Penetrating eye injuries at the workplace : case reports and discussion

We describe two recent cases of penetrating eye injury seen at Mater Dei Hospital, both resulting in a poor final outcome and illustrating the importance of wearing appropriate eye protection at the workplace. In case 1, injury was caused by a metal chip produced by a hammer and chisel that penetrated the sclera and lodged in the vitreous cavity, requiring vitrectomy and lens extraction and ultimately resulting in severe visual loss. In case 2, injury was caused by a shard released from an angle grinder disc that struck the orbit and caused severe disruption of the globe, which required enucleation. This is followed by a discussion on penetrating eye injuries, summarising the initial assessment and management of a suspected penetrating injury in the primary care setting and highlighting the need for urgent ophthalmic referral in cases where the nature of work was suggestive of high-speed projectiles, even where the trauma is seemingly trivial.peer-reviewe

Holistic obstetrics: the origins of "natural childbirth" in Britain.

The term "natural childbirth" denotes an approach to childbirth characterised by a bias towards physical and mental hygiene in the management of pregnancy and labour. It emerged in Britain in the interwar period, partly as a response to the growing interventionism of mainstream obstetrics. Its appeal since then has rested on the belief that it could provide a holistic approach to maternity care, capable of addressing the needs of the "whole" patient. At the same time, "natural childbirth" has provided a means of expressing anxieties about the social, economic and political upheavals of the 20th century. Long-lived space observatories for astronomy and astrophysics

NASA's plan to build and launch a fleet of long-lived space observatories that include the Hubble Space Telescope (HST), the Gamma Ray Observatory (GRO), the Advanced X Ray Astrophysics Observatory (AXAF), and the Space Infrared Telescope Facility (SIRTF) are discussed. These facilities are expected to have a profound impact on the sciences of astronomy and astrophysics. The long-lived observatories will provide new insights about astronomical and astrophysical problems that range from the presence of planets orbiting nearby stars to the large-scale distribution and evolution of matter in the universe. An important concern to NASA and the scientific community is the operation and maintenance cost of the four observatories described above. The HST cost about $1.3 billion (1984 dollars) to build and is estimated to require$160 million (1986 dollars) a year to operate and maintain. If HST is operated for 20 years, the accumulated costs will be considerably more than those required for its construction. Therefore, it is essential to plan carefully for observatory operations and maintenance before a long-lived facility is constructed. The primary goal of this report is to help NASA develop guidelines for the operations and management of these future observatories so as to achieve the best possible scientific results for the resources available.
