Investigating Factor V (G1691a), Prothrombin (G20210a) and Methylenetetrahydrofolate Reductase (C677t) Gene Polymorphisms in Recurrent Pregnancy Loss

Habitüel Abortus birbirini izleyen en az iki yada daha fazla 20. gebelik haftasından
önce gebeliğin spontan olarak sonlanmasıdır.
Tekrarlayan gebelik kayıplarında çeşitli etyolojik faktörler sayılmaktadır.
Yapılan çalışmalarda yaklaşık olarak %7 kromozomal anomaliler, %10 anatomik
sorunlar, %15 hormonal düzensizlikler, %6 açıklanamayan sebepler ve %55-62
koagülasyon protein/platelet kusurları olarak bildirilmektedir. Koagülasyon
sisteminde aksaklığa neden olacak genetik kusurların son dönemde önemi daha iyi
anlaşılmıştır. Bunlar arasında en sık ilişkilendirilen Faktör V, Protrombin ve
MTHFR genlerinde yer alan bazı mutasyonlardır.
Bu çalışmada iki ve üzerinde gebelik kaybı olan 110 kadın ve gebelik kaybı
olmayan, sağlıklı çocuk sahibi olan 30 kadında FV Leiden (G1691A), Protrombin
(G20210A) ve MTHFR (C677T) gen mutasyonlarının varlığını araştırmayı
amaçladık. Tüm olguların değerlendirilmesine anamnez alınması ve hasta onay
formu doldurularak başlandı. Kan örnekleri alınıp DNA izolasyonu yapılan
örneklere Hind III ve Hinf I restriksiyon enzimleri kullanılarak PCR-RFLP yöntemi
ile mutasyon taraması yapıldı.
FV Leiden (G1691A) mutasyonu HA grubunda %13.6, kontrol grubunda
%6.7, Protrombin (G20210A) mutasyonu HA grubunda %6.4, kontrol grubunda
%6.7, MTHFR (C677T) mutasyonu HA grubunda %55.5, kontrol grubun da %53.3
olguda tespit edilmiştir. Mutasyonların bulunması sıklığına bakıldığında HA ve
kontrol grubu arasında anlamlı fark tespit edilmemiştir.
Gebelik kaybı riskinin, sipesifik bir mutasyondan çok trombofilik
mutasyonların kombine birikimine bağlı olabileceği düşünülmüştür. Belkide HA’lu
kadınlarda daha fazla trombofilik gen mutasyonunun prevalansının araştırılması
daha anlamlı olacaktır.In 75% of women trying to be pregnant, early pregnancy loss occurs.
Habitual abortion is the termination of two or more consecutive pregnancies before
20th gestational week.
Various etiologic factors are responsible for recurrent pregnancy loss. In the
performed studies these reasons are reported to be, 7% chromosomal abnormalities,
10% anatomic problems, 15% hormonal irregularities, 6% unclear reasons and 55-
62% coagulation protein/platelet problems, approximately. The importance of
genetic defects causing defficiency in the coagulation system are better understood
recently. Among them the most frequently related ones are some of the mutations
take place in the Factor V, Prothrombin and the MTHFR genes.
In our study we objected to investigate the existence of the FV Leiden
(G1691A), Prothrombin (G20210A) and MTHFR (C677T) gene mutations in 110
women with recurrent pregnancy loss and in 30 women with no pregnancy loss and
having healthy children. Evaluation of all the cases was begun by taking anamnesis
and filling patient approval form. Mutation screening was perfomed by PCR-RFLP
method using Hind III and Hinf I restriction enzymes for the blood samples of
which DNAs were isolated.
FV Leiden (G1691A) mutation was detected to be 13.6 % in the case and
6.7% in the control group, Prothrombin (G20210A) mutation was detected to be
6.4% in the case and 6.7% in the control group, MTHFR (C677T) mutation was
detected to be 55.5% in the case and 53.3% in the control group. No significant
differences were detected between the case and the control group according to the
mutation frequencies.
It is thought that the risk of pregnancy loss is related to the combined
augmentation of the thrombophilic mutations rather than a specific mutation. Probably, investigating prevalence of more thrombophilic mutations in women with
habitual abortion will be more significant
Genetic anomalies detected in patients with non-obstructive azoospermia and oligozoospermia
Genetik faktörler erkek infertilitesi nedenleri arasında
önemli bir yer tutmaktadır. Sayısal ve yapısal
kromozomal düzensizliklere, sebebi bilinmeyen
oligozoospermik ve azoospermik olgularda sık rastlanmaktadır.
Bu çalışmada 1995-2005 yılları arasında
infertilite kıliniğine başvuran 383 azoospermik ve 436
şiddetli oligozoospermik toplam 819 Türk erkek incelemeye
alındı. Bir ay ara ile iki kez spermogram ve basal
hormon profilleri (FSH-Testosterone) çalışıldı. Tüm olguların
karyogram analizleri yapıldı.
İncelenen 383 azoospermik olgunun 47(%12,3) tanesinde,
436 oligozoospermik olgunun 20(%4,6) tanesinde
kromozomal düzensizlik tesbit edilmiştir. Azoospermik
olgularda tesbit edilen kromozomal düzensizliklerin
9(%19,1) tanesi otosomal ve 38(%80,9) tanesi gonozomal
kromozomlara ait bulunmuştur. Oligozoospermik olgularda
ise tespit edilen kromozomal düzensizliklerin 8(%40)
tanesi otozomal, 12(%60) tanesi gonozomal kromozomlara
aittir.
Erkekte infertiliteye neden olan genetik nedenlerin belirlenmesi
ve yardımcı üreme teknikleri kullanılarak elde edilecek
bu bireylere ait çocukların taşıyabilecekleri genetik
riskin saptanması amacıyla infertilite kliniğine başvuran
azoospermik, oligozoospermik infertil erkeklerde genetik
danışma verilmesi ve sitogenetik inceleme yapılması
doğru tanının konmasına yardımcı olacaktır.Genetic factors have major importance in
male infertility etiology. Numerical and structural
chromosomal abnormalities seem to be frequent
inoligospermia and azoospermia cases with unknown
etiology. In this study 819 patients with azoospermia (383)
and oligospermia (436) who attended to infertility
depantinent between 1995-2005 were evaluated.
Spermogram and basic hormone proties (FSHTestesterone)
were studied two times with one month
interval from each patient and all the cases were evaluated
cytogenetically.
The 47(12,3%) of 383 azoospermia patients and the
20(4,6%) of 436 oligospermia patients were found to have
chromosomal abnormalities. The 9(19,1%) of the
chromosomal abnormalities found in azoospermia patients
were autosomal and the 38(80,9%) were gonosomal. In
oligospermia cases the 8(40%) of the chromosomal
abnormalities were autosomal and 12(60%) were
gonosomal.
Cytogenetic analysis and genetic counseling would be
heepful in infertile males with azoospermia and
oligospermia by determining the genetic factors causing
infertility and by assesing the genetic risks of the offsprigs
prowided by assisted reproductive techniques
Prevalence of consanguineous marriage in Afyonkarahisar and its relation with the occurence of congenital anomalies
Bu çalışma Afyonkarahisar ili merkez ve ilçelerindeki akraba evliliklerinin sıklığı, nedenleri ile bu evliliklerin spontan abortus ve konjenital anomalili doğumlar üzerine etkisini araştırmak üzere yapılmıştır. Çalışmada rastgele örnekleme yöntemi ile her aileden yalnızca bir kişi olacak şekilde örnek şeçilmiştir ve yüzyüze anket yöntemi uygulanmıştır. Çalışmaya alman 1940 evli kişinin 381’inde (% 19.6) akraba evliliği tesbit edilmiştir. Akraba evliliği yapanlar arasında en sık %14.8 ile 1. yeğen evlili¬ği, daha sonra ise %4.8 ile diğer yeğen evlilikleri olduğu bulunmuştur. İlk evlenme yaşına bakıldığında erken yaş evliliklerinde, özellikle 17 yaş ve altında akraba evliliği sıklığının yüksek, 31 yaş ve üzerindeki bireylerde ise oldukça nadir olduğu tespit edilmiştir. Ebeveynler arasında akrabalığın bulunması ve düşük eğitim seviyesinin akraba evliliği sıklığını artırdığı tespit edilmiştir. Akraba evliliği yapan ailelerde spontan abortus ve konjenital anomali görülme sıklığı, yabancı ile evlilik yapmış gruba göre anlamlı derecede yüksek bulunmuştur.This study was performed to search the frequency and the causes of consanguineous marriages and their effects on spontaneous abortions and births with congenital abnormalities. Consanguineous marriage was detected in 381 (19,6%) of 1940 married people in the families studied. It is detected that fist cousin marriage was the most frequent one with 14,8% and the second was other cousin marriages with 4,8% frequency. When first marriage age was evaluated, especially the frequency of consanguineous marriage was detected to be high at the ages of 17 and less, while it was rare at the ages of 31 and over. It was detected that consanguinity between parents and low education level in-creased the frequency of spontaneous abortion and congenital abnormalities. The frequency of spontaneous abortion and congenital abnormalities in families with consanguineous marriage was found to be significantly higher than the group made foreign marriage

Researching p53 gene mutation in breast carcinomas by polymerase chain reaction and enzyme restriction and displaying p53 protein in tissue by immunohistochemical method
Kanser bir seri progressif genetik değişiklikler sonucunda
oluşan çok basamaklı bir hastalıktır. Şimdiye kadar
bir grup protoonkogen ve tümör supressör gen üzerinde
meydana gelen değişikliklerin kanser oluşumunda etkili
olduğu ortaya konmuştur. Tümör supressör gen grubundan
olan ve kanser oluşumunda etkili olan genlerin başında
p53 gelmektedir. p53 meme kanserinin gelişiminde olduk-
ça etkili olduğu bilinmektedir, bu nedenle p53 gen mutasyonlarının
tespitinin hastalığın takibinde ve erken tanısında
önemli bir faktör olabileceği düşünülmektedir. Bu çalışmada,
meme kanserlerinde p53 genindeki ekson 5 ve
7’ye ait mutasyonların görülme sıklığı ve
immunohistokimyasal yöntem ile p53 proteininin varlığı-
nın tespitini amaçladık. Buna bağlı olarak meme kanseri
tanısı konmuş 20 olguya ait tümör dokusundan izole edilen
DNA örneklerinden PCR tekniği kullanılarak ekson 5
ve 7 amplifiye edildi ve enzim kesimi ile mutasyon varlığı
araştırıldı. Çalışma sonunda 20 olguya ait p53 genindeki
ekson 5 ve 7’ye ait mutasyon oranı toplam %30 olarak bulundu.
Ekson 5’e ait mutasyon oranı %20 ve ekson 7’ye
ait mutasyon oranı %11,8 olarak saptandı.
Aynı örneklere immunohistokimyasal yöntem ile p53 antikoru
uygulandı. Boyanma sonrasında 4 olguya ait örnekte
%90 ve üzeri, 1 olguda %3, 2 olguda %1 ve 2 olguda
%0,5 p53 protein pozitifliğinin varlığı tespit edildi. Olguların
11 tanesinde p53 protein varlığı gözlenmedi.Cancer is a multi-step mechanism occuring
as a result of series of progressive genetic alterations. So
far, molecular genetic studies revealed that a group of
tumor supressor gene and protooncogen alterations were
effective in cancer formation. p53 is the most important
tumor supressor gene that is effective in cancer formation.
p53 is very effective in progression of breast cancer. Due
to this, detection of p53 mutations are tought to be an
important factor in prognosis and early diagnosis. In this
study, we aimed to detect the frequency of mutations of
p53, exon 5 and 7 and their relations with progression of
the disease. Relevant to this, by PCR, amplification of 5th
and 7th exons were performed from isolated DNA
samples of tumor tissues of breast cancer diagnosed in 20
cases and mutation existence was tested by enzyme
restriction. As result of the study, mutation rate of 20
cases was found to be 30%, whereas mutation rates of
exon 5 and exon 7 were found to be 20% and 11,8%,
respectively.
Same samples were evaluated by immunohistochemistry
method using p53 antibody. After staining, in samples of 4
cases 90% and over; in sample of 1 case 3,2%; in samples
of 2 cases 1% and in samples of 2 cases 0,5% p53 protein
positivity were detected. In samples of 11 cases p53 protein
existence was not detected

Fetal chromosomal analysis of pregnancies following ıntracytoplasmic sperm ınjection with amniotic tissue culture
Ocak 1996 - Aralık 2000 tarihleri arasında İntrastoplasmik Sperm Injeksiyonu (ICSI) ile gebelik elde edilmiş ardışık 98 olgu çalışıldı. Bu olgulara ait 142 fetus amniyotik hücre kültürü ile tarandı. 142 fetusun 6 tanesinde ( %4,2 ) kromozom anomalisi saptandı. Bu anomaliler 46, XX / 69, XXX / 92, XXXX, 46, XY / 69, XXY / 92, XXYY, 47, XY+21, 47,XY+7, 47,XXY, 45,X0 şeklinde gözlendi. Biri dışında tüm gebelikler çiftlerin onayı ile soıılandırıldı. Gebeliğin sonlandırılmasını takiben yapılan fetal cilt fibroblast kültürü ile 1 olgu dışında prenatal tanı doğrulandı.
ICSI gebeliklerinde kromozomal düzensizlik oranının hafifçe artmış olduğu görülmektedir. Morfolojik anomalili spermlerin kullanılmasıyla paternal faktörler, ileri anne yaşı ile maternal faktörler ve sıklıkla de novo oluşan sex kromozomal düzensizlikler bu konuda sorumlu tutulabilir.From January 1996 to December 2000, 98 consecutive patients who had become pregnant after Intracytoplasmic Sperm Injection (ICSI) were studied. Hundred and forty-two fetuses of these patients were screened with fetal amniotic tissue culture. Chromosomal anomalies were detected from 6 out of 142 (4.2%) fetuses. Anomalies were as follows, ‘46,XX / 69,XXX / 92,XXXX’, ‘46,XY / 69, XXY / 92,XXYY’, ‘47,XY+21’, ‘47,XY+7’, ‘47,XXY’ and ‘45,X0’. All except one pregnancies were terminated with the consent of the couples. Fetal skin fibroblast cultures were also studied after termination of the pregnancy in order to confirm prenatal diagnosis. The ratio of chromosomal anomalies seems to be slightly increased in ICSI pregnancies, paternal factors due to morphology abnormal spermatozoa maternal factors due to increased mother age and more frequently, de novo occuring sex chromosome abnormalities may be responsable for this outcome
p53 gene mutation analysis in cases with bladder cancer
p53 geni, insan malignitelerinin yaklaşık
%50’sinde mutasyona uğrayan bir tümör supresör gendir.
p53 geni, mesane kanserinde en fazla mutasyona uğrayan
tümör supresör geni olma özelliğini taşımaktadır. Bu yüzden
mesane kanserinin prognozunun belirlenmesinde
önemli bir biomarker olabilir. Bu araştırmada, mesane
kanseri teşhisi konmuş 22 olguya ait mesane tümör doku
örneklerinden DNA izole edilip, PCR metoduyla 5. ve 7.
ekson bölgeleri amplifiye edildikten sonra, enzim kesimi
yapılarak 5. ve 7. eksonlarda p53 geni mutasyon analizi
gerçekleştirilmiştir. Mutasyon analizi gerçekleştirilen olguların
%45’inde p53 geni mutasyonu saptanmış olup,
saptanan mutasyonların %60’ının 5. eksonda bulunan 175.
kodonda, %40’ının ise 7. eksonda bulunan 249. kodonda
olduğu belirlenmiştir.p53 gene is a tumor supressor gene mutated
in approximately 50% of human malignities. p53 gene
has the property of being the most mutated tumor
supressor gene in bladder cancer. Due to this, it may be an
important biomarker in bladder cancer prognosis. In this
study, DNA was isolated from bladder tumor tissue
samples of 22 cases diagnosed to be bladder carcinoma.
After then, 5th and 7th exons of p53 were amplified by
PCR method and p53 mutation analysis was performed by
enzyme restriction in 5th and 7th exons. In 45% of cases
performed mutation analysis, p53 gene mutation was
detected. 60% of mutations were in 5th exon, codon 175
and 40% of mutations were in 7th exon, codon 249
Anesthesia approach in case of mosaic turner syndrome (case report with review of literature)
Mozaik turner sendromu (TS) büyüme geriliği,
gonadal disgenezi ve infertilite ile karakterize
kromozomal endokrin bir bozukluktur. 45, X/46, XX
kromozomal patern hastalığın en sık rastlanan mozaik tipidir
(%36). Turner sendromunda yeniden düzenlenmiş X
kromozomunun olduğu durumlar tolere edilebilir. Ancak
X halka kromozomu ve X otozom translokasyonlarında
mental retardasyon ve konjenital anomalilerin insidansı
belirgin şekilde artmıştır. Bu anormal fenotipler bozulmuş
veya kısmi inaktivasyona uğramış X kromozomuna bağlı-
dır. Biz burada 22 yaşında kolesistektomi ve tiroidektomi
geçirmiş, sitogenetik analiz yapılmış turner sendromlu olguyu
rapor ettik. Turner sendromunun triatlarına uygun
kısa boylu ve normal zekalı olan hastanın sitogenetik analiz
sonucunda 45,X[10]/46,XX[75] mozaik karyotipi gö-
rüldü. Beraberinde taşıdığı anomalilerden dolayı anestezik
açıdan önemli sayılan sorunlara yola açan mozaik turner
sendromlu olguda operasyonlarda izlenecek anestezi yaklaşımını
gözden geçirmeyi amaçladık.Mosaic Turner’s Syndrome is an
uncommon chromosomal endocrine disorder characterized by
retarded growth, gonadal dysgenesis and infertility. The
45,X/46,XX chromosomal pattern is the most frequent
mosaic type of this disease (36%). Rearranged X
chromosome in Turner syndrome (TS) are generally welltolerated,
but in cases of ring X chromosomes and of
X/autosome translocations the incidence of mental retardation
and other congenital abnormalities can be significantly
higher. These abnormal phenotypes can be ascribed to failed
or partial X inactivation. Here, we reported a 22-year-old
female who was referred for a cytogenetic analysis because
she developed an kolelitiasis and tyroid disorder. The patient,
with normal intelligence had the traits of TS, especially short
height and also her cytogenetic analysis showed a mosaic
45,X[10]/46,XX[75] karyotype. Mosaic Turner’s Syndrome
can lead to anesthesic complications because of
accompanying disorders. We aimed to determine anesthesic
manegement in Mosaic Turner’s Syndrome

dır. Biz burada 22 yaşında kolesistektomi ve tiroidektomi
geçirmiş, sitogenetik analiz yapılmış turner sendromlu olguyu
rapor ettik. Turner sendromunun triatlarına uygun
kısa boylu ve normal zekalı olan hastanın sitogenetik analiz
sonucunda 45,X[10]/46,XX[75] mozaik karyotipi gö-
rüldü. Beraberinde taşıdığı anomalilerden dolayı anestezik
açıdan önemli sayılan sorunlara yola açan mozaik turner
sendromlu olguda operasyonlarda izlenecek anestezi yaklaşımını
gözden geçirmeyi amaçladık.Mosaic Turner’s Syndrome is an
uncommon chromosomal endocrine disorder characterized by
retarded growth, gonadal dysgenesis and infertility. The
45,X/46,XX chromosomal pattern is the most frequent
mosaic type of this disease (36%). Rearranged X
chromosome in Turner syndrome (TS) are generally welltolerated,
but in cases of ring X chromosomes and of
X/autosome translocations the incidence of mental retardation
and other congenital abnormalities can be significantly
higher. These abnormal phenotypes can be ascribed to failed
or partial X inactivation. Here, we reported a 22-year-old
female who was referred for a cytogenetic analysis because
she developed an kolelitiasis and tyroid disorder. The patient,
with normal intelligence had the traits of TS, especially short
height and also her cytogenetic analysis showed a mosaic
45,X[10]/46,XX[75] karyotype. Mosaic Turner’s Syndrome
can lead to anesthesic complications because of
accompanying disorders. We aimed to determine anesthesic
