Effect of an integrated therapeutic family management program on the family functioning of schizophrenic families

간호학과/석사[한글] 본 연구는 정신분열증환자와 가족이 정신분열증에 대한 올바른 이해를 가지도록 하고, 가족내의 효율적인 대화 및 문제해결과정을 통해서 전반적인 가족기능을 향상시키도륵 하기 위하여 시도되었다. 본 연구는 이러한 목적하에 정신분열증환자와 가족을 대상으로 정신건강교육과 의사소통기술훈련, 그리고 문제해결기술훈련으로 구성된 통합적 가족치료관리프로그램을 효과적으로 구성하고 적용하여 그 프로그램이 정신분열증환자의 가족기능에 영향을 미치는지를 알아보고자 단일군 사전사후의 실험연구설계로 진행되었다. 연구대상 및 연구기간은 1997년 3월 3일부터 1997년 5월 24일까지였고, 1개 종합병원의 정신과 병동에 입원하였다가 퇴원하여 낮병원에 다니고 있는 정신분열증환자와 그 환자를 주로 돌보는 가족중에서 의도표출하였으며 총 10가족을 연구의 대상자로 하였다. 통합적 가족치료 관리프로그램은 Falloon(1984)과 Bernheim(1982)의 모델을 본 연구자가 문헌고찰을 통해 수정 보완한 것으로 1회 2시간씩 10주간 10회로 구성되었다. 연구진행절차는 실험 1주일전에 사전조사가 이루어졌으며 10주간의 실험처치가 끝난 후 사후조사가 실시되었다. 연구도구는 Epstein과 Bishop(1983)이 개발한 가족기능 측정도구(Family Assessment Device)를 본 연구자가 번역하여 우리나라 실정에 적합하게 수정한 후 타당도, 신뢰도를 검증하여 사용하였다. 자료분석은 SPSS를 이용하였다. 통합적 가족치료 관리프로그램이 가족기능에 미치는 영향을 분석하기 위해 Wilcoxen Sign Rank Test를 하였다. 연구결과는 다음과 같다. 1. 주가설인 '정신분열증환자의 가족기능은 통합적 가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 사전 점수 2.27에서 사후에 2.15로 전후차이를 보였으나 통계적으로 유의하지 않았다(Z=-1.68, P=.09). 따라서 주가설이 지지 받지는 않았으나 통합적 가족치료 관리프로그램이 정신분열증환자의 가족기능을 향상시키는데 영향을 주는 경향성을 보인다고 할 수 있다. 2. 제1부가설인 '정신분열증환자가족의 문제해결기능은 통합적 가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 사전 점수 2.23에서 사후점수 2.17로 전후차이를 보였으나 통계적으로는 유의한 차이를 보이지 않았다(Z=-0.76, P=.44). 제2부 가설인 '정신분열증환자가족의 의사소통기능은 통합적 가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 사전점수 2.39에서 사후점수 2.30으로 역시 전후차이를 보였으나 통계적으로 유의한 차이를 보이지 않았다(Z=-1.37, P=.16). 제3부가설인 '정신분열증환자가족의 역할기능은 통합적 가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 사전값 2.33에서 사후점수 2.40으로 오히려 부정적인 결과를 보였다.(Z=-1.17, P=.24). 제4부가설인 '정신분열증환자가족의 정서적 관여기능은 통합적 가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 통계적으로 유의한 차이(Z=-2.60, P=.009)를 보여 지지되었다. 제5부가설인 '정신분열증환자가족의 정서적 반응기능은 통합적가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 사전점수 2.58에서 사후점수 2.45로 낮아져 전후차이를 보이고 있으나 통계적으로 유의한 차이를 보이지 않았다(Z=-1.66, P=.09). 제6부가설인 '정신분열증환자가족의 행동통제기능은 통합적 가족치료 관리프로그램 참여 전과 참여 후에 차이가 있을 것이다'는 통계적으로 유의한 차이를 보여 지지되었다(Z=-2.19, P=.02). 3. 추가분석을 통하여 환자와 가족이 가족기능에 대한 지각의 차이를 보이고 있어 환자와 가족의 평균값으로 프로그램의 전후차이를 보기가 어려웠을 것으로 나타났다. 한편 10회동안 매회 주어진 정서적 지지및 정신분열증에 대한 지식의 습득으로 인해 가족들의 죄책감 및 수치감등의 심리적 부담감의 감소가 나타났다. 또한 환자와 가족간의 상호유대감이 증진됨과 함께 정신재활에의 동기가 강화되었음이 나타났다. 이상의 결과를 종합해 볼 때 통합적 가족치료 관리프로그램이 정신분열증환자의 가족기능에 영향을 미칠것이라는 가설은 지지받지 못했으나 '역할기능'을 제외한 모든 가족기능의 하위차원에서 실험 전후에 변화를 보여 프로그램을 장기적으로 보다 더 많은 대상자에 게 실시한다면 그 효과가 더욱 입증되리라 기대한다. 또한 치료와 재활에 있어 환자와 가족, 그리고 정신건강전문가 사이에 동반자적 관계가 구축되는 계기가 되어 정신분열증환자와 가족을 위한 재활프로그램으로서 기여했다는 점에서 그 의의가 크다고 하겠다. [영문] The main purpose of this study was to identify the effect of an Integrated Therapeutic Family Management Program on the family functioning of schizophrenic families. The design was a pre-post experimental study. The data were collected from March 3 to May 24, 1997. The subjects were 10 schizophrenic patients and 10 patients' primary caregivers who participated in the psychiatric day hospital program of the Yonsei University, Seoul. The Integrated Therapeutic Family Management Program consisted of psycho-education, communication skills training, problem- solving training and emotional support. This program applied a cognitive, behavioral and emotional approach. The program consisted of ten sessions provided over ten weeks with each session lasting two hours. The effect of this progam was measured by the Family Assessment Device developed by Epstein & Bishop(1983) which was translated by a researcher. The reliability and validity had been tested(Cronbach Alpha, with results of .77 -.84). The data were analyzed using Wilcoxen Sign Rank Test. The results are as follows: 1. The Integrated Therapeutic Family Management Program showed a difference between pre and post test score on the general family functioning, but was not statistically significant(Z=-1.68, P=.09). 2. However, the Integrated Therapeutic Family Management Program had a significant effect on the functioning of 'affective involvement'(Z=-2.60, P=.009) and 'behavioral control'(Z=-2.19, P=.02) among the six dimensions of family functioning. 3. By the content analysis, it was found that the families' psychological burden such as guilty feeling and shame were lessened and the cohesion between patients and their families was improved. In conclusion, it was found that The Integrated Therapeutic Family Management Program was partially effective in promoting the functioning of schizophrenic families and further served as a launching pad for increased partnership in treatment and rehabilitation among patients, families, and professionals. Based on these findings, the following suggestions are made: 1. It is necessary to replicate this study using a larger sample and a longer period of time. 2. It is necessary to identify the effect of this program on burden, quality of life, stress, and coping strategies. 3. It is suggested to do a comparative study with FAD and FACES Ⅲ.restrictio

A Study on Self-Image of Korean Nurses

The two purpose of this study was to find the self· image of Korean Nurses ￦o r k i n g in hospitals. camm· unity areas and in nursing schools; the other was to explore the possible influence of certain factors on the levels in nursing image For this research SINS (Self· Image of the Nurse Scale) was administered from May 1978 to October 1978 to a sampling of 559 nurses. The major findings of this study were as follows; a) The mean score of nursing image of Korean Nurses was 3.18. b) The related factor of SINS with the highest mean score (3.58) is the nature and role of nursing, and the factor with the lowest nursing image is about job satisfaction such as salary (2.34) or hard task (2.55) , c) There was the significant difference among the mean scores of nursing image in nurses, those who are older (r=.182), those who hold higher positions (r=.183), but no significant difference of nursing image in nurses those have a different level of nursing education (r=.055) d) Married nurses, nurses with religion , nurses working in community. had more positive image in nursing than single. nurses without religion. nurses working in hospitals. e) According to salary satisfaction, there was the significant difference in nursing image; satisfied group in salary had the most positive nursing image. (F = 3.561, P<.05) f) The last reasons among the selected motivation of career as a nursing were 1) a chance of going abroad, 2) low cost of nursing education, 3) recognition on the societys demands, 4) economic independence, 5) religious belief, 6) service interest. Among them, there was a significant difference in the image of nursing.
