    Profiling risk factors for chronic uveitis in juvenile idiopathic arthritis: a new model for EHR-based research.

    BackgroundJuvenile idiopathic arthritis is the most common rheumatic disease in children. Chronic uveitis is a common and serious comorbid condition of juvenile idiopathic arthritis, with insidious presentation and potential to cause blindness. Knowledge of clinical associations will improve risk stratification. Based on clinical observation, we hypothesized that allergic conditions are associated with chronic uveitis in juvenile idiopathic arthritis patients.MethodsThis study is a retrospective cohort study using Stanford's clinical data warehouse containing data from Lucile Packard Children's Hospital from 2000-2011 to analyze patient characteristics associated with chronic uveitis in a large juvenile idiopathic arthritis cohort. Clinical notes in patients under 16 years of age were processed via a validated text analytics pipeline. Bivariate-associated variables were used in a multivariate logistic regression adjusted for age, gender, and race. Previously reported associations were evaluated to validate our methods. The main outcome measure was presence of terms indicating allergy or allergy medications use overrepresented in juvenile idiopathic arthritis patients with chronic uveitis. Residual text features were then used in unsupervised hierarchical clustering to compare clinical text similarity between patients with and without uveitis.ResultsPreviously reported associations with uveitis in juvenile idiopathic arthritis patients (earlier age at arthritis diagnosis, oligoarticular-onset disease, antinuclear antibody status, history of psoriasis) were reproduced in our study. Use of allergy medications and terms describing allergic conditions were independently associated with chronic uveitis. The association with allergy drugs when adjusted for known associations remained significant (OR 2.54, 95% CI 1.22-5.4).ConclusionsThis study shows the potential of using a validated text analytics pipeline on clinical data warehouses to examine practice-based evidence for evaluating hypotheses formed during patient care. Our study reproduces four known associations with uveitis development in juvenile idiopathic arthritis patients, and reports a new association between allergic conditions and chronic uveitis in juvenile idiopathic arthritis patients

    Girls in Mining: Research Findings from Ghana, Niger, Peru, and United Republic of Tanzania

    [Excerpt] Research carried out by the International Labour Organization’s International Programme on the Elimination of Child Labour (ILO–IPEC) between April and December 2006 has produced evidence that girls as well as boys are involved in hazardous work in the small-scale mining industry. Due to the fact that boys are statistically more likely to be involved in hazardous child labour than girls, 1 the appalling work of girls is often overlooked. In the small-scale mining industry especially, little is understood about the roles and activities of girls and the effect that this has on their lives and livelihoods. Not much is known of the dynamics that brought them into this type of employment and consequently what could lead them out of it. The issue of girl child labour in mining is largely unknown, it is often not fully recognized by the law, and missed by the intervention services and the media. New evidence presented in this paper challenges the general understanding of gender roles in small-scale mining communities. It forces us to acknowledge a more intricate reality for boys and girls as the evidence shows that the involvement of girl child labour in mining is much more frequent and far-reaching than was previously recognized. The assumption that girls are only involved in prostitution and domestic work is incorrect; girls are involved in tasks related to the extraction, transportation and processing stages of mining as well as in other mining-related jobs such as selling food and supplies to the miners. The gender balance appears to be shifting. Girls are involved in more and more hazardous occupations deeper into the interiors of the mine, but at the same time they are also upheld to their traditional female responsibilities in the home. The result is that girls in mining communities are forced to juggle their domestic tasks with other paid or non-paid work. Often, girls are performing just as hazardous tasks as boys, working longer hours, with a greater workload and often have a lesser chance of schooling, withdrawal or rehabilitation

    An artificial neural network approach for assigning rating judgements to Italian Small Firms

    Based on new regulations of Basel II Accord in 2004, banks and financial nstitutions have now the possibility to develop internal rating systems with the aim of correctly udging financial health status of firms. This study analyses the situation of Italian small firms that are difficult to judge because their economic and financial data are often not available. The intend of this work is to propose a simulation framework to give a rating judgements to firms presenting poor financial information. The model assigns a rating judgement that is a simulated counterpart of that done by Bureau van Dijk-K Finance (BvD). Assigning rating score to small firms with problem of poor availability of financial data is really problematic. Nevertheless, in Italy the majority of firms are small and there is not a law that requires to firms to deposit balance-sheet in a detailed form.     Early hospital mortality prediction using vital signals

    Early hospital mortality prediction is critical as intensivists strive to make efficient medical decisions about the severely ill patients staying in intensive care units. As a result, various methods have been developed to address this problem based on clinical records. However, some of the laboratory test results are time-consuming and need to be processed. In this paper, we propose a novel method to predict mortality using features extracted from the heart signals of patients within the first hour of ICU admission. In order to predict the risk, quantitative features have been computed based on the heart rate signals of ICU patients. Each signal is described in terms of 12 statistical and signal-based features. The extracted features are fed into eight classifiers: decision tree, linear discriminant, logistic regression, support vector machine (SVM), random forest, boosted trees, Gaussian SVM, and K-nearest neighborhood (K-NN). To derive insight into the performance of the proposed method, several experiments have been conducted using the well-known clinical dataset named Medical Information Mart for Intensive Care III (MIMIC-III). The experimental results demonstrate the capability of the proposed method in terms of precision, recall, F1-score, and area under the receiver operating characteristic curve (AUC). The decision tree classifier satisfies both accuracy and interpretability better than the other classifiers, producing an F1-score and AUC equal to 0.91 and 0.93, respectively. It indicates that heart rate signals can be used for predicting mortality in patients in the ICU, achieving a comparable performance with existing predictions that rely on high dimensional features from clinical records which need to be processed and may contain missing information.Comment: 11 pages, 5 figures, preprint of accepted paper in IEEE&ACM CHASE 2018 and published in Smart Health journa

    A Statistical Measure of a Population's Propensity to Engage in Post-Purchase Online Word-of-Mouth

    The emergence of online communities has enabled firms to monitor consumer-generated online word-of-mouth (WOM) in real-time by mining publicly available information from the Internet. A prerequisite for harnessing this new ability is the development of appropriate WOM metrics and the identification of relationships between such metrics and consumer behavior. Along these lines this paper introduces a metric of a purchasing population's propensity to rate a product online. Using data from a popular movie website we find that our metric exhibits several relationships that have been previously found to exist between aspects of a product and consumers' propensity to engage in offline WOM about it. Our study, thus, provides positive evidence for the validity of our metric as a proxy of a population's propensity to engage in post-purchase online WOM.     Classification of Stabilometric Time-Series Using an Adaptive Fuzzy Inference Neural Network System

    Stabilometry is a branch of medicine that studies balance-related human functions. The analysis of stabilometric-generated time series can be very useful to the diagnosis and treatment balance-related dysfunctions such as dizziness. In stabilometry, the key nuggets of information in a time series signal are concentrated within definite time periods known as events. In this study, a feature extraction scheme has been developed to identify and characterise the events. The proposed scheme utilises a statistical method that goes through the whole time series from the start to the end, looking for the conditions that define events, according to the expertsÂż criteria. Based on these extracted features, an Adaptive Fuzzy Inference Neural Network (AFINN) has been applied for the classification of stabilometric signals.
