    Towards case-based medical learning in radiological decision making using content-based image retrieval

    <p>Abstract</p> <p>Background</p> <p>Radiologists' training is based on intensive practice and can be improved with the use of diagnostic training systems. However, existing systems typically require laboriously prepared training cases and lack integration into the clinical environment with a proper learning scenario. Consequently, diagnostic training systems advancing decision-making skills are not well established in radiological education.</p> <p>Methods</p> <p>We investigated didactic concepts and appraised methods appropriate to the radiology domain, as follows: (i) Adult learning theories stress the importance of work-related practice gained in a team of problem-solvers; (ii) Case-based reasoning (CBR) parallels the human problem-solving process; (iii) Content-based image retrieval (CBIR) can be useful for computer-aided diagnosis (CAD). To overcome the known drawbacks of existing learning systems, we developed the concept of image-based case retrieval for radiological education (IBCR-RE). The IBCR-RE diagnostic training is embedded into a didactic framework based on the Seven Jump approach, which is well established in problem-based learning (PBL). In order to provide a learning environment that is as similar as possible to radiological practice, we have analysed the radiological workflow and environment.</p> <p>Results</p> <p>We mapped the IBCR-RE diagnostic training approach into the Image Retrieval in Medical Applications (IRMA) framework, resulting in the proposed concept of the IRMAdiag training application. IRMAdiag makes use of the modular structure of IRMA and comprises (i) the IRMA core, i.e., the IRMA CBIR engine; and (ii) the IRMAcon viewer. We propose embedding IRMAdiag into hospital information technology (IT) infrastructure using the standard protocols Digital Imaging and Communications in Medicine (DICOM) and Health Level Seven (HL7). Furthermore, we present a case description and a scheme of planned evaluations to comprehensively assess the system.</p> <p>Conclusions</p> <p>The IBCR-RE paradigm incorporates a novel combination of essential aspects of diagnostic learning in radiology: (i) Provision of work-relevant experiences in a training environment integrated into the radiologist's working context; (ii) Up-to-date training cases that do not require cumbersome preparation because they are provided by routinely generated electronic medical records; (iii) Support of the way adults learn while remaining suitable for the patient- and problem-oriented nature of medicine. Future work will address unanswered questions to complete the implementation of the IRMAdiag trainer.</p

    Structuring cooperative nuclear risk reduction initiatives with China

    The Stanford Center for International Security and Cooperation engaged several Chinese nuclear organizations in cooperative research that focused on responses to radiological and nuclear terrorism. The objective was to identify joint research initiatives to reduce the global dangers of such threats and to pursue initial technical collaborations in several high priority areas. Initiatives were identified in three primary research areas: 1) detection and interdiction of smuggled nuclear materials; 2) nuclear forensics; and 3) radiological (“dirty bomb”) threats and countermeasures. Initial work emphasized the application of systems and risk analysis tools, which proved effective in structuring the collaborations.     Keeping the Organization in the Loop as a General Concept for Human-Centered AI: The Example of Medical Imaging

    This study emanates from work on human-centered AI and the claim of “keeping the organiza-tion in the loop”. A previous study suggests a sys-tematic framework of organizational practices in the context of predictive maintenance, and identified four cycles: using AI, customizing AI, original task handling with support of AI, and dealing with con-textual changes. Since we assume that these findings can be generalized for other kinds of applications of Machine Learning (ML), we contrast the manage-ment activities that support the four cycles and their interplay with a widely different domain: the usage of AI for radiology. Our literature analysis reveals a series of overlaps with the existing framework, but also results in the need for extensions, such as holis-tic consideration of workflows or supervision and quality assurance

    Now in its 11th edition, The Global Risks Report 2016 draws attention to ways that global risks could evolve and interact in the next decade. The year 2016 marks a forceful departure from past findings, as the risks about which the Report has been warning over the past decade are starting to manifest themselves in new, sometimes unexpected ways and harm people, institutions and economies. Warming climate is likely to raise this year's temperature to 1° Celsius above the pre-industrial era, 60 million people, equivalent to the world's 24th largest country and largest number in recent history, are forcibly displaced, and crimes in cyberspace cost the global economy an estimated US$445 billion, higher than many economies' national incomes. In this context, the Reportcalls for action to build resilience – the "resilience imperative" – and identifies practical examples of how it could be done.The Report also steps back and explores how emerging global risks and major trends, such as climate change, the rise of cyber dependence and income and wealth disparity are impacting already-strained societies by highlighting three clusters of risks as Risks in Focus. As resilience building is helped by the ability to analyse global risks from the perspective of specific stakeholders, the Report also analyses the significance of global risks to the business community at a regional and country-level

    An examination of the development of standards, protocols and standard operating procedures in forensic radiography

    Forensic Radiography has seen a rapid development over the last 20 years with the evolution of new technologies, but perhaps more significantly development of policy and guidelines for radiographers, the adoption of standards and standard operating procedures and changes in education and training. This thesis explores the author’s contribution to the body of knowledge in this discipline through the use of participatory action research which was undertaken through a sequence of iterative cycles over time. The author, as practitioner-researcher, examined the practice of forensic radiography through these cycles of data gathering, studying a number of different operational situations, each of which presented specific organisational issues requiring resolution. Practitioners and service users as members of the organisational system were active participants in the cyclical process of planning, taking action and evaluating the actions. Longitudinal, cross sectional and case study research designs employed observation and reflection, focus groups and multidisciplinary group discussions, questionnaires and semi-structured interviews. These led to collaborative findings and the development of new knowledge that was applied to subsequent situations, which were then further examined and refined within an overarching action research model. The thesis highlights the author’s new contributions to the body of knowledge from the respective studies and resultant publications with specific reference to the development of standards, protocols and standard operating procedures in forensic radiography

    Intense Collaboration In Globally Distributed Teams: Evolving Patterns Of Dependencies And Coordination

    As multi-national firms and major offshore outsourcing companies develop experience with global work, their globally distributed teams face the challenge of collaborating intensely without the common interaction advantages associated with collocated work. This chapter analyzes the sources of intense collaboration. It then introduces strategies that organizations have developed to reduce the intensity of collaboration (sequentializing work, using mediating artifacts, modularity), or to enable intense teamwork (real time contact, boundary spanners). Strategy properties and deployment opportunities and constraints are indicated in order to equip managers and researchers with a framework for handling or analyzing globally distributed teamwork

    This paper presents two empirical usability studies based on techniques from Human-Computer Interaction (HeI) and software engineering, which were used to elicit requirements for the design of a hypertext generation system. Here we will discuss the findings of these studies, which were used to motivate the choice of adaptivity techniques. The results showed dependencies between different ways to adapt the explanation content and the document length and formatting. Therefore, the system's architecture had to be modified to cope with this requirement. In addition, the system had to be made adaptable, in addition to being adaptive, in order to satisfy the elicited users' preferences
