재, 평가에 관련된 내용으로 분류한 후 분석하였으며, 통증관리에 대한 빈도와 백분율 분석을 하였다. 연구결과를 요약하면 다음과 같다. 1. 통증사정은 간호사의 경우 특정한 사정도구없이 환자가 서술한 것을 토대로 기술 하고 있어 포괄적으로 간호기록지에 정확히 서술되지 않고 있었다.. 의사들의 경우에는 통증의 부위와 유무를 간단히 기록하는 것 외에는 통증에 대한 기록이 없었고, 통증 양상, 강도,부위 등에 대한 계속적인 사정기록이 없어 정확하게 통증정도를 측정하지 못하였으며, 계속적인 사정이 이루어지지 않았다. 2. 통증중재는 통증관리에 대한 체계적인 계획없이 상황에 따라 관리되고 있었으며 약물을 이용한 중재방법에서 통증조절 약물은 대부분 24시간 지속적으로 투여되었으나 잘 조절되지 않았으며, 정규액물 투여시간 이외에 통증을 호소하는 경우 산발적으로 Demero l, Nubain, Tridol 등을 일시적으로 투여하였다. 약물이외에 보조적인 통증완화법으로 간호사들은 8명에게 더운 물주머니를 배나 허리등 통증이 있는 부위에 해 주었고 쑥찜팩1명, 복대를 1명에게 해 주었는에 이는 대부분 환자의 요구에 의해 이루어졌다. 3. 통증경감 중재에 대한 평가는 약물의 효과에 대한 정확한 평가와 규칙적이 평가가 이루어지지 않고 있었으며, 마약성 진통제는 반드시 발생하는 변비와 같은 부작용이 있음에도 불구하고 부작용에 대한 적절한 사전교욱이 행해지지 않았고 수동적으로 환자가 호소하는 경우에만 그에 반응하였다. 4. 의사와 간호사의 통증에 대한 지식은 총 13점 중 평균 6.8점으로 52.5%의 정답률을 보였으며, 의사와 간호사의 마약성 진통제 사용에 대한 지식은 총 19점중 평균 11.5점으로 60.4%의 정답룰을 보여 의사 및 간호사가 통증지식 및 마약성 진통제 사용에 대한 지식이 낮은 것으로 나타났다. 결론적으로 병동에서 암환자의 통증관리가 계획적으로 이루어지지 않고 있으며, 간호사와 의사의 통증사정 및 중재방법에 대한 지식 부족, 환자의 진통제 사용에 대한 잘못된 지식, 부정확한 통증사정으로 인해 효과적으로 통증관리가 이루어지지 않고 있음을 알 수 있었다. 그러므로 보다 효과적인 통증관리를 위해 통증사정 도구의 이용과 게획성 있는 사정, 수행, 평가 및 약물 이외의 통증완화법을 사용해야 할 것이다. [영문] Even with the development of modern medical technologies, many cancer patients suffer from unrelieved pain. It's mainly related to a knowledge deficit in pain management and the misconceptions about pain control. This study was done to describe the process of pain management in cancer patient unit. Data collection was done by the patient charts review, interview with nurses, patients, and a pain knowledge questionnaire. The subjects were cancer patients who had pain and nurses and doctors who worked on cancer patients unit in teaching hospital. The number of subjects was 31 cancer patients, 14 nurses, and 6 doctors. Data were collected from March 16, 1995 to April 30, 1995. Analysis of the data was done using descriptive statistics. The results of this study are as follows: 1. Pain assessment by nurses was only documented by the patients' descriptions of their pain without any specific questions or measurements. Physicians' notes only recorded the pain site. And there was no continuous assessment about pain related to degree, site or pattern. 2. Pain management was done only when the patient complain pain without any specified plan. 3. In the case of pain control with analgesics, pain was nat well relieved because the type and dose of analgesics were not well considered for each case. But in some cases, continuous analgesic infusions were done even though dose were not high enough to control the pain. Additional noninvasive methods to relieve pain were hot packs, mugwort hot packs and abdominal bandage when patient requested. 4. There were no systematic evaluations to the effects of the analgesics on the pain and little preventive measure for the side effect in the analgesics. 5. Overall knowledge levels about concer pain management in nurses and doctors shwed only a 52.5% accuracy rate about knowledge of pain and a 60.4% accuracy rate about knowledge of analgesics. In conclusion, it was found that cancer pain management was not done effectively in cancer patient unit. The reasons can be sen to be caused by firstly, lack of systematic pain assessment, planning, implementation and evaluation and, secondly, ignorance and misconception toward pain management by patients, nurses and doctors.restrictio
