Prognosis of sporadic resected small (<= 2 cm) nonfunctional pancreatic neuroendocrine tumors - a multi-institutional study

Background: Malignant potential of small ( Methods: Patients were identified from databases of 16 centers. Outcomes and risk factors for recurrence were identified by uni-and multivariate analyses. Results: sNF-PNET was resected in 210 patients, and 66% (n = 138) were asymptomatic. Median age was 60 years, median tumor size was 15 mm, parenchyma-sparing surgery was performed in 42%. Postoperative mortality was 0.5% (n = 1), severe morbidity rate was 14.3% (n = 30), and 14 of 132 patients (10.6%) with harvested lymph nodes had metastatic lymph nodes. Tumor size, presence of biliary or pancreatic duct dilatation, and WHO grade 2-3 were independently associated with recurrence. Patients with tumors sized Conclusions: In sNF-PNETs, the presence of biliary or pancreatic duct dilatation or WHO grade 2-3 advocate for surgical treatment. In the remaining patients, a wait-and-see policy might be considered.Peer reviewe

La teoria política de l'austriacisme: El comte del S.R.I. Juan Amor de Soria

Sovint quan es parla de la guerra de Successió sembla que les diferencies polítiques entre els dos bandols no fossin cIares. En d'altres casos es parla d'una conflagració internacional sense que els factors interns siguin rellevants. Penso que ambdues posicions són erronies. Unes posicions a les que no repugnaria una frase com la següent: «els trets diferencials del projecte polític austriacista resulten extremadament difícils d'identificar». Discrepo basicament d'aquesta frase. En tots els posicionaments austriacistes hi ha molts punts comuns. Una comparació del contingut del Tractat de Genova (1705) amb Via fora als adormits (1734) i Record de i'alianr;a (1736) o amb el pensament teoric de Juan Amor de Soria posa de manifest la coincidencia en una Espanya polisinodial o composta basada en les diferents Corts, de la que la Corona d' Aragó i Catalunya n'eren vertebradors. A l'interior de l' austriacisme hi havia dos corrents doctrinals. El primer estava format per un tradicionalisme foralista molt subratllat i el segon els que volien fer compatibles el respecte a les Corts amb l' eficacia del govern. Quan Via fora ais adormíts, una magnífica mostra del primer corrent, fou jutjada pels homes de I'Emperador fou condemnada i immediatament amnistiada pel fet de pert~myer al mateix ordre d'idees. Ningú representa millar el pensament polític i economic austriacista que I'aragones Juan Amor de Soria sobre el que presentem la nostra visió en dos aspectes fonamentals del seu pensament.
