    Developing Evidence-Based Practice in Chaplaincy: A Study of Unit and Chaplain-Specific Integration

    Health care chaplains provide spiritual care across diverse hospital units. As a result of the complex and interprofessional nature of health care services, different units are associated with unique integration and collaboration needs. Effective chaplain practice and patient-centered care are enhanced by sensitivity to unit differences. Our research, aimed at quality improvement, examined unit and chaplain integration to promote unit-specific evidence-based practice. Integration was conceptualized by five dimensions: interdependence, newly created professional activities, flexibility, collective ownership of goals, reflection on process. data was collected using the Interprofessional Integration and Collaboration Instrument (Bronstein, 2002), which has appropriate measurement quality (Bainbridge et al., 2015). Addition questions captured chaplain-specific integration and methods of chaplain engagement (charting, referrals). The survey was available in electronic and paper format. Over 150 staff from 10 units participated in the 2017 convenience survey. Survey results were used to develop profiles of unit and chaplain-specific integration; of chaplain engagement; and of perceived contributions of chaplains to patient care. Demographic information was summarized to determine representativeness. The findings contribute to quality improvement and evidence-based practice by identifying how chaplains can effectively integrated within specific units.     Hopf bifurcation in a gene regulatory network model: Molecular movement causes oscillations

    Gene regulatory networks, i.e. DNA segments in a cell which interact with each other indirectly through their RNA and protein products, lie at the heart of many important intracellular signal transduction processes. In this paper we analyse a mathematical model of a canonical gene regulatory network consisting of a single negative feedback loop between a protein and its mRNA (e.g. the Hes1 transcription factor system). The model consists of two partial differential equations describing the spatio-temporal interactions between the protein and its mRNA in a 1-dimensional domain. Such intracellular negative feedback systems are known to exhibit oscillatory behaviour and this is the case for our model, shown initially via computational simulations. In order to investigate this behaviour more deeply, we next solve our system using Greens functions and then undertake a linearized stability analysis of the steady states of the model. Our results show that the diffusion coefficient of the protein/mRNA acts as a bifurcation parameter and gives rise to a Hopf bifurcation. This shows that the spatial movement of the mRNA and protein molecules alone is sufficient to cause the oscillations. This has implications for transcription factors such as p53, NF-kappakappaB and heat shock proteins which are involved in regulating important cellular processes such as inflammation, meiosis, apoptosis and the heat shock response, and are linked to diseases such as arthritis and cancer

Contributing factors include: Capacity chaplain in the mastery of the yellow book, remarkable spirit of chaplain to lead the lodge, the spirit of religious teachers in teaching the students a good presence of input, the motivation of the students to study religion, presence of co-operation with schools lodges in the BabakanCiwaringin. The support of parents and community students. b. Inhibiting factors include: Lack of system recitation yellow book, lack of availability of facilities and infrastructure, less the maximum support from the alumni, Lack of support from the local government, limited professional educators, (d). Implications of managerial leadership chaplain and chaplain efforts in improving the quality of Islamic education and the students are: Positive because Az-Ziyadah schools have scored alumni who became role models when they come back to take part in the community.     Multiscale modelling of cancer progression and treatment control : the role of intracellular heterogeneities in chemotherapy treatment

    Cancer is a complex, multiscale process involving interactions at intracellular, intercellular and tissue scales that are in turn susceptible to microenvironmental changes. Each individual cancer cell within a cancer cell mass is unique, with its own internal cellular pathways and biochemical interactions. These interactions contribute to the functional changes at the cellular and tissue scale, creating a heterogenous cancer cell population. Anticancer drugs are effective in controlling cancer growth by inflicting damage to various target molecules and thereby triggering multiple cellular and intracellular pathways, leading to cell death or cell-cycle arrest. One of the major impediments in the chemotherapy treatment of cancer is drug resistance driven by multiple mechanisms, including multi-drug and cell-cycle mediated resistance to chemotherapy drugs. In this article, we discuss two hybrid multiscale modelling approaches, incorporating multiple interactions involved in the sub-cellular, cellular and microenvironmental levels to study the effects of cell-cycle, phase-specific chemotherapy on the growth and progression of cancer cells.PostprintPeer reviewe

    Around the world, chaplains provide specialist spiritual care for people with complex healthcare needs. If the nature of chaplain interventions was better understood then multidisciplinary colleagues could both improve their own skills in spiritual care and better understand when to refer people to chaplains. A survey was constructed to establish what aspects of the chaplain/patient relationship were most important for patients in Scotland and Australia. Outcomes were measured with the Scottish Patient Reported Outcome Measure (Scottish PROM©). Results from 610 respondents showed the strongest correlation was between ‘being able to talk about what is on my mind’ and the Scottish PROM (rs(452) = .451, p \u3c .0005). ‘Being able to talk about what is on my mind’ proved more important than being listened to, having faith/beliefs valued, or being understood. Given the importance placed on listening and understanding by clinicians, this original and counterintuitive finding goes some way to explaining the unique role and function of healthcare chaplaincy

    Predicting the Importance of Hospital Chaplain Care in a Trauma Population

    Background. The purpose of this exploratory study was to determine if the importance of chaplain care is associated with and could be predicted by patient or injury characteristics. Methods. A telephone survey of recently discharged trauma patients was conducted. Logistic regression analyses were conducted to determine what factors are associated with the importance of chaplain care and satisfaction with chaplain care. Results. Self-reported religious affiliation was associated with the importance of chaplain care and importance of chaplain care was associated with satisfaction with chaplain care. Conclusions. The value of chaplain care cannot be measured by patient characteristics, therefore, chaplain care should be offered to all patients and families

    On December 2, 1862, just eleven days before the Battle of Fredericksburg, Virginia, Jeremiah Mickly said goodbye to his wife and two children and reported for duty with the 177th Pennsylvania Infantry to become a Civil War chaplain. The only known photograph ofMickly shows him dressed in the standard chaplain\u27s uniform of the day: a plain black frock coat with a standing collar and black buttons with plain black pantaloons. Like many other Civil War soldiers, Mickly re-enlisted for service after his stint with the 177th ended, becoming chaplain of the 43rd Regiment, United States Colored Troops. Impressed with the educational progress and courage of the black soldiers he served with, Mickly wrote a history of the 43rd Regiment. The 88-page booklet was published in 1866 in Gettysburg by J. E. Wible, Printer. Mickly\u27s book and correspondence prove that his Civil War experience shaped his belief that black people are entitled to equal rights. [excerpt
