Diagnosis and treatment of peripheral arteriovenous malformations

Abstract

Cilj ovog rada je pregled, klasfikacija, kliničkih prezentacija, dijagnostike i modaliteta liječenja perifernih arteriovenskih malformacija s naglaskom na dijagnostičke i terapijske postupke. AVM su kongenitalne vaskularne malformacije koje predstavljaju patološke poveznice arterija i vena koje komuniciraju putem centralnog nidusa (mreže displastičnih žila) bez prisustva kapilara i predstavljaju najrjeđi oblik vaskularnih malformacija. Periferne AVM su AVM lokalizirane izvan središnjeg živčanog sustava. Klinički simptomi perifernih AVM ovise o stupnju proširenosti i lokalizaciji, a većina perifernih AVM ima tendenciju progresije s izrazito teškim posljedicama na okolna tkiva. Različite se klasifikacije koriste za klasificiranje vasklularnih anomalija, no temeljnom i najbolje prihvaćenom smatra se Mulliken i Glowacki klasifikacija, a najraširenijom i najčešće korištenom smatra se International Society for the Study of Vascular Anomalies (ISSVA) klasifikacija. U dijagnostičke svrhe koriste se različite radiološke tehnike, no zlatni standard je magnetna rezonancija koja je revolucionarizirala diferencijaciju i karakterizaciju vaskularnih malformacija te procjenu težine bolesti. Kirurško liječenje rezervirano je za lokalizirane periferne AVM, a endovaskularno liječenje je izbor liječenja za sve ostale. U liječenju se koriste različita sklerozacijska/embolizacijska sredstva, od kojih je najpoznatiji etanol.The purpose of my thesis is an overview of the classification, clinical presentation, diagnostic procedures and treatment of peripheral arteriovenous malformations (AVMs) while emphasizing the diagnostic and treatment modalities. AVMs are congenital vascular malformations composed of pathological communications between arteries and veins that communicate through a net of dysplastic vascular vessels, called the nidus, which bypasses the capillary bed. They also represent the rarest form of vascular malformations. Peripheral AVMs are AVMs located outside the central nervous system. The clinical presentation of peripheral AVMs depends on the clinical staging and localization, and the majority of lesions have a tendency to progress with severe complications. Many classification systems are used for the categorization of vascular anomalies, but the most widely accepted and fundamental one is the Mulliken and Glowatski classification, as well as the International Society for the Study of Vascular Anomalies (ISSVA) classification. Many radiological techniques are used for the diagnosis, but the gold standard is magnetic resonance imaging, which revolutionized the differentiation, description, and staging of peripheral AVMs. Surgical treatment is used only for localized peripheral AVMs, while endovascular treatment is the treatment of choice for all other forms. Different types of sclerosing and embolic agents are used in the treatment, among which the best known is ethanol
