(A) study on the level of medical record documentation and agreement in the information on the patient's past history

병원행정학과/석사[한글] 이 연구는 환자 과거력 정보의 각 항목별로 작성되어야 할 기재수준을 설정하여 이 기준에 근거하여 실제 의무기록을 검토함으로써 그 완전성을 분석하고, 서로 다른 기록자 집단이 동일한 정보를 각자 기록하는 서식 간 내용의 일치도를 통해 그 신뢰성을 분석하고자 하였다. 2004년 연구대상병원에 위암으로 최초 입원한 403명의 인턴 기록, 전공의 기록, 간호사 기록의 과거력 정보를 대상으로 하여 기재여부, 기재정도, 연구대상병원의 기재 기대수준 충족정도, 내용 일치도를 분석하였다.분석 결과는 다음과 같다.첫째, 각 항목의 기재율은 주소 및 현질병상태, 과거병력은 높았고, 입원력, 알레르기, 투약력은 낮았다. 알레르기의 경우 전공의 기록에는 거의 기재되고 있지 않았고, 투약력은 세 서식 모두에서 작성되지 않아 그 내용을 알 수 없는 경우가 10%에 달하였다. 기록자 집단과 입원과에 따라 항목별로 기재율에 큰 차이가 있었다.둘째, 과거력 항목의 기재정도를 각 항목별로 ‘기재정도 1’에서 ‘기재정도 4’까지 구분하여 1점에서 4점으로 점수화 하였다. 그 수준을 측정할 결과 입원력, 과거병력, 가족력이 ‘3 점 대’였고 흡연력, 투약력, 주소 및 현질병상태, 음주력, 알레르기는 ‘2 점 대’를 보였다. 작성자 집단에 따라 항목별 기재정도의 수준에 차이가 있었으며 입원과별로는 내과계는 인턴 기록이, 외과계는 전공의 기록이 우수하였다. 연구대상병원의 기록자 집단과 입원과에 따른 업무절차의 차이가 기재정도에 반영된 결과를 보였다. 셋째, 기재형태에 따른 기재율과 기재정도에 있어서는 기재율은 체크-서술 혼합형태가 높았으나, 내용의 기재정도는 완전서술형태가 우수하였다.넷째, 연구대상병원의 의무기록 위원이 정한 과거력 정보의 항목별 기재 기대수준과 실제 기록의 기재정도를 비교하여 그 충족정도를 본 결과 가족력이 제일 높고 다음은 입원력, 흡연력, 과거병력, 음주력, 투약력, 알레르기, 주소 및 현질병상태 순이었다. 기재 기대수준이 ‘기재정도 4’로 상세하게 기재될 것을 요구하는 항목인 과거병력, 알레르기, 주소 및 현질병상태의 기대수준 충족율은 다른 항목보다도 저조하였다.다섯째, 환자 입원 시 세 기록자 집단이 면담을 통해 각자 기록하는 인턴 기록, 전공의 기록 및 간호사 기록의 과거력 정보를 ‘기재정도 1’의 해당사항 ‘있음’ 또는 ‘없음’ 수준에서 Kappa 계수로 일치도를 분석하였다. 전공의 기록-간호사 기록의 일치도와 인턴 기록-전공의 기록의 일치도는 ‘아주 좋음’에서 ‘보통’의 수준이었고, 간호사기록-인턴 기록의 일치도는 ‘약간 좋음’에서 ‘나쁨’까지의 수준이었다.과거력 정보의 완전성과 신뢰성 수준은 기록자 집단과 입원과별로 차이를 보였다. 기록자 집단과 입원과에 따라 행해지는 진료과정의 절차가 의무기록의 질에 반영되는 결과를 보여, 의무기록 작성의 과정과 기록의 질에 관한 상세한 연구가 필요하였다. 양적으로 높은 기재율을 나타내는 항목이 기재정도는 낮아 의무기록에 기재된 내용을 질적으로 점검하고 평가할 수 있는 도구의 개발이 요구되었다. 또한 기재정도가 상세하여야 하는 항목의 기재수준은 오히려 낮은 결과를 보여 임상과별 특성에 맞춘 구체적인 의무기록 작성에 대한 지침과 교육이 필요하였다. [영문]This study was conducted to evaluate the quality in medical records by analyzing its completeness through setting up the level of record on the patient''s past history and through examining the actually medial records based on this standard, and by analyzing its reliability through the level of agreement in the contents between records that mutually different recorder groups do record the same information, respectively. Targeting the information on the patient''s past history in interns'' records, residents'' records and nurses'' records toward 403 inpatients who were admitted first in 2004 at university hospital due to stomach cancer, it analyzed the whether or not recording, the recording level, the satisfaction with the expectant level of the records in the hospital targeted for a research and the level of agreement .The results are as follows.First, as for the rate of recording each item, it was high in the chief complaint & present illness and the past illness history, and low in the admission history, allergy, and the medication history. Depending on a group of recorders and on the admission department, the recording rate showed big difference by item. This results showed that important items were different by a group of recorders and by the admission departmentSecond, as a result of measuring the level after dividing the recording level of items for the patient''s past history from Level 1 to Level 4 by each item, the admission history, the past illness history, and the family history were about Level 3, and the smoking history, the medication history, the chief complaint & present illness, the drinking history and allergy were about Level 2. By admission department, it was excellent in the interns'' records for the medical department, and in the residents'' records for the surgical department.Third, as a result of analyzing the recording rate and the recording level in contents depending on a recording form, the recording rate was high in the mixed form of check and description, but the recording level in contents was excellent in the completely descriptive form.Fourth, as a result of its satisfactory level by comparing the expectant level of a record and the actual record by item in information on the patient''s past history, which was expected by the medical-record committee members of the hospital targeted for a study, it was low in the satisfactory rate of the expectant level in the patient''s past history , in allergy, and in the chief complaint & the present illness, which are items of requiring to be recorded in detail as Level 4 in the expectant level of a record.Fifth, it analyzed the level of agreement with Kappa score in the level of ‘Yes’ or ‘None’ related to the corresponding matter in Level 1, in terms of information on the past history in the intern''s record, the resident''s record, and the nurse''s record. The level of agreement in the resident''s record & the nurse''s record, and in the intern''s record & the resident''s record was from "excellent" to "a little good", and the level of agreement in the nurse''s record & the intern''s record was from "a little good" to "poor".There was difference in the level of completeness and in reliability for the information on the past history by recorder group or by admission department. The encounter process that was performed by the admission department or the recorder group, indicated the result that was directly reflected on the quality of medical records, thus it was required further study about the medical record documentation process and quality. The items that showed the high recording rate quantitatively were rather low, thus it was required the development of tool for the qualitative inspection and evaluation in the medical record. And the items which were needed to be detailed the record level were rather low, thus there needed a documentation guideline and education by clinical department.ope
