Experiences of Laryngectomized Individuals in Rediscovering Ways to Enjoy Life as Themselves

Abstract

目的:本研究は,喉頭摘出者が摘出手術を決断した時から,自分らしく生活を楽しめるように なるまでの体験のプロセスを明らかにすることを目的とした。 方法:喉頭摘出術を受け発声会に所属し「自分らしく生活を楽しんでいる」6 名に 半構成的面接を行い,逐語録を質的統合法(KJ 法)で分析した。 結果:自分らしく生活を楽しめるようになるまでの体験について【手術の決断と失声を自覚する】 ,【代用音声を獲得する】,【新たな声で伝える】,【気管孔を持って生活する】, 【障がいを受け入れる】,【生活を楽しむ】,【周囲からの支え】,【持ち前の考え方】の 9 つのシンボルマークが抽出された。 結論:求められる看護として,術前からの心理的支援と情報提供,代用音声獲得や生活再構築を 支える継続的な看護,さらにピアサポートや患者会への橋渡しが重要であり,特にピアとの 関わりは自己効力感を高め,回復過程を促進することが示唆された。Purpose: This study aimed to clarify how patients who undergo laryngectomy learn to enjoy life in their own way from the time they decide to undergo surgery. Methods: Semi-structured interviews were conducted with six individuals who had undergone laryngectomy, belonged to a voice club, and reported “enjoying life in their own way.” Verbatim transcripts were analyzed using the KJ method. Results: Nine symbolic themes were identified: deciding to undergo surgery, becoming aware of voicelessness, acquisition of alternative voice, communicating with a new voice, living with a tracheostoma, acceptance of disability, enjoying life, support from those around them, and innate mindset. Conclusion: It was suggested that nursing care aimed at helping laryngectomy patients enjoy life in their own way should include psychological support and information provision before surgery, continuous assistance for acquiring alternative speech and rebuilding daily life, and bridging to peer support and patient associations.
