Fingolimod (FTY-720) is Capable of Reversing Tumor Necrosis Factor Induced Decreases in Cochlear Blood Flow
Hypothesis: The potential of Fingolimod (FTY-720), a sphingosine-1-phosphate analogue, to revoke the changes in cochlear blood flow induced by tumor necrosis factor (TNF) was investigated. Background: Impairment of cochlear blood flow has often been considered as the common final pathway of various inner ear pathologies. TNF, an ubiquitous cytokine, plays a major role in these pathologies, reducing cochlear blood flow via sphingosine-1-phosphate-signaling. Methods: Fifteen Dunkin-Hartley guinea pigs were randomly assigned to one of three groups (placebo/placebo, TNF/placebo, TNF/FTY-720). Cochlear microcirculation was quantified over 60 minutes by in vivo fluorescence microscopy before and after topical application of placebo or TNF (5 ng/ml) and after subsequent application of placebo or FTY-720 (200 mu g/ml). Results: Treatment with TNF led to a significant decrease of cochlear blood flow. Following this, application of placebo caused no significant changes while application of FTY-720 caused a significant rise in cochlear blood flow. Conclusions: FTY-720 is capable of reversing changes in cochlear blood flow induced by application of TNF. This makes FTY-720 a valid candidate for potential treatment of numerous inner ear pathologies
Multiple sclerosis drug FTY-720 toxicity is mediated by the heterotypic fusion of organelles in neuroendocrine cells
FTY-720 (Fingolimod) was one of the first compounds authorized for the treatment of multiple sclerosis. Among its other activities, this sphingosine analogue enhances exocytosis in neuroendocrine chromaffin cells, altering the quantal release of catecholamines. Surprisingly, the size of chromaffin granules is reduced within few minutes of treatment, a process that is paralleled by the homotypic fusion of granules and their heterotypic fusion with mitochondria, as witnessed by dynamic confocal and TIRF microscopy. Electron microscopy studies support these observations, revealing the fusion of several vesicles with individual mitochondria to form large, round mixed organelles. This cross-fusion is SNARE-dependent, being partially prevented by the expression of an inactive form of SNAP-25. Fused mitochondria exhibit an altered redox potential, which dramatically enhances cell death. Therefore, the cross-fusion of intracellular organelles appears to be a new mechanism to be borne in mind when considering the effect of FTY-720 on the survival of neuroendocrine cells

Fingolimod (Gilenya) to jedyny doustny lek immunomodulujący,
który uzyskał rekomendację w Stanach Zjednoczonych w 2010
roku — w postaci rzutowo-remitującej stwardnienia rozsianego
(RRMS, relapsing-remitting multiple sclerosis) i w Unii Europejskiej
(UE) w 2011 roku — w wysoce aktywnej postaci choroby
oraz jako lek drugiego rzutu w RRMS u chorych nieodpowiadających
na leczenie interferonem β. Mechanizm jego działania polega na
blokowaniu receptorów S1P1, 3, 5 (sphingosine 1-phosphate) na
limfocytach T, co ogranicza migrację limfocytów z węzłów chłonnych
do krwi obwodowej, zmniejszając ich liczbę bez upośledzenia
funkcji CD4+ i B. Hamujący wpływ fingolimodu na przebieg
stwardnienia rozsianego wynika również z bezpośredniego działania
antyapoptycznego na komórki oligodendrogleju poprzez hamowanie
cytokin prozapalnych oraz aktywację mikrogleju. Przeprowadzono
dwie zasadnicze próby kliniczne III fazy z zastosowaniem
fingolimodu w RRMS. Pierwsza to 24-miesięczna próba

Sclerosis (FREEDOMS), w której porównano skuteczność
dwóch dawek fingolimodu (0,5 mg/d. i 1,25 mg/d.) z placebo,
zaś druga — Trial Assessing Injectable Interferon Versus FTY 720
Oral in Relapsing-Remitting Multiple Sclerosis (TRANSFORMS)
— to 12-miesięczna, wydłużona do 24 miesięcy próba, w której
oceniano skuteczność i bezpieczeństwo fingolimodu podawanego
doustnie w dawkach 0,5 mg/dobę i 1,25 mg/dobę w porównaniu
z interferonem β-1a (IFNβ-1a) podawanym w dawce 30 mg,
domięśniowo, raz w tygodniu. Podobnie jak w pierwszym badaniu
roczny wskaźnik rzutów był istotnie niższy w obu grupach —
leczonych fingolimodem lub IFNβ-1a, podawanym domięśniowo
(0,16 v. 0,20 v. 0,33). Korzystny efekt potwierdzono w badaniu rezonansu magnetycznego mózgu. W obu próbach oceniono objętość
mózgu; ograniczenie zaniku mózgu w badaniu FREEDOMS wynosiło
38% w porównaniu z grupą przyjmującą placebo, natomiast
w TRANSFORMS — 40% w porównaniu z leczonymi IFNβ-1a po
12 miesiącach terapii. W grupie otrzymującej 1,25 mg/dobę fingolimodu
w 1. przypadku wystąpił zgon z powodu ciężkiej infekcji
wirusem opryszczki powikłanej opryszczkowym zapaleniem mózgu,
a w 2. zgon nastąpił w wyniku infekcji wirusem ospy wietrznej,
mimo odstawienia fingolimodu i zastosowania dożylnej terapii
przeciwwirusowej. W innym przypadku po 339 dniach leczenia
fingolimodem wystąpiło opryszczkowe zapalenie mózgu. Pozostałe
objawy niepożądane obejmowały łagodne postacie infekcji
wirusem opryszczki oraz: infekcje dróg oddechowych, dróg moczowych,
bradykardię, blok przedsionkowo-komorowy w czasie
rozpoczynania leczenia fingolimodem, nadciśnienie tętnicze, obrzęk
plamki żółtej, zwiększenie aktywności enzymów wątrobowych oraz
zmniejszenie liczby limfocytów we krwi obwodowej, wynikające
z mechanizmu działania fingolimodu. Należy podkreślić, że poważne
działania niepożądane (rak skóry, rak piersi, ziarnica złośliwa)
występowały w przypadku stosowania większej dawki fingolimodu,
tj. 1,25 mg/dobę. U leczonych dotychczas fingolimodem
chorych nie stwierdzono występowania wieloogniskowej postępującej
leukoencefalopatii. Poza wyżej wymienionymi objawami
niepożądanymi, ujawniającymi się w przypadku stosowania większej
dawki leku, inne objawy niepożądane były podobne, jak
w grupie przyjmującej placebo. W celu poprawy bezpieczeństwa
leczenia fingolimodem w dawce 0,5 mg/dobę zalecono następujące
postępowanie: kontrolę morfologii krwi obwodowej w czasie
6 miesięcy od rozpoczęcia terapii, ocenę aktywności enzymów
wątrobowych oraz stężenia bilirubiny, badanie elektrokardiograficzne
(kontrola u chorych przyjmujących b-adrenolityki, antagonistów wapnia, leki przeciwarytmiczne klas I–III, u chorych z blokiem
przedsionkowo-komorowym II stopnia i wyższym w wywiadzie,
chorobą węzła zatokowego, wydłużonym odstępem Q-T, chorobą
niedokrwienną serca, rytmem serca < 55/min lub niemiarowością
pracy serca). Przed rozpoczęciem leczenia u wszystkich chorych
powinno być przeprowadzone podstawowe badanie okulistyczne
(w UE zalecane tylko u chorych obciążonych czynnikami ryzyka). U pacjentów, którzy nie chorowali na ospę wietrzną bądź
nie byli przeciw niej szczepieni, w przypadku braku przeciwciał,
należy rozważyć wykonanie szczepienia przeciw tej chorobie
(pierwsze podanie leku miesiąc po szczepieniu). Przed i po podaniu
pierwszej dawki fingolimodu przez 6 godzin należy kontrolować
tętno i ciśnienie tętnicze

Defective sphingosine-1-phosphate receptor 1 (S1P1) phosphorylation exacerbates TH17-mediated autoimmune neuroinflammation
Sphingosine-1-phosphate (S1P) signaling regulates lymphocyte egress from lymphoid organs into systemic circulation. Sphingosine phosphate receptor 1 (S1P1) agonist, FTY-720 (Gilenya™) arrests immune trafficking and prevents multiple sclerosis (MS) relapses. However, alternative mechanisms of S1P-S1P1 signaling have been reported. Phosphoproteomic analysis of MS brain lesions revealed S1P1 phosphorylation on S351, a residue crucial for receptor internalization. Mutant mice harboring a S1pr1 gene encoding phosphorylation-deficient receptors [S1P1(S5A)] developed severe experimental autoimmune encephalomyelitis (EAE) due to T helper (TH) 17-mediated autoimmunity in the peripheral immune and nervous system. S1P1 directly activated Janus-like kinase–signal transducer and activator of transcription 3 (JAK-STAT3) pathway via interleukin 6 (IL-6). Impaired S1P1 phosphorylation enhances TH17 polarization and exacerbates autoimmune neuroinflammation. These mechanisms may be pathogenic in MS

The development by the Edmonton group of a sirolimus-based, steroid-free, low-tacrolimus regimen is a significant breakthrough that allows the rate of insulin independence after islet transplantation to increase from 13% to 80% at 1 year ; however, the rate is reduced to 50% at 3 years, attributed to prolonged tacrolimus exposure. Recently, immunosuppression agents such as cyclosporine, mycophenolate mofetil, and the novel agent FTY 720 have been used instead of tacrolimus. Lymphocytedepleting antibodies such as anti-thymocyte globulin, alemtuzumab, and hOKT3gamma 1 (ala, ala) have been launched, and a costimulatory blockade of anti-CD40 monoclonal antibodies and CTLA4-Ig will be attempted in the near future. Moreover, the potential of a novel immunosuppressing peptide could now be realized using new technology called the protein transduction system. Pericytes regulate vascular immune homeostasis in the CNS.
Pericytes regulate the development of organ-specific characteristics of the brain vasculature such as the blood-brain barrier (BBB) and astrocytic end-feet. Whether pericytes are involved in the control of leukocyte trafficking in the adult central nervous system (CNS), a process tightly regulated by CNS vasculature, remains elusive. Using adult pericyte-deficient mice (Pdgfb ret/ret ), we show that pericytes limit leukocyte infiltration into the CNS during homeostasis and autoimmune neuroinflammation. The permissiveness of the vasculature toward leukocyte trafficking in Pdgfb ret/ret mice inversely correlates with vessel pericyte coverage. Upon induction of experimental autoimmune encephalomyelitis (EAE), pericyte-deficient mice die of severe atypical EAE, which can be reversed with fingolimod, indicating that the mortality is due to the massive influx of immune cells into the brain. Additionally, administration of anti-VCAM-1 and anti-ICAM-1 antibodies reduces leukocyte infiltration and diminishes the severity of atypical EAE symptoms of Pdgfb ret/ret mice, indicating that the proinflammatory endothelium due to absence of pericytes facilitates exaggerated neuroinflammation. Furthermore, we show that the presence of myelin peptide-specific peripheral T cells in Pdgfb ret/ret ;2D2 tg mice leads to the development of spontaneous neurological symptoms paralleled by the massive influx of leukocytes into the brain. These findings indicate that intrinsic changes within brain vasculature can promote the development of a neuroinflammatory disorder
