Development of the nursing intervention protocol for home health care clients with respiratory function problems

간호학과/석사[한글] 간호실무의 표준화된 언어와 이의 체계적인 분류는 간호가 건강관리체계에 기여하는 영향을 가시화하고 간호업무의 효율성을 높이기 위하여 필요하며 특히 간호사가 모든 활동을 독자적으로 수행하는 가정간호에서 보다 절실하게 요구된다. 따라서 본 연구의 목적은 가정간호의 주요 대상자인 호흡기능장애 환자를 중심으로 최근에 Iowa 대학에서 개발하여 그 타당성이 검증되고 있는 간호중재 분류체계인 NIC(Nursing Interventions Classification)의 분류틀에 기초하여 가정간호 중재를 수행하는데 지침이 되는 표준화된 간호중재 프로토콜을 개발하는 것이다. 연구방법은 1992년 4월부터 1997년 S월까지 Y의료원 가정간호사업소에 등록된 호흡기능장애 대상자 전수인 157명의 가정간호 기록지를 통하여 호흡기능장애 가정간호 대상자에게 주요한 5개 간호진단으로 기도개방유지불능, 가스교환장애, 비효율적 호흡양상, 활동의 지속성 장애, 영양부족을 선정한 후 NIC에서 제시하는 간호중재 중 해당하는 중재를 선별하고, 5개 진단을 포함한 87명의 가정간호 기록지를 대상으로 중재내용을 조사하여 비교 수정, 보완을 통해 예비 가정간호 중재목록을 작성하였다. 이에 대해 두 번의 설문지를 통하여 전문가 집단의 내용타당도 검증단계를 거친 후 최종 가정간호 중재 프로토콜을 완성하였다. 본 연구의 연구결과를 요약하면 다음과 같다. 1. 선정된 5개 간호진단에 따라 NIC에서 제시한 중재와 기록지에서 조사한 중재를 비교, 수정, 보완하여 작성한 예비 가정간호 중재목록으로 이루어진 1차 설문지에 대해 전문가 집단의 타당도를 본 결과에 따라 전체 30개의 간호중재와 이에 따른 491개의 간호활동 중 내용상 관련이 적은 중재와 간호활동은 제외하였고, 한 중재 아래 너무 자세히 서술되어 있는 활동은 관련 활동을 통합하였고, 필요하나 실정에 맞지 않는 활동은 수정하였으며, 번역이 어색한 항목은 용어를 재선정하였다. 제외 및 추가될 활동으로 표시된 항목을 재검토하여 2차 설문지는 25개 중재에 따른 329개의 간호활동으로 구성하였다. 2. 1차 내용타당도 분석결과를 바탕으로 각 항목에 대해 5점 척도로 구성된 2차 설문지 분석결과 전체 25개 간호중재의 내용타당도 점수는 .82에서 .94의 분포를 이루었고, 각 중재의 간호활동에 대한 내용타당도 점수는 1.00인 문항이 5개(1.52%), .90∼.99인 항목이 135개(41%), .80∼ .89인 항목이 165개(50.2%), .70∼ .79인 항목이 23개(6.99%), .68인 항목이 1개로 나타났다. 그 중 1차 설문지에서 추가되어야 할 항목으로 제안된 9개 중재에 따른 16개 간호활동에 대해서는 5개 항목이 .90∼ .93, 11개 항목이 .80∼ .85를 나타내어 주요한 간호활동으로 합의하였다. 3. Fehring의 내용타당도 검증방법에 근거하여 타당도 점수가 .50보다 낮은 간호활동은 탈락시키고 .80이상인 간호활동은 "주요 간호활동"으로 .80미만인 간호활동은 "부수적인 간호활동"으로 구분한 결과 각 중재에 따른 간호활동의 타당도 점수가 모두 .68이상이었으므로 이를 모두 프로토콜에 포함하기로 하였고 전체 간호활동의 92.72%가 "주요 간호활동"에 해당하였다. 이상을 내용으로 최종 가정간호 중재 프로토콜을 완성하였다. [영문] The purpose of this study was to develop a standardized nursing intervention protocol based on Nursing Interventions Classification(NIC) system for home health care clients with respiratory function problems. For this Study, Clients having five nursing diagnoses were selected from 157 home health care records between April, 1992 and August, 1997 at Yonsei Medical Center, Seoul. A sample of 87 records of clients with one or more of five diagnoses was selected. Appropriate intervention lists for the five nursing diagnoses were selected from the diagnosis-intervention linkage lists from NIC and the home health care records. The two sets of nursing interventions were compared with each other. For the content validity of the intervention protocol, Fehring's two-round Delphi technique was used. Ten experts evaluates a set of nursing intervention lists from the preliminary protocol and compared them two times using a written questionnaires. The results of this study were summarized as follows : 1. Five important nursing diagnoses of the clients extracted in a meeting of experts were Ineffective Airway Clearance: Impaired Gas Exchange; Ineffective Breathing Pattern; Activity Intolerance; Altered Nutrition: Less than body requirements. 2. After the first questionnaire analysis with 30 edited intervention labels and 491 corresponding nursing activities, several changes were made : excluding activities, changing the words of activities, adding new activities and merging two or more relative interventions or activities. 3. After two round analysis with 25 interventions arts 329 nursing activities, the Intervenention Content Validity scores(ICV) of nursing interventions were .82∼.94 and nursing activities showed weighted ratios of .68∼ 1.00, respectively. 4. All activities received ICV scores of .50 or higher indicating all activities were included. Activities with weighted ratios greater than or equal to .80 were labeled as "major" and considered to be critical activities for that intervention and activities with ratios .50 greater but less than .80, were labeled as "minor" or supporting activities, and 92.72% of activities were labeled "major nursing activities". 5.
