    Manipulating Particles for Micro- and Nano-Fluidics Via Floating Electrodes and Diffusiophoresis

    The ability to accurately control micro- and nano-particles in a liquid is fundamentally useful for many applications in biology, medicine, pharmacology, tissue engineering, and microelectronics. Therefore, first particle manipulations are experimentally studied using electrodes attached to the bottom of a straight microchannel under an imposed DC or AC electric field. In contrast to a dielectric microchannel possessing a nearly-uniform surface charge, a floating electrode is polarized under the imposed electric field. The purpose is to create a non-uniform distribution of the induced surface charge, with a zero-net-surface charge along the floating electrode\u27s surface. Such a field, in turn, generates an induced-charge electro-osmotic (ICED) flow near the metal strip. The demonstrations by using single and multiple floating electrodes at the bottom of a straight microchannel, with induced DC electric field, include particle enrichment, movement, trapping, reversal of motion, separation, and particle focusing. A flexible strategy for the on-demand control of the particle enrichment and positioning is also proposed and demonstrated by using a locally-controlled floating metal electrode. Then, under an externally imposed AC electric field, the particle deposition onto a floating electrode, which is placed in a closed circular cavity, has been experimentally investigated. In the second part of the study, another particle manipulation method was computationally investigated. The diffusiophoretic and electrodiffusiophoretic motion of a charged spherical particle in a nanopore is subjected to an axial electrolyte concentration gradient. The charged particle experiences electrophoresis because of the imposed electric field and the diffusiophoresis is caused solely by the imposed concentration gradient. Depending on the magnitude and direction of the imposed concentration gradient, the particle\u27s electrophoretic motion can be accelerated, decelerated, and even reversed in a nanopore by the superimposed diffusiophoresis. Based on the results demonstrated in the present study, it is entirely conceivable to extend the development to design devices for the following objectives: (1) to enrich the concentration of, say, DNA or RNA, and to increase their concentrations at a desired location. (2) to act as a filtration device, wherin the filtration can be achieved without blocking the microfluidic channel and without any porous material. (3) to act as a microfluidic valve, where the particles can be locally trapped in any desired location and the direction can be switched as desired. (4) to create nanocomposite material formation or even a thin nanocomposite film formation on the floating electrode. (5) to create a continuous concentration-gradient-generator nanofluidic device that may be obtained for nanoparticle translocation process. This may achieve nanometer-scale spatial accuracy sample sequencing by simultaneously controlling the electric field and concentration gradient

    In this paper, we present a high performance and low cost hardware architecture for real-time implementation of an SAD reuse based hierarchical motion estimation algorithm for H.264 / MPEG4 Part 10 video coding. This hardware is designed to be used as part of a complete H.264 video coding system for portable applications. The proposed architecture is implemented in Verilog HDL. The Verilog RTL code is verified to work at 68 MHz in a Xilinx Virtex II FPGA. The FPGA implementation can process 27 VGA frames (640x480) or 82 CIF frames (352x288) per second

    Recently, a new international standard for video compression named H.264 / MPEG-4 Part 10 is developed. This new standard offers significantly better video compression efficiency than previous international standards. The variable block size motion estimation is the most compute-intensive part of an H.264 video encoder. The full search method is impractical for real-time implementations since it requires a high computational complexity. Therefore, many fast motion estimation algorithms have been developed for real-time implementations. In this thesis, we used an SAD reuse based hierarchical motion estimation algorithm for real-time H.264 / MPEG-4 Part 10 video coding. This algorithm uses the Lagrangian cost parameter (SAD+λR) for selecting the best motion vector. We designed a high performance and low cost hardware architecture for real-time implementation of this algorithm. We have considered several alternative designs and decided on this architecture based on a cost/performance analysis. This architecture uses a novel data flow resulting in a low cost and high performance hardware. This hardware is designed to be used as part of a complete H.264 video coding system for portable applications. The proposed architecture is implemented in Verilog HDL. The Verilog RTL code is verified to work at 63 MHz in a Xilinx Virtex II FPGA. The FPGA implementation can process 25 VGA frames (640x480) or 76 CIF frames (352x288) per second

    Ruptured Granulosa Cell Tumor of the Ovary as a Cause of Acute Abdomen in Postmenopausal Woman

    Acute abdomen with hemoperitoneum is a very rare entity in postmenopausal women due to gynecologic conditions. A 54-year-old, postmenopausal woman was brought to emergency department with severe abdominal pain. Physical examination revealed acute abdomen findings with 15 cm pelvic mass on the right adnexal region. Immediate exploratory laparotomy was performed. During laparotomy 1000 cc of bloodstained fluid, ruptured and actively bleeding large mass arising from right ovary was observed. Right salpingo-oopherectomy was performed in emergency conditions, and pathology report revealed an adult type of granulosa cell tumor. After this result, staging surgery was performed and patient was diagnosed as granulosa cell tumor stage 1 c. Cisplatin, etoposide, and bleomycin chemotherapy was given. Clinicians should be aware of granulosa cell tumors which may occur at any age and prone to rupture. Frozen section will be helpful in order to avoid incomplete surgeries especially in postmenopausal women presented with intra-abdominal bleeding

    Evaluation of atrial electromechanical conduction delay in case of hemodynamically insignificant rheumatic heart disease: A tissue Doppler study

    Background: Atrial electromechanical delay (AEMD) that reflects delayed conduction may show us the clinical reflection of pathological changes in the atria. The main objective of the present study is to investigate AEMD in patients who had previous rheumatic carditis but without hemodynamically significant valvular disease. Methods: A total of 40 patients, previously diagnosed as rheumatic carditis but without significant valvular stenosis/regurgitation and atrial enlargement; and 39 age- and-sex matched controls were enrolled for the present study. Parameters of AEMD (lateral mitral annulus electromechanical delay, septal mitral annulus electromechanical delay and lateral tricuspid annulus electromechanical delay) were measured with tissue Doppler echocardiography and left intra-atrial and inter-atrial conduction times were calculated accordingly. A 24h ambulatory Holter monitoring was used in both groups to detect atrial fibrillation episodes and quantify atrial extrasystoles. Results: Parameters of AEMD, including left intra-atrial and inter-atrial conduction times of subjects in the study group were longer compared to the control group (23.7 ± 7.0 vs. 18.3 ± 6.2). Conclusions: Increased AEMD is observed in patients with previous rheumatic carditis and no significant valvular stenosis/regurgitation and atrial enlargement, which may partly explain the increased incidence of atrial fibrillation observed in these patients

    30-day morbidity and mortality of sleeve gastrectomy, Roux-en-Y gastric bypass and one anastomosis gastric bypass: a propensity score-matched analysis of the GENEVA data

    Background: There is a paucity of data comparing 30-day morbidity and mortality of sleeve gastrectomy (SG), Roux-en-Y gastric bypass (RYGB), and one anastomosis gastric bypass (OAGB). This study aimed to compare the 30-day safety of SG, RYGB, and OAGB in propensity score-matched cohorts. Materials and methods: This analysis utilised data collected from the GENEVA study which was a multicentre observational cohort study of bariatric and metabolic surgery (BMS) in 185 centres across 42 countries between 01/05/2022 and 31/10/2020 during the Coronavirus Disease-2019 (COVID-19) pandemic. 30-day complications were categorised according to the Clavien–Dindo classification. Patients receiving SG, RYGB, or OAGB were propensity-matched according to baseline characteristics and 30-day complications were compared between groups. Results: In total, 6770 patients (SG 3983; OAGB 702; RYGB 2085) were included in this analysis. Prior to matching, RYGB was associated with highest 30-day complication rate (SG 5.8%; OAGB 7.5%; RYGB 8.0% (p = 0.006)). On multivariate regression modelling, Insulin-dependent type 2 diabetes mellitus and hypercholesterolaemia were associated with increased 30-day complications. Being a non-smoker was associated with reduced complication rates. When compared to SG as a reference category, RYGB, but not OAGB, was associated with an increased rate of 30-day complications. A total of 702 pairs of SG and OAGB were propensity score-matched. The complication rate in the SG group was 7.3% (n = 51) as compared to 7.5% (n = 53) in the OAGB group (p = 0.68). Similarly, 2085 pairs of SG and RYGB were propensity score-matched. The complication rate in the SG group was 6.1% (n = 127) as compared to 7.9% (n = 166) in the RYGB group (p = 0.09). And, 702 pairs of OAGB and RYGB were matched. The complication rate in both groups was the same at 7.5 % (n = 53; p = 0.07). Conclusions: This global study found no significant difference in the 30-day morbidity and mortality of SG, RYGB, and OAGB in propensity score-matched cohorts

    30-Day morbidity and mortality of bariatric metabolic surgery in adolescence during the COVID-19 pandemic – The GENEVA study

    Background: Metabolic and bariatric surgery (MBS) is an effective treatment for adolescents with severe obesity. Objectives: This study examined the safety of MBS in adolescents during the coronavirus disease 2019 (COVID-19) pandemic. Methods: This was a global, multicentre and observational cohort study of MBS performed between May 01, 2020, and October 10,2020, in 68 centres from 24 countries. Data collection included in-hospital and 30-day COVID-19 and surgery-specific morbidity/mortality. Results: One hundred and seventy adolescent patients (mean age: 17.75 ± 1.30 years), mostly females (n = 122, 71.8%), underwent MBS during the study period. The mean pre-operative weight and body mass index were 122.16 ± 15.92 kg and 43.7 ± 7.11 kg/m2, respectively. Although majority of patients had pre-operative testing for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (n = 146; 85.9%), only 42.4% (n = 72) of the patients were asked to self-isolate pre-operatively. Two patients developed symptomatic SARS-CoV-2 infection post-operatively (1.2%). The overall complication rate was 5.3% (n = 9). There was no mortality in this cohort. Conclusions: MBS in adolescents with obesity is safe during the COVID-19 pandemic when performed within the context of local precautionary procedures (such as pre-operative testing). The 30-day morbidity rates were similar to those reported pre-pandemic. These data will help facilitate the safe re-introduction of MBS services for this group of patients

    Risk portfolio on modern finance has become increasingly technical, requiring the use of sophisticated mathematical tools in both research and practice. Since companies cannot insure themselves completely against risk, as human incompetence in predicting the future precisely that written in Al-Quran surah Luqman verse 34, they have to manage it to yield an optimal portfolio. The objective here is to minimize the variance among all portfolios, or alternatively, to maximize expected return among all portfolios that has at least a certain expected return. Furthermore, this study focuses on optimizing risk portfolio so called Markowitz MVO (Mean-Variance Optimization). Some theoretical frameworks for analysis are arithmetic mean, geometric mean, variance, covariance, linear programming, and quadratic programming. Moreover, finding a minimum variance portfolio produces a convex quadratic programming, that is minimizing the objective function ðð¥with constraintsð ð 𥠥 ðandð´ð¥ = ð. The outcome of this research is the solution of optimal risk portofolio in some investments that could be finished smoothly using MATLAB R2007b software together with its graphic analysis
