A Study on Family Health Conditions and Health Problems of Urban Low Income Families Requiring Visiting Health Care

보건간호학과/석사[영문] [한글] 본 연구는 저소득 방문간호사업 대상 가족의 주요 건강 문제와 그 실태를 가족을 중심 으로 파악한 건강문제 기초 조사 연구이다. 본 연구에서 저소득 방문간호사업 대상 가족의 건강 문제와 실태를 파악하는 것은 그 대상 가족들에게 필요한 양질의 의료서비스를 제공하고자 하는 방문 간호사업 기획에 기 초자료를 제공하는데 의의가 있다고 보고 본 연구를 시도 하였다. 연구 대상은 S구 영세 지역 3개동 저소득 방문간호사업 대상 가족 326세대 전수를 대상 으로 하였으나 면접 조사가 가능했던 총 128 가구가 자료 분석에 사용 되었다. 연구 도구로는 가족 건강 측정을 위한 설문지로 1990년 비슷한 영세 지역저소득 주민을 대상으로 실시한 김의숙의 건강문제 기초 조사(survey)에서 사용하였던 설문지를 참조하 여 본 연구 내용에 맞도록 수정한 것이다. 설문지 내용은 가구원 조사, 환경 상태, 가정내 건강문제, 가족역등, 부부문제, 의료보 호, 건강 관련 행위의 내용으로 구성 되었으며 총 65문항 으로 이루어 졌다. 자료 수집 기간은 1992년 4월 6일 부터 5월 10일 까지 총 35일간 실시 되었으며 자료 수집 방법은 방문면접 설문지 조사법을 사용하였다. 자료 분석 방법으로서는 수집된 자료는 대상자 개인단위, 부부단위 및 가구단위로 나누 어 분석 하였으며 대상자의 일반적 특성은 각 문항에 대해 실수와 백분율을, 건강문제는 개인 단위로 분석하여 성별 신체기관(System)별 건강문제, 사용한 치료 방법에 대해서는 실수와 백분율을 구하였다. 가족역동은 가구 단위로 분석하고 부부문제 변수는 부부단위 로 분석하여 실수와 백분율을 산출 하였으며 건강에 관련된 가정의 문제는 실수를 구하였 다. 모든 자료는 Spss로 전산 통계 처리 하였다. 본 연구조사 대상가족의 일반적 특성, 환경위생상태, 건강문제, 가족역동, 부부문제, 건강과 관련된 가정의 문제를 조사하여 파악된 결과를 토대로 다음과 같은 결론을 얻었다 . 1. 조사대상 가족중 면접 조사 가능한 128 가구 365명의 건강문제에서 만성건강문제를 가진 가족은 전체가구의 79.7% 였으며, 만성건강문제가 급성건강문제의 4.5배나 되었으며 , 만성질환의 56.7%가 30-50대 연령층에 분포되어 있으며, 주요 건강문제는 중년층의 만 성질환으로, 생업에 종사할 수 없게 된 이들에 대한 방문간호사업이 우선 되어야 한다고 본다. 2. 조사대상 가족구성원의 신체기관(System)별 건강문제는 급성건강문제로는 호흡기계 가 44.4%로 가장 많았으며 만성건강문제로는 근 골격계 23.9%, 순환기계 19.6%, 소화기계 11.0%, 호흡기계 8.0%, 손상 및 중독이 7.4%의 순으로 많았다. 평균 유병기간은 7.4년 으로 나타나, 근 골격계질환과 순환계질환이 상위를 차지하고 있으므로 거동이 불편한 환 자가 많다는 것을 시사하고 있으며 이에 대한 방문진료 방문간호가 활성화 되어야 한다고 생각한다. 3. 의료이용 상태는 급성 건강문제의 경우 병의원, 약국이용(70.0%)으로 해결 하였으나 , 만성 건강문제의 경우 초기에는 주로 병·의원을 이용(55.2%) 했으나 나중에 치료처를 변경 하거나(20.7%) 현재는 치료 받고 있지 않은 경우가 23.4%으로 나타나, 치료 효과가 뚜렷하지 않음으로 증상처리만 하는 것으로 사려된다. 의료이용상 문제는 2,3차 전문의료 기관의 접근이 용이 하지않아 인근 의원이나 약국이용에 의존하고 있으므로 근본적인 치 료가 늦어지고 있다고 본다. 4. 가족역동 및 가정문제에 있어서 가사운영의 결정은 대부분 부인이 하고 있는것(47 4 %)으로 나타났는데, 가사운영이 단순하고 남편의 수입이 낮기 때문이라고 본다. 친척과의 왕래는 큰일때만 있고(47.7%) 가족이 함께하는 시간은 적은편으로 인지하고 있었으며(42 .8%) 가정내에 불화를 일으키는 내용은 건강상의 문제(40.4%)와 경제적인 문제(37.4%)가 대부분이었다. 가족 결속력이 약하다고 보며 지역사회 주민의 관심이 요구 된다고 본다. 5. 부부문제로는 가족원에 대한 건강문제, 배우자의 생활무능력, 부부간의성격 차이의 문제가 주된 것으로 나타났으며 부부간의 대화시간은 제대로 갖지 못하는 것으로 나타났 다(54.4%). 대화 내용은 일상 생활 이야기 살림살이의 내용에 대한 것으로 조사 되었고, 자녀와의 대화 내용은 교육문제에 대한 것이 가장 많은 것으로 나타났다(35 8%). 이혼에 대한 태도는 매우 부정적으로 나타났으며(90.2%), 자녀교육에 대한 관심이 높은 것으로 보아 자활가능성이 있다고 본다. 6. 건강과 관련된 가정의 문제로는 입원비 문제, 2,3차 의료기관의 치료비 문제, 건강 문제로 인한 직업 상실, 신체 장애자의 바깥 출입 문제, 자녀의 가출, 주거 공간 부족, 간병인 문제, 의료이용 정보부족, 전문의료기간의 제한등의 문제가 발견되었다. 7. 본 조사대상 가족의 건강문제와 건강과 관련된 가정의 문제를 해결하기위한 방안으 로 지역보건간호사의 활동 영역으로서는 보건 교육, 직접 간호 서비스 제공, 상담자 역할 , 자원활용과 의뢰 활동, 연구자 활동의 간호사업이 요구되며 정책지원 방안으로는 공공 전문의료기간의 확보, 행정체계상의 지원, 사회복지체계상의 지원, 지역사회 주민과 의료 인들의 관심과 참여로 문제해결 방안을 모색해 보았다. A STUDY ON FAMILY HEALTH CONDITIONS AND HEALTH PROBLEMS OF URBAN LOW INCOME FAMILIES REQUIRING VISITING HEALTH CARE Kim, Young Ja Graduate School of Health Science and Management Yonsei University (Directed by Professor Cho, Won Jung. Ph.D.) This study was survey of family health conditions and health problems of low income families requiring visiting health care. This study was done to provide for visiting health care plan basic data and supply quality medical services to the subject families. The subject were 326 families requiring visiting health care in an urban poor area of three blocks in S District. Data collection was completed for 128 families which were surveyed for analysis. The study tool used was a questionnaire for family health exam which was a modification tool of the health problems survey by EuiSook, Kim which was designed in 1990 for use with the urban poor. The questionnaire was composed of 65 items including demographic information on family mebers, environmental conditions, families health problems, family dynamics, husband and wife problems medicaid and health related activities. The period for data collection was a total of 35 days from April. 6, 1992 to May 10, 1992 and the method used was a home visiting interview using the questionnaire. The data was analyzed according to several units; individual, couple and household. Incidence and percentage were used to analyze the characteristics of the family for each item of the questionnaire. Health problems were analyzed for the individuals. Actual numbers and percentages were used to analyze the method of the treatment used and the health problems according to body system, and to sex. Family dynamics were analyzed for households unit; variables regarding husband and wife problems were analyzed for couples using actual numbers and percentages. Health related family problems were analyzed using actual numbers. The following conclusions are based on the result of the survey for general characteristics, environmental sanitary conditions, health problems, family dynamics, husbands and wife problems and the health related family problems. 1. The result showed that 79.7% of the families had a meber with a chronic disease. Chronic health problems were 4.5 times higher than acute ones, 56.7% of the chronic diseases were found in the 30-50 age group, people who must participate in economic activities. The main health problems was chronic disease in the middle aged group. 2. In the survey of health problems according to the physical system for family mebers, chronic health problems in the muscle andskeletal system showed the most(23.9%). The circulatory system showed next(19.6%). These results indicate that there are a high number of mobility disabled who need visiting health care. 3. In case of utilization medical facilities, acute health problems were solved using care in hospitals and drug stores (70.0%). But early chronic health problems were solved mainly using care in hospitals or medical clinics(55.2%) but changing medical facilities was frequent(20.7%) and 23.4% were not receiving treatment. They stopped receiving care for symptoms because of the progress made under the hospital treatment was poor. 4. Decisions of household affairs were generally made by wives (47.0%). Mainly because the household affairs were very simple and husbands' incomes were very low. Relatives visited for big events (47.7%). They spent few hours together as family mebers(42.8%). Health problems(40.4%) and economic problem were the main causes of home troubles. The result showed that the family dymamics were poor. 5. The main problems between husband and wife were caused by family health problems, incompetent spouses and differences of the character between husband and wife. The number of hours spent in conversation between the couples were very low(54.4%). The main subjects of the conversations were life and household affairs. The main subject of the conversation with children was education(35.8%). These couples' attitudes to divorce were negative(90.2%). This result showed that these families were interested in their children's education and had traditional attitudes. 6. Health related family problems were hospital charges, medical fees for secondary and tertiary class medical services, loss of job due to health problems, physical handicapped persons' the outdoor life, children who run away from home, lack of space in residence, caretaker's problem, lack of medical information and limited professional medical organizations. 7. Nursing care addressing solutions to the health problems and health related family problems should include health education, direct nuring service, the role of counsellor, the use of resources and requiring activity, and the researcher's activity are required. Policy support plans, such as, increasing public professional medical organizations, governmental help, social welfare expansion and expanding the interest and participation of medical personnel and the local community people are needed.restrictio

(A) comparative study of knowledge on tuberculosis between the delinquent and non-delinquent of the Yonhee Community Health Service Center

간호학교육전공/석사[한글] 결핵퇴치를 위한 범 국가적인 결핵관리사업에도 불구하고 결핵치료 도중에 중단하게 되는 치료중단자의 문제는 결핵관리의 중대한 문제로 대두되고 있다. 본 연구는 결핵치료중단자와 계속자의 결핵에 관한 지식을 비교하기 위하여 시도되었다. 연구의 목적은 1. 결핵치료 중단자와 치료계속자의 일반적인 특성과 결핵치료력의 특성을 비교, 조사한다. 2. 결핵치료중단자와 치료계속자의 결핵에 관한 일반적인 지식을 비교 조사한다. 연구대상은 서울시내 Y 보건소 결핵실에 등록된 환자중 중단자 41명과 계속자 42명, 총 83명을 대상으로 하였다. 연구조사는 1977년 9월 1일부터 10월 10일 사이에 실시되었으며 연구도구는 저자가 작성한 면접지와 결핵실에 비치된 환자기록부를 참고하였다. 자료수집은 본 연구조사를 위하여 훈련된 면접자가 면접지를 가지고 대상자를 직접면접하여수집하였으며 수집된 자료는 대상자의 자료수집은 본 연구조사를 위하여 훈련된 면접자가 면접지를 가지고 대상자를 직접면접하여 수집하였으며 수집된 자료는 대상자의 일반 적인 특성과 결핵치료력은 백분율과 χ^^2 - test, 결핵의 일반적인 지식에 관한 문항은 전체적 정답율로 비교하고 대상자 1인의 총정답수를 문항별, 영역별로 구분하여 t-test로 처리하였다. 본 연구에 의해 얻어진 결과를 요약하면 다음과 같다. 1. 대상자의 일반적 특성과 결핵치료력 대상자의 일반적인 특성에서는 연령, 성별, 결혼상태, 교육수준, 직업에 대해 조사하였으며 중단자군과 계속자군간에 유의한 차이를 보이는 것은 직없의 유,무,관계 뿐이었으며 다른 특성에서는 유의성이 없어 두집단의 비교가 타당하였다. 대상자 83명의 연령분포는 16세에서 71세까지로, 평균연령은 40.4세였고 남녀의 비는 1.5:1로 남자가 많았다. 결혼상태는 기혼자가 많았고 교육수주는 과반수가 국졸이하였으며 자가소유율은 54.2%였다. 직업의 분포는 무직이 제일 많아서 계속자가 85.8%로 중단자 60.9%보다 높았고 통계적인 유의성이 있었다(χ^^2 = 6.59, d.f. = 1, P < 0.05). 대상자의 결핵치료력을 보면 결핵진단을 받은 기관과 치료시작기관은 보건소가 가장 많아서 대부분의 환자가 결핵진단 받은기관에서 치료를 시작하는 양상을 알 수 있었다. Y보건소 등록당시 결핵의 상태는 79.5%가 중등중이상이었고 치료후 객담상태가 좋아질대 치료를 중단하는 양상을 보였다. 치료시작후 1년내와 2년이상 기간이 길어질때 중단하는 율이 높았으며 치료중단의 이유는 환자측의 이유가 높았고 특히 자각증상이 없을때 치료중단율이 가장 높아 자각증산만 없으면 병이 완치되는 것으로 알고 치료를 중단하게 되는 경향을 알 수 있었다. 2. 대상자의 결핵에 관한 지식 결핵에 관한 일반적인 지식인 31문항에 대한 지식정도는 전 대상자의 57.9%의 정답율을 보여 과반수이상이 결핵에 관한 일반적인 지식에 대해 알고 있었다. 중단자군에서는 55.5%, 계속자군에서는 60.3%의 정답율을 보이고있어 계속자군에서 약간 높은 결과였으나 유의한 차이는 없었다. 5개의 영역별 지식으로 구분하여 비교해본 결과 결핵환자의 간호에 관한 지식영역이 78.7%로 가장 높은 정답율을 보였고 그다음은 진ㄷ단에 관한 영역, 감염에 관한 영역, 치료 및 투약에 관한 영역의 순이었고 결핵예방에 관한 영역이 37.3%로 가장 저조한 정답율을 보여주고 있다. 31 문항에 대한 정답수의 분포는 최고가 28이고 최하는 4로서 전 대상자의 평균정답수와 표준편차는 17.3 ± 5.10 이었다. 31 문항에 대한 중단자군의 평균정답수와 표준편차는 중단자군에서 16.4 ± 5.13, 계속자군은 18.2 ± 5.63 으로 계속자군이 약간 높았으나 두집단간에 통계적인 유의성은 없었다(t = 1.54, d.f. = 81, P > 0.05). 영역별 정답수 분석에서 중단자군과 계속자군간에 유의한 차이를 보이는 것은 결핵예방에 관한 영역(t = 1.67, d.f. = 81, P < 0.05) 과 결핵치료 및 투약에 관한 영역(t=2.16, d.f. = 81, P < 0.05) 이었다. 본 연구의 결과를 기반으로 하여 다음과 같이 제언한다. 1. 지식이 결핵치료의 계속과 중단에 미치는 영향에 대한 종단적 연구. 2. 대상자 개인의 과거경험이나 교육이 결핵치료의 태도에 미치는 영향에 관한 연구 3. 간호원을 비롯한 건강요원의 역할이 결핵환자의 지식에 미치는 영향에 관한 연구. [영문] The fact that there still exist a numerous pulmonary tuberculosis patients who discontinue proper tuberculosis treatment inspite of the national tuberculosis control system. Thisstudy was designed to find out how those who discontinued tuberculosis treatment(the delinquents) and who are continuously under proper treatment (the non-delinquents) know on tuberculosis and to compare knowledge between those two groups. The specific objectives of this study are: 1. To find out the general characteristics and treatment history of tuberculosis of the delinquents and the non-delinquents. 2. To compare the extent of knowledge on tuberculosis between the delinquents and the non-delinquents. The subjects of this study was 83 consist of 41 delinquents and 42 non-delinquents. The subject was sample from the patient registerd at the tuberculosis clinic of "Y" Community Health Service Center in Seoul. The data was obtaioned by personal interview from September 1 to October 10, 1977. The auther developed a interview schedule that is consist of thirty one items which are devided into five categories, all items were answered by true and false. The general characteristics and treatment history of tuberculosis was analized by the percentage and χ^^2-test and the knowledge on tuberculosis were figured by percentage and t-test for statistical significance. The results of this study are as follows: 1. General characteristics and teatment history of tuberculosis: The age range of the 83 subjects extends from 16 to 71 with on average of 40.4 years, and sex ratio was male 1.5: female 1. Inmarriage status the married was over 50% and 49.4% of the subjects was educated in the elementary school. 60.9% was unemployees in the delinquents as compared with the non-deliquents 85.8% and the fact showed statistical significance (χ^^2=6.59, d.f.=1 p<0.05) The most of the subjects were diagnosed and treated at a Health Center. The diagnostic level of tuberculosis at the "Y" Health Center was mostly modrate and far advanced tuberculosis. It was found that tuberculosis treatment was stopped when the subjects did not have subjective symptoms and 53.2% delinquents have had tuberculosis treatment over two years. 2. Knowledge on tuberculosis: The initial step in the data analysis was to make a percentge analysis of a right answers to each item which would determine the subject's knowlege to each of the thirty one items. 57.9% of the total answer were correct. The fact implies that more than a half of the items were correctly known by the subjects. In detail, the delinquents revealed 55.5% of correct answer while that of the non-deliquents were 60.3% Among the five cathegories of the knowledge, as follows: The subjects showed 78.7% of correct answer on cathegory of nursing care of tuberculosis that was the highest. Preventure measure of tuberculosis was the lowest cathegory. Only 37.3% of the subjectives cheched right answer on these items of the cathegory. Numer of correct answer for one subject ws scored twenty eight out of thirty one items were the highest correct answer and the lowest correct answer was four. Mean and standard deviation was figured as 17.3 ± 5.10. The mean and standard deviation of the delinquent group was 16.4 ± 5.13 and in the group of delinquent was a slightly higher as being 18.2 ± 5.63, and thus no remarkable significance noted between the two groups (t=154, d.f.=81, p<0.05) In analysis of correct answer by each cathegory, a remarkable difference the delinquent and the non-delinquents was on the cathegory of prevention of tuberculosis ( t=1.67, d.f.=81, p>0.05)restrictio
