Azonnali, postmastectomiás emlőrekonstrukciókkal szerzett tapasztalatok. Száz eset klinikopatológiai utánkövetése és a kozmetikai eredmények felmérése | Evaluation of clinicopathological findings and cosmetic outcome of 100 immediate postmastectomy breast reconstruction cases
Absztrakt
Bevezetés: Az azonnali emlőhelyreállító műtétek – biztosítva az
onkológiai radikalitást és a megfelelő kozmetikai értéket – nem késleltetik az
adjuváns kezeléseket, lehetővé teszik a képalkotó kontrollt, hosszabb műtéti
időt igényelve. Célkitűzés: Hazai betegek körében először, az
azonnali, postmastectomiás emlőhelyreállító műtétek eredményeinek felmérése és
összehasonlítása a nemzetközi irodalom adataival. Módszer: A
szerzők 2011. május és 2014. szeptember között 100 betegen végzett 121 terápiás
és kockázatcsökkentő profilaktikus, postmastectomiás, azonnali emlőhelyreállító
műtét klinikopatológiai adatainak retrospektív feldolgozását végezték. A műtétek
onkológiai, műtéttechnikai és kozmetikai adatait statisztikai módszerekkel
elemezték. Eredmények: A betegek átlagéletkora 42,6 év, az
utánkövetési idő 29,4 hónap, a műtéti idő 132 perc volt. Leggyakrabban
bőrtakarékos mastectomiát végeztek (64%). Az emlőhelyreállítás 70%-ban
submuscularis szöveti expanderrel történt. Korai posztoperatív szövődményt 18
esetben, lokoregionális kiújulást egy betegnél észleltek. Az adjuváns kezelések
megkezdésének átlagideje 4,8 hét volt. A kozmetikai végeredménnyel a betegek
89%-a volt elégedett. Következtetések: Az azonnali
emlőhelyreállítás technikái biztonságosan és eredményesen végezhetők, megfelelve

Abstract
Introduction: Immediate breast reconstruction provides
oncological safety, requires longer operation time. It does not influence the
initiation of adjuvant therapy and radiological control, and results in
favourable cosmetic outcome. Aim: Assessing the Hungarian data
of immediate postmastectomy breast reconstructions, and comparing them to
international findings. Method: Between May, 2011 and
September, 2014 121 therapeutic and prophylactic, postmastectomy immediate
breast reconstructions were performed in 100 patients. The clinico-pathological
findings were assessed retrospectively, and surgical, oncological and cosmetic
outcomes were evaluated statistically. Results: The mean age of
patients was 42.6 years, the follow up time was 29.4 months, and the duration of
operation was 132 minutes. Skin-sparing mastectomy was performed most commonly
(64%) with submuscular tissue expander placement (70%). Early postoperative
complication was identified in 18 patients, and loco-regional recurrence in 1
patient. Most patients (89%) were satisfied with the cosmetic outcome. The
average initiation time of adjuvant therapy was 4.8 weeks.
Conclusions: Immediate breast reconstruction is a safe and

Absztrakt
Bevezetés: A European Society of Breast Cancer Specialists
létrehozta az emlőrákellátást végző onkológiai központok és szakorvosok európai
minőségbiztosítási feltételrendszerét, hogy egységes minimumstandardok
bevezetésével csökkentse az emlőrák mortalitását a kontinensen.
Célkitűzés: Jelen vizsgálat célja, hogy az Országos
Onkológiai Intézet Emlő- és Lágyrészsebészeti Osztályán operált, illetve az
intézetben multidiszciplinárisan kezelt emlődaganatok komplex onkológiai
ellátását a European Society of Breast Cancer Specialists minőségbiztosítási
kívánalmai szerint elemezze és a nagy esetszámú, reprezentatív minta révén a
magyar emlőrákos populációról részletes klinikopatológiai adatokat szolgáltasson
az onkológiai ellátórendszer számára. Módszer: 2011. június 1.
és 2012. május 31. között multidiszciplinárisan kezelt emlődaganatok részletes
klinikopatológiai retrospektív adatfeldolgozását végezték a European Society of
Breast Cancer Specialists egységes nemzetközi kritériumrendszere szerint.
Eredmények: A vizsgált időszakban 906 betegnél történt
malignus, illetve jóindulatú emlődaganat miatt műtét. Az EUSOMA kötelező
minőségbiztosítási minimumelvárásainak az Országos Onkológiai Intézet eredményei
mindenben megfeleltek. Következtetések: Az Országos Onkológiai
Intézet emlődiagnosztikával és multidiszciplináris onkológiai kezelésével
foglalkozó emlőrák-ellátási szervezeti egysége a European Society of Breast

Abstract
Introduction: The European Society of Breast Cancer Specialists
has created quality indicators for breast units to establish minimum standards
and to ensure specialist multimodality care with the conscious aim of improving
outcomes and decreasing breast cancer mortality. Aim: The aim
of this study was to analyse the breast cancer care in the National Institute of
Oncology according to the European Society of Breast Cancer Specialists
requirements and in a large number of cases in order to present representative
clinico-pathological data on the incidence of breast cancer in Hungary.
Method: According to the European Society of Breast Cancer
Specialists uniformed criteria clinico-pathological data of multimodality
treated breast cancer cases were retrospectively analysed between June 1, 2011
and May 31, 2012. Results: During the period of interest 906
patients underwent breast surgery for malignant or benign lesions. According to
the European Society of Breast Cancer Specialists quality indicators the breast
cancer care of the National Institute of Oncology is eligible.
Conclusions: The diagnostic modalities and multimodality
care of breast cancer of the National Institute of Oncology breast unit meets
the critical mass and minimum standards of the European Society of Breast Cancer

Sentinel lymph node biopsy following previous axillary surgery in recurrent breast cancer
Ipsilateral breast recurrence or second primary breast cancer can develop in patients who have undergone breast conserving surgery (BCS) and axillary surgery. The purpose of this study was to examine the feasibility of a reoperative sentinel lymph node biopsy (SLNB) as a repeated axillary staging procedure.From August 2014 through January 2017 patients with locally recurrent breast cancer or with BRCA mutation requiring risk reduction mastectomy as a second surgical procedure, underwent repeat SLNB in three Hungarian Breast Units with a radiocolloid (and blue dye) technique.Hundred and sixty repeat SLNBs were analysed, 80 after previous SLNB and 80 after previous total or partial axillary lymph node dissection (ALND). SLN identification was successful in 106 patients (66%); 77/80 (77.5%) and 44/80 (55%) in the SLNB and ALND groups, respectively. (p < 0.003). Extra-axillary lymph drainage was more frequent in the ALND group (19/44, 43,2% versus 7/62, 11,3%; p < 0.001). Lymphatic drainage to the contralateral axilla was observed in 14 patients (11 in the ALND group, p = 0.025), isolated parasternal drainage was detected in 4 patients (p = 0.31). Only 9/106 patients with successful repeat SLNB (8,8%, all with 1 SLN removed) had SLN metastases CONCLUSIONS: Repeat SLNB is feasible in patients with ipsilateral breast tumor recurrence or new ipsilateral primary tumor after previous BCS and axillary staging. Repeat SLNB should replace routine ALND as the standard axillary restaging procedure in recurrent disease with a clinically negative axilla. Preoperative lymphoscintigraphy is important to explore extra-axillary lymphatic drainage in this restaging setting
A módosított Regnault „B” emlőmegtartó műtét mint II. szintű standard onkoplasztikus emlősebészeti technika
Bevezetés és célkitűzés: A szerzők a Regnault „B” típusú emlőfelvarrás sebészi technikájának módosításával végzett onkoplasztikus műtéti technikát és az új emlősebészeti módszer alkalmazásához kapcsolódó retrospektív klinikopatológiai vizsgálat eredményeit mutatják be. Módszer: 2012. április és 2018. október között, emlőrák miatt, módosított Regnault „B” technikával operált 215 nőbeteg klinikopatológiai adatait prospektíven vezetett adatbázis alapján retrospektív módon vizsgáltuk. A betegek életminőségét validált kérdőív segítségével mértük fel, míg a műtétek esztétikai eredményét a szintén validált Breast Cancer Conservative Treatment (BCCT.core) számítógépes program és az 5 pontos Likert-skála alapján értékeltük. Eredmények: A betegek átlagéletkora 53 év (szórás: 29–81 év) volt. A medián utánkövetési idő 47 hónap (szórás: 7–85 hónap) volt. Az átlag műtéti idő 47 perc (szórás: 35–85 perc) volt, míg a patológiai tumorméret átlagosan 33 mm-nek (szórás: 18–58 mm) bizonyult. Pozitív sebészi szél miatt 13 (6%) esetben irányított reexcisióra, míg 3 (1,4%) esetben mastectomiára kényszerültünk. Az összesített szövődményarány 7,4% (n = 16) volt. Az esztétikai eredmények Likert-skála szerinti átlagértéke 4,2 (szórás: 2–5), míg a BCCT.core program alapján 1,3 pont (szórás: 1–4 pont) volt. Az életminőséggel kapcsolatos kérdőívek eredményei magas betegelégedettséget igazoltak. Következtetés: A módosított Regnault „B” emlőmegtartó technika biztonságos és hatékony standard ’level II.’ onkoplasztikus emlősebészeti technika. A műtét magas betegelégedettséggel, illetve kozmetikai eredménnyel képes a közepes vagy nagyobb térfogatú emlők külső, illetve külső-felső negyedeinek T1–T3-as tumorai miatt az emlők akár 20–50%-ának eltávolítására, majd egyidejű parenchymaáthelyezéssel történő rekonstrukciójára. A módszer előnye, hogy nem igényel ellenoldali szimmetrizációs műtétet, míg hátránya, hogy az emlő bőrpalástján vezetett metszések komplettáló mastectomia esetén az azonnali rekonstrukciót nehezítik
Modern Breast Cancer Surgery 1st Central-Eastern European Professional Consensus Statement on Breast Cancer
This text is based on the recommendations accepted by the 4th Hungarian Consensus Conference on Breast Cancer, modified on the basis of the international consultation and conference within the frames of the Central-Eastern European Academy of Oncology. The recommendations cover non-operative, intraoperative and postoperative diagnostics, determination of prognostic and predictive markers and the content of cytology and histology reports. Dermatofibrosarcoma protuberans multidiszciplináris kezelésével szerzett hosszú távú eredmények 26 betegnél = Long-term experiences with multidisciplinary therapy of twenty-six patients with dermatofibrosarcoma protuberans and review of the literature
A dermatofibrosarcoma protuberans alacsony vagy közepes malignitású, ritka, rosszindulatú daganat. A tumort a lassú, de agresszív lokális növekedés, az alacsony áttétképzési és a magas helyi kiújulási arány jellemzi. Az elsődleges kezelés a radikális sebészi eltávolítás hagyományos kimetszéssel vagy Mohs-féle sebészi technikával. Pozitív sebészi szél, recidív tumor esetén radio-, kemoterápia, illetve újabban imatinib mesylat alkalmazható.
Célkitűzés:
A szerzők 26, dermatofibrosarcoma protuberans miatt onkológiai centrumban multidiszciplinárisan kezelt beteg hosszú távú klinikopatológiai utánkövetését végezték.
Módszer és eredmények:
A betegek átlagéletkora 44,7 év volt. Az utánkövetés átlagideje 60,57 hónap volt. Tizenöt betegnél (57,7%) sikerült R0 eltávolítást végezni, míg 11 betegnél (42,3%) csak R1 reszekciót. Az R0 reszekciók eléréséhez átlagosan 1,87 kimetszésre volt szükség. Adjuváns kezelésként a primer tumor eltávolítását követően 6 beteg (23%) részesült radioterápiában és 2 (7,6%) beteg kemoterápiában. Tizenhat betegnél nem észleltünk kiújulást. Tíz betegnél (38,4%) alakult ki recidíva, amely miatt további kezeléseket folytattunk. Egy beteg távoli áttétek miatt elhunyt. Statisztikai módszerekkel vizsgáltuk az irodalomban prognosztikai faktornak számító 50 év feletti életkor és a sebészi radikalitás hatását a helyi kiújulásra.
Következtetések:
A dermatofibrosarcoma protuberans multidiszciplináris kezeléssel eredményesen kezelhető daganat. Pontosabb következtetések levonásához nagyobb esetszám és multicentrikus randomizált vizsgálatok szükségesek.

Dermatofibrosarcoma protuberans is a low or moderate grade malignant, uncommon soft tissue tumor. The tumor is characterized by slow, but locally aggressive growth, low metastatic potential and high recurrence rate. Initial treatment is the radical surgical excision, using traditional wide excision or Mohs surgery. In case of positive surgical margin or local recurrence, radio-chemotherapy and recently imatinib mesylate is used as adjuvant therapy.
Aims:
Twenty-six patients treated multidisciplinary for dermatofibrosarcoma protuberans were followed up.
Methods and Results:
Mean age of the patients was 44.7 years; mean follow-up time was 60.57 months. In fifteen cases (57.7%) R0 resection was performed, while eleven patients (42,3%) received only R1 resection. An average of 1.87 resections was necessary in order to achieve R0 resection. Six patients (23%) received adjuvant radiotherapy and two patients (7.6%) adjuvant chemotherapy following the removal of the primary tumor. Sixteen patients had no local recurrence. Ongoing treatments were needed in the case of ten patients (38.4%) who developed local recurrence. One patient has deceased due to distant metastases. Using statistical methods we examined the effects indicated as prognostic factors in the literature on local recurrence, precisely, the effect of age above 50 years and surgical radicalism.
Conclusions:
Dermatofibrosarcoma protuberans can be successfully treated with multidisciplinary therapy. A larger number of cases and randomized multicenter investigations are needed in order to reach more accurate conclusion
A SPECT/CT szerepe az őrszemnyirokcsomó preoperatív meghatározásában, emlőrákban = The potential role of SPECT/CT in the preoperative detection of sentinel lymph nodes in breast cancer
A regionális nyirokcsomóstátus az emlőrák legfontosabb prognosztikai faktora. A korai, klinikailag nyirokcsomó-negatív emlőrákoknál az őrszemnyirokcsomó-biopszia az axillaris staging standard módszere. A preoperatív limfoszcintigráfia az őrszemnyirokcsomók számának és elhelyezkedésének meghatározásával pontosítja a biopsziát. Az utóbbi években az idős, túlsúlyos betegeknél, atípusos vagy extraaxillaris nyirokelvezetésnél, nem ábrázolódó, illetve az izotópbeadási helyhez közeli őrszemnyirokcsomók meghatározásához egy új, hibrid képalkotó eljárás, a komputertomográfiával kombinált single photon emissziós komputertomográf (3D SPECT/CT) nyújt segítséget. A szerzők magyarul elsőként tekintik át az emlőrák-őrszemnyirokcsomó meghatározásával foglalkozó, a 3D SPECT/CT teljes – máig mindössze 14 közleményből álló – angol nyelvű szakirodalmát. Összefoglalásként elmondható, hogy a 3D SPECT/CT növeli a preoperatív őrszemnyirokcsomó-meghatározás eredményességét, minőségét, segítve ezzel a rutin klinikai gyakorlatban az emlőrákos betegek pontosabb stagingjét. Regional lymph node status is the most important prognostic factor in breast cancer. Sentinel lymph node biopsy is the standard method of axillary staging in early breast cancer patients with clinically negative nodes. Preoperative lymphoscintigraphy might support refining biopsy findings by determining the number and location of sentinel lymph nodes. In aged or overweight patients, in the presence of atypical or extra-axillary lymphatic drainage, non-visualized lymph nodes, or sentinel lymph nodes close to the isotope injection site, detection could be aided by a new, hybrid imaging tool: the single-photon emission computed tomography combined with computed tomography (3D SPECT/CT). For the first time in Hungarian language, authors overview the literature: all 14 English-language articles on the implementation of 3D SPECT/CT in sentinel lymph node detection in breast cancer are included. It is concluded that 3D SPECT/CT increases the success rate and quality of preoperative sentinel node identification, and is capable of providing a more accurate staging of breast cancer patients in routine clinical practice. A magyar emlőrákos betegek igényei a korszerű onkoplasztikus emlősebészeti ellátásra
The significant need for breast reconstruction resulting from the spread of oncoplastic breast surgery raises a number of systemic issues. Clarification and regulation of the indications are needed for aesthetic changes of the reconstructed breast due to oncotherapy treatments, ageing and technical problems of implants; a number of operations, targeted aesthetic goals as well as surgical capacities and financial background should also be determined.Our aim was to conduct a survey on the opinions and needs of the Hungarian breast cancer population about a modern breast reconstruction system.A study was conducted enrolling 500 patients who underwent mastectomy with immediate or delayed reconstruction. A structured questionnaire containing eleven questions was used to measure the attitude for loss and reconstruction of breast, the expectation of cosmetic outcome and qualification of the operating surgeon and the needs relating to the health system and funding.The median age was 47 years (min.-max.: 26-73), 59% (n = 294) was married and 52% (n = 260) had graduated in university. The majority of women (70%; n = 348) would like to have nakedly also similar breasts after the reconstruction process. To achieve this, 43% (n = 217) and 37% (n = 184) would undergo maximum two or four procedures, respectively, supported by the national health insurance company. 86% (n = 430) would like to choose qualified breast surgeon for her treatment.The modern oncoplastic treatment raises complex, systemic issues. Women with breast cancer would like to have qualified breast surgeons restoring their breasts by two operations, all funded by the national health insurance company. Association of Axillary Dissection With Systemic Therapy in Patients With Clinically Node-Positive Breast Cancer.
The role of axillary lymph node dissection (ALND) to determine nodal burden to inform systemic therapy recommendations in patients with clinically node (cN)-positive breast cancer (BC) is currently unknown.
To address the association of ALND with systemic therapy in cN-positive BC in the upfront surgery setting and after neoadjuvant chemotherapy (NACT).
This was a prospective, observational, cohort study conducted from August 2018 to June 2022. This was a preplanned study within the phase 3 randomized clinical OPBC-03/TAXIS trial. Included were patients with confirmed cN-positive BC from 44 private, public, and academic breast centers in 6 European countries. After NACT, residual nodal disease was mandatory, and a minimum follow-up of 2 months was required.
All patients underwent tailored axillary surgery (TAS) followed by ALND or axillary radiotherapy (ART) according to TAXIS randomization. TAS removed suspicious palpable and sentinel nodes, whereas imaging-guidance was optional. Systemic therapy recommendations were at the discretion of the local investigators.
A total of 500 patients (median [IQR] age, 57 [48-69] years; 487 female [97.4%]) were included in the study. In the upfront surgery setting, 296 of 335 patients (88.4%) had hormone receptor (HR)-positive and Erb-B2 receptor tyrosine kinase 2 (ERBB2; formerly HER2 or HER2/neu)-negative disease: 145 (49.0%) underwent ART, and 151 (51.0%) underwent ALND. The median (IQR) number of removed positive lymph nodes without ALND was 3 (1-4) nodes compared with 4 (2-9) nodes with ALND. There was no association of ALND with the proportion of patients undergoing adjuvant chemotherapy (81 of 145 [55.9%] vs 91 of 151 [60.3%]; adjusted odds ratio [aOR], 0.72; 95% CI, 0.19-2.67) and type of systemic therapy. Of 151 patients with NACT, 74 (51.0%) underwent ART, and 77 (49.0%) underwent ALND. The ratio of removed to positive nodes was a median (IQR) of 4 (3-7) nodes to 2 (1-3) nodes and 15 (12-19) nodes to 2 (1-5) nodes in the ART and ALND groups, respectively. There was no observed association of ALND with the proportion of patients undergoing postneoadjuvant systemic therapy (57 of 74 [77.0%] vs 55 of 77 [71.4%]; aOR, 0.86; 95% CI, 0.43-1.70), type of postneoadjuvant chemotherapy (eg, capecitabine: 10 of 74 [13.5%] vs 10 of 77 [13.0%]; trastuzumab emtansine-DM1: 9 of 74 [12.2%] vs 11 of 77 [14.3%]), or endocrine therapy (eg, aromatase inhibitors: 41 of 74 [55.4%] vs 36 of 77 [46.8%]; tamoxifen: 8 of 74 [10.8%] vs 6 of 77 [7.8%]).
Results of this cohort study suggest that patients without ALND were significantly understaged. However, ALND did not inform systemic therapy recommendations
