    Speech Illusions in People at Clinical High Risk for Psychosis Linked to Clinical Outcome

    Background and hypothesis Around 20% of people at clinical high risk (CHR) for psychosis later develop a psychotic disorder, but it is difficult to predict who this will be. We assessed the incidence of hearing speech (termed speech illusions [SIs]) in noise in CHR participants and examined whether this was associated with adverse clinical outcomes. Study design At baseline, 344 CHR participants and 67 healthy controls were presented with a computerized white noise task and asked whether they heard speech, and whether speech was neutral, affective, or whether they were uncertain about its valence. After 2 years, we assessed whether participants transitioned to psychosis, or remitted from the CHR state, and their functioning. Study results CHR participants had a lower sensitivity to the task. Logistic regression revealed that a bias towards hearing targets in stimuli was associated with remission status (OR = 0.21, P = 042). Conversely, hearing SIs with uncertain valence at baseline was associated with reduced likelihood of remission (OR = 7.72. P = .007). When we assessed only participants who did not take antipsychotic medication at baseline, the association between hearing SIs with uncertain valence at baseline and remission likelihood remained (OR = 7.61, P = .043) and this variable was additionally associated with a greater likelihood of transition to psychosis (OR = 5.34, P = .029). Conclusions In CHR individuals, a tendency to hear speech in noise, and uncertainty about the affective valence of this speech, is associated with adverse outcomes.     Does Maturity Signals High Risk and High Return?

    The objective of this study is to examine the interaction between firm maturity and firm growth opportunities over risk and its impact on returns. This study uses 135 firms listed in Indonesia Stock Exchange during 2010 to 2016 as sample which gives 945 as total observed data. This study conducts path analysis in term for hypothesis testing and finds that firm maturity has significant role to increase the risk which gives impact on increasing the returns. In context of Indonesian firms, the findings imply that mature firms will have higher risk and higher returns

    Impact of adverse childhood experiences on educational achievements in young people at clinical high risk of developing psychosis

    BACKGROUND: Adverse childhood experiences (ACE) can affect educational attainments, but little is known about their impact on educational achievements in people at clinical high risk of psychosis (CHR). METHODS: In total, 344 CHR individuals and 67 healthy controls (HC) were recruited as part of the European Community'sSeventh Framework Programme-funded multicenter study the European Network of National Schizophrenia Networks Studying Gene-Environment Interactions (EU-GEI). The brief version of the Child Trauma Questionnaire was used to measure ACE, while educational attainments were assessed using a semi-structured interview. RESULTS: At baseline, compared with HC, the CHR group spent less time in education and had higher rates of ACE, lower rates of employment, and lower estimated intelligence quotient (IQ). Across both groups, the total number of ACE was associated with fewer days in education and lower level of education. Emotional abuse was associated with fewer days in education in HC. Emotional neglect was associated with a lower level of education in CHR, while sexual abuse was associated with a lower level of education in HC. In the CHR group, the total number of ACE, physical abuse, and neglect was significantly associated with unemployment, while emotional neglect was associated with employment. CONCLUSIONS: ACE are strongly associated with developmental outcomes such as educational achievement. Early intervention for psychosis programs should aim at integrating specific interventions to support young CHR people in their educational and vocational recovery.     In today’s modern world, cybercrime is skyrocketing globally, which impacts a variety of organizations and endpoint users. Hackers are using a multitude of approaches and tools, including ransomware threats, to take over targeted systems. These acts of cybercrime lead to huge damages in areas of business, healthcare systems, industry sectors, and other fields. Ransomware is considered as a high risk threat, which is designed to hijack the data. This paper is demonstrating the ransomware types, and how they are evolved from the malware and trojan codes, which is used to attack previous incidents, and explains the most common encryption algorithms such as AES, and RSA, ransomware uses them during infection process in order to produce complex threats. The practical approach for data encryption uses python programming language to show the efficiency of those algorithms in real attacks by executing this section on Ubuntu virtual machine. Furthermore, this paper analyzes programming languages, which is used to build ransomware. An example of ransomware code is being demonstrated in this paper, which is written specifically in C sharp language, and it has been tested out on windows operating system using MS visual studio. So, it is very important to recognize the system vulnerability, which can be very useful to prevent the ransomware. In contrast, this threat might sneak into the system easily, allowing for a ransom to be demanded. Therefore, understanding ransomware anatomy can help us to find a better solution in different situations. Consequently, this paper shows a number of outstanding removal techniques to get rid from ransomware attacks in the system

    Early Detection of High Risk Pregnancy

    There are 30.939 pregnant women in Banyumas, with 6.206 cases referred due to high-risk pregnancies. Petahunan village in Pekuncen has the the highest incidence of high-risk pregnancies compared with other villages. The purpose of this study is to describe the implementation of early detection of high-risk pregnancies in Petahunan village, Pekuncen. This study used qualitative research methods with case study approach. Research instruments used in-depth interviews and focus group disscussion toward early detection of high-risk pregnancy issue. The informants were 7 people include pregnant women, health workers, midwives, head of village and village health forum chairman. The result showed only antenatal care implemented to detect high-risk pregnancy and no programs and community empowerment efforts in early detection of high-risk pregnancies

    High Risk of HIV Towards Sexual Transaction, Use of Drugs, and Lingual Symbol of Day-pay Workers

    The number of people suffering from HIV/AIDS increases from year to year in Padang City. The two main causes are free sex and drug use. One of the most frequent subpopulations of free sex and drug use is a day-pay worker in Teluk Bayur Seaport. They have their own lingual behaviors, patterns, and symbols for the sexual transaction and drug use. These are the aspects discussed in this paper. A qualitative study with a trans-sectional non-experimental design was applied. Data were collected through interviews, existing documents, and social networks. A thematic content analysis is used to analyze. The results proved the opportunity to commit free sex and drug use in Teluk Bayur Seaport is in the waiting period of the ship arrival. The most frequently used locations are on boats, on trucks, and wild lodges around the seaport. Types of drugs with the use of hypodermic needles are thought of to be potentially high in HIV infection. Research has also proven that day-pay workers at Teluk Bayur Seaport use lingual symbols and numeric symbols in sexual transactions and drug use

    Post-Transplant Outcomes in High-Risk Compared with Non-High-Risk Multiple Myeloma: A CIBMTR Analysis.

    Conventional cytogenetics and interphase fluorescence in situ hybridization (FISH) identify high-risk multiple myeloma (HRM) populations characterized by poor outcomes. We analyzed these differences among HRM versus non-HRM populations after upfront autologous hematopoietic cell transplantation (autoHCT). Between 2008 and 2012, 715 patients with multiple myeloma identified by FISH and/or cytogenetic data with upfront autoHCT were identified in the Center for International Blood and Marrow Transplant Research database. HRM was defined as del17p, t(4;14), t(14;16), hypodiploidy (-Y) or chromosome 1 p and 1q abnormalities; all others were non-HRM. Among 125 HRM patients (17.5%), induction with bortezomib and immunomodulatory agents (imids) was higher compared with non-HRM (56% versus 43%, P \u3c .001) with similar pretransplant complete response (CR) rates (14% versus 16%, P .1). At day 100 post-transplant, at least a very good partial response was 59% in HRM and 61% in non-HRM (P = .6). More HRM patients received post-transplant therapy with bortezomib and imids (26% versus 12%, P = .004). Three-year post-transplant progression-free (PFS) and overall survival (OS) rates in HRM versus non-HRM were 37% versus 49% (P \u3c .001) and 72% versus 85% (P \u3c .001), respectively. At 3 years, PFS for HRM patients with and without post-transplant therapy was 46% (95% confidence interval [CI], 33 to 59) versus 14% (95% CI, 4 to 29) and in non-HRM patients with and without post-transplant therapy 55% (95% CI, 49 to 62) versus 39% (95% CI, 32 to 47); rates of OS for HRM patients with and without post-transplant therapy were 81% (95% CI, 70 to 90) versus 48% (95% CI, 30 to 65) compared with 88% (95% CI, 84 to 92) and 79% (95% CI, 73 to 85) in non-HRM patients with and without post-transplant therapy, respectively. Among patients receiving post-transplant therapy, there was no difference in OS between HRM and non-HRM (P = .08). In addition to HRM, higher stage, less than a CR pretransplant, lack of post-transplant therapy, and African American race were associated with worse OS. In conclusion, we show HRM patients achieve similar day 100 post-transplant responses compared with non-HRM patients, but these responses are not sustained. Post-transplant therapy appeared to improve the poor outcomes of HRM
