韓方物理療法의 利用實態에 關한 硏究

보건정책및관리학과/석사[한글] 본 연구는 한방물리요법실의 입원·외래 이용자의 한방물리요법실 이용실태를 파악하고, 한방물리요법 의료보험 실시에 대한 필요성의 인식과 태도를 파악하여 한방물리요법의 의료보험 적용의 필요상을 제시 하고자 시도된 연구이다. 연구대상은 서울과 수도권지역에 있는 7개 한방병원 물리요법실 입원·외래 환자 210명이며, 자료수집 기간은 1998년 10월 19일부터 11월 4일까지 실시하였다. 연구의 도구는 연구자가 개발한 자기기입식 설문지로, 내용은 과거 한·양방 물리요법실 이용 실태와 한방물리요법 의료보험 실시의 필요성에 대한 사항으로 구성했다. 본 연구의 결과는 다음과 같다. 1) 현재 한방물리요법을 산택한 이유에 대해서는 양방물리치료 경험군과 무경험군 사이의 관련성은 없지만 두 집단 모두 치료효과가 좋아서라고 응답한 경우가 가장 많았다. 2) 한방물리요법 치료비에 대해서는 비싸다고 답한 경우는 양방물리치료 경험군이 70.6%, 무경험군이 63.7%로 나타났다. 통계적으로는 유의하지 않았지만 두집단 모두 비싸다고 인식하였다. 3) 학력을 보면 양방물리치료 경험군은 대졸이상, 무경험 군은 고졸이하에 높은 분포를 차지하였다(P<0.028). 즉, 학력수준이 높을수록 양방물리치료 경험율이 높은 것으로 나타났다. 4) 연령에 있어서는 나이가 젊을수륵 양방물리치료 이용률이 높았으며, 나이가 많을수록 양방물리치료 경험율이 낮았다(P<0.034). 5) 양방물리치료 경험군과 무경험군 사이에는 치료받는 질병의 구조가 달랐다(P<0.001). 즉, 무경험군의 경우 중풍의 분율이 높고, 경험군의 경우 중풍외의 질환인 관절염, 견비통이 차지하는 분율이 높았다. 6) 한방물리요법 의료보험 도입에 대한 인식과 태도에서 양방물리요법 경험군과 무경험군간에 통계학적으로 유의한 차이는 볼 수 없었으나, 두 집단 모두 한방물리요법 의료보험의 필요성을 인지하고 있었다. 이상의 연구결과를 종합해 보면 한방물리요법실을 이용하는 환자의 치료비 부담을 경감시키는 방향으로 가기 위해서 한방물리요법에도 의료보험을 적용하는 정책을 세워야 한다고 생각한다. [영문] The purpose of this study is to analyze the current status for utilization of the physiotherapy deparment in oriental hospitals, to measure levels of knowledge and acceptance of the medical insurance system of oriental physiotherapy. Such findings obtained from this study will provide valuable information to establish the base of the application of medical insurance system for oriental physiotherapy. The sample group was a proportional stratified sample of 210 patients in 7 oriental hospitals located in seoul and the metropolitan area. The period for data collection was from October 19 to November 4, 1998. The method of investigation was a self report questionnaire developed by the investigator. An Univariate analysis was performed to determine the general characteristics of the population and the current status for utilization of the oriental physiotherapy by frequency distribution and percentage analysis. The statistical differences of the utility status and levels of knowledge and attitude on the medical insurance system of oriental physiotherapy between the experienced and inexperienced group in western physiotherapy were performed using chi-square test. Finally, a multivariate analysis was carried out by the conditional stepwise logistic regression. The results of this study are as follows; 1. Age and educational level were found to be significantly different variables between the experienced and inexperienced group in western physiotherapy in multivariate analysis. The experienced group in western physiotherapy had significantly higher proportion with university education level, whereas the inexperienced group had mostly lower level(P<0.028). 2. As classified by age, the younger were high and the older were low the rate of utility in western physiotherapy(P<0.034). 3. The structure of disease between the experienced and inexperienced group in western physiotherapy was differnt(P<0.001). The rate of cerebral hemorrhage was high in the experienced group and the proportion of arthritis, shoulder pain was high in the inexperienced group. 4. With regard to the knowledge and attitude on the medical insurance system of oriental physiotherapy, the differences between the experienced and inexperienced group were not statistically significant, but both groups were recognized a necessity of the medical insurance system of oriental physiotherapy. This study was limited because the number of subjects was small for practical implications. However, like as other results, this study suggest that health policy planner would be more attentive to cut down the expenses of the oriental physiotherapy services. In addition, efficient medical insurance system of oriental physiotherapy can be achieved by a chosen medical insurance plan with supporting counseling by the health policy planner.restrictio
