북한과 한국의 핵개발 정책결정요인 비교분석

학위논문 (석사)-- 서울대학교 행정대학원 : 정책학과, 2014. 2. 노경수.요약(국문초록) 동 연구 마무리에 한창이던 지난 2013년 11월 24일, 제네바에서 P5+1국가들(미국, 러시아, 중국, 영국, 프랑스, 독일)과 이란이 이란 핵문제 해결을 위한 초기 단계에 합의하여 이란이 우라늄 농축과 중수로 활동을 동결하고 IAEA(국제원자력기구)에 의한 검증을 강화하기로 하였다는 보도를 보았다. 이란의 핵문제가 해결됨으로써 이제 국제사회에서 미결의 핵문제는 한반도, 즉 제2차 세계대전 이후 냉전의 파생으로 분단된 한반도의 반쪽, 조선민주주의인민공화국에만 남게 된다. 동 연구는 북한 핵문제 해결을 위한 연구의 일환으로 북한 핵개발 정책의 결정요인 분석에 주목하는데, 그중 제반 선행연구에서 간과하고 있었던 과거 한국 정부의 핵개발 정책과 북한의 핵개발 사례를 비교분석함으로써 북한의 핵개발 정책결정에 있어 주요결정요인이 무엇이었는가를 규명해 보고자 한다. 20여년의 격차를 두고 국가정책으로 가시화되었던 한국과 북한의 핵개발 정책결정에 대한 주요결정요인 분석은 동 결정요인의 해결이 바로 현 북한 핵문제 해결의 시작이라는데 그 연구의의가 있다. 한국 박정희 정부가 추진한 핵개발 정책과 북한 김일성, 김정일 정부가 추진한 핵개발 정책은 Waltz의 신현실주의 시각이 말하는 위협에 대한 자국 안보 확보가 그 정책적 목표였다는 점에서 공통점이 있다. 그러나 차이점은 바로 양국의 핵개발 정책결정에 대한 위협의 정도가 상이하다는 것이다. 한국의 경우 비록 1960~70년대 데탕트라는 국제체제변수의 영향이 있기는 하였으나 당시 한국이 자국의 안보를 한미동맹에 거의 전부 의지하고 있던 상황에서 동맹관계변수는 체제변수를 능가하는 효과가 있었다. 따라서 동맹이 국제체제변수의 영향을 상쇄할 수 있는 것이다. 문제는 존슨행정부, 닉슨행정부, 포드행정부 시기 데탕트로 인한 양국의 위협 인식에 대한 불일치와 더 나아가 카터행정부 시기 인권과 민주주의라는 정치적 가치를 둘러싼 박정희 대통령과 카터대통령 사이의 개인적 갈등이 동맹변수에 있어 위협으로 대두하였다는 점이다. 박정희 정부는 동맹국 미국에 대한 배신감과 더 이상 미국에 안보를 의지할 수 없다는 인식하에 이러한 위협에 대한 '자국 안보 추구의 방법으로 자주국방 정책을 추진하게 된 것이다. 그리고 동 자주국방정책의 최종 목표는 당시 동맹국 미국이 한국에게 제공하고 있던 핵우산의 대체물, 즉 핵무기 보유였다. 북한의 경우는 동맹관계변수 보다는 체제변수 차원의 위협이 핵개발 정책결정에 있어 가장 큰 요인이었다. 한국에 비해 북한은 중국과 소련 사이에서 시소외교를 펼치면서 동맹변수에 자국의 안보를 전부 의지하고 있지 않았던바, 동맹변수의 영향은 1980~90년대 탈냉전이라는 체제변수의 영향을 능가하지 못하였다. 북한은 사실 제2차 세계대전 이후부터 핵무기에 대한 정책적 관심을 가지고 있었는데, 1980~90년대 경제난과 국가체제 유지, 정권이양이라는 국내정치경제환경요인 하에서, 체제변수 차원에서 전개되었던 탈냉전이라는 위협에 대응하기 위해 핵개발 정책결정을 가시화하게 되는 것이다. 공산주의권이 해체되고 소련과 중국이 사실상 공산주의를 포기하고 한국과 수교를 맺는 상황은 북한 김일성, 김정일에게는 자신들이 아는 세계관 자체가 무너진 것과 같은 상황이었으며, 이러한 국가의 근본이념과 최고지도자들의 세계관 자체가 무너지는 위협에 직면하여 북한은 안보확보 차원에서 핵개발을 추진하게 되는 것이다. 동 연구는 상기와 같은 한국과 북한의 핵개발 정책결정에 대한 주요결정요인을 설명함과 더불어, 정책결정과정 측면에서 핵개발 정책결정에 대한 주요 정책결정행위자를 살펴보았다. 한국의 경우 박정희 대통령이라는 최고지도자의 영향력이 절대적이었으며, 상대적으로 군부는 박정희 대통령의 군부분할통제방식으로 인해 국가정책에 대한 영향력이 제한되었다고 보았다. 또한 군부 일부에서는 박정희 대통령의 핵개발 정책에 대한 반대의견도 제시하는 등 동 정책결정에 있어 군부의 참여는 예상보다 제한적이었다. 외부행위자 미국도 한국의 핵개발 정책결정에 있어 주요행위자이다. 박정희 정부의 핵개발 정책은 미국과의 상호반응(feedback)을 통한 정책결정이라는 측면이 존재하기에, 미국이라는 주요행위자를 결코 간과할 수 없다. 북한의 경우는 김일성의 유일체제 당시는 한국과 같이 최고지도자의 정책결정력이 강력하였으나, 김정일로의 정권 이양과 탈냉전 시기에는 경제난 극복을 위한 사회내부통제 필요성에서 군부의 역할이 커지게 된다. 이는 북한이 대내정책으로 선군정치를 공개적으로 표방하고 있고, 김정일 역시 국가주석이 아니라 국방위원장의 직함으로 최고지도자의 역할을 수행하였다는 점에서도 증명된다. 한편, 한국의 경우와는 달리 북한의 핵개발 정책결정과정에 외부행위자를 포함하기는 어렵다. 북한에게는 미국과 같은 전적인 안보제공자가 존재하지 않았기 때문이기도 하다. 따라서 양국은 위협에 대한 대응이라는 정책결정요인 측면 뿐 아니라 정책결정과정상 주요행위자 측면에서도 차이점을 보이고 있음을 알 수 있다. 동 연구를 통한 북한 핵문제에 대한 해결책은 결국 탈냉전 하 국가생존의 위협을 느끼고 있는 북한에게 북한의 국가체제를 보장함으로서 핵개발 정책의 결정요인을 해소하고 또한 이러한 해결책을 북한 최고지도자 및 군부와 협상해야 함을 암시하고 있다고 할 수 있는데, 이는 여타 다른 가치의 문제, 다른 방향의 정책문제를 수반한다. 즉, 북한을 정상국가로 인정할 것인가, 북한을 정상국가로 인정함으로써 현재의 한반도 분단 상태를 영원히 고착시킬 것인가 하는 정책문제들이 제기되는 것이다. 이들 문제에 대한 입장을 먼저 결정하는 것이 바로 북핵문제 해결을 위한 대전제라는 점을 제시하면서, 동 정책문제와 관련된 또 다른 연구는 향후 별도의 연구과제로 남겨둔다.Abstract Comparative Study on the Decision Factors in North Korea's Nuclear Policy in the 1990s and the Factors in South Korea's Nuclear Policy in the 1970s Suh, Minjeong Public Policy Major Graduate School of Public Administration Seoul National University In a long way to resolve the North Korean nuclear crisis, overwhelming volumes of research have been conducted in regards to Pyongyangs continuous nuclear driveyet, the academia has largely overlooked South Koreas own nuclear policy during the 1970s. North and South Koreas launched their nuclear programs in a time-gap of approximately 20 years, respectively. What led to both governments on the Korean Peninsula to initiate their nuclear policies? What factors have made not only North Korea, a repressive authoritarian regime, but also the democratic South to pursue nuclear armament? It should be noted that while South Korea has eventually given up its nuclear policy, the North continues to hold fast to the position that its nuclear program is non-negotiable. Indeed, Pyongyang continues to pursue nuclear weapons regardless of tremendous pressure and crippling economic sanctions from the international community. If this difference can be sufficiently explained, I may argue that such explanation could provide an insightful hint in resolving North Korea's nuclear problem. To analyze the similarities and differences between the two Koreas' nuclear policies, this study aims to find out what was the most crucial factor in Seoul and Pyongyangs decision-making for their nuclear programs. In conclusion, as Kenneth Waltzs Neo-realism theory suggests, both Koreas sought to ensure their national security against potential threats by acquiring nuclear weapons. Clearly, dissimilarities exist between the factors that led Seoul and Pyongyang to initiate the policy of nuclear armament. For example, in the era of "détente" between the Free World and the Communist bloc in the 1960s and 70s, South Korea perceived that perception gap between U.S. and South Korea on external threat was another threat for the South. Further, the bigger threat lies in the so-called "alliance factor" on the R.O.K.-U.S. Bilateral Alliance. If the bilateral ties between the two countries had been solid, the alliance would have been sufficient to defend Seoul not only from the shifting international environment under détente, but also from the constant military threats from North Korea. However, South Korea and the U.S. underwent a crisis in their alliance during the 1970s, first due to the Nixon Doctrine, and finally due to personal disagreements between South Korean President Park Chung-hee and his American counterpart President Jimmy Carter on their individual values. On the other hand, the biggest threat to the North Korean regime in the 1990s was the newfound international system of Post-Cold War framework per se. All other communist countries disappeared on the earth only except for the North. The whole new world was ever first chapter for North Korea in its history. Further, North Korea did not fully rely on its alliance with the Peoples Republic of China(PRC) or with the Soviet Union in a way to fully ensure its national security. North Koreas lack of external security guarantee was a big difference from the situation of South Korea. Facing the demise of the entire communist bloc, North Korea did not have the diplomatic leverage to counterbalance the threat from the shifting international system, and as a result, Pyongyang had no choice but to initiate its nuclear policy to ensure its own national security. This study also analyzes the roles of main actors behind the decision-making of Seoul and Pyongyangs nuclear drive. In the case of South Korea, President Park held absolute power and authority over both policy-making and policy-implementation, while the South Korean military was restricted in action due to the Presidents "divide-and-control policy" over the men-in-uniform. Also, the United States took a significant role in South Korea's nuclear policy by exerting influence and pressure on the Park administration. In North Korea, the ultimate authority lies in the hands of the head-of-state, but throughout the era of Kim Jong-Il, the Korean Peoples Army(KPA) enlarged its role to the sphere of policy making under the banner of "Military-First Politics." Unlike the case of South Korea, the North did not experience significant influence or pressure from external actors, which could have reined in the regimes aggressive drive for nuclear armament. After all, this study suggests that, in order to resolve the North Korean nuclear crisis, Pyongyang needs to be secured against the threats that it perceives from the international community. Yet, the suggestion above raises additional policy issue that North Korea should be recognized as a normal state. Comparison of Job Performance, Job Satisfaction and Job Stress of Child Health Nurse Practitioners by Roles in the Work Place

PURPOSE: Child Health nurse practitioners (CHNPs) in Korea have important roles in disease management and health promotion for children and adolescents. Yet, practices of CHNPs licensed and employed in hospitals have not been adequately identified. Thus, in this study the scope of practice by CHNPs and job satisfaction and stress were investigated and compared according to the CHNPs' position in the working place. METHODS: A descriptive cross-sectional survey design was used. All 53 licensed CHNPs participated in the mail survey which included a 71-item questionnaire on job performance and job satisfaction and a job contents questionnaire. Descriptive statistics, chi2 test, independent t-test and Mann-Whitney U test were used to compare CHNPs employed as nurse practitioner (NP) and CHNPs employed as staff nurses. RESULTS: Compared to CHNPs employed as staff nurses, CHNPs employed as NPs more frequently provided education, environment management, coordination and research in their practice areas. No significant difference was found in job satisfaction between the two groups except for the administration and income subdomains. Only the physical exertion subdomain in job stress was stressful to CHNP employed as staff nurse. Conclusions: Job performance of CHNPs in Korea needs to be revised to include more practical practice in education, coordination, and research related areas. Experience of Treatment Adherence in Korean Patients with HIV

Purpose: The purpose of this study was to explore the subjective experience of Highly active Antiretroviral therapy (HAART) adherence in Korean patients with HIV. Methods: A phenomenological methodology was used for the study. Data were collected from March to December, 2013 using open-ended questions during in-depth interviews. Participants were taking HAART for HIV, and were contacted through purposive techniques. Results: Four men and 4 women whose average treatment period was 5.9 years participated in this study. Seven themes emerged from the analysis using Colazzi's method: (a) Seizing life in a deep sense of despair, (b) Struggling with medication, (c) Facing harsh treatment from health care providers, (d) Spoiling healthy pattern, (e) Hungering for communication and emotional support, (f) Turning to accepting HIV, (g) Becoming adherent to HIV treatment. Conclusion: Prejudice from health care providers and lack of emotional support were barriers to HAART in Korea. Intervention strategies are needed to decrease prejudices from health care providers and to increase family support.ope

A Study on Requirement of Suicide for Occupational Accident: Focusing on State of Mental Disorder

본 연구에서는 우리나라 산재보험법이 자살의 업무상 재해 인정요건으로 명시한 ‘정신적 이상 상태’가 갖는 의미를 살펴보고, 자살의 업무상 재해를 판단하는데 ‘정신적 이상 상태’라는 요건의 필요성 여부를 알아보고자 시행하였다. 법원의 정신적 이상 상태 판단 경향은 초기에는 1993년부터 최근까지 4단계로 변화하였다. 1단계는 심신상실 내지 정신착란 상태만 인정했지만, 다음에는 정신병적 증상이 있는 경우를 인정했다. 이후 정신질환의 병력이 없는 경우에도 합리적인 판단을 기대할 수 없을 정도의 상황이라면 정신적 이상 상태를 인정하였고, 최근에는 극심한 업무상 스트레스가 있다면 판단력 상실이라고 보기 어려운 경우에도 업무와 자살 사이의 상당인과관계를 인정하였다. 이제까지 연구자들도 자살의 동기와 업무와 상당인과관계가 있다면 정신적 이상 상태가 없더라도 업무상 재해를 인정해야 한다는 점에 의견이 일치하였다. 본 연구에서 실시한 설문조사에서도 전체 75% 이상이 정신적 이상 상태가 없더라도 업무상 재해를 인정할 수 있다고 하였다. 프랑스나 벨기에, 독일과 같은 유럽국가에서도 업무 관련성이 있으면 자살의 업무상 재해가 인정되고, 정신적 이상 상태 여부가 그 전제요건이 되지 않는다. 이러한 결과를 바탕으로 볼 때, 정신적 이상 상태를 업무상 재해 인정요건으로 두어 사후에 검증할 것이 아니라 재해자의 업무와 자살 사이에 상당인과관계가 있는지 여부를 기준으로 업무상 재해를 판단해야 한다. 따라서 정신적 이상 상태를 자살의 업무상 재해의 인정요건으로 명시한 산재보험법 시행령 제36조는 개정이 필요하다. This research was done to examine the meaning of 'state of mental disorder' stipulated as requirement for occupational accident in dealing with suicide in the Industrial Accident Compensation Insurance Act in Korea, and assess the necessity of the requirement called 'state of mental disorder' in deciding whether a certain suicide comes under occupational accident or not. The trend of judgement by Korean courts has changed four times since 1993 regarded as the first one regarding. First, the suicide caused only by insanity or mental derangement was accepted as satisfying the requirement. Second, the case where symptoms of mental disease was accepted. Third, even if the person who committed suicide did not have mental disease, if it is difficult to expect rational judgement from the situation where the person committed suicide, the case was accepted as satisfying the requirement. Forth, recently, if there is reason to believe that the suicide happened under severe work stress of the person, courts recognize the relationship between work and suicide, even if it is difficult to view that the suicide was caused by loss of judgement. Researchers have also agreed that if suicide is considerably related with suicide, the suicide should be admitted as occupational accident. In the survey to related specialists conducted by this research as well, 75% of respondents view that suicide without mental disorder can be admitted as satisfying the requirement of occupational accident. In European countries like France, Belgium, and Germany, if a suicide case is related with work, it is recognized as occupational accident, and state of mental disorder is not precondition of it. Based on such findings, we can agree that occupational accident should be decided on the criteria of whether there is considerable causal relationship between the person's work and his or her suicide, rather than trying to establishing state of mental disorder as requirement for occupational accident, and trying to test whether a suicide case satuisfies the requirement afterwards. Therefore, Article 36 of the Enforcement Decree of the Industrial Accident Compensation Insurance Act in Korea should be revised.open석

Burnout and coping type of oncology nurse.

간호학과/석사[한글]본 연구는 암환자를 간호하는 간호사의 대처유형에 따른 소진을 파악하는 서술적 조사연구로서, 긍정적이고 적절한 대처 방법을 사용하여 소진의 요인이 되는 스트레스, 갈등 상황, 위기 상황을 완화시키는데 필요한 기초자료를 제공하고자 한다. 본 연구의 예비조사를 포함한 자료수집은 서울시에 소재한 3개 대학부속 병원에서 2000년 10월 10일부터 10월 19일까지 암환자, 특히 말기 암환자가 많은 병동에 근무하는 9개월 이상 근무한 간호사를 대상으로 내과, 외과, 중환자실, 산부인과, 암센터(호스피스 병동)를 선정하여 130명에게 질문지를 배부하여 조사하였으며 5개 이상 항목에서 답하지 않는 질문지를 제외하고 106개의 자료를 분석하였다. 본 연구에서는 암환자를 간호하는 간호사의 소진 정도를 측정하기 위하여 Pines(1981)이 개발한 소진 척도를 픽은희(1983)가 번역하여 수정, 보완한 측정도구를 사용하였으며, 암환자를 간호하는 간호사의 대처유형 측정도구는 Lazarus와 Folkman(1983)이 개발한 것을 한정석, 오가실(1990)이 번역하여 수정, 보완한 도구를 사용하였으며 자료분석은 SPSS/PC를 이용하여 전산통계 처리하였다. 연구결과는 다음과 같다. 1. 암환자를 간호하는 간호사의 소진과 대처유형의 항목별 평균을 분석한 결과 신체적 소진(2.89)과 긍정적 관점(2.94)을 가장 많이 사용하는 것으로 나타났다. 소진합은 평균 2.72점, 대처유형합은 평균 2.76점으로 이는 임상 간호사를 대상으로 한 조영순(1999)의 연구결과 소진합 2.34, 대처유형합 2.63보다 높게 나타났다. 2. 암환자를 간호하는 간호사의 일반적 특성에 따른 소진정도의 차이를 검증한 결과, 종교가 있는 경우, 신앙이 차지하는 비중이 클수록, 근무번이 고정번일 수록, 간호직을 이상적인 직업으로 생각할수록, 근무 병동에 대한 만족도가 높을수록, 연령이 높을수록, 간호직에 종사한 총경력이 길수록 소진경험이 낮은 것으로 나타났다. 3. 암환자를 간호하는 간호사의 일반적 특성에 따른 대처유형 중 종교에 따른 전반적인 대처유형의 정도에서만 통계적으로 유의한 차이가 있는 것으로 나타났으며(F=3.26, p<.05), 추후 검정 결과 천주교 집단이 타종교 집단과 비교하여 차이를 보였고 대처유형을 많이 사용하고 있는 것으로 나타났다. 4. 암환자를 간호하는 간호사의 대처유형 중 무관심의 영역은 정서적 소진 영역에서 유의한 상관관계가 있다고 나타났다(r=-.303, p<.01). 대처유형의 사회적지지 탐색 영역과 긍정적 관점 영역에서는 정서적 소진(r=-.300, p<.01), 정신적 소진(r=-.264, p<.01)에서 긍정적 관점에서는 정서적 소진(r=-.264, p<.01), 정신적 소진(r=-.219, p<.05)에서 상관관계가 있다고 나타났다. 5. 암환자를 간호하는 간호사의 소진 경험과 대처유형의 영향요인으로는 신앙이 차지하는 비중이 8.8%로 나타났고, 여기에 대처유형 중 무관심 영역과 긍정적 관점가 추가될 경우 갖는 설명력은 총 17.6%였다. 이상의 결과를 종합해 볼 때, 암환자를 간호하는 간호사들은 신체적 소진을 가장 많이 겪는 것으로 나타났으며 종교가 중요한 변수로 작용하고 있음을 알수 있다. 이는 종교가 주는 철학적, 윤리적 기준이 간호활동시 일어날 수 있는 갈등 상황에서 판단의 기준을 만들어 주고 있으며 지지체계의 역할을 하기 때문인 것으로 사료된다. 그러므로 종교가 없는 간호사들에게도 암환자를 간호하면서 발생할 수 있는 여러 문제와 갈등 상황에서 바른 인식과 대처를 할 수 있도록 간호사를 도울 수 있는 적절한 교육이 필요하다고 생각된다. [영문]The purpose of this study was to identify burnout according to coping type in oncology nurses. This study was designed on the basis of cross-sectional and descriptive approaches. One hundred thirty nurses participated in this study from October 10-19, 2000. These participants were from three University Teaching Hospitals in Seoul, Korea with at least 9 months experience on oncology units. 114 data was collected but only 106 data was analyzed. The research instruments were a questionnaire to gather demographic data, a Burnout scale developed by Pines (1981) and translated by Pick, Eun Hee(1983), and a Coping type scale developed by Folkman & Lazarus (1983) and revised by Han, Chung Seock and Oh, Ga Sil(1990). Data was analyzed using the SPSS program. The obstetrics nurses' average burnout degree was 2.72 out of 5 and total coping degree was 2.76 out of 4, this is higher compared with the result of Cho, Young Sook (1999). Physical burnout (2.89) and a positive-view coping type(2.94)is the highest subcategory in each. According to demographics, religion, working a fixed time, a high satisfaction in the ward, old age, and experience contribute to a lower burnout rate. Catholics have a higher coping score than other religions, this suggests that they use many kinds of coping type. Emotional burnout (r=-.303, p<.01)is higher in nurses who have more compassion. A searching-for-social support coping type and positive-view coping type are related to emotional burnout (r=.300, p<.01; r=.264, p<.01 respectively) and mental burnout(r=.264, p<.01; r=.219, p<.05 respectively). In conclusion, oncology nurses mainly undergo physical burnout, yet religion is a significant factor in the type of burnout and coping. It is inferred that philosophical and ethical principle is influence on decision making in nursing conflict situations on caring for cancer patients. 생애말기 암환자의 의료적 의사결정에 대한 문화기술지

간호학과/석사현재 우리나라 생애말기 의료결정의 가장 중요한 화두는 환자가 배제된 채 의사결정이 진행된다는 것이다. 이에 대한 구조적, 문화적 배경과 원인을 알기 위해서는 생애말기 암환자의 의료적 의사결정 과정에서 환자, 가족 및 의사와 간호사의 상호작용과 의사소통에 대한 연구가 필요하다. 본 연구는 암환자를 대상으로 생애말기 의사결정 상황에서의 환자, 가족, 의사, 간호사 사이의 역동과 담론을 밝혀내기 위한 문화기술지 연구(ethnography)이다. 연구 방법은 심층 면담, 참여관찰을 사용하였으며 포스트모더니즘 문화기술지 방법과 상징적 상호작용이론을 사용하여 담론을 해석하였다. 자료수집은 2014년 1월~8월까지 서울에 소재한 Y대학병원 암센터 외래진료와 병동회진을 중심으로 진행되었다. 본 연구의 참여자는 환자 11명, 가족 13명, 의사6명, 간호사 7명으로 총 37명이며, 연구 설명 후 자발적 동의에 의해 참여하였다. 본 연구의 주요 결과는 다음과 같다. 생애말기 암환자의 의료적 의사결정 과정은 생애말기에 대한 확인, 증상악화로 인한 입원, 구두 사전의료지시서, 임박한 임종, 서면 사전의료지시서, 임종 등 6단계로 나누어진다. 의사결정 논의의 시작은 대부분 의료적 치료를 결정하는 의사가 주도하였다. 환자에게 예후를 고지하는 문제에 있어서는 가족들의 결정을 따르거나 가족에게 맡기는 경향이 있었다. 대부분의 설명이 구두 혹은 서면 사전의료지시서를 설명하는 시점에 집중되어 있었고 의사- 가족간의 의사소통이 위주가 되었다.환자-가족-의사-간호사 사이의 상호작용은 환자의 죽음 인식상태에 따라 다르게 나타났다. 본인의 예후를 아는 경우는 죽음을 받아들이고 의사결정에 적극적으로 대처하는 경우, 죽음을 받아들이기는 하나 의료적 결정을 가족들에게 맡기는 경우, 직면하지 못하고 회피하거나 우울해 하는 경우, 환자-가족-의료진 사이에 상호 암묵적 위장상태가 지속되는 경우, 희망적이 대화를 원하는 경우 등으로 나뉘었다. 자신의 예후를 알고도 생의 마무리를 하기 보다 가족들에게 맡기기는 것, 환자-가족-의료진 모두 죽음과 관련된 대화를 기피하려는 상호 암묵적 위장 상태를 지속하는 특징을 보였다.생애말기 암환자를 둘러 싼 사회 문화적 텍스트는 차이 속에 공존하기, 불확실성, 침묵으로 나누어 설명될 수 있었다. 차이 속에 공존은 환자-가족, 의료진 사이에 병을 바라보는 시각의 차이, 급속 근대화 이후 동양 사상과 서양 생명윤리관의 공존, 서양의료와 보완대체의료의 공존을 확인할 수 있었다. 생애말기 의사결정과정 맥락 속에 있는 침묵은 죽음으로 인한 불확실성 외에 문화적, 구조적, 개인적 차원으로 설명될 수 있었다. 침묵 속에는 차이와 갈등도 있었지만 죽음을 앞둔 생애말기 암환자에 대한 배려 역시 확인할 수 있었다. 생애말기 의사결정은 복합적인 사회 문화적 요인과 환자, 가족의 삶의 맥락 속에서 고려 되어야 하는 과정이다. 성숙한 죽음 문화를 만들기 위해서는 사회적, 개인적 노력과 더불어 의사결정을 위한 적절한 시기와 치료 범위에 대한 의료진 사이의 공유가 함께 이루어지는 것이 필요하다.ope

Pediatric nurse practitioners' clinical competencies and knowing patterns in nursing: Focus group interviews

BACKGROUND: The generic competency domains of advanced nursing practice have been reported on in numerous countries, but rather few studies have examined competencies specific to pediatric nurse practitioners (PNPs). OBJECTIVES: We identified the core clinical competencies of PNPs in South Korea and related these identified competencies to the five patterns of knowing in nursing. DESIGN: Focus group interviews were conducted with five PNP students and four PNPs using two thematic questions, one on clinical competencies required for PNPs and the other on competencies specific to Korean PNPs. A purposive sampling method was used to choose nurses with varying work experience and age from different hospital units. The inclusion criterion for PNP students was having at least two years of clinical experience and that for PNPs was having at least two years of clinical experience as a PNP in pediatric units in tertiary hospitals. The verbatim transcriptions of these interviews were analysed by two researchers using inductive content analysis. RESULTS: Six clinical competency domains were identified including advanced pediatric-specific knowledge and clinical skills, education and counseling, utilization and engagement in research, professional identity development, clinical and professional leadership, and holistic care. Some competencies identified were related to empirical and ethical knowledge that could be taught in nursing, whereas others were based on esthetic and personal knowledge, which can be mastered through professional experience. CONCLUSIONS: To provide holistic care for children and families, PNPs must acquire all necessary patterns of knowing through continuing education and individual reflection on personal practice.restrictio
