    A Case of Chronic Alveolar Abscess Extending into the Maxillary Sinus

    We report a case of chronic alveolar abscess extending into the maxillary sinus of a female, age 19 years. While signs of abscess formation and sinusitis were not visible an clinical examination, an X-ray graph showed a spherical slightly radiopaque area of mucosal enlargement without destuction of bone in the right maxillary sinus. The clinical diagnosis was suspected radicular cyst, but due to the histopathological finding of a cavity lined by granulation tissue, an alveolar abscess was diagnosed. As is well known the maxillary sinus is easily affected by odontogenic disease, because of the very close anatomic relation between this sinus and dental apices. Therefore, we have to differentiate radicular cysts from mucous cysts, and from postperative maxillary cysts, which look like radicular cysts in the X-ray findings. Roentogenographic features and the formation of alveolar abscess in maxillary sinus are discussed

    糖尿病患者における口腔粘膜の創傷治癒が遅延するメカニズムについて、口腔粘膜の創傷治癒とそれに影響を及ぼす因子、糖尿病での血流・血管新生、免疫能の変化、糖尿病での唾液量と成分の変化、糖尿病での成長因子の変化、糖尿病患者の心理的ストレスに分けて考察した。創傷治癒遅延には様々な現象が関わると考えられ、これらの現象が影響しあい遅延を引き起こしているものと考えられる

    Because the Champy\u27s miniplate can easily be shaped and molded either by binding pliers or by hand, it can be adapted to all facial bones, especially in the case of mandibular fractures in the condyle, zygomatic complex, and upper or lower orbital rim. It can also be used in mandibular reconstruction with autogenous bone grafts after resection of the mandible. We reported on 4 cases, which were representative of 12 patients who were treated suitably with this technique during the past 6 months. Consequently Champy\u27s miniplate is thought to be very valuable for the osteosynthesis of facial bone

    A Case of Tertiary Syphilis with Glossitis and Aneurysm at the Carotid

    Although the oral lesions of acquired syphilis were described as long ago as the year 1900, more recent interest in the oral manifestations of this disease had waned with the rarity of these lesions. With the spectacular development of various antibiotics, tertiary syphilis is rarely seen in modern clinics of oral and maxillofacial surgery. A case of tertiary syphilis found at the tip of the tongue and the carotid of a 55-year-old man is presented. The swelling at the tip of the tongue revealed a 10×40mm lesion, which was hard and firm with burning and aching. Extraoral examination revealed freely movable nodes in the left submaxillary area. There were no cervical nodes palpable, bat the carotid body revealed enlargement, which was soft and without pain. Biopsy was performed on the oral lesion, revealing granulomatous inflammation. Treponemal serologic tests showed lues, and the two symptoms stated above were thought to be affected with lues. Luic aneurysm are discussed regarding the frequency and the mechanism of formation

    Problems in Clinical Diagnosis of Neurilemmoma : Report of a case

    We report a case of neurilemmoma of the trigonum caroticum occurring in a 34-year-old female. She was referred to our clinic by a general practitioner because of a painless swelling in the right side of the submandibular region persisting for the previous 10 years. The mass in the submandibular region appeared to be unattached to the skin and freely movable. There were no palpable submandibular lymph nodes. This lesion was clinically diagnosed as a benign tumor of the submandibular gland. The removed mass, measuring 50×30×30mm, was encapsulated by a thin fibrous membrane. Histopathologically it was diagnosed as neurilemmoma (Antoni of type A and B). No recurrences have been noted for two years after the surgical procedure. Discussion mainly focuses on the differential diagnosis between neurilemmoma and other lesions of the trigonum caroticum
