Usefulness of Comparing of Magnetic Resonance Imaging (MRI) Findings Prior to Repeat Biopsy and Negative Initial Biopsy as a Decision-Making Method for Repeat Prostate Biopsy
We retrospectively reviewed the data from a cohort of 44 patients with one initial negative transrectal ultrasound-guided prostate biopsy who underwent a repeat biopsy from 2006 to 2013. At each biopsy session, we checked patient age, serum prostate specific antigen (PSA), prostate volume, PSA density, PSA velocity, months from the initial biopsy session, multiparametric magnetic resonance imaging (MRI) findings (T2-weighted, dynamic contrast-enhanced and diffusion-weighted, 1.5 Tesla pelvic-phased array) prior to repeat biopsy and initial negative biopsy. Mean age was 68.2±8.82 years. PSA was 11.5±7.65 ng/ml before repeat biopsy. Prostate cancer was detected in 15 (34.0%) patients at repeat biopsy. In univariate and multivariate analysis, positive MRI findings before repeat biopsy were significant independent predictors of a positive repeat biopsy. At per patient analysis, the sensitivity, specificity, positive and negative predictive values were 66.6, 68.9, 71.4 and 80.0% for MRI before repeat biopsy. No suspicious lesion on MRI before repeat biopsy was relevant to negative biopsy. According to the comparison of MRI findings prior to repeat biopsy and negative initial biopsy, suspicious MRI findings at the peripheral zone before repeat biopsy and initial negative biopsy were relevant to a high cancer detection rate (83.3%) at repeat prostate biopsy. These results suggested that the absence of a suspicious lesion on MRI before repeat biopsy could guide the avoidance of repeat biopsy and suspicious MRI findings at the peripheral zone before repeat biopsy and initial negative biopsy could guide repeat biopsy
Great East Japan Earthquake and Crisis of Nuclear Power Plant as TV Archive
和文:テレビアーカイブは、テレビ番組の「流れ」から番組を召喚することができる。そのとき、番組を構成する出来事の「流れ」も、出来事を表象している映像と音声の「流れ」も召喚される。そして、どのような出来事が、どのような映像と音声の「流れ」によって表象されていたのかを検証することができる。さらに、他の映像と音声の「流れ」を接続することによって、それらが表象する出来事の時間と空間が拡大し、出来事の記録と記憶としての可能性も広がっていく。東日本大震災にかんするテレビの初報は、主に中継映像の「流れ」によって表象された津波の襲来という衝撃的な出来事であった。テレビアーカイブでは、そのような中継映像の「流れ」も、震災後の出来事を表象する映像と音声の「流れ」と接続される。そのとき、接続された映像と音声の「流れ」が表象する出来事は、その時間と空間をひろげながら震災の記録と記憶となっていく。同様にして、福島第一原発の危機にかんする初期報道も、メディア環境における記録と記憶としての可能性がテレビアーカイブによって明らかになる。そこでは、原発と原子力にかんする知の未成熟、そして福島の破局的事態の時間と空間が、日本における原子力開発の史相へとつながっていることが浮き彫りになる。英文:The TV archive can summon a certain program from “flow” of TV programs. The “flow” of events to constitute a TV program and the “flow” of images and sounds representing such events are summoned then. And it comes to be examined what kind of the “flow” events is represented by what kind of the “flow” of images and sounds. Furthermore, the time and space of event that the “flow” of images and sounds represents spread out by connecting other “flows” of images and sounds, and thereby the possibilities of “flows” of images and sounds are developed as a record and memory of events. The first TV report about Great East Japan Earthquake was a shocking event of tsunami represented by the “flow” of live images mainly. In the TV archive, the “flow” of such live images is connected to some “flows” of images and sounds representing even ts after the earthquake. And such “flows” of images and sounds become a record and memory of the earthquake then while spreading out the time and space of events represented. Similarly, in the TV archive, the initial TV report about the crisis of Fukushima Dai-Ichi Nuclear Power Plant can be a record and memory in media environment. It becomes apparent that immaturity of the knowledge about nuclear power generation and atomic energy and the time and space of catastrophic situation of Fukushima lead to historical phases of the atomic energy development in Japan
