    SOMWeb: Supporting a Distributed Clinical Community of Practice Using Semantic Web Technologies

    This thesis concerns supporting the collaboration and knowledge sharing of distributed clinicians of oral medicine, a sub-discipline of dentistry. The Swedish Oral Medicine Network (SOMNet) holds monthly telephone conferences where a group of clinicians discuss interesting and difficult cases, which distinguishes it from one-to-one teleconsultations. SOMNet can be seen as a distributed community of practice, that is, a group of people sharing a concern and who interact regularly to extend their individual and collective expertise. Related to this, several topics need further investigation: How can geographically distributed clinical collaboration be characterized? What is appropriate functionality for a Web-based system supporting such collaborations? What are the impacts of such systems on collaboration? Further, Semantic Web technologies, such as the Web Ontology Language (OWL), have been proposed as a means of enhancing knowledge sharing. What are benefits and limitations of using these technologies to encode domain knowledge in oral medicine and to support clinical collaboration, and what practical issues face developers? The developed system, SOMWeb, focuses on functionality for meetings and structured cases, and has been regularly used for three years. Interviews, observations, a questionnaire, system log analysis, and case analysis were used to study SOMNet's collaboration and identify system impacts. The documentation of the forms of collaboration in SOMNet can serve as a model for other groups of clinicians wishing to establish a distributed collaboration. SOMNet's meetings provide a necessary rhythm for the community and the cases give context to the clinicians' learning which point toward that the centrality of meetings and cases in a tool will benefit collaboration. Impacts on SOMNet's collaboration include enabling the participation of a wider range of clinics. Factors influencing this are the more accessible submission process as well as the increased tangibility of the collaboration. The thesis also provides recommendations for developers of systems supporting clinical collaboration and knowledge sharing. The use of OWL in examination descriptions has enabled reasoning over cases in the system to provide improved case browsing. At the same time, limitations were found in using OWL for examination templates. Based on the lessons learned in this development, the thesis provides recommendations for using Semantic Web technologies, which can be of value for other developers and to guide future research

    Semantic Integration of Cervical Cancer Data Repositories to Facilitate Multicenter Association Studies: The ASSIST Approach

    The current work addresses the unification of Electronic Health Records related to cervical cancer into a single medical knowledge source, in the context of the EU-funded ASSIST research project. The project aims to facilitate the research for cervical precancer and cancer through a system that virtually unifies multiple patient record repositories, physically located in different medical centers/hospitals, thus, increasing flexibility by allowing the formation of study groups “on demand” and by recycling patient records in new studies. To this end, ASSIST uses semantic technologies to translate all medical entities (such as patient examination results, history, habits, genetic profile) and represent them in a common form, encoded in the ASSIST Cervical Cancer Ontology. The current paper presents the knowledge elicitation approach followed, towards the definition and representation of the disease’s medical concepts and rules that constitute the basis for the ASSIST Cervical Cancer Ontology. The proposed approach constitutes a paradigm for semantic integration of heterogeneous clinical data that may be applicable to other biomedical application domains

    The use of computer-interpretable clinical guidelines to manage care complexities of patients with multimorbid conditions : a review

    Clinical practice guidelines (CPGs) document evidence-based information and recommendations on treatment and management of conditions. CPGs usually focus on management of a single condition; however, in many cases a patient will be at the centre of multiple health conditions (multimorbidity). Multiple CPGs need to be followed in parallel, each managing a separate condition, which often results in instructions that may interact with each other, such as conflicts in medication. Furthermore, the impetus to deliver customised care based on patient-specific information, results in the need to be able to offer guidelines in an integrated manner, identifying and managing their interactions. In recent years, CPGs have been formatted as computer-interpretable guidelines (CIGs). This enables developing CIG-driven clinical decision support systems (CDSSs), which allow the development of IT applications that contribute to the systematic and reliable management of multiple guidelines. This study focuses on understanding the use of CIG-based CDSSs, in order to manage care complexities of patients with multimorbidity. The literature between 2011 and 2017 is reviewed, which covers: (a) the challenges and barriers in the care of multimorbid patients, (b) the role of CIGs in CDSS augmented delivery of care, and (c) the approaches to alleviating care complexities of multimorbid patients. Generating integrated care plans, detecting and resolving adverse interactions between treatments and medications, dealing with temporal constraints in care steps, supporting patient-caregiver shared decision making and maintaining the continuity of care are some of the approaches that are enabled using a CIG-based CDSS

    The Relationship of Somatosensory Perception and Fine-Force Control in the Adult Human Orofacial System

    The orofacial area stands apart from other body systems in that it possesses a unique performance anatomy whereby oral musculature inserts directly into the underlying cutaneous skin, allowing for the generation of complex three-dimensional deformations of the orofacial system. This anatomical substrate provides for the tight temporal synchrony between self-generated cutaneous somatosensation and oromotor control during functional behaviors in this region and provides the necessary feedback needed to learn and maintain skilled orofacial behaviors. The Directions into Velocity of Articulators (DIVA) model highlights the importance of the bidirectional relationship between sensation and production in the orofacial region in children learning speech. This relationship has not been as well-established in the adult orofacial system. The purpose of this observational study was to begin assessing the perception-action relationship in healthy adults and to describe how this relationship may be altered as a function of healthy aging. This study was designed to determine the correspondence between orofacial cutaneous perception using vibrotactile detection thresholds (VDT) and low-level static and dynamic force control tasks in three representative age cohorts. Correlational relationships among measures of somatosensory capacity and low-level skilled orofacial force control were determined for 60 adults (19-84 years). Significant correlational relationships were identified using non-parametric Spearman’s correlations with an alpha at 0.1 between the 5 Hz test probe and several 0.5 N low-level force control assessments in the static and slow ramp-and-hold condition. These findings indicate that as vibrotactile detection thresholds increase (labial sensation decreases), ability to maintain a low-level force endpoint decreases. Group data was analyzed using non-parametric Kruskal-Wallis tests and identified significant differences between the 5 Hz test frequency probe and various 0.5 N skilled force assessments for group variables such as age, pure tone hearing assessments, sex, speech usage and smoking history. Future studies will begin the processing of modeling this complex multivariate relationship in healthy individuals before moving to a disordered population

    Conflict resolution in clinical treatments

    Dissertação de mestrado integrado em Engenharia InformáticaCurrently, in the health area, there is a need for systems that provide support for the decision of health professionals through specific recommendations for each patient based on Clinical Practice Guidelines (CPGs) for automatic interpretation. CPGs are documents that have enormous importance in the daily life of health professionals, playing a key role in reducing variations in medical practice, improving the quality of health care, and reducing health care costs. These documents reflect knowledge about how best to diagnose and treat diseases in the form of a list of clinical recommendations. However, there may be conflicts and interactions in the application of these clinical recommendations, that which in their maximum exponent may impair the patient’s clinical condition. These conflicts are transported to decision support systems, creating the need to develop computational methods to solve these same conflicts. In the case of multimorbid patients, this resolution of conflicts can be very problematic because these patients suffer from several pathologies at the same time, and that the use of a drug for one particular pathology may have a detrimental effect on the application of another drug in another pathology. Therefore, the objective of this dissertation topic is the determination of conflicts and interactions between drugs and the determination of these same alternatives.Atualmente na área da saúde, existe uma necessidade de existirem sistemas que forneçam apoio à decisão dos profissionais de saúde através de recomendações específicas para cada paciente com base em protocolos clínicos para interpretação automática. Os protocolos clínicos são documentos que têm enorme importância no dia-a-dia dos profissionais de saúde, desempenhando um papel fundamental na redução das variações na prática médica, na melhoria da qualidade dos cuidados de saúde e na redução dos custos de saúde. Estes documentos reflectem o conhecimento sobre a melhor forma de diagnosticar e tratar doenças na forma de uma lista de recomendações clínicas. Contudo, podem existir conflitos e interações na aplicação destas recomendações clínicas, que no seu expoente máximo poderão levar a um agravamento do estado clínico do paciente, nomeadamente no caso da aplicação de diferentes fármacos. Estes conflitos são transportados para os sistemas de apoio à decisão, criando a necessidade de desenvolver métodos computacionais de resolução destes mesmos conflitos. No caso dos pacientes multimórbidos esta resolução de conflitos pode ser bastante problemática devido ao facto destes pacientes sofrerem de várias patologias ao mesmo tempo, e que a utilização de um fármaco para uma determinada patologia possa vir a ter um efeito nocivo na aplicação de outro fármaco noutra patologia. Sendo assim, o objetivo deste tema de dissertação é a determinação dos conflitos e interações entre fármacos e a determinação dessas mesmas alternativas
