    BackgroundPatients with symptomatic intracranial atherosclerosis (ICAS) of ≥ 70% luminal stenosis are at high risk of stroke recurrence. We aimed to evaluate the relationships between hemodynamics of ICAS revealed by computational fluid dynamics (CFD) models and risk of stroke recurrence in this patient subset.MethodsPatients with a symptomatic ICAS lesion of 70-99% luminal stenosis were screened and enrolled in this study. CFD models were reconstructed based on baseline computed tomographic angiography (CTA) source images, to reveal hemodynamics of the qualifying symptomatic ICAS lesions. Change of pressures across a lesion was represented by the ratio of post- and pre-stenotic pressures. Change of shear strain rates (SSR) across a lesion was represented by the ratio of SSRs at the stenotic throat and proximal normal vessel segment, similar for the change of flow velocities. Patients were followed up for 1 year.ResultsOverall, 32 patients (median age 65; 59.4% males) were recruited. The median pressure, SSR and velocity ratios for the ICAS lesions were 0.40 (-2.46-0.79), 4.5 (2.2-20.6), and 7.4 (5.2-12.5), respectively. SSR ratio (hazard ratio [HR] 1.027; 95% confidence interval [CI], 1.004-1.051; P = 0.023) and velocity ratio (HR 1.029; 95% CI, 1.002-1.056; P = 0.035) were significantly related to recurrent territorial ischemic stroke within 1 year by univariate Cox regression, respectively with the c-statistics of 0.776 (95% CI, 0.594-0.903; P = 0.014) and 0.776 (95% CI, 0.594-0.903; P = 0.002) in receiver operating characteristic analysis.ConclusionsHemodynamics of ICAS on CFD models reconstructed from routinely obtained CTA images may predict subsequent stroke recurrence in patients with a symptomatic ICAS lesion of 70-99% luminal stenosis

    Stent-assisted reconstructive endovascular repair of intracranial aneurysms: long-term clinical and angiographic follow-up

    Abstract Background and Purpose: The development of self-expanding stents dedicated to intracranial use has significantly widened the applicability of endovascular therapy to many intracranial aneurysms. The purpose of this study was to report the angiographic and clinical outcomes of wide-necked intracranial aneurysms treated with stent. Methods: Between January 2007 and October 2011 we deployed 22 stents in 20 patients with wide-necked cerebral aneurysms. Inclusion criteria restricted the group to adult patients with wide-necked intracranial aneurysms (ruptured and unruptured lesions). Immediate post-procedural angiographic studies were performed to evaluate successful occlusion of the aneurysm as well as patency of the parent vessel. We assessed long term angiography follow-up to detect in-stent stenosis, progressive thrombosis, recurrence and need for retreatment. Clinical outcome was assessed with the modifing Ranking Scale (mRS). Results: Technical success was obtained in all 22 (100%) cases. Angiography immediately after treatment procedure showed complete occlusion in 7 aneurysms (35%), neck remnant in 11 (55%), incomplete occlusion in 1 (5%) and partial occlusion in 1 (5%). During the endovascular embolization procedure no rupture of the sac or bleeding complication occurred; none of the patients needed undergoing surgical crossover. Procedure-related adverse events occurred in one (5%) patient: a brachial artery pseudoaneurysm. Three (15%) patients had neurological complications after procedure, whose 1 (5%) transitory complication spontaneusly resolved. Two patients (10%), had acute complete in-stent thrombosis which resolved after intraarterial administration of abciximab and placement of a new stent in-stent. Of the 20 patients treated with stent deployment, a follow-up imaging study was available in all 19 surviving patients (95%) at an average of 16.2 months (range, 6 to 50 months). The first follow-up DSA, compared with initial angiography, showed no changes in 14 aneurysms (73.7%), progressive thrombosis in 3 (15.7%), and major recurrence in 2 (10.5%). The overall rate of succesful procedure to 6 months is 89.5%; there was 1 case of asintomatic moderate endothelial hyperplasia. The further follow-up imaging study, showed no changes in 17 (89.5%) of the 19 surviving patients, 1 progressive thrombosis and 1 minor recurrence. One month- and long term (average of 16.2 months; range, 6 to 50 months) clinical follow-up showed no worsening in the mRS in 18 (90%) of 20 patients, 1 (5%) mRS 2 and 1 (5%) mRS 6. All the survived patients are alive and we did not observe periprocedural or long-term intracranial bleeding events or symptomatic stent related stenosis/occlusion complication. Conclusions: Our findings suggest that the endovascular treatment of intracranial aneurysms by stenting is feasible, effective and safe; follow-up results proved intact parent arteries and stable occlusion rates in the majority of treated aneurysms. Nevertheless, long-term data on safety and efficacy and larger patient groups are necessary

    Airway management is basic for anesthesia practice, and sometimes it can represent a really dramatic scenario for both the patient and the physicians. Laryngoscopy has been the gold standard of airway visualization for more than 60 years, showing its limitations and failure rates with time. New technology has made available an opportunity to move the physician’s eye inside patient airways thanks to video laryngoscopy and video assisted airway management technique. Undoubtedly, we have entered a new era of high resolution airway visualization and different approach in airway instrumentation. Nevertheless, each new technology needs time to be tested and considered reliable, and pitfalls and limitations may come out with careful and long lasting analysis, so it is probably not the right time yet to promote video assisted approach as a new gold standard for airway visualization, despite the fact that it certainly offers some new prospects. In any case, whatever the visualization approach, no patient dies because of missed airway visualization or failed intubation, but due to failed ventilation, which remains without doubt the gold standard of any patient safety goal and airway management technique.Održavanje dišnog puta je osnovna anesteziološka vještina koja ponekad predstavlja u pravom smislu dramatičan scenarij za bolesnika i liječnika. Laringoskopija, sa svim svojim ograničenjima i neuspjesima, predstavlja zlatni standard vizualizacije dišnog puta već više od 60 godina. Nove tehnologije, zahvaljujući videolaringoskopiji i video asistiranim tehnikamaodržavanja dišnog puta, omogućile su pomicanje očiju liječnika unutar dišnog puta bolesnika. Bez sumnje, ušli smo u novu eru visoke rezolucije vizualizacije dišnog puta i različitog pristupa instrumentalizaciji dišnog puta. Međutim, svaka nova tehnologija zahtijeva vrijeme da bi se testirala i smatrala pouzdanom. Zapreke i ograničenja mogu se iznjedriti nakon pažljive i dugotrajne analize, stoga vjerojatno još nije vrijeme da se promovira video asistirani pristup kao novi zlatni standard u vizualizaciji dišnog puta, iako on definitivno predstavlja novu budućnost. U svakom slučaju, koji kod bio pristup vizualizaciji, nijedan bolesnik ne umire zbog propuštene vizualizacije ili neuspjele intubacije, već umire zbog neuspješne ventilacije koja ostaje bez sumnje zlatni standard bilo kojeg cilja za sigurnost bolesnika i tehnike zbrinjavanja dišnog puta
