An Analytical Study on Determinants of Utilization of Maternal Child Health Clinic in Rueal Area

간호학과/석사[영문] [한글] 본 연구는 정부 말단기관으로서 농촌지역의 영세민을 위해 모자보건 사업을 시행하고 있는 모자보건크리닉 대상자들의 산전관리 이용실태와 크리닉이용에 영향을 미치는 요인 들은 규명해 보고자 시행되었다. 특히 임신 분만관리를 위한 효율적인 크리닉 활용을 도 모하기 위해 1978년도와 1979년도 2년에 걸쳐 모자보건크리닉에 등록한 대상부인의 사회 경제 및 인구학적 특성과 임신·출산력을 조사분석하고 과거크리닉 경험도 및 현 크리닉 이용실태를 분석하였다. 조사대상은 경남 울산시 울주군 부녀자 분만센타에 1978년 1월부터 1979년 12월까지 등 록한 전체부인중 1/2 계통적 단순 무작위 표본 추출방법을 이용하여 413명을 추출하였으 며 연구자료는 크리닉 사업기록지를 이용하였다. 분석방법은 대상부인의 크리닉이용도를 알아보기 위해, 분석모형을 개발하여 사용하였 다. 즉 산전관리나 분만개조를 위해 모자보건크리닉을 이용하는 양상은 그들이 지닌 사회 ·경제·인구학적특성과 부인의 임신 및 출산의 경험, 과거 동일크리닉의 이용경험에 따 라 영향을 받을 것으로 사려되어 일반적 특성을 독립변수로, 출산력과 과거크리닉 이용경 험을 2차적 독립변수인 중간변수로, 크리닉 이용실태를 본 연구의 종속변수로 설정하여 2 1개의 변수를 가지고 관련성을 분석하였다. 조사 성적을 요약하면 다음과 같다. 1. 대상부인의 크리닉이용실태는 총 방문횟수가 평균 1.7회로서 대상부인의 주거지역이 크리닉 위치가 가까울수록, 남편의 교육수준이 높을수록, 자연 유산 경험이 많을수록 방 문횟수가 증가함을 보였다. 크리닉 첫 방문시의 임신주기는 산전관리를 언제 시작하는가를 결정짓는 주요변수로 사 려되나 본 연구결과 임신7 1/2∼분만시의 임신말기에 전체부인의 52%가 방문한 것으로 나 타나 농촌지역의 모자보건 실태는 아직도 낮은 실정임을 보여주었다. 첫 방문시의 임신주 기는 주거지여과 자연유산 횟수 등이 의의있는 변수로 선정되었다 임신주기 구분별 크리닉 방문횟수는 임신초∼7개월 사이가 평균 0.4회, 임신7 1/2∼9개 월 사이가 0.8회, 임신9 1/2∼분만시가 1.3회로 거의 임신말기에 가서 크리닉 방문빈도가 증가함을 보였다. 대체로 임신주기별 크리닉 방문횟수는 자연유산횟수가 증가할수록 방 문도가 증가함을 볼 수 있었다. 2. 과거 크리닉에서 분만한 경험은 전체 부인 중 자녀가 1명이라도 있는 부인에게서 38 .1%로 나타났으며, 현재 임신한 부인 중 크리닉에서 분만한 경우는 47.9%로 이러한 결과 는 일반적인 농촌지역의 의료기관 분만율과 현격한 대조를 나타내었다. 이러한 사실로 미 루어 보아 농촌지역이라도 적절한 의료기관과 산전관리를 제공해준다면 보다 효율적인 모 자보건사업을 전개할 수 있을 것으로 사려된다. 3. 크리닉이용실태를 결정짓는 변수들, 즉 크리닉 총 방문횟수와 첫 방문시의 임신주기 , 임신주기 구분별 방문횟수, 과거 크리닉 이용경험 및 현재 임신한 아기의 크리닉에서의 분만여부를 종합해서 크리닉이용을 대표할 수 있는 종합평가 지수를 산출한 결과, 총점 10점 만점에 대상부인의 크리닉이용 종합평가지수 평균은 4.5점으로 나타났으며 주거지역 이 가까울수록, 부인의 연령이 젊을수록, 남편의 교육수준이 높을수록, 남편의 직업이 농 업보다는 비 농업일 때 크리닉이용 종합평가지수 점수가 높은 경향을 보였다. 4. 크리닉이용 종합평가지수에 영향을 미친 변수로는 대상부인의 전체유산횟수, 남편의 직업, 자녀수가 전체 14.3%를 설명했으며, 그 중 대상부인의 전체유산횟수, 남편의 직업 이 유의있는 변수로 나타나 이두개의 변수로는 전체 11.7%를 설명할 수 있었다. We also have the Maternal Child Health Clinic by almost every 'Myun' area. But, in fact, the Maternal Child Health Service is regarded as the weakest field among many other health services in most of the developing countries. It cannot be said Korea is not exempted among these countries. For example, more than 90 percent of the total delivery have taken place at hospital in western countries including the United States of America, while less than only 20 percent of them have done in Korea. The main purpose of this study is to find out possible determinants which can influence on the clients' utilization of Maternal Child Health Clinic. And the results of this study will be able to provide us with basic materials countributing not only to the improvement of MCH Services at national level but also to be better utilization of MCH Clinic at community level. Data was obtained from service records written by a mid-wife working at the MCH Clinic on Aunyang Myun located in thirteen women, which is a half of total registered housewives from 1978 through 1979, were selected by systematic sampling with random start. The analytical framwork used for this study can be diagramed as follows: independent variables intermediate variables dependent variables +-------------------+ +-----------------------+ +---------------------+characteristics of+-------------------------+ This was assumed that the clients' visiting to the clinic for the prenatal care and delivery could be influenced by their socio-economic and demographic backgrounds which were taken as independent variables in this study. And fertility and past experence of the clinic visits was regarded as intermediate variables that also having effects on the clients' visiting to the clinic. For the analysis of data, statistical methods such as X**2-text, F-test and Multiple Stepwise Regression Test are employed. The findings of this study can be summarized as follows: 1. When compared the utilizers of MCH Clinic with other married women various studies, they were relatively younger, better educated and of fewer parity. About 80 percent of them were aged between 20 and 30 years and the averaged age was 26.9 years which are far younger than that of the married women in rural areas(34-35 years old). 2. Among the married women with more than a child, only 38.1 percent of them had experienced the delivery at the clinic. 3. The averaged frequences of client's visits to the clinic was 1.7 times during their conception period. The nearer they lived from the clinic, the higher their husband have educational status and the more they experienced spontaneous abortion, the more frequently they visited the clinic. 4. Two hundred and fourteen women(52%) made their first visit to the clinic at the gestation age between 7 1/2 months and just before the delivery. Their first visit in terms of gestation period varied significantly depending upon distance to the clinic from their residence area and number of experience of spontaneous abortion. 5. When dividing gestation age into three categories as from the onset to 7 months, from 7 1/2 to 9 months and from 9 1/4 months to delivery, the averaged frequences of visits to the clinic by each category were 0.4, 0.8, and 1.3 times respectively. 6. The rates of delivery at the clinic was 47.9 percent among women who have made a visit to clinic before delivery. 7. The average score of the composite index of clinic utilization was 4.5 and the average score was increased as the age of women decreased, as the educational status of husband in higher and as the occupation status of husband is white colar. 8.
