Görög a görög válság
Görögország az eurózóna legsúlyosabb válságban lévő országa, szinte minden lényeges gazdasági mutatója a legrosszabb a 17 ország közül. A válság kirobbanása óta a görög GDP volumene 17,4 százalékkal esett vissza, 2013-ra 4 százalék körül lehet a csökkenés, és a növekedés 2014 előtt a legjobb esetben sem indul meg.
A visszaesésben egyértelműen a belföldi fogyasztás drasztikus (2007 és 2012 között 17,3 százalékos) visszaesése játszotta a fő szerepet. A lakossági fogyasztás mellett a beruházások is csökkennek, 2007 és 2011 között éves szinten mintegy 25 milliárd euróval, a GDP 25,9 százalékáról 14,9 százalékára, és fellendülésük a közeljövőben sem várható. A munkanélküliség a válság alatt megháromszorozódott, 2012 júniusában 1,22 millió főt (24,4%) tett ki. A fiatalok helyzete még aggasztóbb, munkanélküliségi rátájuk 55,4 százalék.
A költsĂ©gvetĂ©s hiánya 2012-ben 6,7 százalĂ©kra várhatĂł, Ă©s mĂ©g 2013-ban is nagyobb lesz, mint 4 százalĂ©k. Az államadĂłsság a privát adĂłsság egy rĂ©szĂ©nek leĂrása mellett is meghaladja a GDP 160 százalĂ©kát
A kĂĽlsĹ‘ egyensĂşly már rögtön az euróövezethez valĂł csatlakozás után jelentĹ‘sen romlott, de romlás 2005 után felgyorsult. A fizetĂ©si mĂ©rleghiány növekedĂ©se nagyrĂ©szt a jövedelemmĂ©rleg (bĂ©rek, beruházási jövedelmek egyenlege) 2000 után gyorsan növekvĹ‘ passzĂvumának köszönhetĹ‘. 2000 elejĂ©tĹ‘l 2012 közepĂ©ig 115,7 milliárd eurĂł hagyta el az országot, amit csak 42,3 milliárd eurĂł beáramlás ellentĂ©telezett. A legnagyobb tĂ©telt a portfĂłliĂł Ă©s egyĂ©b beruházási jövedelmek kiáramlása adta, ezek nettĂł összĂ©rtĂ©ke a tárgyalt idĹ‘szakban megközelĂtette a 73 milliárd eurĂłt.
A kereskedelem terĂ©n Görögország egyre inkább kifordul az EU-bĂłl, ami az eurĂł utĂłbbi idĹ‘ben törtĂ©nt gyengĂĽlĂ©sĂ©vel magyarázhat. Az eurĂł leĂ©rtĂ©kelĹ‘dĂ©se ugyanis javĂtja versenykĂ©pessĂ©gĂ©t más valutájĂş országokkal szemben, de nincs hatással az eurĂł-partnerekkel szembeni versenykĂ©pessĂ©gĂ©re.
Hogy a nyĂlt államcsĹ‘döt elkerĂĽlje, Görögország kĂ©t mentĹ‘csomagban, összesen 238 milliárd eurĂłt kapott az eurĂłzĂłnátĂłl, az EU-tĂłl Ă©s az IMF-tĹ‘l. Az ezekhez kapcsolt, a költsĂ©gvetĂ©s egyensĂşlyba hozatalát cĂ©lzĂł intĂ©zkedĂ©sekkel azonban nem halad, mert azok társadalmi ára sĂşlyos. Szinte semmi elĹ‘relĂ©pĂ©s nem törtĂ©nt az adĂłelkerĂĽlĂ©s vagy a vállalkozási környezet javĂtása terĂ©n, Ă©s nem sikerĂĽl Ă©rtĂ©kesĂteni a görög állami termelĹ‘alapokat sem, amelyekbĹ‘l pedig 50 milliárd eurĂłt kellene beszedni 2014-ig.
Bár a bĂ©rek, minimálbĂ©rek, nyugdĂjak, juttatások már jelentĹ‘sen csökkentek, további 13,5 milliárd eurĂłnyi kiigazĂtásra van szĂĽksĂ©g 2013-14-ben, amelybĹ‘l 3 milliárd eurĂł adĂłemelĂ©sbĹ‘l, 6,7 milliárd nyugdĂj- Ă©s bĂ©rcsökkentĂ©sbĹ‘l, a többi az egĂ©szsĂ©gĂĽgyi, az oktatási Ă©s a hadikiadások megnyirbálásábĂłl adĂłdik.
Bár a kialakult helyzetĂ©rt a görög elit felelĹ‘tlensĂ©ge mellett a világgazdasági válság Ă©s az eurĂłzĂłna belsĹ‘ egyensĂşlytalanságai a felelĹ‘sek, a terhek dandárját a görög lakosságnak kell viselnie. A görögök eurĂłzĂłnábĂłl valĂł kilĂ©pĂ©se továbbra is napirenden van, mivel azonban ez az eurĂłzĂłnát is romba döntenĂ©, az EU vezetĹ‘i valĂłszĂnűleg mindent megmozgatnak ennek elkerĂĽlĂ©sĂ©Ă©rt. Music therapy in Greece and its applications in dementia and end-of-life care

dementia and end-of-life care. Initially we offer an overview of music therapy
in the country and outline key developments in terms of professionalisation,
education, research and scholarship. Exploring these developments from a
critical perspective, we acknowledge contemporary debates and their
implications for the advancement of the field. This exploration offers a platform
for understanding the current applications of music therapy in dementia and
adult end-of-life care in Greece. We provide examples of recent projects and
initiatives alongside broader considerations regarding the status of dementia
and end-of-life care. We conclude by offering some perspectives for future

Ponovni pregled predstavbne Dodone
The aim of this paper is to put together the currently dispersed information about the occurrence of architectural remains at the archaeological site of Dodona during the so-called prebuilding phase, mainly at the end of the Late Bronze Age-Beginning of Early Iron Age through the reports and diaries of the site’s excavators. Moreover, the combination of the architectural remains with the portable finds will lead to suggestions about the site’s character during late prehistory. Was it a humble village inhabited by stockbreeders, or a hypaethral sanctuary?Namen članka je združiti trenutno razpršene podatke o pojavu arhitekturnih ostankov na arheološkem najdišču Dodona v času t. i. predstavbne faze, kar pomeni med pozno bronasto in zgodnjo železno dobo, in sicer s pomočjo poročil in dnevnikov izkopavalcev najdišča. Poleg tega bo kombinacija arhitekturnih ostankov s prenosnimi najdbami vodila do predlogov o značaju tega najdišča v pozni prazgodovini. Vascular conditioning prevents adverse left ventricular remodelling after acute myocardial infarction: a randomised remote conditioning study
Aims:
Remote ischemic conditioning (RIC) alleviates ischemia–reperfusion injury via several pathways, including micro-RNAs (miRs) expression and oxidative stress modulation. We investigated the effects of RIC on endothelial glycocalyx, arterial stiffness, LV remodelling, and the underlying mediators within the vasculature as a target for protection.
Methods and results:
We block-randomised 270 patients within 48 h of STEMI post-PCI to either one or two cycles of bilateral brachial cuff inflation, and a control group without RIC. We measured: (a) the perfusion boundary region (PBR) of the sublingual arterial microvessels to assess glycocalyx integrity; (b) the carotid-femoral pulse wave velocity (PWV); (c) miR-144,-150,-21,-208, nitrate-nitrite (NOx) and malondialdehyde (MDA) plasma levels at baseline (T0) and 40 min after RIC onset (T3); and (d) LV volumes at baseline and after one year. Compared to baseline, there was a greater PBR and PWV decrease, miR-144 and NOx levels increase (p  15% (odds-ratio of 3.75, p = 0.029). MiR-144 and PWV changes post-RIC were interrelated and associated with LVESV reduction at follow-up (r = 0.40 and 0.37, p < 0.05), in the single-cycle RIC.
Conclusion:
RIC evokes “vascular conditioning” likely by upregulation of cardio-protective microRNAs, NOx production, and oxidative stress reduction, facilitating reverse LV remodelling
Indications and practical approach to non-invasive ventilation in acute heart failure
In acute heart failure (AHF) syndromes significant respiratory failure (RF) is essentially seen in patients with acute cardiogenic pulmonary oedema (ACPE) or cardiogenic shock (CS). Non-invasive ventilation (NIV), the application of positive intrathoracic pressure through an interface, has shown to be useful in the treatment of moderate to severe RF in several scenarios. There are two main modalities of NIV: continuous positive airway pressure (CPAP) and pressure support ventilation (NIPSV) with positive end expiratory pressure. Appropriate equipment and experience is needed for NIPSV, whereas CPAP may be administered without a ventilator, not requiring special training. Both modalities have shown to be effective in ACPE, by a reduction of respiratory distress and the endotracheal intubation rate compared to conventional oxygen therapy, but the impact on mortality is less conclusive. Non-invasive ventilation is also indicated in patients with AHF associated to pulmonary disease and may be considered, after haemodynamic stabilization, in some patients with CS. There are no differences in the outcomes in the studies comparing both techniques, but CPAP is a simpler technique that may be preferred in low-equipped areas like the pre-hospital setting, while NIPSV may be preferable in patients with significant hypercapnia. The new modality 'high-flow nasal cannula' seems promising in cases of AHF with less severe RF. The correct selection of patients and interfaces, early application of the technique, the achievement of a good synchrony between patients and the ventilator avoiding excessive leakage, close monitoring, proactive management, and in some cases mild sedation, may warrant the success of the technique
