Aquisição e processamento de sons crepitantes para auxÃlio ao diagnóstico de enfermidades pulmonares
Tese (doutorado) - Universidade Federal de Santa Catarina, Centro Tecnológico, Programa de Pós-Graduação em Engenharia Elétrica, Florianópolis, 2011O presente trabalho aborda o desenvolvimento e a implementação de um sistema de aquisição de sons pulmonares de baixo custo para investigar técnicas para a aquisição e processamento de sons crepitantes que estão presentes em pacientes com fibrose, pneumonia e insuficiência cardÃaca congestiva (ICC). O objetivo é captar sons que portem mais informação de diagnóstico, bem como analisá-los para melhor caracterizar parâmetros de diagnóstico propostos na literatura. Visa, assim, contribuir para a obtenção de parâmetros quantitativos que possam auxiliar o diagnóstico e o acompanhamento do estado clÃnico de pacientes. Os resultados obtidos mostraram que a utilização de filtro passa- altas e ganhos adequados possibilitam o registro de sons com maior frequência máxima. É também discutido o efeito da atenuação causada pelo percurso do som entre os pulmões e a superfÃcie do tórax comprometendo a análise do crepitante, pois altera as caracterÃsticas morfológicas e espectrais desses sinais. Para recuperar o sinal submetido ao efeito desse canal, técnicas de equalização FLSI (Fir Least Square Inverse) e EVA (Eigenvector Approach) foram aplicadas elevando as frequência máxima e recuperando a morfologia do sinal. Este trabalho também mostra que a utilização das técnicas DPWD (Pseudo Discrete Wigner-Ville Distribution) e MGM (Método Geométrico Modificado), adequada a sinas não estacionários, aos crepitantes possibilita a detecção objetiva da sua máxima frequência em diferentes enfermidades, possibilitando a diferenciação de pacientes com fibrose e pneumonia e entre pacientes com fibrose e ICC (Insuficiência CardÃaca Congestiva). Com a técnica foi possÃvel estimar a frequência máxima maior nas enfermidades estudadas do que as apresentadas na literatura. A variação da taxa respiratória e volume relativo medidos nessas taxas não alterou a estimativa da frequência máxima de maneira estatististicamente significante. This paper discusses the development and implementation of a system of acquisition of lung sounds of low-cost techniques to investigate the acquisition and processing of crackling sounds that are present in patients with cystic fibrosis, pneumonia and congestive heart failure (CHF). The goal is to capture sounds that carry more diagnostic information and analyze them to better define diagnostic criteria proposed in the literature. Therefore aims to contribute to the achievement of quantitative parameters that may assist the diagnosis and monitoring of clinical status of patients. The results showed that using high-pass filter and allow adequate returns the record sounds with higher maximum frequency. It also discussed the effect of attenuation caused by the sound path between the lungs and chest area compromising the analysis of crackling, because it alters the morphological and spectral those signals. To recover the signal subjected to the effect of channel equalization techniques have been applied FLSI (Fir Least Square Inverse) and EVA (Eigenvector Approach) and raising the maximum frequency and morphology of recovering the signal. This work also shows that the use of techniques and DPWD (Pseudo Discrete Wigner-Ville Distribution) and MGM (Modified Geometric Method) appropriate to non-stationary signal, the crackling allows the objective detection of its maximum frequency in different diseases, enabling the differentiation of patients with fibrosis and pneumonia among patients with fibrosis and congestive heart failure. With the technique it was possible to estimate the maximum frequency higher in the studied illnesses reported in the literature. The variation of relative volume and respiratory rate measured in these rates did not change the estimate of the maximum frequency was statistically significantly so
