[์˜๋ฌธ] To elucidate the genetic events which may play important roles in hepatocarcinogenesis, allelotype study was undertaken initially on 22 hepatocellular carcinomas (HCC) using 68 highly polymorphic micsatellite markers. And 10 blown tumor suppressor genes (TSGs), including VHL, APC, EXTI, WTI, Rbl, p53, BRCAI, nm23, DPC4, and DCC, were examined in 29 HCCs using PCR-LOH (loss of heterozygosity) studies. Frequent LOH (>50%) was found on chromosome arms 1q,4q, 8p, 8q, and 16q. Then, 61 microsatellite markers were used to narrow down the common LOH regions on these chromosonle arms. Specific deletions were found at APOA2, D4S1615, D4S1554, DBS264, DBS261, DBS565, and D16S752 loci. The associations between clinicopathological findings and LOH at these loci and p53 locus were also analyzed in 41 HCCs. Significant associations were found between FALG (fractional allelic lass of putative tumor suppressor genes) and tumor differentiation (p=0.01, by Fisher's exact test), between FALG and LOH at APOA2, D4S1554, D8S565(P=0.04, p=0.04, and p=0.03, respectively), and between tumor differentiation and LOH at D4S1554, D8S261,and D8S565 loci (p=0.04, p=0.04, and p=0.01, respectively), indicating that accumulation of genetic alterations may contribute to the development and/or the progression of HCC. There were significant associations between LOH at D4S1554 and D8S565 (p=0.03), between LOH at D8S264 and D8S261(P=0.03), and between LOH at D8S261 and D8S565(P=0.05), suggesting that LOH at some loci may cooperate with each ocher during the development of HCC. The frequent LOH on these specific chromosomal arms indicates that novel TSGs might locale at these specific regions, and in addition to p53, the accumulation of LOH on these legions might play a significant role during the development and the progression of HCC.restrictio

Homeostasis of immune function plays a key role in the development of some of these complications, and regulatory T cells (Tregs) are known to have an important role in maintaining the immune homeostasis. i Expression of Forkhead BOX P3 (FOXP3) is essential for the development of Tregs. The main aim of this study was to evaluate the potential influence of FOXP3 rs3761548 polymorphisms of donor on the outcomes of the HSCT. Methods: A total of 171 patients were enrolled in this study and genotyping was done with PCR and direct sequencing using the post-transplant samples, which showed the complete replacement of hematopoietic cells by donor. We grouped the patients according to the genotype and compared the clinical outcomes using the Log- rank test and Gray test. Results: The patients who received HSCT from the donor with rs3761548 CC genotype had higher incidence of hepatic veno- occlusive disease (HVOD) and cytomegalovirus (CMV) infection compared to those of AA or AC genotype group (24.9% vs. 8.5%, P=0.011 and 66.3% vs. 47.6%, P=0.023). Treatment-related mortality (TRM) rate of patients with AA or AC genotype was lower than that of the other patients with CC genotype (3.4% vs. 14.2%, P=0.044) resulting in the difference of overall survival. There was no difference in graft-versus-host disease (GVHD), relapse or blood stream infection (BSI) according to the rs3761548 ii genotype. In multivariate analysis, CC genotype maintained its statistical significance in the development of HVOD (HR=3.97, 95% CI=1.47-10.74) and CMV infection (HR=1.88, 95% CI=1.19- 2.97), showing lower overall survival. Concerning wound repair of the cartilage, many studies had directed to the wound repair of the articular cartilage and its application. However, some random studies in cauliflower ear deformity and cartilage transplantation had been reported regarding the ear cartilage until 1972, when Skoog & coworkers revealed the perichondrial potential for cartilage regeneration. Since them, many reports about the cartilage studies had pulished. In order to clarify the better appearance of the ear after the certain surgical repair of the ear cartilage, this study was carried out using rabbit's ear as the experimental animal as follows: A different surgical wounds were created in each ear and repaired in different way respectively, first group created about 0.5ใŽร—1.5ใŽ of cartilage defect with its perichondrum and overlying skin closure, second group created the same sized cartilage defect leaving perichondrium intact and overlying skin closure, third group created the cartilage gap with its perichondrium and gap closure by overlapping edges, and forth group created gap closure by dege-to-edge approximation with figure of eight suture. On the 3rd, 6gh, 12th, 24th,30th, and 42nd days following surgery, the gross appearance of the each as well as the histologic findings of the cartilages were observed, and the results were summerized. 1. Marked ear bulkiness(thickening) was in the group 3. and normal to least thinning of the ear was shown in group 1 and 2, but in group 4, smooth appearance fo the ear without change in thickness was observed. 2. In all groups, the primitive mesenchymal cells and fibroblasts were aggregated in early period of the healing process and decreased in number gradually changing to the loose connectives tissue in 12th post-surgical day. The inward growing of the immature chondrocytes from the perichondrium to the cartilage side was observed in 24th post-surgical day, particularly in group 2 and some degree at the cut edge of cartilage in other groups. 3. In conclusion, it may be indicated that the regeneration of the cartilage is originated from the perichondrium in the defect of cartilage, and edge-to-edge approximation with intact perichondrium is the best method to attain the original gross appearance of the ear.restrictio

Intestine is known to be much more vulnerable to ischemic injury than other tissues, and many means have been directed to protect the graft from ischemic injury incurred prior to restoration of blood flow. Several experimental animal studies have been reported on the effects of ischemia on intestinal tissue, but many of theme have dealt with the effects of ischemia on the various metabolic functions and their reversibility as found in such clinical conditions as superior mesentericvascular obstruction and revascularization. But in the case of denervated intestinal segments used as a vascularized free crafts for the reconstruction of head and neck defects. the presence or absence of metabolic function is not so critical if the grafted intestine can survive as a conduit or coverage. In order to clarify the time related degree of injury and its reversibility according to the varying periods of warm ischemia in a vascularized jejunal free graft, and also to define critical warm ischemic time to survive, this study, using the dogs as experimental animals was carried out as follows: A cervical esophagus defect was created by segmental resection, and replaced with a free jejunal graft by microscopic individual vascular anastomosis in the neck with varying periods of warm ischemic time ranging from one to five hours. The dogs were sacrificed after three to five months and the grafted jejunums ware excised for observation of grass and histological appearance. In another group of dogs, in vivo, non-transplanted free jejunal segments were exposed and seven hourly excisions of tissue were made and their gross and histological appearance observed to note immediate warm ischemic change sequence. The results were summerized : 1. In the immediate time periods of one to seven hours following warm ischemia, the mucosal change was most striking. Deepithelialization of villi was observed beginning in the superficial farer of mucosa at one hour. It deepened gradually and by seven hours had invaded at the entire thickness of mucosa. 2. All free jejunal grafts with a warm ischemic time up to four and a half hours survived for a long time(three to five months). 3. Once the free grafts survive, there was no gross and histological difference in the long term observation of three to five months among all the surviving grafts, independent of the warm ischemic time periods up to four and a half hours. In conclusion, jejunal free grafts can survive, if thor are revascularized within warm ischemic time of four and a half hours.
