Key Indicators of a Skilled Midwife’s Emphasizes During Delivery Assistance

Abstract

目的：熟練助産師が分娩介助時において重要視している考えを明らかにする。 方法：熟練助産師3 名を対象に，フォーカスグループインタビューを実施した。 本研究は，所属大学研究倫理審査委員会の承認を受けて実施した。 結果：熟練助産師 3 名の年齢 47～66 歳，経験年数 24～34 年，分娩介助件数 1000～3000 件であった。 熟練助産師は，【常に母児の安全リスクを助産診断した上でできるだけ自然な分娩を目指し見守る】， 【妊娠期からの産婦の分娩に対する前向きな姿勢は母児に良い影響をもたらす】， 【分娩体位や分娩環境は安楽と安全を考えて選択する】， 【超音波を活用しつつ，自律した助産診断ができるようにする】， 【分娩進行の管理についてすべてのスタッフとディスカッションをする】の 5 カテゴリーを重視していた。 結論：妊婦の産む力を最大限に引き出し，正常分娩に導くための臨床判断力を向上するための 臨場感がある教育の工夫が必要である。Objective: To clarify the key considerations of experienced midwives in providing delivery assistance. Method: A focus group interview was held with three experienced midwives. This study was approved by the research ethics committee of the affiliated university. Results: The participants were 47–66 years old, had 24–34 years of experience, and had assisted in 1,000–3,000 deliveries. The midwives highlighted five main categories: (1) consistently evaluating maternal and fetal safety risks while striving for a natural birth, (2) staying continuously involved during pregnancy to promote a positive attitude during labor and enhance maternal and fetal health, (3) choosing labor positions and environments based on comfort and safety, (4) using ultrasound while forming independent midwifery diagnoses, and (5) discussing labor progress management with all staff members. Conclusion: Education that improves clinical decision-making skills is crucial for optimizing women’s birthing capabilities and supporting normal deliveries.departmental bulletin pape
