HMMT effect analysis of first delivery women's labor time and cesarean section ratio

역학 및 건강증진학과/석사[한글] 본 연구의 목적은 분만중인 산부에게 한서생체자기경락요법이 효과적인가를 검정하기 위한 것이다. 실험군은 2001년 1월1일부터 2001년 4월 30일까지 경기도 군포시 소재 J병원 분만실에 입원한 초산부 중 무작위 편의 추출한 49명을 대상으로 실시된 자료를 기초로 산출하였다. 실험군과의 동질성을 확보하기 위해 2001년 1월1일부터 2001년 4월30일까지 동 병원에서 출산한 총 1341명중 자석분만이 실시된 매주 월요일·수요일·금요일에 출산한 초산부 256명 전원을 대조군으로 하였다. 실험처치는 실험군의 산부에게 자궁경관이 2-6㎝ 개대되었을 때 한서생체자기경락요법에 의거해 산부 각 개인에게 필요한 경락을 정하고, 손목과 손끝·발목과 발끝에 경락의 유주방향에 따라 자석을 의료용 반창고를 이용하여 부착시킨 후 시간과 제왕절개 유무를 기록하였다. 자료수집은 분만 후 J병원의 분만대장과 진료기록부, Nurse`s Record, Labor Room Record 등에서 수술여부를 우선 분류하여 기록하였다. 분만 중 제왕절개술을 받지 않은 초산부의 기록 중 자궁경관의 완전 개대 시점이 명확히 기재되어 있는 자료를 조사하여 산모 연령, 임신횟수, 임신주수, 신생아 체중, 신생아의 성별, 진통제 투여량, 촉진제 투여량을 기록하였고, 자궁경관의 완전 개대에서부터 신생아의 만출 까지(분만 2기) 소요된 시간은 단위를 분으로 환산하여 기록하였다. 그 결과 실험군의 경우 초산부 49명중 제왕 절개술을 받은 6명과 쌍둥이 출산 1명, 자궁의 완전 개대 시점이 명확히 기록되어 있지 않은 9명을 제외한 33명이 분만 2기 소요시간의 통계적 분석대상이 되었다. 최종적으로 정리된 자료는 SAS Package Program을 활용하여 분석하였으며, 출산에 미치는 한서생체자기경락요법의 효과를 알아보기 위하여 산모 연령, 임신주수, 신생아 체중, 촉진제 투여량, 분만실에 입실하고 있던 시간과 초산부의 제왕절개 및 분만 2기 소요시간과의 관계를 기술통계, x2-Test, T-test를 통하여 비교하였다. 혼란변수들의 영향을 통제한 가운데 초산부의 분만 2기 소요시간에 미치는 한서생체자기경락요법의 효과를 알아보기 위하여 다중회귀분석( Multiple Regression Analysis)을 실시하였다. 연구결과는 다음과 같다. 초산부의 평균연령은 대조군이 28.82±2.97세, 실험군이 28.59±2.94세로 거의 차이가 없었고, 통계적으로도 유의한 차이가 없었다.(p=0.628) 날수로 환산한 평균 임신주수는 대조군이 278.36±10.27일, 실험군이 276.22±11.16일로 두 집단간 통계적으로 유의한 차이가 없었다.(p=0.405) 두 집단 간 신생아의 평균체중은 대조군이 3.252±0.420㎏, 실험군이 3.295±0.415㎏으로 약간 무거웠으나 통계적으로 유의한 차이는 없었다(p=0.513). 출산을 위하여 분만실에 입실하고 있던 평균시간은 대조군이 평균 562.60±404.24분(9.38시간)이었고, 실험군은 평균 1057.43±824.38분(17.62시간)으로 약 8.24시간 정도 더 오래 분만실에서 산통을 겪고 있는 초산부에게 한서생체자기경락요법을 적용한 것으로 조사되었으며, 통계적으로도 유의한 차이를 보였다(p=0.001). 분만 2기에 소요된 평균시간은 대조군이 51.25±33.95분, 실험군이 40.85±20.13분으로 대조군보다 10.40분 정도 단축된 것으로 나타났고, 통계적으로도 유의한 차이를 보였다(p=0.032). 산모 연령, 분만실에 입실하고 있던 기간, 신생아의 체중, 임신주수를 통제한 다변량 분석에서도 한서생체자기경락요법(p=0.022)은 초산부의 분만 2기 소요시간에 영향을 미치는것으로 나타났으며, 다섯 가지변수는 초산부의 분만 2기 소요시간을 23.9％ 설명하였다(p=0.017). 실험군의 초산부 49명 중 제왕절개술을 받은 초산부는 6명(12.24％)으로 대조군의 초산부 256명 중 제왕절개술 56명(21.88％)보다 제왕절개술에 의한 복식분만비율이 9.64％ 정도 낮았으나 통계적으로 유의한 차이를 보이지는 않았다.(p=0.125) 결론적으로 본 연구결과 한서생체자기경락요법을 산모의 출산과정에 활용하였을 때 분만소요시간의 단축과 단축시간에 따른 통증의 감소라는 긍정적인 효과를 기대할 수 있을 것으로 분석되었고, 한서생체자기경락요법은 정상적인 질식분만을 요구하는 산모들과 사회 경제적 분위기에 부합하는 새로운 한국적 출산문화 성립의 가능성을 제시할 수 있을 것이다. [영문] The purpose of this research is to examine the effect of Han-seo Meridian Magnetic Therapy (HMMT) on the process of women's delivery. The sample for the data is chosen from a hospital located in Kunpo, Kyongki-Do. The experiment group is arbitrarily chosen 49 patients among 256 who are hospitalized for their first delivery from the period of January 1, 2001 to April 30, 2001. In the experiment, the therapeutical magnets are attached to certain meridian points of the experimentee's body such as wrist, fingertips, ankle, tiptoe, and so on to their individual need according to the method of HMMT, when the cervix of uterus opens 2-6 centimeters wide. Then the duration of labor and the ratio of caesarean section among the experimentees are measured. The basic data about patients' age, time of pregnancy, weight and sex of new babies, volume of anodyne and catalyzer applied, and whether the group have section or not are collected from the nurse's record and the delivery room record. And the duration of labor in the second stage originally measured by hour is recalculated in terms of minute. The record of six experimentees that have caesarean section and ten whose time of cervical opening is not specified is ignored in this data collection. The analysis tools are X-test and T-test applied with SAS Package Program. Multiple Regression Analysis is used with complex variables under control to check how effective the HMMT is for the reduction of the duration of labor in the second stage. The average age of the experiment group is 28.59 2.94 while that of the control group 28.82 2.97(p=0.628), and the number of days pregnancy lasts in the experiment group is 276.22 11.16 while 278.36 10.27 in the control group (p=0.405), which is almost the same. The weight of new babies in each group is 3.295 0.415 kilograms and 3.2522 0.420 kilograms, which also doesn't show much difference (p=0.513). But the average time the experiment group stay in delivery room, 1057.43 824.38 minutes contrasts with that of the control group, 562.60 404.24, which draws statistical attention (p=0.001). The difference in the duration of labor in the second stage between the experiment group and the control group attracts strong statistical attention (p=0.032). The average time the experiment group are in the second stage of labor is 40.88 20.13 while the control group 51.25 33.95. In Multiple Regression Analysis with four complex variables such as age, weight of new babies, the days pregnancy lasts, and the time in delivery room under control, the HMMT proves to affect the reduction of the duration of labor in the second stage (p=0.022), and the five factors explain the duration of labor in the second stage to the degree of 23.9％ (p=0.017). The ratio of caesarean section in the experiment group is 12.24 %, a half of the ratio shown in the control group, 21.88 %, but it doesn't mean the number draws statistical attention (p=0.125). In conclusion, the HMMT shows positive effect on the reduction of the duration and also the pain of labor in the process of delivery. It is possible to say that the HMMT will correspond to and enhance women's awareness and preference of the benefit of normal vaginal delivery.ope
